It's Just a Fingertip! Yet Controversy Exists: Standardizing a Treatment Pathway for Pediatric Fingertip Injuries.

BACKGROUND: Pediatric fingertip injuries are very common; however, treatment varies greatly depending on geographic location and surgeon. A paucity of data exists on how emergency department practitioners and hand surgeons should treat these injuries. We sought to develop consensus around treatment guidelines for these injuries. METHODS: We conducted a Delphi survey of self-identified pediatric hand surgeons. Participants answered care delivery questions about 11 scenarios of fingertip injuries. These included minor (e.g., <50% subungual hematomas without fractures) to severe injuries (e.g.,

Compartment-specific effects of muscle strength on bone microarchitecture in women at high risk of osteoporosis.

BACKGROUND: It is well known that skeletal integrity is influenced by the musculature. Poor muscle strength (i.e. sarcopenia) is considered a major predictor of fragility fractures. While this observation appears particularly relevant for older women with increased risk of osteoporosis, there has been no comprehensive investigation to determine the influence of muscle performance on compartment-specific bone microarchitecture in multiple body regions. METHODS: We retrospectively analysed data from different muscle performance and bone microarchitecture assessments in 230 women (aged 21 to 87

Early surgical site infection in adult appendicular skeleton trauma surgery: a multicenter prospective series.

INTRODUCTION: Surgical site infections (SSI) studies rely on an imprecise and debatable definition. The term "wound healing problems" (WHP), not necessarily septic, is also frequently cited. This study had the objectives of determining the frequency of early SSIs in traumatology, these terms eventual correlation, and the factors influencing onset. PATIENTS AND METHODS: A multicenter prospective observational study was conducted in 12 centers. The exclusion criteria were open lesions as well as multiple injuries and multiple fractures (more than two fractures treated surgically). All patients

Comparative study of different intraoperative 3-D image intensifiers in orthopedic trauma care.

INTRODUCTION: Recently, isocentric C-arm fluoroscopy (Iso-C 3D) has been introduced as a precise imaging modality for intraoperative evaluation and management of fractures and osteosyntheses. The Siemens Iso-C 3D collects multiple fluoroscopic images during a 190-degree arc of rotation around the anatomic region of interest and reconstructs them into sagittal, axial, and coronal planes. Like the Iso-C 3D, the new Ziehm Vario 3D imaging system reconstructs images in multiple planes, but only requires a 136-degree arc of rotation. The purpose of this study was to compare the image quality and ra

Knee laxity associated with a proximal diaphyseal tibial and fibular fracture.

If operative treatment of a proximal diaphyseal tibial and fibular fracture is required, in most cases, it involves reduction and stabilization of only the tibial fracture. In this case report, after stabilization of the tibial fracture with an intramedullary nail, the patient continued to demonstrate significant varus knee laxity, despite an intact proximal tibiofibular joint and undisplaced fibular head. The stability of the knee was achieved only with internal fixation of the segmental fibular fracture. This case shows the importance of assessing knee laxity in all cases of proximal tibial

Fixation Strategies for Metacarpal Shaft Fractures: A Comparison of Plate-and-Screw, Intramedullary Screw, and Percutaneous Wire Techniques.

PURPOSE: The optimal fixation for metacarpal shaft fractures remains a topic of debate. This study compares clinical outcomes of three fixation methods-closed reduction with percutaneous Kirschner wire pinning, open reduction internal fixation (ORIF), and intramedullary fixation (IMN)-focusing on time to motion, range of motion (ROM), and complications. We hypothesized that IMN would allow earlier and more reliable return of ROM without increasing complications. METHODS: A retrospective review of 84 patients (104 diaphyseal fractures) treated at a level I trauma center between 2018 and 2024 w

Pregnancy and lactation-associated osteoporosis in a postpartum woman with twin pregnancy: a 5-year follow-up case report.

INTRODUCTION: Pregnancy and lactation-associated osteoporosis (PLO) is a rare but potentially disabling metabolic bone disorder that typically emerges in late pregnancy or the early postpartum period. Because its symptoms often present as common postpartum musculoskeletal complaints, diagnosis is frequently delayed until vertebral fractures are detected. This case report describes a postpartum woman with a twin pregnancy who developed multiple vertebral compression fractures, illustrating the diagnostic challenges, clinical features, and long-term outcomes of PLO. CASE PRESENTATION: A 32-year

Risky Driving Behaviours and Their Association With Fracture Injury Severity in Road Traffic Accident Patients.

BACKGROUND: Road traffic injuries (RTIs) are an increasingly significant population health problem in Pakistan. Such accidents cause fractures and are linked to unsafe driving behaviours. Determining the predictors of injury severity through behavioural means can reinforce the need for specific prevention methods. The study aimed to determine the correlation between risky driving behaviours and the severity of injuries in patients with road traffic-related fractures. PROCEDURES: The cross-sectional study was conducted from July 2024 to May 2025 at tertiary care hospitals in Islamabad, Rawalpi

Biconcave vertebral fractures as a possible distinctive feature of indolent systemic mastocytosis compared with primary osteoporosis.

UNLABELLED: Indolent systemic mastocytosis (ISM) is a rare cause of skeletal fragility. In this study, patients with ISM showed a markedly higher prevalence of biconcave vertebral fractures compared with primary osteoporosis. Assessing fracture morphology may aid in differentiating ISM, reducing diagnostic delay, and guiding appropriate management. PURPOSE: Indolent systemic mastocytosis (ISM) is a rare disorder frequently associated with bone fragility and vertebral fractures (VFx). While the morphology of VFx has been studied in other metabolic bone diseases (e.g., osteomalacia), its role i

Relationship Between Malunion and Short-Term Outcomes of Nonsurgical Treatment of Distal Radius Fractures in the Elderly: Differences Between Early- and Late-Geriatric Patients.

PURPOSE: Previous studies have suggested little association between radiographic malalignment and long-term functional outcomes of nonsurgical treatment of distal radius fractures in geriatric patients. However, no report has stratified the elderly by age and focused on short-term outcomes. The purpose of this study was to determine how the relationship between malunion and patient outcomes differs between early- and late-geriatric patients in the short and long terms after injury, thereby informing explanations and decision-making on treatment options for geriatric patients with distal radius

Study of double button plate and cannulated screw fixation for posterior cruciate ligament avulsion fracture.

BACKGROUND: The posterior cruciate ligament (PCL) plays an important role in maintaining the stability of the knee joint. To date, researchers have not reached agreement on which type of fixation material should be used to treat PCL tibial avulsion fractures. The aim of this study was to investigate the effects of double button plate and cannulated screw fixation in the treatment of PCL avulsion fractures. METHODS: We retrospectively reviewed our database, which was collected prospectively. From January 2019 to January 2020, 46 patients with posterior cruciate ligament avulsion fractures who w

Comparison between screw fixation and plate fixation via sinus tarsi approach for displaced intra-articular calcaneal fractures: a systematic review and meta-analysis.

BACKGROUND: Optimal surgical fixation for displaced intra-articular calcaneal fractures (DIACF) remains a subject of debate, particularly regarding the superiority between screw fixation and plate fixation via the sinus tarsi approach (STA). This review aims to determine the preferred treatment for DIACF and compare the outcomes of minimally invasive surgery options. METHODS: Our study involved thorough searches across multiple electronic databases, including PubMed, Cochrane, Embase, and Web of Science, to identify all relevant publications on distal intra-articular fractures of the calcaneu

DeepBackRib: Deep learning to understand factors associated with readmissions after rib fractures.

BACKGROUND: Deep neural networks yield high predictive performance, yet obscure interpretability limits clinical applicability. We aimed to build an explainable deep neural network that elucidates factors associated with readmissions after rib fractures among nonelderly adults, termed DeepBackRib . We hypothesized that DeepBackRib could accurately predict readmissions and a game theoretic approach to elucidate how predictions are made would facilitate model explainability. METHODS: We queried the 2017 National Readmissions Database for index hospitalization encounters of adults aged 18 to 64
