Whiplash injuries of the temporomandibular joint in motor vehicle accidents: speculations and facts.

Referring to the temporomandibular joint (TMJ) of the human mandibular locomotor system, it has been asserted that displacement of the TMJ disc and inflammation of TMJ tissues are the results of acute and indirect trauma to the TMJ; on occasion this is allegedly experienced in motor vehicle accidents and commonly known as a TMJ whiplash injury. It is postulated that the TMJ whiplash injury is released in the occupant or occupants of a target vehicle when its rear end is impacted by the front end of a bullet vehicle. On the basis of detailed analyses of TMJ trauma/pain histories and TMJ magneti

Pattern of external birth trauma in southwestern Saudi Arabia.

The pattern and associations of 62 instances of external birth trauma (EBT) in 57 of 2222 consecutive hospital-born neonates were studied. Macrosomia and vaginal instrumental or breech delivery were identified as significant risk factors for EBT. In contrast to previous reports, subgaleal hemorrhage (SGH) was the predominant form of trauma (n = 33) and it was associated with vacuum application in 28 of 33 (85%) neonates. Brachial plexus palsy (BPP) (n = 10) occurred with the highest frequency (6.4%) after breech delivery. Fractures were uncommon. Multiple injuries (10 items in 5 babies) were m

Rigid facial skeletal fixation: advances in treatment.

Rigid facial skeletal fixation with miniplates, combined with craniofacial reconstruction techniques, has dramatically changed the treatment of facial fractures. Our use of these newer techniques in 142 facial fractures treated during the past 14 months has resulted in improved aesthetics and function, with a low complication rate. Advances over other techniques include (1) craniofacial incisions with wide subperiosteal exposure, (2) rigid skeletal fixation that virtually eliminates the need for continued intermaxillary fixation, suspension wires, or tracheostomy, (3) primary bone grafting wit

[Obstetrical injuries of the newborn infant. Retrospective study apropos of 20,409 births].

We have been able to find 291 cases of obstetric trauma when we analysed the 20,409 deliveries which took place in the Pitié maternity between 1971 and 1980. It has been shown that bony lesions did well in the long and short term but nerve lesions had a far less good prognosis. This pathology should become far less when it is possible to recognize better when the fetus is large and when such traumatising manoeuvres as high forceps and assisted deliveries of large breech babies are stopped.

Facial scars resulting from vehicular accidents.

The surgeon will be better able to cope with the problem of scars as he understands the nature of wound healing and as advances are made in the knowledge of the physiology and biochemistry of tissues. Still there do remain the all-important careful handling of tissues, adequate wound repair, and adequate postoperative care of the wound and the patient.

Sixteen years of cochlear implant surgery in cochleovestibular malformation and cochlear nerve deficiency: Insights from northern Malaysia.

INTRODUCTION: Congenital sensorineural hearing loss associated with cochleovestibular malformations (CVM) and/or cochlear nerve deficiency (CND) presents distinct surgical and audiological challenges. Patients with CVM face elevated risks of intraoperative cerebrospinal fluid (CSF) gushers and aberrant facial nerve courses, while those with CND have historically been viewed as poor candidates for cochlear implantation (CI) due to concerns regarding neural sufficiency. This study aims to bridge this gap by evaluating the surgical safety and audiological outcomes of CI in a focused cohort of pat

Feasibility and Safety of Cervical Kinematic Magnetic Resonance Imaging in Patients with Cervical Spinal Cord Injury without Fracture and Dislocation.

OBJECTIVE: To evaluate the feasibility and safety of cervical kinematic MRI (KMRI) in patients with cervical spinal cord injury without fracture and dislocation (CSCIWFD). METHODS: This was a single-institution case-only study. Patients with CSCIWFD were enrolled in our institution from February 2015 to July 2019. Cervical radiography and CT were performed first to exclude cervical tumors, and major fracture or dislocation. Then neutral static and kinematic (flexion and extension) MRI was performed for patients who met the inclusion criteria under the supervision of a spinal surgeon. Any adve

Craniofacial Injuries Associated With Micromobility Vehicles: A Retrospective Analysis Using the NEISS Database (2014-2023).

This retrospective analysis examines craniofacial injuries resulting from micromobility vehicle use based on National Electronic Injury Surveillance System data from 2014 to 2023. The study analyzed injury patterns from electric scooters, hoverboards, and e-bikes across ~100 US hospital emergency departments. Among 308,068 recorded craniofacial injuries, a significant gender disparity emerged with males comprising 69.4% of cases versus 30.6% for females. Injury rates peaked in males aged 20 to 29 and females aged 0 to 9. Males experienced injuries at 2.4 times the female rate. Powered and unsp

A two-year retrospective cohort study investigating the occurrence and short-term outcomes of dog-bite injures in regional Western Australia.

BACKGROUND: Dog bite injuries are a preventable yet common cause of animal related hospitalisation. Dog bites in metropolitan areas have been well characterised however there is limited information regarding dog bites in regional areas. This study sought to describe the demographics, clinical presentation and short-term outcomes of patients presenting with dog bite related injuries to Broome Regional Hospital (BRH). METHODS: A retrospective cohort study examined all dog bite related injuries presenting to BRH Emergency Department (ED) between July 1st 2021 - June 30th 2023, with the terms "do

Factors associated with first-pass success of emergency endotracheal intubation.

STUDY OBJECTIVE: Endotracheal intubation is frequently performed in emergency departments (EDs). First-pass success is important because repeated attempts are associated with poor outcomes. We sought to identify factors associated with first-pass success in emergency endotracheal intubation. METHODS: We analyzed emergency orotracheal intubations on adult patients in an ED located in South Korea from Jan. 2013 to Dec. 2016. Various operator-, procedure- and patient-related factors were screened with univariable logistic regression. Using variables with P-values less than 0.2, a multiple logist

Pneumomediastinum, ST elevation and urgent cardiac catheterisation: a crucial triad?

Pneumomediastinum (PNMD) entails the presence of air or other gas in the mediastinum and is also known as mediastinal emphysema. PNMD may cause a wide variety of signs and symptoms, as well as ECG abnormality, including ST segment changes. We present a 56-year-old man admitted to our hospital after a facial trauma. After undergoing tracheostomy, he complained of chest discomfort. A chest X-ray in the posteroanterior view showed PNMD, and an ECG was suggestive of inferior-lateral wall myocardial infarction. An urgent cardiac catheterisation identified a critical obstruction at the origin of the

Stereotactic Radiosurgery Treatment of Trigeminal Neuralgia: Clinical Outcomes and Prognostic Factors.

BACKGROUND: Stereotactic radiosurgery (SRS) is a minimally invasive surgical option for the treatment of trigeminal neuralgia (TN). Here we review our institutional experience to identify prognostic factors associated with pain relief after SRS. METHODS: 263 patients with TN treated at the University of California, San Diego/San Diego Gamma Knife (2001-2013) were followed for more than 6 months. Univariate and multivariate Cox proportional hazard models analysis of factors associated with outcome was performed. RESULTS: Of the 263 patients, 229 (87%) presented with classical idiopathic TN, 3

[Orofacial pain - Trigeminal neuralgia and posttraumatic trigeminal neuropathy: Common features and differences].

Neuropathic pain is the result of a lesion or disease of the somatosensory system in the peripheral or central nervous system. Classical trigeminal neuralgia and posttraumatic trigeminal neuropathy are pain disorders which oral and maxillofacial surgeons and dentists are confronted with in the differential diagnostics in routine daily practice. The etiopathogenesis of classical trigeminal neuralgia is attributable to pathological blood vessel-nerve contact in the trigeminal nerve root entry zone to the brain stem. The typical pain symptoms are characterized by sudden stabbing pain attacks. The
