BACKGROUND: Early reports of both contact and noncontact transscleral Nd:YAG laser cyclophotocoagulation have been encouraging; however, recent evidence indicates a significant incidence of hypotony, visual loss, and phthisis with the noncontact technique with more than 6 months of follow-up. The authors sought to determine the intermediate term effects of contact transscleral Nd:YAG laser cyclophotocoagulation (CYC).

Noncontact transscleral Nd:YAG laser thermal mode cyclocoagulation is known to reduce intraocular pressure (IOP). Thirty-five eyes were treated at a mean of 2.5 Joules and followed for a mean of 28 months. The mean preoperative IOP of 39.8 mmHg (standard deviation, 11.7) was consistently decreased by 45% to 55% over the follow-up period (standard deviation, 19.9 to 30.8), along with a significant reduction of medical therapy. Late-onset angle-closure glaucoma secondary to retinopathy of prematurity (ROP) occurred in ten eyes of ten patients. The age at presentation ranged from 12 to 45 years (mean, 32 years). Fifty-one soft hydrogel posterior chamber intraocular lenses (IOLs) were implanted in the ciliary sulcus after planned extracapsular cataract extractions. In nine cases the IOLs were positioned with one haptic in the capsular bag and the other in the ciliary sulcus. The cases of 23 patients with untreated cilioretinal artery obstruction were reviewed. Three distinct groups were found: (1) isolated cilioretinal artery obstruction, (2) cilioretinal artery obstruction associated with central retinal venous obstruction, and (3) cilioretinal artery obstruction in conjunction with ischemic optic neuropathy.
