[Clinical study of gene locus heterogeneity in hereditary olivopontocerebellar atrophy (OPCA)--report of 2 pedigrees affected with non SCA1 type OPCA].

From the linkage study of D6S89, we previously reported that hereditary OPCA in Japan is genetically heterogenous. Two pedigrees, P2 and P35, reported in this report, were not linked to D6S89. In order to examine possible correlation between OPCA genotypes and disease phenotypes, we studied clinically eight cases in these two pedigrees. One autopsied case in pedigree P2 was proven to have marked neuronal degeneration in the inferior olivary nuclei, pontine nuclei, cerebellar cortex, and substantia nigra. Dentate nucleus and oculomotor nuclei were free from neuronal degeneration. Clinical featu

A study of perceptual and verbal skills of disabled readers in grades 4, 5 and 6.

This investigation addresses the role of the optometrist in diagnosing and treating children in grades 4, 5, and 6 who have been identified as reading disabled. Fifty-one subjects with average intelligence, but whose reading comprehension skills were below the 31st percentile (mean, 20th percentile), were evaluated using verbal and perceptual tests. When the performance of this experimental group was compared with the mean scores from standardized test norms for each of the various tasks, the disabled readers scored significantly lower in seven of the eight perceptual and five of the six verba

The anti-motion sickness mechanism of ginger. A comparative study with placebo and dimenhydrinate.

A controlled, double-blind study was carried out to determine whether nystagmus response to optokinetic or vestibular stimuli might be altered by some agent contained in powdered ginger root (Zingiber officinale). For comparative purposes, the test subjects were examined after medication with ginger root, placebo and with dimenhydrinate. Eye movements were recorded using standard ENG equipment and evaluation was performed by automatic nystagmus analysis. It could be demonstrated that the effect of ginger root did not differ from that found at baseline, or with placebo, i.e. it had no influence

[Thresholds of long-latency evoked potentials and of the sensations of motion occurring during human exposure to linear accelerations].

Forty-seven healthy test subjects were exposed several times each to linear acceleration of varying value. Depending on the acceleration value, they reported subjectively three types of sensations at the threshold level: nondiscriminated in terms of direction (P1, a = 8.0 +/- 2.5 cm/sec2), inverted (P2, a = 12.4 +/- 2.5 cm/sec2) and discriminated (P3, a = 16.1 +/- 3.5 cm/sec2). The acceleration value at which P2 was recorded caused most typical responses in different test subjects whereas that at which P3 was recorded induced greatest individual variations. Evoked potentials in response to lin

Unilateral left paramedian infarction of thalamus and midbrain: a clinico-pathological study.

In a patient with a unilateral embolic infarct in the left posterior thalamo-subthalamic paramedian artery territory, neuropathological studies showed involvement of the intralaminar, dorsomedial, and internal part of the ventral posterior nuclei of the thalamus, of the rostral part of the mesencephalic reticular formation, and of the posterior commissure. The patient showed upgaze palsy for voluntary saccades, smooth pursuit and vestibulo-ocular movements, sustained downgaze, right-sided motor hemineglect and facio-brachial hypaesthesia, motor transcortical aphasia and anterograde amnesia. Measuring motor intent for BCI control-A comparative analysis of signal quality of simultaneously recorded vECoG and scalp EEG.

Objective.Stent-electrode arrays enable endovascular brain-computer interfaces (BCI) by recording cortical neural activity from within the superior sagittal sinus and have recently been evaluated in an early feasibility clinical trial in the United States (ClinicalTrials.gov: NCT05035823). For a BCI to be viable, the signals need to be high quality to enable accurate decoding of user intent. Compared to electrodes placed on the scalp for electroencephalography (EEG), stent-electrode arrays lie closer to the cortical surface and would presumably offer higher signal quality yet a direct comparis

Aquatic Exercise Enhances Functional Movement and Sensorimotor Performance in Athletes With Chronic Ankle Instability: A Randomized Controlled Trial.

CONTEXT: Chronic ankle instability (CAI) is prevalent among athletes and is associated with impaired sensorimotor function, reduced dynamic stability, and limited functional movement, increasing the risk of reinjury. Aquatic exercise offers a low-impact alternative to land-based rehabilitation by reducing joint loading and promoting neuromuscular adaptations. This study evaluated the effectiveness of an 8-week aquatic exercise program on functional outcomes in athletes with CAI. DESIGN: Randomized controlled trial with preintervention and postintervention assessments. METHODS: Forty male ath

Tourette disorder and sleep.

Healthy sleep is of utmost importance for growth, development, and overall health. Strong evidence shows that sleep is affected negatively in patients and particularly children with Tourette Disorder (TD). There is also a frequent association of TD with Attention Deficit Hyperactivity Disorder (ADHD) which alone has negative effects on sleep and cumulatively worsens the associated sleep findings. The most consistent polysomnographic findings in patients with TD is decreased total sleep time, lower sleep efficiency and an elevated arousal index. Polysomnography studies have confirmed the presen

Clinical features and genotypes of six patients from four families with horizontal gaze palsy with progressive scoliosis.

BACKGROUND: Horizontal gaze palsy with progressive scoliosis (HGPPS) is a rare disorder mainly involved in ocular movement and spinal development. It is caused by a roundabout guidance receptor 3 (ROBO3) gene mutation. This study aimed to describe the clinical features of six patients with HGPPS and investigate the corresponding ROBO3 gene mutations. METHODS: Patients underwent detailed clinical and imaging examinations. Whole-exome sequencing was performed to detect nucleotide variations in the disease-causing genes of HGPPS. RESULTS: Six pathogenic variants were detected in the ROBO3 gene

Disruption of Brainstem Structural Connectivity in REM Sleep Behavior Disorder Using 7 Tesla Magnetic Resonance Imaging.

BACKGROUND: Isolated rapid eye movement (REM) sleep behavior disorder (iRBD) is one of the earliest manifestations of α synucleinopathies. Brainstem pathophysiology underlying REM sleep behavior disorder has been described in animal models, yet it is understudied in living humans because of the lack of an in vivo brainstem nuclei atlas and to the limited magnetic resonance imaging (MRI) sensitivity. OBJECTIVE: To investigate brainstem structural connectivity changes in iRBD patients by using an in vivo probabilistic brainstem nuclei atlas and 7 Tesla MRI. METHODS: Structural connectivity of

Medical Management, Orofacial Findings, and Dental Care for the Patient with Parkinson's Disease.

Parkinson's disease (PD) is the second most prevalent neurodegenerative disease in North America, next to Alzheimer's disease. Patients who suffer from PD typically present with neuromuscular, cognitive, postural and psychiatric deficits, which make oral hygiene challenging, but extremely important. Although the cardinal signs of PD are movement-related, manifestations in the orofacial complex are ubiquitous. Weakened facial musculature, gaunt appearance, tremors of the tongue, lips and eyes, erratic mandibular movements, bruxism, xerostomia, sialorrhea, dysphagia, dysgeusia and glossitis are

Overview of sleep disturbances and their management in Parkinson plus disorders.

Sleep disturbance is one of the commonly reported non-motor symptoms in patients with Parkinson's disease (PD) as well as in Parkinson plus disorders such as multiple system atrophy (MSA), dementia with Lewy bodies (DLB), progressive supranuclear palsy (PSP), and corticobasal syndrome (CBS). Although there is a wealth of literature on sleep disturbances in PD, the same is not robust on the Parkinson plus disorders. This article aims to comprehensively review the sleep disturbances in Parkinson plus disorders. The literature review was as per the guidelines of the preferred reporting items for

A model of face selection in viewing video stories.

When typical adults watch TV programs, they show surprisingly stereo-typed gaze behaviours, as indicated by the almost simultaneous shifts of their gazes from one face to another. However, a standard saliency model based on low-level physical features alone failed to explain such typical gaze behaviours. To find rules that explain the typical gaze behaviours, we examined temporo-spatial gaze patterns in adults while they viewed video clips with human characters that were played with or without sound, and in the forward or reverse direction. We here show the following: 1) the "peak" face scanpa

Sleep and aging: 2. Management of sleep disorders in older people.

The treatment of sleep-related illness in older patients must be undertaken with an appreciation of the physiologic changes associated with aging. Insomnia is common among older people. When it occurs secondary to another medical condition, treatment of the underlying disorder is imperative. Benzodiazepines, although potentially effective, must be used with care and in conservative doses. Daytime sedation, a common side effect, may limit use of benzodiazepines. Newer non-benzodiazepine drugs appear to be promising. Rapid eye movement (REM) sleep behaviour disorder can be treated with clonazepa

Visuo-vestibular biofeedback in patients with peripheral vestibular disorders.

It is well known that diseases of the vestibular system can be compensated by increased spontaneous activity of other systems engaged in maintaining equilibrium, i.e. proprioceptive and visual systems. A complex approach using multisensory stimulation is the optimal way to achieve vestibular compensation. The aim of our study was to determine the effect of vestibular rehabilitation therapy as measured by posturography in a group of 72 patients suffering from vestibular disorders: Ménière's disease (n = 31), neuritis vestibularis (n = 21) and vertebrobasilar insufficiency (n = 20). Patients und

The lateralizing value of ictal clinical symptoms in uniregional temporal lobe epilepsy.

In order to assess the lateralizing value of several ictal and postictal clinical symptoms in temporal lobe epilepsy (TLE), we analyzed 89 seizures of 20 left dominant patients with intractable left (n = 9) versus right (n = 11) TLE who had undergone successful anterior temporal lobectomy. In left TLE, movement arrest at seizure onset, postictal dysphasia > 120 s and postictal dyslexia > 180 s were the most typical findings and associated with a sensitivity of 94, 94, and 100%, respectively. The highest specificity of 100% each was evident for contralateral versions of eyes and head and dyston

Seizure semiology of occipital lobe epilepsy in children.

PURPOSE: Occipital lobe epilepsy in children occurs as an idiopathic form, i.e., Benign Epilepsy with Occipital Paroxysms (BEOP), and a symptomatic form. This study attempted to determine whether seizure semiology could distinguish between the two forms. METHODS: Fifty children (34 boys, 16 girls) with clinical seizures and interictal EEGs presenting occipital spikes were included consecutively in the study. Seizure onset was between the ages of 2 months and 15 years. Epilepsy was considered symptomatic when psychomotor retardation and/or abnormalities at neurological and/or neuroradiological

Analysis of precordial movements.

Thorough analysis of precordial movements in patients with known or suspected heart disease provides valuable diagnostic information in most patients and is an important, although often overlooked, part of the initial cardiac examination. As with all physical diagnostic methods, information obtained by analysis of precordial movements must be carefully integrated with the clinical history and other physical findings. Further diagnostic studies may be necessary for certain patients, but failure to obtain readily available information during the initial examination can lead to important mistakes
