The RCPCH care pathway for children with drug allergies: an evidence and consensus based national approach.

AIMS: The Royal College of Paediatrics and Child Health (RCPCH) was commissioned by the Department of Health to develop a drug allergy pathway. It focuses on defining the competences to improve the equity of care received by children. METHOD: The drug pathway was developed by a multidisciplinary working group and was based on a comprehensive review of evidence. The team decided to focus on IgE-mediated reactions as these have the greatest potential to be life-threatening. RESULTS: The results are presented in four parts: evidence review, pathway mapping, external review and core knowledge do

Childhood abuse uncovered in a palliative care audit.

OBJECTIVE: This article aims to highlight potentially high levels of childhood sexual abuse within Cancer and Palliative Care Service users. METHODS: During a routine audit, data was collated to quantify a perceived high level of disclosure of pre-existing psychological trauma within the palliative care caseload of a Macmillan Children and Families Therapist. Families comprised adults (generally parents or step-parents), one of whom was terminally ill or recently deceased who had direct responsibility for children and young people aged under 20 years old. Each family had at least two members

The association between time of birth and fetal injury resulting in death.

OBJECTIVE: In obstetrics, the care of patients in labor or with emergencies takes place day and night. Birth-related injury is among the worst of obstetric outcomes. This study sought to examine the relationship between time of birth and fetal injury resulting in death. STUDY DESIGN: The Birth-Related Neurologic Injury Compensation Association (NICA) is a Florida organization that pays for the care of infants >2500 g with birth-related brain or spinal cord injury resulting in permanent impairment. We conducted a case-control study using all deaths from the NICA database from 1989 to 2002. Dat

Prevention of spread of hepatitis C.

Hepatitis C virus (HCV) is transmitted by percutaneous or permucosal exposure to infectious blood or blood-derived body fluids. Based on the results of cohort and acute case control studies, risk factors associated with acquiring HCV infection in the United States have included transfusion of blood and blood products and transplantation of solid organs from infected donors, injecting drug use, occupational exposure to blood (primarily contaminated needle sticks), birth to an infected mother, sex with an infected partner, and multiple heterosexual partners. Nosocomial and iatrogenic transmissio

Childhood sexual abuse and affective symptoms in women: a general population study.

BACKGROUND: Child sexual abuse (CSA) is associated, after controlling for other adversities, with a range of non-psychotic disorders in adult life. There is a need to clarify whether CSA contributes to risk of disorder in the absence of such adversities, and given that associations may be accounted for by genetic mechanisms, whether they are seen where the perpetrator of CSA is not a biological relative, and where there has been only one incident. METHODS: A questionnaire-based study of a socio-economically representative sample of women age 25-36 (N = 862) was carried out. Parental care and

The ecological context of child health in Saudi Arabia.

The general background to child health in Saudi Arabia is reviewed. Information is provided on the social and demographic characteristics of the population, on common health indicators, on the health care system and its utilization, and on the general pattern of childhood morbidity and mortality. The unprecendented socioeconomic development has transformed the health care system. In 15 years the number of nurses have increased from 3261 to 29896, physicians from 1172 to 14335, primary health care centers from 591 to 1821, and hospital beds from 9036 to 30707. In spite of this progress, the dis

A decade of screening in the hemoglobinopathies: is a national program to prevent sickle cell anemia possible?

The increased attention given to sickle cell anemia for the past decade has enhanced public and professional understanding of the disease. The need to counsel a large segment of the general public has resulted in the production of a large amount of informative educational material on the subject. Despite the large amount of effort expended in population screening and counseling, very few studies of the counseling process have been done. It is still not known whether people use the knowledge gained for decisions regarding their reproduction. The development of direct antenatal diagnosis of sick

"We are running on the fumes of goodwill" Professionals' experiences of delivering 24/7 end-of-life care to children and their families: a qualitative study.

BACKGROUND: Despite recent improvements, there is still stark inequity in the funding and provision of 24/7 end-of-life care for children, resulting in many families not receiving the support they need. To inform, plan and implement service changes it is important to take account of what works in current contexts and existing models of care, and to learn how professionals 'on the ground' are currently experiencing 24/7 care delivery. METHODS: The study aimed to explore professionals' perspectives of delivering 24/7 paediatric palliative care and their expectations and needs of a new service.

Cross-cultural adaptation of the Paediatric Palliative Screening Scale for the early referral of pediatric patients to palliative care teams.

Introduction. The Paediatric Palliative Screening Scale (PaPaS Scale) is a tool designed to help identify children aged 1 to 18 who would benefit from pediatric palliative care (PPC) and to facilitate early referral. No similar tool exists in Spanish. The objective was to translate and cross-culturally adapt the PaPaS Scale into Spanish for screening and early referral to PPC teams for pediatric patients aged 12 months to 18 years with life-threatening or life-limiting illnesses. Methods. Translation and cultural adaptation of the PaPaS Scale into Spanish, consisting of five phases: obtaining

Evaluation of emergency department utilization for dermatologic conditions in the pediatric population within the United States from 2009-2015.

BACKGROUND: Dermatologic conditions comprise a significant number of emergency department visits in the pediatric population in the United States. Understanding key predictors of emergency department utilization for dermatologic conditions is important to reduce inappropriate use. METHODS: A total of 44 554 sampled patient emergency department visits, consisting of patients less than 18 years of age, were collected from the National Hospital Ambulatory Medical Care Survey between 2009 to 2015. ICD-9 codes were used to define dermatologic conditions versus non-dermatologic conditions with univ

Long-term grief experiences of parents who lost a child to cancer - Results from a multicenter cross-sectional study.

BACKGROUND: Childhood cancer deaths may be associated with persistent parental grief. Evidence on grief symptoms beyond ten years post-loss remains limited. AIMS: We described grief symptoms, identified grief profiles, and examined associations between grief intensity and sociodemographic, cancer-related characteristics, and time since death. DESIGN: This multicenter cross-sectional survey was conducted in Switzerland (July 2022-July 2023). We used PG-13. Items were rated on a 5-point scale (1-5). We calculated the sum score to assess grief intensity (range 11-55). We employed descriptive st

Hypertrophic cardiomyopathy associate with a PLN gene mutation in a child: a case report.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a common hereditary disease of the myocardium, caused by mutations in at least 20 genes, encoding cardiac sarcomere proteins and sarcomere-associated proteins, while genes related to calcium metabolism, such as phospholamban (PLN), have been reported rarely, in less than 1% of positive patients. This study aims to report a unique case of child's HCM caused by a missense variant in the PLN gene on chromosome 6 (NM_002667.5:c.106T>C, p.Cys36Arg). This mutation in children, not previously reported in the literature except for an adult, expand the k

Needs and experiences of families after a sudden unexplained death in childhood: a qualitative study.

BACKGROUND: Sudden unexplained death in childhood (SUDC) is a rare and devastating experience for families. In the UK, multi-agency investigation by police, health and social care of sudden, unexpected child deaths is a statutory requirement aiming to identify full causes for deaths. Families should be allocated bereavement keyworkers for support throughout the investigative process which can take several months. Previous research has focused on multi-agency investigation of sudden infant deaths, with little known about parents' experiences for deaths of older children. METHODS: Bereaved pare

A Case Report of a 14-year-old Boy with Mixed-type Autoimmune Hemolytic Anemia.

Autoimmune hemolytic anemia (AIHA) is uncommon in the pediatric population, particularly when it manifests as severe anemia. AIHA is characterized by a positive direct antiglobulin test (DAT) and immune-mediated red blood cell (RBC) destruction. AIHA is subclassified on the basis of the thermal characteristics of autoantibody into warm, cold, and mixed. Mixed AIHA shows both the common types and characteristics of warm and cold types. A 14-year-old male, born of nonconsanguineous marriage, admitted with complaints of dizziness and hematuria. It was not associated with decreased urine output an

A Comprehensive Review on Postpartum Depression.

One of the most common psychological effects following childbirth is postpartum depression. Postpartum depression (PPD) has a significant negative impact on the child's emotional, mental as well as intellectual development if left untreated, which can later have long-term complications. Later in life, it also results in the mother developing obsessive-compulsive disorder and anxiety. Many psychological risk factors are linked with PPD. The pathophysiology of the development of PPD is explained by different models like biological, psychological, integrated, and evolutionary models, which relate

Surgically Acquired Vocal Cord Palsy in Infants and Children with Congenital Heart Disease (CHD): Description of Feeding Outcomes.

Impaired swallowing in infants can impact upon the ability to feed orally, often resulting in dependency on supplementary feeding. Such difficulties can lead to an increased burden of care and associated costs. The primary aim of this study was to investigate the impact of vocal cord palsy (VCP), acquired intraoperatively during cardiac surgery, on the feeding outcomes of infants at a tertiary metropolitan children's hospital. An additional aim was to obtain preliminary information on the impact of feeding difficulties in this group on the quality of life of parents and families. A review of 4
