Conventional salvage therapy in Hodgkin's disease is appropriate in patients who relapse after a first complete remission of greater than one year many of whom who will have good long term survival with no further therapy. Patients who do not achieve CR, who relapse within one year of CR1 or who have a second relapse will have a survival of less than 20% at 5 years and these patients are candidates for high dose therapy (HDT) and Autologous Bone Marrow Transplantation (ABMT) or a second line salvage protocol. Current published and registry data suggests that HDT and ABMT may be superior and th

A male infant with DiGeorge syndrome had hypogammaglobulinaemia with a normal number of B cells. CD3(+) T cells were reduced and the CD4(+)/CD8(+) ratio was reversed. Proliferative responses of T cells to mitogens and to allogeneic cells were low. The pokeweed mitogen (PWM)-induced B cell differentiation assay revealed a higher than normal suppressor T cell activity. This suggests that some T cells had differentiated into functionally mature cells resulting in an imbalance of regulatory T cell functions and that excess suppressor activity might play a role in hypogammaglobulinaemia. This study examines the suitability of the intracranial route of transplantation of human cell lines in nude mice for preclinical testing of cytostatic drugs. Tumours were generated by inoculation of human hematopoietic cell lines. Animals in which intramuscularly or intracranially transplanted human tumours developed were treated with cytostatic drugs. Three different cell line generated tumours (T-ALL L735, Hodgkin cell line L540 and Burkitt Lymphoma Line BJAB) were treated with four different drugs (CY, ADM, VCR, VP16). The data show a good correspondence between the results of treatment of

Twenty-nine patients with diffuse large cell lymphoma who failed traditional chemotherapy were treated with high dose chemotherapy with or without total body irradiation followed by infusion of cryopreserved autologous marrow. Complete response was achieved in 11/29 patients (38%), partial response in 13/29 patients (45%) and 5/29 patients (17%) had no response. Six complete responders remain well and free of disease for 5+, 6+, 9+, 10+, 18+ and 25+ months, 3 relapsed at 2, 3 and 8 months after marrow infusion, and 2 died from infectious complications. Complete response was seen more frequentl

Non-relapse mortality (NRM) remains a major and often unavoidable adverse event following allogeneic hematopoietic stem cell transplantation (allo-HSCT), particularly among older adults and patients with multiple comorbidities. Although the hematopoietic cell transplantation-specific comorbidity index (HCT-CI) is widely used for risk assessment, its predictive performance for NRM is limited, particularly for capturing functional status and physiological reserve. Sarcopenia, a marker of physical frailty and reduced physiological reserve, has recently emerged as a potential prognostic marker in

High-dose post-transplant cyclophosphamide (PTCy) for graft versus host disease (GVHD) prophylaxis after HLA-matched allogeneic hematopoietic cell transplantation (HCT) in adults with hematologic malignancies is standard of care when combined with tacrolimus and mycophenolate mofetil (MMF) after reduced intensity conditioning (RIC). However, use of PTCy after HLA-matched HCT in pediatrics is not well described. To show that PTCy allows for successful engraftment, survival, and low rates of GVHD, a retrospective analysis of patients ≤21 years treated at Johns Hopkins (n = 22) who underwent HLA-

Considering that post-transplant cyclophosphamide (PTCy) has emerged as an effective GVHD prophylaxis across donor types, we conducted a multicenter retrospective study to evaluate outcomes of PTCy-based allo-HCT in patients with MDS. A total of 291 patients undergoing allo-HCT with PTCy between 2014 and 2024 were included. Median age was 61 years (range, 20 to 76), with 28.2% aged >65 years. Reduced-intensity conditioning was used in 73.5% of patients, and donor sources included matched sibling (20.3%), matched unrelated (25.8%), mismatched unrelated (14.4%), and haploidentical donors (39.5%)

Haploidentical hematopoietic cell transplantation (haplo-HCT) with post-transplant cyclophosphamide (PTCy) is a viable option for patients lacking a matched donor. In this setting, early inflammatory complications (EICs), such as cytokine release syndrome (CRS) and engraftment syndrome (ES), represent significant complications. Their risk factors and clinical impact, particularly in pediatric populations, remain poorly defined. This multicenter, retrospective study analyzed 102 consecutive pediatric haplo-HCT with PTCy for acute leukemia. It aimed to define EICs incidence, risk factors, and tr

Outcomes of allogeneic hematopoietic stem cell transplantation (allo-HSCT) are heavily influenced by donor type. While matched sibling donors (MSD) have traditionally been the preferred source, the use of haploidentical (haplo) donors has expanded following the introduction of post-transplant cyclophosphamide (PTCy). This study compares clinical outcomes between MSD and haplo-HSCT recipients treated with uniform PTCy-based prophylaxis. In this retrospective multicenter study utilizing the CIBMTR registry, we analyzed 875 adult patients with acute myeloid leukemia, acute lymphoblastic leukemia,

BACKGROUND: Late-onset non-infectious pulmonary complications (LONIPCs) following hematopoietic stem cell transplantation (HSCT) are a known indication for need of lung transplantation. This study aimed to clarify the clinical characteristics of patients with LONIPCs after HSCT who were registered for lung transplantation and reveal the risk factors for waiting list mortality.
METHODS: We retrospectively reviewed the clinical data of patients with LONIPCs after allogeneic HSCT who were referred to Okayama University Hospital and registered in the Japan Organ Transplant Network for deceased-do

Hematopoietic stem cell transplant (HSCT) recipients are at high risk for opportunistic infections due to profound immunosuppression and graft-versus-host disease (GvHD). Molds and nontuberculous mycobacteria (NTM) pose diagnostic and therapeutic challenges, especially when infections overlap. A 42-year-old woman with prior allogeneic HSCT for acute myeloid leukemia (AML) developed pulmonary infections with Microascus spp. and Mycobacterium chimaera, later complicated by Aspergillus calidoustus and RSV infection. Initial therapy included voriconazole, amphotericin B, and a macrolide-based mult

BACKGROUND: Post-transplant cyclophosphamide (PTCy) is a standard graft-versus-host disease (GVHD) prophylaxis in allogeneic hematopoietic cell transplantation (allo-HCT). While effective, concerns remain about cyclophosphamide-related cardiotoxicity, especially in patients with pre-existing cardiac morbidity, a population often underrepresented in clinical trials.
OBJECTIVES: To assess the incidence and outcomes of early (ECE, ≤100 days) and late (LCE, >100 days) cardiac events in acute myeloid leukemia (AML) patients with and without baseline cardiac morbidity undergoing allo-HCT with PTCy.

The role of ABO blood group system mismatch on allogeneic hematopoietic cell transplantation (allo-HCT) outcomes is controversial since current publications of large datasets are lacking. We retrospectively analyzed 30,487 patients transplanted between 2010 and 2021 using the EBMT registry to assess ABO incompatibility's effect on non-relapse mortality (NRM), overall survival (OS), progression-free survival (PFS), relapse incidence (RI), acute GvHD (aGvHD), chronic GvHD (cGvHD), and neutrophil engraftment. Transplantations were classified as ABO-compatible (56.3%), major (18.1%), minor (20.1%)

PURPOSE: There are no clinical guidelines for micronutrient supplementation and monitoring during the post-acute phase of Stem cell transplantation (SCT) and during graft versus host disease (GVHD). Hence this comprehensive systematic review aimed to evaluate the evidence for vitamin and mineral supplementation and monitoring to inform clinical practice.
METHODS: Eligible studies included adults who had undergone SCT more than 100 days ago or were experiencing GVHD and were prescribed a micronutrient supplement and/or had micronutrient levels monitored. Human studies published in English were

Mycophenolate mofetil (MMF) is commonly included in post-transplant cyclophosphamide (PTCy) based graft-versus-host disease (GVHD) prophylaxis after haploidentical (haplo) hematopoietic cell transplant (HCT). In the non-PTCy setting, higher MMF dose/kg has been shown to reduce rates of acute graft-versus-host disease (GVHD). When used in conjunction with PTCy, MMF is dosed at 15 mg/kg three times daily up to a maximum dose of 3 g/day. Thus, patients who weigh ≥67 kg receive 3 g/day and a variable dose/kg of MMF. We investigated the impact of MMF dose/kg on clinical outcomes following haploiden

BACKGROUND AIMS: In HLA-identical hematopoietic stem cell transplantation (HSCT), HLA-C1 group killer cell immunoglobulin-like receptor (KIR) ligands have been linked to graft-versus-host disease, whereas C2 homozygosity was associated with increased relapses. The differential impact of the recipients versus the donor's HLA-C KIR ligands cannot be determined in HLA-identical HSCT but may be elucidated in the haploidentical setting, in which HLA-C (including the HLA-C KIR ligand group) mismatching is frequently present.
METHODS: We retrospectively investigated the effect of recipient versus do

Gastrointestinal bleeding (GIB) is a serious complication following allogeneic hematopoietic stem cell transplantation (HSCT), with limited data on its incidence and characteristics, particularly for upper gastrointestinal bleeding (UGIB) of gastric origin. We aimed to evaluate the incidence, clinical, endoscopic, and histopathologic features, and outcomes of UGIB, with a focus on gastric vascular ectasias (GVEs) in patients undergoing HSCT with graft-versus-host disease (GVHD) prophylaxis using post-transplant cyclophosphamide (PTCY), sirolimus or calcineurin inhibitors, and mycophenolate mof

Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is used in the treatment of high-risk acute myeloid leukemia (AML) and myelodysplastic syndromes (MDS); however, the treatment has high risk of severe transplantation-related mortality (TRM). In this study, we examined pretransplantation serum samples derived from 92 consecutive allotransplant recipients with AML or MDS. Using nontargeted metabolomics, we identified 1274 metabolites including 968 of known identity (named biochemicals). We further investigated metabolites that differed significantly when comparing patients with and

Stenotrophomonas maltophilia (S. maltophilia) is an aerobic nonfermenting Gram-negative bacillus widely distributed in the environment that has inherent multidrug resistance to beta-lactam and carbapenem antibiotics. S. maltophilia infection (SMI) is known as an important fatal complication following allogeneic hematopoietic stem cell transplantation (HSCT), but its clinical characteristics have not been well clarified. A retrospective study to identify the incidence, risk factors, and outcomes of SMI after allogeneic HSCT was performed using the database of the Japanese nationwide registry, i
