Design of a pragmatic randomized implementation effectiveness trial testing a health system wide hypertension program for older adults.

Hypertension control remains poor. Multiple barriers at the level of patients, providers, and health systems interfere with implementation of hypertension guidelines and effective lowering of BP. Some strategies such as self-measured blood pressure (SMBP) and remote management by pharmacists are safe and effectively lower BP but have not been effectively implemented. In this study, we combine such evidence-based strategies to build a remote hypertension program and test its effectiveness and implementation in large health systems. This randomized, controlled, pragmatic type I hybrid implementa

Personality traits as predictors of depression across the lifespan.

BACKGROUND: Depression is a major public health concern. A barrier for research has been the heterogeneous nature of depression, complicated by the categorical diagnosis of depression which is based on a cluster of symptoms, each with its own etiology. To address the multifactorial etiology of depression and its high comorbidity with anxiety, we aimed to examine the relations between personality traits, diverse behavioral, cognitive and physical measures, and depression and anxiety over the lifespan. METHOD: Our sample was drawn from the NKI-RS, a community-based lifespan sample (N = 1494 par

The importance of individual beliefs in assessing treatment efficacy.

In recent years, there has been debate about the effectiveness of treatments from different fields, such as neurostimulation, neurofeedback, brain training, and pharmacotherapy. This debate has been fuelled by contradictory and nuanced experimental findings. Notably, the effectiveness of a given treatment is commonly evaluated by comparing the effect of the active treatment versus the placebo on human health and/or behaviour. However, this approach neglects the individual's subjective experience of the type of treatment she or he received in establishing treatment efficacy. Here, we show that

Prediction by Young Autistic Children from Visual and Spoken Input.

Recent theoretical accounts suggest that differences in the processing of probabilistic events underlie the core and associated traits of autism spectrum disorder (ASD). These theories hypothesize that autistic individuals are differentially impacted by disruptions in probabilistic input relative to neurotypical peers. According to this view, autistic individuals assign disproportionate weight to prediction errors such that novel input is overweighted relative to the aggregation of prior input; this is referred to as 'hyperplasticity' of learning. Prediction among autistic individuals has prim

Eye movement disorders: A new approach to preliminary screening of Parkinson's disease.

To investigate the characteristics and diagnostic values of the eye movement disorders in patients with Parkinson's disease (PD-EMDs), this cross-sectional study enrolled 127 Chinese patients with PD and 80 healthy controls, and divided them into training and validation sets based on enrollment time. Performance in the five oculomotor paradigms was assessed using an infrared pupil and a corneal reflection tracking device. The primary characteristics of PD-EMDs were elucidated as inaccurate fixation with high deviation (frequency and total quantity); inaccurate saccades with delayed reaction an

Investigating the relationship of trunk and postural control with pulmonary functions in subacute stroke patients.

Stroke is a disease with high mortality and morbidity that not only causes weakness in the extremities, loss of balance, and disturbances in trunk and postural control, but also affects respiratory function. The aim of this study was to investigate the relationship between trunk and postural control and pulmonary function in subacute stroke patients. Herein, 32 volunteer patients who were diagnosed with hemiplegia by a competent physician after unilateral hemorrhagic or ischemic stroke and who met the inclusion criteria participated in the study. Functional independence of the participants was

Risk Prediction of Alzheimer's Disease Conversion in Mild Cognitive Impaired Population Based on Brain Age Estimation.

Alzheimer's disease (AD) is one of the most common neurodegenerative diseases in the world. To reduce the incidence of AD, it's essential to quantify the AD conversion risk of mild cognitive impaired (MCI) individuals. Here, we propose an AD conversion risk estimation system (CRES), which contains an automated MRI feature extractor, brain age estimation (BAE) module, and AD conversion risk estimation module. The CRES is trained on 634 normal controls (NC) from the public IXI and OASIS cohorts, then it is evaluated on 462 subjects (106 NC, 102 stable MCI (sMCI), 124 progressive MCI (pMCI) and 1

Early Diagnosis and Biomarkers of Alzheimer's Disease Based on Spatio-temporal Graph Convolution Network.

Functional magnetic resonance imaging (fMRI) could detect the dynamic activity of brain function and communication. Previous studies have found reduced brain functional connectivity in Alzheimer's disease (AD) patients. In this study, we proposed to process fMRI data by spatio-temporal graph convolution network (ST-GCN) to achieve an early differential diagnosis of AD and to extract image markers using gradient-weighted class activation mapping (Grad-CAM). The data used in this study were from the Alzheimer's Disease Neuroimaging Initiative (ADNI) database, Xuanwu Hospital, and Tongji Hospital

Cerebellar Direct Current Stimulation Reveals the Causal Role of the Cerebellum in Temporal Prediction.

Temporal prediction (TP) influences our perception and cognition. The cerebellum could mediate this multi-level ability in a context-dependent manner. We tested whether a modulation of the cerebellar neural activity, induced by transcranial Direct Current Stimulation (tDCS), changed the TP ability according to the temporal features of the context and the duration of target interval. Fifteen healthy participants received anodal, cathodal, and sham tDCS (15 min × 2 mA intensity) over the right cerebellar hemisphere during a TP task. We recorded reaction times (RTs) to a target during the task in

Optimising verbal fluency analysis in neurological patients with dysarthria: examples from Parkinson's disease and hereditary ataxia.

BACKGROUND: Verbal fluency tests (VFTs) are widely used to assess cognitive-linguistic performance in neurological diseases. However, the influence of dysarthria on performance in tests requiring oral responses is unclear in ataxia and Parkinson's disease. OBJECTIVES: To determine the impact of dysarthria on VFT performance and evaluate the validity and reliability of alternative methods for analyzing VFT data. METHOD: Trained raters evaluated dysarthria using VFT recordings in people with ataxia (N = 61) or Parkinson's disease (PD; N = 69). Total Correct Items scores and qualitative paramet

Early response to cognitive behavioral therapy for body dysmorphic disorder as a predictor of outcomes.

Individuals with body dysmorphic disorder (BDD) suffer from distressing or impairing preoccupations with perceived imperfections in their appearance. This often-chronic condition is associated with significant functional impairment and elevated rates of psychiatric comorbidity and morbidity, including depression, substance use disorders, and suicidality. Cognitive behavioral therapy (CBT) for BDD has been shown to be efficacious. However, this intervention is long (up to 24 weeks) relative to many manualized approaches for other related conditions, there is a significant shortage of clinicians

Perirhinal and Postrhinal Damage Have Different Consequences on Attention as Assessed in the Five-Choice Serial Reaction Time Task.

The perirhinal (PER) and postrhinal (POR) cortices, structures in the medial temporal lobe, are implicated in learning and memory. The PER is understood to process object information and the POR to process spatial or contextual information. Whether the medial temporal lobe is dedicated to memory, however, is under debate. In this study, we addressed the hypothesis that the PER and POR are also involved in non-mnemonic cognitive functions. Rats with PER or POR damage and SHAM surgical controls were shaped, trained, and tested on the five-choice serial reaction time (5CSRT) task, which assesses
