Burden of catastrophic costs, income inequalities and associated factors in Guatemala: First national tuberculosis costs survey, 2023.

OBJECTIVES: To estimate the prevalence of catastrophic costs incurred by households of people with tuberculosis (TB) in Guatemala, and to assess the potential wealth inequalities and the associated factors with the catastrophic costs, social consequences and coping strategies. METHODS: A national representative survey, employing a clustered sampling based on health facilities, was conducted in 2023. The participants were people with TB and the data was collected by trained professionals using a standardized questionnaire. Catastrophic costs were defined as costs incurred by TB treatment that

Cost analysis of primary single-level lumbar discectomies using the Value Driven Outcomes database in a large academic center.

BACKGROUND CONTEXT: Improving value is an established point of emphasis to reduce the rapidly rising health care costs in the United States. Back pain is a major driver of costs with a substantial fraction caused by lumbar radiculopathy. The most common surgical treatment for lumbar radiculopathy is microdiscectomy. Research is sparse regarding variables driving cost in microdiscectomies and often limited by cost data derived from payer-based Medicare data. PURPOSE: To identify targets for cost reduction by determining variables associated with significant cost variation in microdiscectomies,

Effectiveness and cost-effectiveness of a guided internet-based Acceptance and Commitment Therapy intervention (MobACT) for adults with Chronic Pain in Italy: study protocol for a randomized controlled trial.

BACKGROUND: Chronic Pain (CP) affects approximately one in five adults and is associated with emotional distress, disability, reduced productivity and substantial healthcare and societal costs. Acceptance and Commitment Therapy (ACT) has demonstrated effectiveness in improving emotional, behavioral and functional outcomes in individuals with CP, but access to ACT is frequently limited by geographic, organizational and workforce barriers. Internet-based interventions may help overcome these limitations by increasing accessibility, reducing treatment costs and improving scalability. OBJECTIVE:

Implementation and effectiveness of a nurse-enabled, shared-care follow-up model for early breast cancer survivors (The IBIS-Survivorship Study): protocol for a stepped-wedge cluster randomised trial.

INTRODUCTION: Breast cancer is the most commonly diagnosed cancer among women worldwide. Survivors often experience physical and psychological effects arising from breast cancer and its treatment, which can last months and years, adversely impacting quality of life. As the number of early breast cancer survivors increases, models of specialist-led follow-up care in hospital settings are not sustainable and evidence suggests that they may not meet survivors' needs. Nurse-enabled, shared-care, follow-up models between cancer specialist and primary care teams have potential to address this need.

Utility of continuous vagal neuromonitoring in thyroid and parathyroid gland surgery: a retrospective study of 500 cases.

BACKGROUND: Injury to the recurrent laryngeal nerve (RLN) during thyroid and parathyroid surgery can cause vocal cord paralysis (VCP), with severe consequences, particularly in bilateral cases. Intraoperative neuromonitoring (IONM) is useful for identifying the nerve, but there are discrepancies regarding its effectiveness in preventing neural damage. Continuous monitoring, which provides real-time information, could improve postoperative outcomes, yet its adoption has been modest. Although over 90% of endocrine surgeons use some form of monitoring, this procedure has not been fully standardiz

Impact of Double-Fortified Salt with Iron and Iodine on Hemoglobin, Anemia, and Iron Deficiency Anemia: A Systematic Review and Meta-Analysis.

Double-fortified salt (DFS) containing iron and iodine has been proposed as a feasible and cost-effective alternative for iron fortification in low- and middle-income countries (LMICs). We conducted a systematic review and meta-analysis from randomized and quasi-randomized controlled trials to 1) assess the effect of DFS on biomarkers of iron status and the risk of anemia and iron deficiency anemia (IDA) and 2) evaluate differential effects of DFS by study type (efficacy or effectiveness), population subgroups, iron formulation (ferrous sulfate, ferrous fumarate, and ferric pyrophosphate), iro

How is a specialist depression service effective for persistent moderate to severe depressive disorder?: a qualitative study of service user experience.

BACKGROUND: A specialist depression service (SDS) offering collaborative pharmacological and cognitive behaviour therapy treatment for persistent depressive disorder showed effectiveness against depression symptoms versus usual community based multidisciplinary care in a randomised controlled trial (RCT) in specialist mental health services in England. However, there is uncertainty concerning how specialist depression services effect such change. The current study aimed to evaluate the factors which may explain the greater effectiveness of SDS compared to Treatment as Usual (TAU) by exploring

The long-term costs for treating multiple sclerosis in a 16-year retrospective cohort study in Brazil.

BackgroundMultiple Sclerosis (MS) is a disease that appreciably impacts on the quality of life of patients and is associated with high expenditure. MS is a chronic multifactorial disease, characterized by inflammation, demyelination and axonal loss. The Brazilian public health system provides pharmacological treatment as well as hospital and outpatient care for patients with relapsing-remitting and secondary progressive multiple sclerosis. However, we are not aware of any previous publications assessing total direct medical costs in patients with a long follow-up within the Brazilian healthcar

Chronic venous insufficiency: the effects of health-care reforms on the cost of treatment and hospitalisation--an Italian perspective.

OBJECTIVES: This study evaluated the evolution of chronic venous insufficiency (CVI)-related costs in Italy over 8 years by focusing on the impact of the Italian health-care reforms (drug de-reimbursement) of 1993-94. RESEARCH AND DESIGN METHODS: The global cost of CVI to the Italian health system (Sistema Sanitario Nazionale, SSN) was divided into three sub-costs: hospitalisation, GP consultation and prescription costs. Indirect costs, such as loss of working days, were not included. CVI-related hospitalisation costs included inpatient treatment for varicose veins, venous leg ulcers and othe

Relation between non-dietary cardiovascular health and costs associated with stroke in the US.

BACKGROUND: Stroke poses a significant challenge in terms of mortality and healthcare expenditure in the United States, yet the role of Cardiovascular Health (CVH) metrics in alleviating these costs post-stroke remains insufficiently studied. This research aims to evaluate the effectiveness of CVH as a potential tool for secondary prevention by reducing overall healthcare expenditures associated with stroke. METHODS: A comprehensive cross-sectional analysis was conducted utilizing data from the Medical Expenditure Panel Survey (MEPS) collected between 2011 and 2022. The analytic sample consis

Predicted effects of the introduction of long-acting injectable cabotegravir pre-exposure prophylaxis in sub-Saharan Africa: a modelling study.

BACKGROUND: Long-acting injectable cabotegravir pre-exposure prophylaxis (PrEP) is recommended by WHO as an additional option for HIV prevention in sub-Saharan Africa, but there is concern that its introduction could lead to an increase in integrase-inhibitor resistance undermining treatment programmes that rely on dolutegravir. We aimed to project the health benefits and risks of cabotegravir-PrEP introduction in settings in sub-Saharan Africa. METHODS: With HIV Synthesis, an individual-based HIV model, we simulated 1000 setting-scenarios reflecting both variability and uncertainty about HIV

Pathway Of Low Anterior Resection syndrome (LARS) relief after Surgery (POLARiS): protocol for an international, open-label, multi-arm, phase 3 randomised superiority trial within a cohort, with economic evaluation, process evaluation and qualitative sub-study, to explore the natural history of LARS and compare transanal irrigation and sacral neuromodulation to optimised conservative management for people with major LARS following a high or low anterior resection for colorectal cancer.

INTRODUCTION: As a result of improving survival rates, the adverse consequences of rectal cancer surgery are becoming increasingly recognised. Low anterior resection syndrome (LARS) is one such consequence and describes a constellation of bowel symptoms after rectal cancer surgery which includes urgency, faecal incontinence, stool clustering and incomplete evacuation. LARS has a significant adverse impact on quality of life (QoL) and symptoms are present in up to 75% of patients in the first year after surgery. Despite this, little is known about the natural history and there is poor evidence

PeRsonalIsed MEdicine in Rheumatoid Arthritis (PRIMERA) trial: a multicentre, open-label, randomised controlled trial comparing routine care with a tailor-made approach.

INTRODUCTION: Rheumatoid arthritis (RA) is a heterogeneous disease, which current treatment guidelines insufficiently accommodate, as they predominantly emphasise the suppression of disease activity. However, a step towards personalised medicine is preferred to further optimise treatment and requires homogeneous subgroups with similarities in pathophysiological mechanisms and treatment responses. Prior research has already demonstrated notable differences in the pathophysiology of patients with autoantibody-positive and autoantibody-negative RA, as well as differences in treatment responses, w
