Diffusion-weighted and conventional magnetic resonance imaging in cerebral cryptococcoma.

A case of cryptococcoma involving the right cerebral hemisphere is reported. The diffusion-weighted image shows hypointensity in the central cavity of the cryptococcoma, while apparent diffusion coefficient maps show hyperintensity. The imaging features of an intracerebral cryptococcoma mimic that of a central necrotic brain tumor, rather than a pyogenic brain abscess.

Mucormycoses in patients with hematologic malignancies: an emerging fungal infection.

BACKGROUND AND OBJECTIVES: Mucormycoses are seen with an increasing incidence in immunocompromised patients. Most common presentations are rhinocerebral and pulmonary. We here report the experience of a single center with mucormycoses in patients with hematologic malignancies. RESULTS: Mucormycoses were diagnosed in six patients, (median age of 52 years; range, 26-74) treated between 2001-2004. Diagnoses included acute myeloid leukemia (AML) (n=3), acute lymphoblastic leukemia (n=1), chronic lymphocytic leukemia (n=1) and multiple myeloma (n=1). Mucormycosis was diagnosed in the neutropenic s

Combining mortality and longitudinal measures in clinical trials.

Clinical trials often assess therapeutic benefit on the basis of an event such as death or the diagnosis of disease. Usually, there are several additional longitudinal measures of clinical status which are collected to be used in the treatment comparison. This paper proposes a simple non-parametric test which combines a time to event measure and a longitudinal measure so that a substantial treatment difference on either of the measures will reject the null hypothesis. The test is applied on AIDS prophylaxis and paediatric trials.

Development of Pyrazole and Pyrazolo[4,5-b]pyridine Analogues with Potent Fungal Biofilm Inhibition Properties.

INTRODUCTION: Fungal infections continue to pose significant health risks because of two factors, which include drug resistance and the restricted availability of treatment options. Molecular hybridization has been an effective method for developing new drugs with improved therapeutic effects. Fused heterocyclic systems provide researchers with useful frameworks for synthesizing compounds that more effectively engage biological targets. The pyrazole and pyrazolo[ 4,5-b]pyridine derivatives have attracted significant scientific interest due to their multiple biological functions and their abili

Comprehensive Profiling of Aseer Medicinal Plants: Connections Between Molecular Identity, Chemical Composition, and Antifungal-Antibiofilm Activity Against Oral Yeasts.

Oral fungal infections resulting from non-albicans Candida species and new opportunistic yeasts are increasingly linked to antifungal resistance, especially in individuals with periodontal disease. Bioactive compounds may serve as potential alternatives; nevertheless, there is a paucity of research that has comprehensively assessed their antifungal and antibiofilm efficacy against clinically defined oral yeast isolates. This study aimed to (i) describe the variety and antifungal resistance profiles of oral yeasts isolated from women with various periodontal diseases; (ii) assess four ethanolic

In Vitro Activity of Rezafungin Against Planktonic and Biofilm Forms of Candida albicans and Nakaseomyces glabratus Clinical Isolates from Vascular Infections in Poland: A Pilot Study.

Background/Objectives: Certain yeast species are recognized as significant opportunistic pathogens, capable of causing severe systemic infections, particularly in immunocompromised individuals or those with disrupted physiological barriers. The rising incidence of invasive candidiasis associated with vascular infections poses a significant clinical challenge due to the high mortality rates and the limited efficacy of conventional antifungal therapies. The formation of resilient biofilms on vascular catheters by species such as Candida albicans and Nakaseomyces glabratus further complicates tre

Characterizing antimicrobial activity of environmental Streptomyces spp. and oral bacterial and fungal isolates from Canis familiaris and Felis catus.

UNLABELLED: Antimicrobials are a pillar of modern medicine, yet our limited arsenal of antibiotics and antifungals is currently threatened by widespread drug resistance. Ongoing efforts are focused on developing strategies to identify compounds that enhance the efficacy of current antimicrobials and develop novel, resistance-evasive therapeutic strategies. In this study, we characterized microbial isolates from two distinct environments to identify those that exhibit antimicrobial activity alone and in combination with current antimicrobials: (i) oral isolates from domesticated animals and (ii

In vitro and in vivo activity of sodium houttuyfonate and sodium new houttuyfonate against Candida auris infection by affecting adhesion, aggregation, and biofilm formation abilities.

UNLABELLED: Candida auris is a rapidly spreading multidrug-resistant fungus that causes fatal infections under certain global conditions. Sodium houttuyfonate (SH) and sodium new houttuyfonate (SNH) are stable derivatives of houttuynin (decyl aldehyde) extracted from Houttuynia cordata, both possessing antifungal and antibacterial pharmacological activities. However, the inhibitory effects of SH and SNH on C. auris remain unclear. Therefore, this study aims to evaluate the potential activity and possible mechanisms of SH and SNH as antifungal agents against C. auris. First, our results showed

Molecular Characterization and Sterol Profiles Identify Nonsynonymous Mutations in ERG2 as a Major Mechanism Conferring Reduced Susceptibility to Amphotericin B in Candida kefyr.

The molecular basis of reduced susceptibility to amphotericin B (rs-AMB) among any yeasts is poorly defined. Genetic alterations in genes involved in ergosterol biosynthesis and total cell sterols were investigated among clinical Candida kefyr isolates. C. kefyr isolates (n = 81) obtained from 74 patients in Kuwait and identified by phenotypic and molecular methods were analyzed. An Etest was initially used to identify isolates with rs-AMB. Specific mutations in ERG2 and ERG6 involved in ergosterol biosynthesis were detected by PCR sequencing. Twelve selected isolates were also tested by the S

Epidemiology and antifungal susceptibility of Candida species in a tertiary care hospital, Kolkata, India.

BACKGROUND AND PURPOSE: The incidence of fungal infection as well as candidemia has increased significantly, contributing to morbidity and mortality in the developed countries. The alarming increase in infections with multidrug resistant bacteria is due to overuse of a broad spectrum antimicrobials, which leads to over growth of Candida spp.; thus, enhancing its opportunity to cause the disease. A shift has been observed in the relative frequency of each Candida spp. Antifungal agents available for the treatment of systemic and invasive candidiasis are restricted to polyenes, allylamines, azol

Pharmacokinetic aspects of treating infections in the intensive care unit: focus on drug interactions.

Pharmacokinetic interactions involving anti-infective drugs may be important in the intensive care unit (ICU). Although some interactions involve absorption or distribution, the most clinically relevant interactions during anti-infective treatment involve the elimination phase. Cytochrome P450 (CYP) 1A2, 2C9, 2C19, 2D6 and 3A4 are the major isoforms responsible for oxidative metabolism of drugs. Macrolides (especially troleandomycin and erythromycin versus CYP3A4), fluoroquinolones (especially enoxacin, ciprofloxacin and norfloxacin versus CYP1A2) and azole antifungals (especially fluconazole

Drug interactions with cisapride: clinical implications.

Cisapride, a prokinetic agent, has been used for the treatment of a number of gastrointestinal disorders, particularly gastro-oesophageal reflux disease in adults and children. Since 1993, 341 cases of ventricular arrhythmias, including 80 deaths, have been reported to the US Food and Drug Administration. Marketing of the drug has now been discontinued in the US; however, it is still available under a limited-access protocol. Knowledge of the risk factors for cisapride-associated arrhythmias will be essential for its continued use in those patients who meet the eligibility criteria. This revie
