BACKGROUND: Anthroposophical Therapeutic Speech uses poems and exercises which typically induce rhythmical breathing. Speaking influences respiration and thus directly heart rate variability (HRV), in particular via respiratory sinus arrhythmia which, together with the slower rhythms of HRV, may be regarded as an indicator of sympathovagal balance. In chronomedicine numerous frequency ratios between physiological rhythms, especially in trophotropic phases, have been established. Integer ratios occur frequently and seem to be associated with the optimization of physiological processes. In large

BACKGROUND: Proximal humerus fracture (PHF) is a common fragility fracture in older adults and can have a substantial impact on upper limb function. Although most patients with PHF can be treated nonsurgically, it is unknown whether older adults benefit from supervised exercise therapy after PHF. Therefore, the objective of this trial was to investigate whether 10 weeks of physiotherapist-supervised exercises once a week were superior to 10 weeks of unsupervised home-based exercises in older adults with a nonsurgically treated displaced 2-part PHF.
METHODS: This was designed as an assessor-bl

Manipulation of the spinal joints for the relief of pain is an ancient remedy that is in widespread use today. Clinical trials investigating the effectiveness of manipulative therapy are briefly reviewed, and some experimental evidence suggesting possible causes of spine pain is presented. The rationale for the use of spinal manipulative therapy within the context of R. A. McKenzie's diagnostic and therapeutic system is detailed and explicitly stated. Spinal mobilization and manipulation are presented as a logical treatment progression from the use of postural instruction and exercise therapy.

Glucosarcopenia, or sarcopenia coexisting with diabetes, is a common condition. Caused by various factors, including poor glucose control, as well as some glucoselowering therapies, sarcopenia negatively impacts quality of life and long-term health. Nandrolone, or 19- nortestosterone, is a selective anabolic receptor modulator (SARM), classified as an anabolic-androgenic steroid (AAS). A review of its pharmacological properties suggests that it may be used as therapy for sarcopenia, along with medical nutrition therapy and resistance exercise training. This review suggests a practical and prag

BACKGROUND: Vestibular rehabilitation therapy (VRT) is an established intervention for patients with acute unilateral peripheral vestibular dysfunction (aUPVD). However, the relative effectiveness of different VRT strategies remains uncertain. This multicenter randomized pilot trial aimed to compare the effects of customized vestibular rehabilitation therapy (CVRT), generic Cawthorne-Cooksey exercises (GVRT), and vestibulo-ocular reflex (VOR) adaptation exercises on vestibular symptom recovery and functional improvement.
METHODS: A total of 34 patients with aUPVD were enrolled across three ce

BACKGROUND: Manual therapy and exercise are recommended for patients with neck pain. In a recent randomized controlled trial, home stretching exercises with or without manual therapy were offered to subjects with persistent or recurrent neck pain. No difference in pain or disability between the treatment groups were found after the two-week intervention period. We aimed to investigate whether these patients had a better outcome after individual tailoring of the treatment content two months after the initial structured intervention period.
METHODS: This manuscript is a secondary analysis of a

BACKGROUND: Equestrian riding is a sport with a high risk of concussion. Currently, the literature guiding rehabilitation for concussions in equestrian athletes is limited, especially for directing return to sport.
CASE DESCRIPTION: In this case report, a 14-year-old female equestrian athlete presented to physical therapy following her third concussion in 3 years. Her primary complaints were headaches, dizziness, difficulty concentrating, light sensitivity, and neck pain. On examination, the patient demonstrated reproduction of symptoms during testing of the vestibular-ocular reflex, showed a

OBJECTIVES: In severe anorexia nervosa, there are a litany of medical complications that affect virtually every body system, and severe weakness is a typical characteristic. To our knowledge, aspiration risk, dysphagia recognition, and dysphagia management and intervention have not been well described in the literature in regard to severe anorexia nervosa. The purpose of this case series is to increase awareness among clinicians of possible oropharyngeal dysphagia symptoms that may present in patients with severe anorexia nervosa.
METHODS: We describe the cases of 3 patients with severe anore

BACKGROUND: People with cystic fibrosis (CF) experience chronic airway infections as a result of mucus buildup within the lungs. Repeated infections often cause lung damage and disease. Airway clearance therapies aim to improve mucus clearance, increase sputum production, and improve airway function. The active cycle of breathing technique (ACBT) is an airway clearance method that uses a cycle of techniques to loosen airway secretions including breathing control, thoracic expansion exercises, and the forced expiration technique. This is an update of a previously published review.

INTRODUCTION: Chronic low back pain (CLBP) is a prevalent global condition that significantly impairs quality of life, reduces work productivity and imposes substantial healthcare burdens. Approximately 70% of adults worldwide experience at least one episode of low back pain during their lifetime. Central sensitisation, a common complexity of CLBP, involves heightened responsiveness of the central nervous system, resulting in amplified neural signalling and increased pain sensitivity. This phenomenon suggests that pain in CLBP extends beyond purely biomedical origins, involving multiple factor

Dementia affects over 55 million people worldwide, with Alzheimer's disease (AD) comprising most cases. Because curative therapies remain unavailable, there is an urgent need for prevention strategies that target modifiable risk factors. Behavioral interventions-particularly sleep improvement and physical activity-can independently benefit cognition, brain function, and cardiometabolic health, yet their combined (potentially synergistic) effects and scalable delivery models remain underexplored. This exploratory, hypothesis-generating, three-arm randomized controlled trial will evaluate: (1) w
