An incidental finding of mesenteric hematoma in a patient with a new diagnosis of inflammatory bowel disease.

Mesenteric hematomas (MHs) are relatively rare and are characterized by localized bleeding in the mesenteric vascular tree presenting with abdominal pain, vomiting, abdominal distension, masses, and hypotension. We report a case of a MH which was incidentally found in a 10-year-old patient with a new diagnosis of ulcerative colitis (UC). This is the second-ever reported case of MH associated with inflammatory bowel disease to our knowledge, with the first being in an adult female with Crohn's disease. The patient presented with painless hematochezia with unknown etiology and underwent an explo

Intussusception in an adult with a history of rectal cancer: a case report.

This case report details the clinical journey of a 37-year-old male patient who had undergone rectal cancer surgery five years prior to symptom onset. The patient presented with an abdominal mass and intermittent abdominal pain that had been present for approximately two weeks prior to the current hospitalization for treatment. Through a comprehensive array of diagnostic procedures, notably abdominal CT scans and colonoscopies, the presence and precise location of intussusception were ascertained. The surgical management entailed a radical right hemicolectomy supplemented by preventive measure

[A Case of Anal Canal Squamous Cell Carcinoma with Extensive Rectal Stricture after Chemoradiotherapy].

A 49-year-old female patient was referred to our obstetrics and gynecology clinic in August 2017 with a chief complaint of vaginal defecation. Internal examination revealed a fistula on the posterior wall of the vagina, and rectal examination revealed an irregular mass on the anterior wall of the anal canal. Colonoscopy revealed an irregular mass spanning the lower rectum and anterior wall of the anal canal. Biopsy revealed squamous cell carcinoma. A diagnosis was made of vaginal invasion by an anal canal squamous cell carcinoma, T4N0M0, Stage ⅢB with no evidence of lymph node or distant metas

An unusual case of isolated brain metastasis with impending brain herniation from sigmoid colon cancer: case report and review of literature.

Colorectal carcinoma with a finding of isolated brain metastasis is rare. Furthermore, neurologic symptoms secondary to oligometastasis to the brain are very rare initial presentations for colorectal cancer. Hence, it is uncommon to do brain imaging in the initial staging of colon carcinoma. We present an unusual case of a patient who initially presented with a headache. Laboratory tests revealed mild anemia. Brain imaging revealed a mass in the parietal lobe; hence, neurosurgery was performed with resection of the mass. Histopathology showed it's colonic in origin. Colonoscopy revealed adenoc

A case report on use of TIPS for ectopic variceal bleeding.

BACKGROUND: Ectopic varices may develop anywhere in the abdomen except in the cardioesophageal junction and cause only 5% of all variceal bleeding. Considering the low prevalence of this disease, management is challenging and low-evidence based. By means of a case report, we reviewed literature on the treatment of ectopic varices using TIPS (Transjugular Intrahepatic Portosystemic Shunt). CASE PRESENTATION: A 65-year old patient presented to the emergency department with massive hematochezia and haemorrhagic shock. A CT angiography reported liver cirrhosis, a portal vein thrombosis (PVT) and

Changes in hormone receptor when breast cancer metastasizes to the colon: case report and literature review.

The metastasis of breast cancer to the colon is a rare occurrence, especially in the presence of changes in estrogen and progesterone receptors. To date, literature has only reported two cases of invasive ductal carcinoma and two cases of invasive lobular carcinoma metastasizing to the colon with concurrent changes in hormone receptors. This report describes a 65-year-old woman with a history of left breast cancer, who presented with symptoms of bloody stools and abdominal pain. CT and colonoscopy results revealed a malignant tumor in the ascending colon, and the patient underwent surgery. Pat

A Rare Subepithelial Lesion of the Proximal Colon Evaluated Using Miniprobe Endoscopic Ultrasonography: A Case Report.

Preoperative diagnosis of submucosal lesions in the colon can be challenging. Total colonoscopy comprising miniprobe endoscopic ultrasonography (mEUS) revealed a 5-mm submucosal lesion in the transverse colon as a hypoechoic area primarily within the submucosa in our patient. However, a definitive diagnosis could not be made after evaluating the biopsy results. Based on the mEUS findings, adhesion to the muscularis was localized; therefore, endoscopic submucosal dissection was performed. A postoperative pathology examination revealed a parasite-like structure at the center of the necrotic tiss

Macro-vacuolar steatosis in a cirrhotic liver mimicking metastatic disease.

Multinodular steatosis represents a relatively uncommon manifestation of fatty liver disease (FLD). Co-morbidities such as metabolic syndrome or cirrhosis are often associated. Despite typical features of imaging (ultrasound, CT, and MRI), core biopsy sometimes remains the gold standard for diagnosis. We describe the case of a 57-year-old male patient with a long history of hepatic cirrhosis and a recent diagnosis of carcinoma of the tongue, successfully treated. Due to the occurrence of nausea, diarrhea and jaundice the patient is admitted to Our Hospital where ultrasound examination and cont

[Successful Resection of a Giant Rectal Gastrointestinal Stromal Tumor after Neoadjuvant Therapy with Imatinib Mesylate-A Case Report].

A 70-year-old woman was admitted to the hospital with incomplete defecation. Colonoscopy and abdominal computed tomography revealed a submucosal tumor of >50 mm in diameter in the lower rectum. The biopsy specimen was positive for CD34 and c-kit. Based on these findings, the tumor was diagnosed as a gastrointestinal stromal tumor(GIST). We decided to administer neoadjuvant therapy with imatinib mesylate(IM)because we considered that the tumor was too large to remove without tumor perforation. After 28 weeks of administering neoadjuvant therapy, the tumor size was reduced by 40%. Robot-assisted

Ulcerated Jejunal Lipoma Causing Gastrointestinal Bleeding: A Case Report.

Lipomas of the small intestine are rare and generally asymptomatic. However, they can present with obstructive symptoms or gastrointestinal bleeding that is difficult to localize. A 63-year-old female patient was admitted to the emergency department with a complaint of melena for four days. Endoscopy and colonoscopy were performed without any abnormalities. The patient showed no further externalization of symptoms and had a suspicion of intussusception on a CT scan. A capsule endoscopy was performed, revealing an elevated and ulcerated lesion in the proximal jejunum near the Treitz angle. An a

[A Case of Laparoscopically Assisted Local Resection of Early-Stage Sigmoid Colon Cancer Arising within a Diverticulum].

The patient was a 79-year-old male in whom a neoplastic lesion was found in the sigmoid colon diverticulum during a follow-up colonoscopy for colorectal adenoma. Endoscopic treatment was difficult, and the patient was referred to our department for surgery. The patient had many underlying diseases, including angina pectoris, interstitial pneumonia, and diabetic nephropathy, and the decision was made to perform local resection as a minimally invasive treatment. The patient was treated with laparoscopic excision of the sigmoid colon, followed by a small laparotomy and intraoperative endoscopy to

Ingestion of 14 Hearing Aid Batteries in an Adult Patient.

Battery ingestion is not a common occurrence in adults. When it occurs in patients of any age, prompt action might be necessary, depending on the type of battery ingested, to prevent damage to the gastric mucosa that is involved in important secreting and absorbing functions required to maintain homeostasis. A 61-year-old Hispanic male presented to the emergency department with the chief concern of shortness of breath and abdominal pain. Immune checkpoint inhibitor associated diarrhoea.

A man in his 80s was undergoing immunotherapy with pembrolizumab, an anti-PD-1 monoclonal antibody, following his diagnosis of adenocarcinoma of primary lung origin. 24 weeks into treatment, the patient reported experiencing loose stools associated with malaise and poor appetite but no further symptoms. This progressed in frequency and a clinical diagnosis of grade 2 immune checkpoint inhibitor colitis was made. Management with oral prednisolone was commenced but symptoms persisted. Common enteric infections had been ruled out, as were coeliac disease and hyperthyroidism. Flexible sigmoidoscop

Massive appendiceal mucinous neoplasm diagnosed after abdominal trauma: Pitfalls in imaging and diagnosis.

Low grade appendiceal mucinous neoplasm (LAMN) is a rare tumor presenting typically with symptoms of acute appendicitis. A 33-year-old gentleman presented with abdominal fullness and discomfort secondary to abdominal trauma obtained during boxing training. Investigations with magnetic resonance imaging (MRI), ultrasound, and colonoscopy revealed a large cystic lesion in the ascending colon mesentery with unclear continuation with the appendix. There were no obvious features of malignancy. Laparoscopy revealed a large, firm mass connected to the appendix with enlarged lymph nodes, and the patie

Pleomorphic leiomyosarcoma of the maxilla with metastasis to the colon: A case report.

BACKGROUND: Pleomorphic leiomyosarcomas make up around 8.6% of all leiomyosarcomas. They behave aggressively and often have poor prognoses. They can affect the gastrointestinal tract and retroperitoneum. To date, pleomorphic leiomyosarcoma involving the mesocolon have been reported in nine patients. CASE SUMMARY: The patient was a 44-year-old man with a history of pleomorphic leiomyosarcoma of the left maxilla with metastasis to the lung and liver. His most recent positron emission tomography-computed tomography (PET-CT) scan showed uptake in the ascending and transverse colons. A colonoscopy

[A Case of Locally Advanced Ascending Colon Cancer in Which a Residual Tumor Was Detected Only in the Subserosal Layer after Neoadjuvant Chemotherapy].

A 68-year-old man diagnosed with ascending colon cancer(cT3[SS], cN2a, cM0)declined surgical treatment; therefore, systemic chemotherapy was administered. After 18 courses of systemic chemotherapy(mFOLFOX or FOLFIRI), the patient underwent laparoscopic right hemicolectomy. Preoperative colonoscopy and computed tomography did not detect any tumor, however histopathological examination revealed a viable residual tumor in the subserosal layer. The patient did not receive adjuvant chemotherapy and is alive with no recurrence 3.5 years after surgery. There is no consensus on the efficacy of neoadju

Diagnosis and management of rectal syphilis-case report.

The incidence and prevalence of syphilis are rising worldwide. Rectal syphilis is a rare condition with few reported cases in the literature and therefore often missed from differential diagnosis of atypical anorectal ulceration. We report a case of a 64-year-old male who presented with change in the bowel habit and a palpable rectal mass on examination. Colonoscopy revealed a small, ulcerated lesion in the rectum. However, histopathological analysis and radiological assessments were inconclusive. A cutaneous ulceration prompted a repeat biopsy and staining for spirochaetes, which was diagnost

A rare cause of rectal bleeding in a 48-year-old lupus patient.

Cavernous hemangiomas represent a rare benign cause of rectal bleeding. It corresponds to a vascular malformation that can be located anywhere in the gastrointestinal tract. Our paper reports the case of a 48-year-old woman treated for cutaneous lupus who presented to our department with intermittent moderate rectal bleeding. The Hemoglobin level was normal. A colonoscopy showed a congestive nodular red-purple vascular formation. At Computerized tomography (CT)-Scan, the lesion appeared as an irregular thickening of the posterior rectal wall, invading the meso-rectum. Magnetic resonance imagin

Colorectal Malignancy Complicated by Arterioenteric Fistula.

Arterioenteric fistulas (AEF) are rare and devastating complications of colorectal/pelvic malignancies. These fistulas can be seen following neoadjuvant or adjuvant therapy but are exceptionally rare de novo. The reported incidence of AEF is less than 1% and iliac artery-enteric fistulas make up less than .1% of all AEF. Here we present a patient in hemorrhagic shock secondary to an advanced colorectal malignancy without adjuvant therapies with local invasion of the right external iliac artery. Following initial resuscitation and hemorrhage control with coil embolization, definitive control wi

Preventive strategies in familial and hereditary colorectal cancer.

Colorectal cancer is a leading cause of cancer-related deaths worldwide. While most cases are sporadic, a significant proportion of cases are associated with familial and hereditary syndromes. Individuals with a family history of colorectal cancer have an increased risk of developing the disease, and those with hereditary syndromes such as Lynch syndrome or familial adenomatous polyposis have a significantly higher risk. In these populations, preventive strategies are critical for reducing the incidence and mortality of colorectal cancer. This review provides an overview of current preventive
