The relationship between biological and psychosocial risk factors and resting-state functional connectivity in 2-month-old Bangladeshi infants: A feasibility and pilot study.

Childhood poverty has been associated with structural and functional alterations in the developing brain. However, poverty does not alter brain development directly, but acts through associated biological or psychosocial risk factors (e.g. malnutrition, family conflict). Yet few studies have investigated risk factors in the context of infant neurodevelopment, and none have done so in low-resource settings such as Bangladesh, where children are exposed to multiple, severe biological and psychosocial hazards. In this feasibility and pilot study, usable resting-state fMRI data were acquired in in

Brief approaches to developmental-behavioral promotion in primary care: updates on methods and technology.

Well-child visits are a critical opportunity to promote learning and development, encourage positive parenting practices, help children acquire behavioral self-control, enhance the development and well-being of children and their families, identify problems not amenable to brief in-office counseling, and refer for services when needed. This article outlines the communication skills, instructional methods, and resource options that enable clinicians to best assist families. Also covered is how to monitor progress and outcomes. A total of 239 articles and 52 Web sites on parent/patient education

Privileging physical activity over healthy eating: 'Time' to Choose?

Physical activity and healthy eating have long been promoted as key strategies in tackling the 'wicked problem' of obesity. Both practices are assumed to go hand-in-hand, but whether one dominates the other has largely remained unexamined. Moreover, time, a dimension beyond the socio-ecological model, is a critical factor of families' busy lives, but related challenges are rarely articulated. We conducted 47 family interviews as part of a mixed methods study examining environmental influences on youth obesity in Nova Scotia, Eastern Canada. Participants were recruited from six schools at the j

[Toward a more rational field-genetic epidemiology].

Genetic dissection of diseases is one of the epoch-making achievements in modern medicine. Positional cloning is a key method to isolate disease-related genes. For positional cloning, there are two conventional methods: family-based studies and case-control studies. In this review, I would like to describe several family-based studies on single gene diseases which I had conducted including those of Akita diabetic mice, systemic carnitine deficiency and Hartnup disease. The study of systemic carnitine deficiency underscored a potential power of the "Carrier state." Furthermore, cultural and pub

CHEK2*1100delC genotyping for clinical assessment of breast cancer risk: meta-analyses of 26,000 patient cases and 27,000 controls.

PURPOSE: CHEK2*1100delC heterozygosity may be associated with an increased risk of breast cancer; however, it is unclear whether the evidence is sufficient to recommend genotyping in clinical practice. PATIENTS AND METHODS: We identified studies on CHEK2*1100delC heterozygosity and the risk of unselected, early-onset, and familial breast cancer through comprehensive, computer-based searches of PubMed, EMBASE, and Web of Science. Aggregated risk estimates were compared with previous estimates for BRCA1 and BRCA2 mutation heterozygotes. RESULTS: By using fixed-effect models for CHEK2*1100delC

A modified indirect method for computing inbreeding coefficients.

The performance of existing methods for computing inbreeding coefficients-including tabular-based methods, Cholesky decomposition-based methods, and indirect methods-has been limited by either memory usage, pedigree depth, or average half-sib family size. In this note, we present a modified indirect method that efficiently identifies, computes, and stores the necessary elements for computing inbreeding coefficients. The algorithm was tested on pedigrees of 8.6 million Japanese Holsteins born from 1901 to 2023, with an average longest ancestral path number of 17 and 42,120 sires with progenies.

Severe familial hypercholesterolemia and pregnancy: handle with care.

BACKGROUND AND AIM: Pregnancy is a period of both physical and hormonal changes that have the function of ensuring the correct development of the child and providing for its needs, as well as preparing the mother for childbirth and breastfeeding. However, the physiological changes that occur during pregnancy may favor a predisposition to metabolic disorders. METHODS AND RESULTS: In the case of a woman affected by familial hypercholesterolemia, a pregnancy must obviously be planned with particular attention. Pregnancy involves important maternal physiological changes that lead to an exacerbati

Interactions With Healthcare Providers Related to Feeding Children With Down Syndrome: Mothers' Perspectives.

BACKGROUND: Feeding is an important health consideration for children with Down syndrome due to their increased risks for feeding challenges and overweight/obesity. Healthcare providers play an important role in providing feeding guidance to families; however, there is limited research on feeding children with Down syndrome and the role of healthcare providers. The purpose of this study is to explore how interactions with healthcare providers contribute to mothers' experiences feeding their children with Down syndrome. METHODS: In this qualitative study, 29 mothers of children with Down syndr

A report of resources used by clinicians in the UK to support motor neuron disease genomic testing.

Genetic testing is a key decision-making point for people with motor neuron disease (MND); to establish eligibility for clinical trials, better understand the cause of their condition, and confirm the potential risk to relatives, who may be able to access predictive testing. Given the wide-reaching implications of MND genetic and predictive testing, it is essential that families are given adequate information, and that staff are provided with appropriate training. In this report we overview the information resources available to people with MND and family members around genetic testing, and th

Considerations for the Future: Family Planning and Infertility During Psychiatry Training.

During medical school, residency, or fellowship, many trainees struggle to balance their careers with starting a family. Some may feel the optimal time for parenthood is after completion of training, but the effect of increasing age on fertility is a real consideration for female physicians.1 Several studies have explored the impact of pregnancy and parental leave during surgical residency, yet little has been published on these topics during psychiatry training. This is surprising as psychiatry residents often address the challenges of integrating work and parenthood with their patients, yet

When a Family Seeks to Exclude Residents From Their Child's Care.

A primary goal of our medical education system is to produce physicians qualified to promote health, prevent and treat disease, and relieve suffering. Although some aspects of the practice of medicine can be learned in classrooms, from textbooks, or with simulators, other aspects can only be learned through the direct provision of patient care. Residency programs therefore offer essential educational experiences that support residents' acquisition of knowledge, skills, and professional judgment through the assumption of progressive responsibility under an appropriate level of supervision. Yet,

Family and community in the lives of UK Bangladeshi parents with intellectual disabilities.

BACKGROUND: Little is known about the lives of parents with intellectual disabilities from minority ethnic communities. Previous research suggests that what it means to live with intellectual disabilities varies across cultural contexts. The current research aimed to explore how cultural values and practices impact upon the experiences of parents with intellectual disabilities within the Bangladeshi community in London, England. METHOD: Six members of the Bangladeshi community, four Bangladeshi parents with intellectual disabilities and four of their family members were interviewed. Thematic

A boy and his AAC team: building instructional competence across team members.

Families and education professionals face unique challenges when working together to support augmentative and alternative communication (AAC) for a child who does not use speech and has only prelinguistic forms of communication. For this study, a family-school team was provided with a specially designed support package comprising training and one-on-one coaching in evidence-based instructional strategies. As part of a larger mixed methods investigation, a single-case experimental design was conducted to explore the effectiveness of the support package on facilitating team members' competence i

The role of genetic testing in unexplained sudden death.

Most sudden deaths are because of a cardiac etiology and are termed sudden cardiac death (SCD). In younger individuals coronary artery disease is less prevalent and cardiac genetic disorders are more common. If sudden death is unexplained despite an appropriate autopsy and toxicologic assessment the term sudden arrhythmic death syndrome (SADS) may be used. This is an umbrella term and common underlying etiologies are primary arrhythmia syndromes with a familial basis such as Brugada syndrome, long QT syndrome, and subtle forms of cardiomyopathy. The first clinical presentation of these conditi

Detection of mild inherited disorders of blood coagulation: current options and personal recommendations.

Although assessment of prior personal and familial bleeding history is an important aspect of the diagnosis of bleeding disorders, patients with mild inherited bleeding disorders are sometimes clinically asymptomatic until presented with a hemostatic challenge. However, bleeding may occur after incursion of trauma or surgery, so detection of these conditions reflects an important facet of clinical and laboratory practice. Mild bleeding disorders may be detected as a result of family studies or following identification of abnormal values in first-line screening tests such as activated partial t

Isolated microscopic haematuria of glomerular origin: clinical significance and diagnosis in the 21st century.

Isolated microscopic, or non-visible, haematuria of glomerular origin was previously regarded a benign finding, but it is now known that, even in the absence of proteinuria, hypertension or renal impairment at presentation, haematuria is associated with increased risk of kidney failure in the long term. The most common causes of isolated microscopic haematuria among children and young adults are IgA nephropathy, Alport syndrome (AS), and thin basement membrane nephropathy (TBMN). AS, which is usually inherited as an X-linked or autosomal recessive trait, and TBMN, which is usually autosomal do
