Advances in understanding the Th17/Treg balance in systemic lupus erythematosus: implications for treatment and management.

Systemic lupus erythematosus (SLE) is a chronic autoimmune disorder characterized by a complex and multifactorial pathogenesis. Recent studies have highlighted the critical role of the disrupted balance between T helper 17 (Th17) cells and regulatory T (Treg) cells in disease initiation and progression. Understanding this imbalance has offered a conceptual framework for developing novel targeted interventions. In this review, we focus on emerging therapeutic strategies aimed at restoring the Th17/Treg balance. We place particular emphasis on the mechanisms and clinical implications of biologic

Challenges and opportunities: CAR-T cell therapy in autoimmune diseases.

Chimeric antigen receptor T (CAR-T) cell therapy is rapidly moving from hematologic malignancies into severe autoimmune disease, particularly B cell-mediated disorders such as systemic lupus erythematosus (SLE), systemic sclerosis (SSc) and inflammatory myopathies. Recent autologous and allogeneic studies show that deep immune-cell depletion can induce drug-free remission or major clinical responses in the refractory patients. However, translating CAR-T cell therapy into autoimmune disease raises challenges that differ from oncology, including target selection in heterogeneous immune ecosystem

Proteomics in Acute Myeloid Leukemia: Current Applications in Precision Medicine and Targeted Immunotherapy.

Acute myeloid leukemia (AML) is a heterogeneous and aggressive malignancy with limited therapeutic options and high relapse rates. Despite advances in genomic profiling, many genetic aberrations remain untargetable, and current risk stratification models often fail to predict treatment responses. Proteomics offers a complementary approach by directly measuring protein abundance, post-translational modifications, and protein-protein interactions, providing mechanistic insights into drug resistance, disease progression, and therapeutic vulnerabilities. In this review, we explore the emerging rol

Case Report: Glofitamab in the treatment of a patient with central nervous system-involved Burkitt lymphoma.

Burkitt lymphoma (BL) is a rare and highly aggressive B-cell non-Hodgkin lymphoma. While targeted combination chemotherapy can be effective, it is associated with a high rate of relapsed or refractory disease and a pronounced propensity for central nervous system (CNS) involvement. Currently, novel therapies such as chimeric antigen receptor T-cell (CAR-T) therapy have not demonstrated established efficacy in BL. Given the poor prognosis and the challenge of managing relapsed/refractory BL, the use of bispecific antibodies, specifically glofitamab, as employed in this case, has yielded a favor

Systemic amyloid light-chain amyloidosis beyond ANDROMEDA: Diagnostic challenges and therapeutic updates.

Systemic immunoglobulin light-chain amyloidosis is a rare but life-threatening plasma cell disorder characterized by the production of misfolded monoclonal light chains that deposit as amyloid fibrils, leading to pathologic tissue remodeling and progressive multiorgan dysfunction. Despite substantial therapeutic advances, delayed diagnosis remains common because of nonspecific presentations and the need for coordinated hematologic, pathologic, and organ-specific evaluation. The introduction of daratumumab-based regimens has transformed frontline therapy, whereas advances in cytogenetic profili

Development of a subunit vaccine candidate (RBD-HA trimer) provides dual protection against multi-subtype avian influenza viruses and QX-type infectious bronchitis virus.

Avian influenza virus (AIV) and infectious bronchitis virus (IBV) are major respiratory pathogens of poultry, and their co-circulation complicates disease control. Here, we developed a chimeric subunit vaccine, RBD-HA, in which the receptor-binding domain (RBD) of the QX-type IBV spike protein replaced the immunodominant head domain of H9N2 AIV haemagglutinin. Structural analyses showed that RBD-HA formed a stable trimeric assembly, supporting the use of the HA stalk as an antigen-presenting scaffold. In chickens, RBD-HA induced humoral responses against both viruses and protected against homo

Neurologic Complications of Cancer Immunotherapy.

The use of oncologic-directed immunotherapies, including immune checkpoint inhibitors (ICIs) and chimeric antigen receptor T cell (CAR-T) therapy, has risen exponentially in recent years. Although these therapies have been shown to improve long-term survival and quality of life in responsive patients, their nonspecific immune system activation can lead to unanticipated neurotoxicity, among other complications. Immune-related adverse events (irAEs) may involve any location in the entire neuroaxis, posing a diagnostic challenge, and may cause severe, life-threatening complications. ICI-related n

[In vivo CAR-T cells: When the patient becomes their own factory].

Over the past decade, ex vivo autologous chimeric antigen receptor (CAR)-T cell therapies have profoundly reshaped the treatment of B-cell malignancies. Despite their remarkable clinical efficacy, their use remains limited by complex manufacturing processes, demanding logistics, long production times, and high costs. In vivo CAR-T approaches are emerging as a potential solution to overcome these obstacles by inducing CAR expression in T-cells directly within the patient's body. This overview provides a comprehensive look at the current development of these therapies. We describe the main deliv

Recombinant antigen-based multiplex serology for the differential diagnosis of human cestodiases: toward integrated immunoprofiling.

The serological diagnosis of human cestodiases, particularly cystic echinococcosis (CE) and neurocysticercosis (NCC), is frequently constrained by cross-reactivity associated with crude parasite extracts, limiting its utility in differential diagnosis. This review evaluates the potential of recombinant antigen-based multiplex serology to improve the differentiation between CE and NCC. We examine key diagnostic targets, including the chimeric antigens DIPOL and 2B2t for CE, and T24H together with lentil lectin-adherent glycoproteins (LLGP) for NCC. We further discuss bead-based multiplex platfo

Clinical research progress and challenges of vaccine for human papillomavirus-associated cancers.

As a major cause of cancer-related morbidity in women, cervical cancer is strongly related to chronic infection by high-risk human papillomavirus (HPV). This viral persistence also underpins the pathogenesis of a broad spectrum of cancers. In this review, we summarize the research progress of prophylactic and therapeutic HPV vaccines in clinical trials. Furthermore, we present a critical analysis of future challenges, specifically discussing the type replacement phenomenon and its potential implications for long-term vaccine efficacy. We also expand the discourse to a pan-gender and pan-cancer

Human iPSC-derived salivary gland cell sheets integrate with injured glands to form glandular structures.

Functional integration of transplanted cells with host tissue remains a major challenge in cell-based therapies for tissue damage in organs with complex structures, such as exocrine glands. In this study, we investigated whether salivary gland organoids derived from human induced pluripotent stem cells (hiPSCs) could be integrated into injured salivary glands using cell sheet engineering. Cell sheet engineering has demonstrated therapeutic potential in a range of organs, including the heart, retina, and lungs. We found that hiPSC-derived salivary gland organoids contain long-term maintainable

Cyclophosphamide for the Treatment of Refractory Immune Effector Cell-Associated Neurotoxicity Syndrome Following CD19-Targeted CAR T-Cell Therapy.

Immune effector cell-associated neurotoxicity syndrome (ICANS) is a serious complication of chimeric antigen receptor T-cell (CAR-T) therapy, associated with significant morbidity and mortality. While corticosteroids and anakinra are cornerstones of treatment, a subset of patients develop severe, steroid-refractory ICANS, highlighting a critical need for more effective therapies. We present the case of a 51-year-old male with relapsed/refractory Philadelphia chromosome-positive (Ph+) B-cell acute lymphoblastic leukemia (B-ALL) who developed grade 4 ICANS following brexucabtagene autoleucel CAR

Can we cure antiphospholipid syndrome?

Antiphospholipid antibody syndrome (APS) is a vasculopathy with recurrent thrombosis and/or miscarriages mediated by autoantibodies against PL-binding proteins (aPL), mainly beta2glycoprotein I (β2GPI). While clotting is the key in vascular APS, thrombosis is not critical for placenta pathology. Despite that, anticoagulant/antiplatelet drugs are the leading treatments, but this is not a 'one-size-fits-all' therapy, and recurrences are reported. Additional therapies (e.g. antimalarials, statins) and a better characterization of the individual risk profile may improve the outcome. Nevertheless,

Therapeutic targeting of the protein tyrosine kinase-7 in cancer: an overview.

The protein tyrosine kinase-7 (PTK7) is an evolutionarily conserved transmembrane receptor that has emerged as a potential therapeutic target for human tumors. PTK7 is a pseudokinase that is involved in the modulation of the Wnt signaling pathway through interactions with other receptors. These interactions result in targeted gene activation that regulates cell polarity, migration, and proliferation during embryogenesis. Aside of this role during development, PTK7 has been shown as overexpressed in numerous cancers including colon carcinoma, leukemia, neuroblastoma, hepatoma, and ovarian cance

Differential conformational expansion of NUP98-HOXA9 oncoprotein from nanosized assemblies to macrophases.

Transcription factors (TFs) play a central role in gene regulation by binding to specific DNA sequences and orchestrating the transcriptional machinery. A majority of eukaryotic TFs have a block copolymer architecture, with at least one block being a folded DNA interaction domain, and another block being highly enriched in intrinsic disorder. In this study, we focus on NUP98-HOXA9 (NHA9), a chimeric TF implicated in leukemogenesis. By integrating experiments and simulations, we examine the structural dynamics of NHA9's FG domain across assembly states. We find that the FG domain has different

Integrated scFv identification and CAR T cell generation for AML targeting in vivo.

Cancer immunotherapy has witnessed remarkable advancements, particularly in the development of chimeric antigen receptor (CAR) T cell therapy. Here, we integrated single chain variable fragment (scFv) development with CAR T cell generation based on a newly developed scFv phagemid library. High-throughput long-read PacBio sequencing identified 4.5 × 107 unique full-length scFv proteins within the generated library. As a proof of principle, we screened for scFvs targeting C-type lectin-like molecule-1 (CLL1) with subsequent cloning into a third generation retroviral CAR backbone. Functional assa

Mechanisms for resistance to BCMA-targeted immunotherapies in multiple myeloma.

Multiple myeloma (MM) remains incurable and patients eventually face the relapse/refractory dilemma. B cell maturation antigen (BCMA)-targeted immunotherapeutic approaches have shown great effectiveness in patients with relapsed/refractory MM, mainly including chimeric antigen receptor T cells (CAR-T), bispecific T cell engagers (TCEs), and antibody-drug conjugates (ADCs). However, their impact on long-term survival remains to be determined. Nonetheless, resistance to these novel therapies is still inevitable, raising a challenge that we have never met in both laboratory research and clinical

Broad-spectrum vaccines against various and evolving viruses: from antigen design to nanoparticle delivery.

Pathogen evolution and narrow vaccine coverage urgently demand broad-spectrum vaccines. This review explores two pivotal technological fronts: structural biology- and immunoinformatics-guided antigen design, and utilizing nanoparticle-based delivery systems to induce broad immune responses. We critically analyze four antigen optimization strategies: (i) structure-based antigen design, (ii) conserved epitope targeting, (iii) consensus sequence-based antigen engineering, and (iv) chimeric immunogen design. Additionally, the common types and characteristics of nanoparticles are described briefly.
