Utility of nerve ultrasound in ulnar neuropathy with abnormal non-localizing electrophysiology in diabetes mellitus.

We studied the utility of ultrasound in the diagnostic workup of ulnar neuropathy with abnormal non-localizing electrophysiology (NL-UN) in patients with diabetes. Eighteen ulnar nerves (15 patients) were scanned from wrist to mid-upper arm. Ultrasound showed: (a) focal nerve enlargement at the elbow (8/18 nerves), either alone (6) or superimposed upon diffuse nerve abnormality (2); (b) diffuse nerve enlargement without focal abnormality (8/18); (c) segmental abnormality in upper-arm or forearm without extrinsic nerve compression (2/18). This study shows a pivotal role for ultrasound in the cl

Unilateral carpal tunnel syndrome: an unusual presentation of nodular fasciitis.

Nodular fasciitis is a benign, self-limiting proliferative disorder of fibroblast of uncertain aetiology, occurs frequently in the forearm. Nodular fasciitis in hand inducing carpal tunnel syndrome is exceptional. There are four cases of non-intraneural nodular fasciitis causing peripheral neuropathy that has been reported previously. We present the case of a 38-year-old man with features of unilateral carpal tunnel syndrome. Decompression of the median nerve performed subsequently along with excision of the lesion in a piecemeal fashion. Histopathological and immunohistochemical findings were

Long Segment, Mixed Sensorimotor Nerve Reconstruction with Allograft: A Case Report of High Radial Nerve Injury.

CASE: A 35-year-old man sustained a Grade I open right mid-diaphyseal humerus fracture and high radial nerve transection from a motor vehicle accident. Acute humeral plate osteosynthesis and radial nerve reconstruction using an intercalary 4-cm processed nerve allograft (PNA) was performed. Five years postoperatively, elbow extension, forearm supination, and wrist extension were 5/5 strength and independent digital extension was 5-/5. Radial nerve sensation recovered to 90% of the uninjured side. CONCLUSIONS: Use of PNA resulted in outcomes comparable with or superior to autograft reconstruct

A case of eosinophilic fasciitis and generalized morphea overlap.

A 60-year old man developed skin hardening and edema on his extremities. Although he had been treated with oral prednisolone at another hospital, skin stiffness relapsed during tapering of prednisolone. At the initial visit to our department, physical examination showed skin hardening of the extremities and also symmetric erythematous macules on the back. Histological examination revealed fasciitis on the forearm and morphea on the back. Eosinophilic fasciitis is occasionally associated with morphea. However, cases of concurrent eosinophilic fasciitis and generalized morphea are rare. In the p

Left Subclavian Artery Occlusion: Femoro-Axillary Artery Retrograde Bypass.

The treatment tactics for subclavian artery occlusion include the more commonly used endovascular therapy rather than surgical intervention. We present a case of a 61-year-old woman with dialysis-dependent chronic renal failure who experienced left finger necrosis in the left upper extremity. To salvage the limb, we performed femoro-axillary (fem-ax) artery bypass using an autologous saphenous vein graft. However, 10 months later, she experienced coldness in the left forearm. Angiography revealed chronic total occlusion of the venous bypass. Despite emergent thrombectomy, redo fem-ax artery by

Ultrasonographic diagnosis of lipofibromatous hamartoma of the median nerve.

Lipofibromatous hamartoma (LFH) is an extremely rare benign tumor, which is characterized by an excessive infiltration of the epineurium and perineurium by fibroadipose tissues.A 27-year-old woman was diagnosed with left carpal tunnel syndrome (CTS) due to (LFH) of the median nerve. LFH was diagnosed by MRI and sonography; the characteristic ultrasonographic feature of LFH showed a good correlation with pathognomonic MRI findings. The median nerve was involved along its course in the forearm; however, the patient needed carpal tunnel release because of severe compression of the median nerve un

Terlipressin-induced ischemic skin necrosis: a rare association.

BACKGROUND: Terlipressin is a synthetic vasopressin analogue that is used in the treatment of bleeding esophageal varices and hepatorenal syndrome in patients with cirrhosis. Serious ischemic adverse events, such as skin necrosis involving the extremities, scrotum, trunk, and abdominal skin, are rarely observed. In the literature to date, 20 cases that developed ischemic skin necrosis due to terlipressin usage have been reported. CASE REPORT: We report a patient with extensive skin necrosis on the infusion site of the right forearm and hand, which developed after the use terlipressin used to

Dramatic alteration of the skull in a uremic patient with leontiasis ossea.

The craniofacial skeleton represents a peculiar target of hyperparathyroidism in patients with end-stage renal disease who exhibit a dramatic pattern of uremic leontiasis ossea. Scant information regarding this condition is available in the renal literature, as the extreme and typical manifestations of leontiasis ossea have been described in only a small series of patients. We herein report a case of significant amelioration of massive modification of the facial appearance of a 30-year-old uremic Chinese woman with severe skeletal deformities who underwent total parathyroidectomy with a forear

Infiltrating angiolipoma of the cheek.

Angiolipoma, spindle cell lipoma, myelolipoma, chondrolipoma and myxolipoma are histological variants of lipomas arising from fat tissue. Although angiolipoma is the most common tumour in the trunk and forearm, it occurs infrequently in the head and neck region. In this report we present a case of angiolipoma occurring in the buccal mucosa of a 33-year-old man. The patient had noticed a painless mass in his buccal mucosa for 2 months. The surgically removed tumour, measuring 4 × 4 cm in diameter was histologically evaluated. The tumour was composed of proliferations of mature fat cells and con

[Holt-Oram syndrome--the importance of early diagnosis and interdisciplinary approach. A case report].

We describe clinical presentation in a male newborn baby who presented with thumb aplasia, forearm hypoplasia and secundum atrial septal defect (ASD II). The child has no other bone anomalies or facial dysmorphism. The ultrasound morphology of the brain is normal. He has patent ductus arteriosus, without abnormalities in the ECG record. Complete blood count (CBC) is normal. In the spectrum of hereditary diseases from the group "heart-hand", clinical presentation in our patient corresponds to Holt-Oram syndrome (HOS). The role of the neonatologist in case of anomalies of the thumb or upper limb

Rhinosporidiosis of the left wrist joint: a case report.

Rhinosporidiosis is a chronic granulomatous disease caused by Rhinosporidium seeberi. It usually occurs in the mucous membranes of nose, nasopharynx, and eyes, and less commonly in extra nasal sites such as skin, bones, genitalia, and even the internal organs. Rhinosporidiosis occurs in the wrist joint with isolated bony involvement is rare. We report one such case in a 50-year-old man who presented with a non-tender, fixed swelling over his anterolateral aspect of left forearm. Radiography and computed tomography showed a lytic destructive lesion involving the distal radius, ulna, carpals, an

One-stage reconstruction for midfacial defect after radical tumor resection.

A serious midface defect involving resection of squamous cell carcinoma originating from the hard palate was treated by an unusual reconstructive strategy. After tumor resection, surgical reconstruction was accomplished in one stage using one free flap with one distant and local flap: a radial forearm flap to reconstruct the upper lip, a forehead flap to reconstruct the external nose, a cantilever calvarial bone graft to replace the nasal skeleton and a nasolabial flap and split thickness skin graft to cover the internal nasal lining. The rationale for this one-stage reconstruction and the pro

Bier spots in two children.

Bier spots are a distinct pattern of vascular mottling. Most reported cases are in young adults, with the youngest case in the literature at age 15 years. We report two children, ages 11 and 12 years, with Bier spots. Patient 1 was an 11-year-old boy who presented with white spots on the dorsal aspect of his hands. Patient 2 was a 12-year-old girl who presented with similar spots on the dorsal aspect of her left forearm and left hand. In both patients, the spots were visible when extremities were placed in a dependent position and disappeared with elevation of the extremity. Both patients were

[Infraclavicular block with ultrasound at amputated upper extremity].

When ultrasound (US) is used in peripheric nerve blocks, successful nerve blocks can be performed even if nerve stimulation is not possible. In this case report, we present a 37-year-old male patient, ASA physical status I, undergoing debridement and grafting for incomplete arm whose upper extremity (forearm) was amputated due to electric shock; motor response to nerve stimulation was not possible. With the help of US, lateral sagittal infraclavicular block was performed with 20 ml local anesthetic mixture (10 ml of 0.5% levobupivacaine and 10 ml 2% lidocaine with 5 mcg/ml epinephrine). After

Successful treatment of severe iatrogenic calcinosis cutis with intravenous sodium thiosulfate in a child affected by T-acute lymphoblastic leukemia.

Sodium thiosulfate has been successfully used to treat calcyphilaxis in adults and children, but its effect on iatrogenic calcinosis cutis secondary to extravasation of calcium solutions is less known. We describe a 5-year-old boy with acute lymphoblastic leukemia who developed severe calcinosis cutis in the right forearm and hand, and in the left leg and foot after extravasation of calcium gluconate during treatment for tumor-lysis-syndrome-related hypocalcaemia. Surgical debridement, local wound care, hyperbaric oxygen therapy, and sodium thiosulfate infusion achieved a complete healing of a

Ligamentous stabilization of the distal radioulnar joint.

Instability of the distal radioulnar joint results from fracture and/or malunions of the forearm bones, disruption, or laxity of the ligaments of the triangular fibrocartilage. Such instability often-times is not diagnosed acutely and presents as a chronic problem. When these ligaments fail to heal adequately after injury, distal radioulnar joint instability develops into mechanical problems resulting in pain, limited range of motion, and decreased grip strength. In this case, reconstruction of the disrupted distal radioulnar joint ligaments is essential to restore proper function. In this pre

Overriding fifth finger: an unusual sign in lower brachial plexopathy.

Hand deformities are often useful clinical signs. To our knowledge, they have not been described in association with brachial plexus lesions. A 28-year-old female presented with sudden left shoulder pain and medial forearm parasthesia. Examination showed the left fifth finger adducting and flexing over the dorsal aspect of the fourth finger. Neurophysiological studies and MRI supported a brachial plexus lesion. The physical sign was not present 5 months post onset and the patient experienced near complete clinical improvement. The unusual physical sign occurred in association with neuralgic am

Longitudinal sliding of the median nerve in patients with carpal tunnel syndrome.

In nerve compression syndromes restricted nerve sliding may lead to increased strain, possibly contributing to symptoms. Ultrasound was used to examine longitudinal median nerve sliding in 17 carpal tunnel syndrome patients and 19 controls during metacarpophalangeal joint movement. Longitudinal movement in the forearm averaged 2.62 mm in controls and was not significantly reduced in carpal tunnel syndrome (CTS) patients (mean=2.20 mm). In contrast, CTS patients had a 40% reduction in transverse nerve movement at the wrist on the most, compared to least, affected side and nerve areas were enlar
