Prolonged intermittent hemodialysis using a standard dialysate flow rate for severe overdose of sustained-release valproic acid: A case report.

BACKGROUND: Valproic acid poisoning can be life threatening and may require urgent extracorporeal elimination. In particular, sustained-release formulations pose a challenge, as conventional short-duration intermittent hemodialysis may fail to remove the drug sufficiently because of delayed and prolonged drug absorption. While prolonged intermittent hemodialysis is a rational alternative, its clinical effectiveness and safety in cases of severe sustained-release sodium valproate overdose have not been reported. CASE: A woman in her 20 s developed coma after ingesting 45 g (approximately 1100

Astrocytes in Sepsis-Associated Encephalopathy: Pivotal Roles in Dual Pathogenesis and Therapeutic Perspectives.

Sepsis-associated encephalopathy (SAE) is a diffuse brain dysfunction secondary to the systemic inflammatory response in sepsis. Clinically characterized by impaired cognition and altered consciousness, SAE contributes to poor patient prognosis. Astrocytes, one of the predominant glial cells in the central nervous system, are essential for maintaining central nervous system homeostasis. Mounting evidence highlights their critical involvement in SAE pathogenesis, driving increasing research interest. This review explores the mechanistic roles of astrocytes in SAE and identifies related therapeu

Reversible Toxic-Metabolic Encephalopathy in Fluroacetamide Intoxication: A Case Report and Review of the Literature.

Fluoroacetamide, a commonly used convulsant rodenticide, can rapidly damage the nervous, digestive, and cardiovascular systems, potentially leading to fatal outcomes if ingested. This study reports the case of a 62-year-old Chinese woman who presented with symptoms of intoxication, including slurred speech, agitation, and seizure-like episodes, accompanied by gastrointestinal symptoms such as vomiting, skin bruising, and mild liver dysfunction. Toxin analysis revealed the presence of fluoroacetate in her blood and urine, and diffusion weighted imaging (DWI) imaging indicated white matter lesio

Xylazine as an emerging new psychoactive substance; focuses on both 5-HT7 and κ-opioid receptors' molecular interactions and isosteric replacement.

Xylazine, traditionally used as a veterinary sedative, has recently emerged as a new psychoactive substance, being typically ingested in combination with fentanyl derivatives and hence raising significant public health concerns. Despite its increasing prevalence, little is known about its molecular interactions with human neuroreceptors, specifically the serotonin 7 (5-HT7R) and kappa-opioid (KOR) receptors, which play critical roles in mood regulation, consciousness and nociception. Hence, the binding affinity and molecular interactions of xylazine with both 5-HT7R and KOR through docking sim

Anti-metabotropic glutamate receptor 5 coexistent anti-N-methyl-D-aspartate receptor encephalitis: a case report and literature review.

Metabotropic glutamate receptor 5 (mGluR5) antibody encephalitis is an infrequent clinical disorder, initially reported in 2011 among two patients presenting with limbic encephalitis and Hodgkin's lymphoma. Mental and behavioral abnormalities are prevalent manifestations, accompanied by cognitive impairment, movement disorders, seizures, and other associated symptoms. In this report, we present the case of a young female patient who presented with abnormal mental behavior, seizures, and disturbances of consciousness. A cell-based assay (CBA) showed positive IgG metabotropic glutamate receptor

Anti-Yo positive paraneoplastic limbic encephalitis associated with nasopharyngeal carcinoma: A rare case.

Paraneoplastic neurological syndromes are rare immune-mediated disorders that may develop independently of tumor stage or metastatic spread. Limbic encephalitis is an uncommon manifestation, and its occurrence in association with head and neck cancers is exceptionally rare. We describe a 42-year-old man who presented with subacute ataxia, tremor, dysarthria, and altered consciousness. Brain magnetic resonance imaging revealed hyperintensities in the left hippocampus, parahippocampal gyrus, and amygdala, while cerebrospinal fluid analysis demonstrated lymphocytic pleocytosis, elevated protein l

A Preclinical Model of Orthotopic Heart Transplantation in Bama Miniature Pigs Using Biatrial Technique.

Although significant progress has been made in heart transplantation, issues such as graft preservation, primary graft dysfunction, cardiac allograft vasculopathy, and long-term immune tolerance remain unresolved. Due to anatomical size and immunological differences, mouse models are unable to effectively replicate clinical heart transplantation surgeries and immune responses. In contrast, pig models, which share more similarities with humans in terms of heart anatomy and immune response, are particularly suitable for orthotopic heart transplantation research. In this study, we established a b

Is Charles Bonnet Syndrome a Harbinger of Neurocognitive Disorder With Lewy Bodies? A Clinical Conundrum.

Charles Bonnet syndrome (CBS) is a clinical condition in which patients with visual impairment experience visual hallucinations (VH) in the presence of clear consciousness. It typically occurs in elderly people and confuses clinicians with multiple differential diagnoses due to VH, which can be present in a variety of clinical conditions ranging from psychosis to neurocognitive disorders (eg, neurocognitive disorder with Lewy bodies). In the latter, the concomitant presence of cognitive decline and parkinsonism aids the diagnosis. Here we report the case of an elderly man with retinitis pigmen

A Case of Acute Epidural Hematoma Successfully Treated with Middle Meningeal Artery Embolization.

We report a case of acute epidural hematoma (AEDH) successfully treated with middle meningeal artery (MMA) embolization. A 59-year-old man presented with headache 2 days after minor head trauma. Head computed tomography (CT) revealed a left-sided AEDH with an associated skull fracture. Although the patient had only mild disturbance of consciousness and no focal neurological deficits, the hematoma volume was significant. Given the absence of mass effect and to prevent further hematoma expansion, emergent MMA embolization was performed via a transradial approach under local anesthesia. Angiograp

Peribacillus simplex and Klebsiella pneumoniae responsible for pyonephrosis with secondary psoas abscess: a case report.

Bacterial urinary tract infections (UTIs) are common, ranging from benign cystitis to complicated pyelonephritis, which can lead to severe complications such as pyonephrosis and sepsis. Pyonephrosis, characterized by the presence of pus in the renal cavities, often requires urgent urological intervention. We report a unique case of pyonephrosis with a psoas abscess caused by Klebsiella pneumoniae and Peribacillus simplex in a 64-year-old diabetic female patient. This is the first case of pyonephrosis caused by P. simplex. The patient presented with acute right lumbar pain, fever and altered co

Spontaneous Abnormal Vertical Eye Movements of Coma.

Spontaneous vertical eye movements in the critical care setting are often a source of confusion and alarm; while their origin remains at least partly theoretical, understanding their classification and associated clinical implications can inform the diagnostic workup and further clinical management. This case describes a patient who demonstrated ocular dipping: slow conjugate downward eye movements with a quick return to primary gaze. Ocular dipping is a rare phenomenon that was initially described in patients with hypoxic brain injury and has since been described in only a handful of cases. D

Under a Spell: Neurologic Evaluation of Presyncope as a Feature of Dysautonomia.

While syncope is characterized by a sudden and temporary loss of consciousness caused by decreased blood flow to the brain and is easily recognized by its clinical features, presyncope involves a sensation of impending fainting, often accompanied by autonomic symptoms. Presyncope is less characterized and studied than syncope, presenting a particular diagnostic challenge in neurology clinics. Neurologists commonly encounter patients with presyncope in outpatient settings or during consultation at the emergency department after cardiopulmonary causes have been excluded. Differential diagnosis o

A Case of Extensive Pneumocephalus Following an Electric Scooter Accident.

The use of electric scooters (e-scooters) has rapidly increased in Japan, accompanied by a rise in associated injuries. Severe head trauma can occur even when the Glasgow Coma Scale score suggests a mild injury, particularly when major clinical signs are present. Therefore, careful observation and appropriate management are essential. A 42-year-old woman injured by an e-scooter accident was transferred to our hospital with facial swelling and right vision loss. Head computed tomography revealed extensive pneumocephalus, skull base fractures, and traumatic subarachnoid hemorrhage. She subsequen
