Is there a benefit from intensified medical and psychological interventions in patients with functional dyspepsia not responding to conventional therapy?

AIM: In a prospective randomized, controlled trial, to compare the long-term outcome of intensive medical therapy (with or without cognitive-behavioural or muscle relaxation therapy) vs. standard medical therapy in patients with refractory functional dyspepsia (FD), referred to a tertiary referral medical center. METHODS: A total of 100 consecutive FD patients were allocated to a standardized symptom-oriented 4 month therapy (SMT, n = 24), intensive medical therapy (IMT, medical therapy with testing-for and targeting-of abnormalities of motor-and-sensory function, n = 28) or IMT plus psycholo

The effect of exercise, cognitive therapy, and nutritional counseling in treating bulimia nervosa.

OBJECTIVE: The aim of this treatment study on bulimia nervosa was (i) to examine the effect of physical exercise as an experimental treatment condition against the well-documented effect of cognitive-behavioral therapy (CBT), and (ii) to compare the effect of CBT versus the effect of nutritional advice as one single treatment component of CBT. METHOD: Normal weight female bulimic patients aged 18-29 yr were randomly assigned to a physical exercise program (N = 15), CBT (N = 16), nutritional advice (N = 17), or a waiting list control group (N = 16). Seventeen healthy female control subjects we

Venlafaxine versus fluoxetine in the treatment of atypical anorectic outpatients: a preliminary study.

The efficacy of venlafaxine and fluoxetine in the treatment of atypical anorexia nervosa (AN) was compared in a controlled trial. A consecutive series of 24 atypical anorectic females was assigned to either venlafaxine (75 mg/day) or fluoxetine (40 mg/day) plus cognitive-behavioural therapy (CBT). Eating Disorder Examination (EDE12.0D), Beck Depression Inventory (BDI) and State and Trait Anxiety Inventory (STAI) scores were compared before and after 6 months of treatment. Venlafaxine and fluoxetine determined an increase of body mass index (BMI) and a significant reduction of EDE12.0D and BDI

Psychotherapy of dysthymia.

OBJECTIVE: The author reviews empirical research on the psychotherapy of dysthymia. Dysthymia, a prevalent mood disorder, has been shown frequently to respond to antidepressant medication. The need for a treatment for dysthymic subjects unable or unwilling to take, or unresponsive to, medication still remains. METHODS: Studies were located by computerized search and the author's knowledge of the literature. All reports of studies on psychotherapy outcome for dysthymic patients, except studies of late-life chronic major depression, were included. RESULTS: Psychotherapy research on dysthymia h

Cognitive distraction and relaxation training for the control of side effects due to cancer chemotherapy.

Sixty cancer chemotherapy patients were randomly assigned to one of six conditions formed by a 3(cognitive distraction, relaxation training, no intervention) x 2(high anxiety, low anxiety) factorial design. All patients were followed for five consecutive chemotherapy sessions. Outcome measures included patient reports, nurse observations, and physiological indices. Results indicated that distraction patients reported less nausea prior to chemotherapy and lower systolic blood pressures after chemotherapy than controls. Relaxation training patients reported less nausea prior to chemotherapy and

KCC2 activation during postnatal development alleviates long-term deficits in CDKL5-deficient mice.

Cyclin-dependent kinase-like 5 (CDKL5) deficiency disorder (CDD) is a severe developmental and epileptic encephalopathy characterized by early onset drug-resistant seizures and later cognitive and social impairments. Existing therapies primarily involve antiseizure medications, which have sedative side effects and lack effective treatments for behavioral impairments. Potassium chloride cotransporter (KCC2) activity is regulated by phosphorylation and is a crucial component of the GABAergic inhibitory system. However, KCC2 dysfunction in CDD remains poorly understood. Here, to investigate poten

Repurposing of Cefotaxime for Its Therapeutic Potential in Alzheimer's Disease: An Explanation of the Possible Mechanism of Action.

Alzheimer's disease (AD) is the leading cause of dementia, characterized by progressive oxidative stress, neuroinflammation, and cognitive decline. Current pharmacological therapies are largely symptomatic, underscoring the need for new disease-modifying strategies. Drug repurposing provides an efficient approach to exploiting clinically approved compounds with established safety. Hence, in the current study, we investigated the neuroprotective potential of cefotaxime (CTX), a third-generation cephalosporin antibiotic, in an intracerebroventricular streptozotocin (ICV-STZ) rat model of AD. Frontotemporal dementia. How to deal with its diagnostic complexity?

INTRODUCTION: Frontotemporal dementia (FTD) encompasses a group of heterogeneous neurodegenerative disorders. Aside from genetic cases, its diagnosis is challenging, particularly in the early stages when symptoms are ambiguous, and structural neuroimaging does not reveal characteristic patterns. AREAS COVERED: The authors performed a comprehensive literature search through MEDLINE, Scopus, and Web of Science databases to gather evidence to aid the diagnostic process for suspected FTD patients, particularly in early phases, even in sporadic cases, ranging from established to promising tools. Updating theoretical frameworks in body dysmorphic disorder: A systematic review and meta-analysis.

BACKGROUND: Real world studies and psychological literature often do not consolidate existing data with theoretical models on body dysmorphic disorder (BDD). OBJECTIVE: This systematic review aims to refine and empirically support the conceptual understanding of BDD through an updated theoretical framework. METHODS: We systematically searched PubMed, Embase, CENTRAL, Scopus, and Web of Science from database inception to 26 November 2023 for studies reporting factors predisposing or associated with a diagnostic and statistical manual of mental disorders IV/V confirmed diagnosis of BDD. Effective Deep Brain Stimulation for Obsessive-Compulsive Disorder Requires Clinical Expertise.

BACKGROUND: Deep brain stimulation (DBS) is an innovative treatment for severe obsessive-compulsive disorder (OCD). Electrodes implanted in specific brain areas allow clinicians to directly modulate neural activity. DBS affects symptomatology in a completely different way than established forms of treatment for OCD, such as psychotherapy or medication. OBJECTIVE: To understand the process of improvement with DBS in patients with severe OCD. METHODS: By means of open-ended interviews and participant observation we explore how expert clinicians involved in the post-operative process of DBS opt

Partner criticism during acute-phase cognitive therapy for recurrent major depressive disorder.

Many patients with major depressive disorder (MDD) are married or in marriage-like relationships that could influence treatment process and outcomes. We clarified relations of patient-reported criticism from partners (perceived criticism) and criticism of partners with psychosocial functioning and changes in cognitive therapy (CT) for depression. Partnered outpatients (N = 219) received a 12-week CT protocol and completed measures repeatedly. As hypothesized, perceived criticism and criticism of partners correlated with personality (e.g., perceived criticism: trait mistrust, self-harm; critici

Future of brain stimulation: new targets, new indications, new technology.

In the last quarter of a century, DBS has become an established neurosurgical treatment for Parkinson's disease (PD), dystonia, and tremors. Improved understanding of brain circuitries and their involvement in various neurological and psychiatric illnesses, coupled with the safety of DBS and its exquisite role as a tool for ethical study of the human brain, have unlocked new opportunities for this technology, both for future therapies and in research. Serendipitous discoveries and advances in structural and functional imaging are providing abundant "new" brain targets for an ever-increasing nu

Marital predictors of symptom severity in panic disorder with agoraphobia.

Twenty-six to forty percent of individuals suffering from panic disorder with agoraphobia (PDA) do not benefit significantly from cognitive-behavior therapy. Marital problems are among risk factors that may explain this limited impact. Some studies suggest that PDA treatment outcome is related to the couple's ability to communicate and solve problems during and after treatment. It may be also useful to further clarify the interplay of marital interpersonal variables with PDA severity before any intervention. This study aims at specifying the links between PDA symptom severity on the one hand a

Predictors of early school age outcomes in very low birth weight children.

The purpose of this study was to examine predictors of outcome in very low birth weight (< 1500 g) children. The very low birth weight sample consisted of 68 children weighing less than 750 g at birth and 65 children weighing 750 to 1499 g at birth who had been matched to the less than 750 g birth weight children in terms of hospital of birth, age, sex, and race. Mean ages for these two groups were 6.7 and 6.9 years, respectively. Outcomes were measured in terms of tests of cognitive function, neuropsychological abilities, and academic achievement and parent and teacher ratings of child behavi

Heart rate variability profiles and exposure therapy treatment outcome in flight phobia.

The goal of this study was to explore why certain patients in a previous study on exposure therapy for flight phobia did not experience an improvement in their conditions. Participants from a treatment study (N = 45) were selected according to post-treatment results and divided into two groups: the unsatisfactory treatment outcome group (UTO, N = 10) and the satisfactory treatment outcome group (STO, N = 10). The differences between these two groups prior to receiving exposure therapy were analyzed at the behavioral, physiological, and cognitive levels. The UTO participants had been avoiding f

Addressing gender disparities in the efficacy of psychological interventions for behavioral addictions: protocol for a systematic review and individual patient data meta-analysis of randomized controlled trials.

BACKGROUND: Behavioral addictions (BAs) are increasingly recognized as significant mental health challenges, but evidence on treatment efficacy remains limited. Psychological interventions, particularly cognitive-behavioral therapy (CBT) and its internet-based formats, are considered first-line treatments. However, it remains unclear whether these interventions are equally effective across genders. Addressing this gap is essential for developing gender-sensitive treatment strategies and improving clinical outcomes in individuals with BAs. METHODS: This systematic review and individual patient
