Semantic Networks, Schema Change, and Reincarceration Outcomes of Therapeutic Community Graduates.

BACKGROUND: Therapeutic community (TC) clinical theory assumes that peer interaction forms a framework for social learning that will displace ingrained cognitive schema that underlie substance abuse. There has been no direct test of this hypothesis. METHODS: We analyzed the content of a large corpus of written affirmations (pushups) and corrections (pull-ups) exchanged between 2342 male TC graduates. We encoded the content of the written communications as semantic networks of words, in which words that appear in the same brief document are connected and are referred to as word combinations. L

Economic and living statuses of community-dwelling older adults and the related factors.

AIM: The health and well-being of older adults are closely related to their economic and living statuses. Socioeconomic inequality has a substantial impact on the healthcare received by these older adults. The present study aimed to understand the current economic and living statuses of community older adults, and examine the relationships among their demographics, economic status and living status. METHODS: This study used a cross-sectional correlational design. Stratified random sampling was applied to recruit 384 community-dwelling older adults. A face-to-face structured questionnaire was

Interventions for improving outcomes in patients with multimorbidity in primary care and community settings.

BACKGROUND: Many people with chronic disease have more than one chronic condition, which is referred to as multimorbidity. The term comorbidity is also used but this is now taken to mean that there is a defined index condition with other linked conditions, for example diabetes and cardiovascular disease. It is also used when there are combinations of defined conditions that commonly co-exist, for example diabetes and depression. While this is not a new phenomenon, there is greater recognition of its impact and the importance of improving outcomes for individuals affected. Research in the area

Antibiotics for treating community-acquired pneumonia in people with sickle cell disease.

BACKGROUND: As a consequence of their condition, people with sickle cell disease are at high risk of developing an acute infection of the pulmonary parenchyma called community-acquired pneumonia. Many different bacteria can cause this infection and antibiotic treatment is generally needed to resolve it. There is no standardized approach to antibiotic therapy and treatment is likely to vary from country to country. Thus, there is a need to identify the efficacy and safety of different antibiotic treatment approaches for people with sickle cell disease suffering from community-acquired pneumonia

The complexities of designing therapy for Māori living with stroke-related communication disorders.

Stroke-related communication disorders can have a substantial impact on Māori whānau (extended family). Timely and appropriate speech-language therapy is required, but there are many challenges in providing this. In this article we discuss the need for a kaupapa Māori approach to speech-language therapy that is designed by Māori for Māori, and undertaken in a Māori way. We report the results of a literature review that revealed a small but significant body of literature describing Māori experiences of stroke, aphasia and speech-language therapy, and evidence that a Māori-specific therapy progr

Community-based, nurse-led post-exposure prophylaxis: results and implications.

HIV medications can be used as post-exposure prophylaxis to efficaciously prevent an HIV-negative person who has come into contact with HIV from becoming HIV-positive. Traditionally, these medications have been available in emergency departments, which have constituted a barrier for the members of many minority groups who are greatly affected by HIV transmission (i.e. gay, bisexual and other men who have sex with men, and persons who use injection drugs). From 5 September 2013 through 4 September 2015, we sought to increase the use of HIV post-exposure prophylaxis by having registered nurses p

Motherhood as a Gendered Entitlement: Intentionality, "Othering," and Homosociality in the Online Infertility Community.

Motherhood is one of the most enduring rites of passage to adult femininity for women. However, not all women have access to motherhood. In this paper, I explore the online infertility community wherein women blog to process their exclusion, expressing incredulity about the prospect of never having a biological child of their own. Women understand their infertility as an injustice, leading them to lay claim to motherhood. I argue that how women lay claim to motherhood changes our understanding of motherhood as a gendered norm. Motherhood is more than a pursuit of a gender identity; it is also

Association between gait abnormality and malnutrition in a community-dwelling elderly population.

AIM: Malnutrition is common in older adults, and contributes to the risk of falls and functional impairment. Gait performance also contributes to falls and functional impairment; however, the association between malnutrition and gait performance remains unclear. The purpose of the present study was to investigate the association between malnutrition risk and gait performance. METHODS: The study participants included 204 community-dwelling older adults with a mean age of 73.4 ± 4.3 years. Nutritional status was evaluated using the short version of the Mini-Nutritional Assessment. A score of 11

Factor Analysis of the Community Balance and Mobility Scale in Individuals with Knee Osteoarthritis.

BACKGROUND AND PURPOSE: The clinical assessment of balance is an important first step in characterizing the risk of falls. The Community Balance and Mobility Scale (CB&M) is a test of balance and mobility that was designed to assess performance on advanced tasks necessary for independence in the community. However, other factors that can affect balancing ability may also be present during performance of the real-world tasks on the CB&M. It is important for clinicians to understand fully what other modifiable factors the CB&M may encompass. The purpose of this study was to evaluate the underlyi

Systemic inflammation, body composition, and physical performance in old community-dwellers.

BACKGROUND: Chronic inflammation, changes in body composition, and declining physical function are hallmarks of the ageing process. The aim of the present study was to provide a preliminary characterisation of the relationship among these age-related phenomena via multivariate modelling. METHODS: Thirty-five old adults (OAs) and 17 young adults (YAs) were enrolled. The volume of skeletal muscle, subcutaneous adipose tissue (SAT), and intermuscular adipose tissue (IMAT) of the thigh was quantified by three-dimensional magnetic resonance imaging. Muscle strength was measured by knee extension s

Clean water, sanitation and diarrhoea in Indonesia: Effects of household and community factors.

Diarrhoea is an important health issue in low- and middle-income countries, including Indonesia. We applied a multilevel regression analysis on the Indonesian Demographic and Health Survey to examine the effects of drinking water and sanitation facilities at the household and community level on diarrhoea prevalence among children under five (n = 33,339). The role of the circumstances was explored by studying interactions between the water and sanitation variables and other risk factors. Diarrhoea prevalence was reported by 4820 (14.4%) children, who on average were younger, poorer and were liv

Waste dumps in local communities in developing countries and hidden danger to health.

AIM: The rapid industrialisation and urbanisation fuelled by a fast-growing population has led to the generation of a huge amount of waste in most communities in developing countries. The hidden disorders and health dangers in waste dumps are often ignored. The waste generated in local communities is usually of a mixed type consisting of domestic waste and waste from small-scale industrial activities. Among these wastes are toxic metals, lead (Pb), cadmium (Cd), arsenic (As), mercury (Hg), halogenated organic compounds, plastics, remnants of paints that are themselves mixtures of hazardous sub
