Symptoms before and after surgery for colorectal cancer.

Questionnaires were completed by a sample (n = 53) of patients at two points in time, prior to surgery for colorectal cancer and three months thereafter. The questionnaires comprised the Nottingham Health Profile, as a general health status measure, and a specific checklist of 30 symptoms. Of the physical symptoms, loss of appetite, rectal bleeding and urgency of bowel movement appeared to decline after treatment, whilst other potential symptoms, such as vomiting, appeared to occur only rarely, both before and after surgery. Pre-treatment symptoms of psychological distress appeared to have dis

[Current trends in clinical urology].

Many recent developments in urologic diagnostic and therapy are also of interest for general surgeons. Flexible and semirigid ureteroscopes (6-8 F) provide easy and atraumatic access to the whole upper urinary tract. Beside extracorporeal shockwave-lithotripsy now various endourologic modalities to treat ureteral stones are available. Although useful, laparoscopic techniques are not yet widely accepted by urologists because there is a lack of indications in urologic laparoscopic surgery. The functional reconstruction of the urinary bladder by means of an ileal neobladder with an anastomosis to

Ondansetron, clinical development for postoperative nausea and vomiting: current studies and future directions.

The clinical development of ondansetron for the prevention and treatment of postoperative nausea and vomiting has been progressing for 5 years, and continues as new directions of research are being addressed. Large multicentre studies have demonstrated the efficacy of ondansetron in the prevention and treatment of postoperative nausea and vomiting, but no large comparator studies have been reported. Several studies are now being undertaken to compare ondansetron with other currently used antiemetics such as droperidol and metoclopramide, assessing efficacy, safety, pharmacoeconomic, and qualit

Tracheoesophageal complications of thyroid disease.

Fine needle aspiration (FNA) biopsy in thyroid diagnosis has significantly reduced the need for diagnostic thyroidectomy. The relative proportion of therapeutic thyroid surgery is therefore increasing and, since a significant number of therapeutic procedures are indicated to alleviate tracheoesophageal complications, surgeons are likely to be confronted with an increasing percentage of potentially challenging cases in the face of declining thyroidectomy caseloads. All thyroidectomies done at the authors' institution from 1987 through 1990 were studied in order to assess the potential impact of

Influence of psychological and clinical factors on postoperative pain and narcotic consumption.

Demographic, psychological and clinical factors influencing postoperative pain and narcotic analgesic requirements in 162 patients undergoing elective operations under general anesthesia were studied. Eysencks Personality Questionnaire, Foulds Hostility Questionnaire, Zung's Anxiety-Depression (self-rating) Scales and the 43 Item Life Events Inventory by Holmes and Rahe were used. Clinical correlates such as surgical department, outcome of the operation, patient's knowledge of the diagnosis, were studied. Using multiple regression analysis the following results were obtained: postoperative pai

A retrospective study of Creutzfeldt-Jakob disease in Italy (1972-1986).

In a retrospective study of Creutzfeldt-Jakob disease (CJD) in Italy from 1972 to 1986, we found 79 cases which fulfilled the diagnostic criteria for CJD. The annual mortality rate was 0.09 cases per million inhabitants. In this series the female to male ratio was 2.59, a value significantly higher than that found in Italian population (1.05). The mean age at death was 62.1 +/- 9.4 years and the mean duration of the disease was 5.3 +/- 3.0 months. No familial cases of CJD were found in our series. Mental deterioration was present in all of our cases, myoclonus in 85% and the other clinical sig

Brain tumor in newborn babies.

5 cases of brain tumor in newborn babies under 2 months are presented. 4 of them were supratentorial teratoma and originated from the midline, and 1 was a glioma at the cerebelloponitine angle. 2 cases died before surgery and 2 cases after surgery. In our 5th case a benign teratoma of 150 g was removed from the third ventricle. He was discharged and enjoyed rather good health for 2 years and 1 month before dying of recurrence of tumor. Internal hydrocephalus associated with the huge tumor was responsible for the sudden deterioration of general condition. Therefore, shunting procedures should b

Dexmedetomidine-Ketamine-Based Multimodal General Anesthesia with Electroencephalographic Spectrogram-Guided Titration Improves Early Recovery After Lumbar Spine Fusion in Older Adults: A Randomized Controlled Trial.

BACKGROUND: Older adults undergoing lumbar spine fusion are vulnerable to impaired postoperative recovery and persistent postsurgical pain. Dexmedetomidine-ketamine-based multimodal general anesthesia may reduce volatile anesthetic and opioid exposure, but its efficacy and tolerability in older adults undergoing high-pain spine surgery remain uncertain. This randomized controlled trial evaluated whether electroencephalographic spectrogram-guided multimodal general anesthesia improves early recovery compared with conventional balanced anesthesia. METHODS: Patients aged ≥60 years undergoing ele

Preoperative Breast MRI: Surgeons' Patient Selection Patterns and Potential Bias in Outcomes Analyses.

OBJECTIVE: The purpose of this study is to determine which patient- and tumor-related and clinical variables influence dedicated breast surgeons' and general surgeons' referrals for preoperative breast MRI for patients with newly diagnosed breast cancer. MATERIALS AND METHODS: Surgeons who perform breast surgery responded to a survey from June 16, 2014, through August 11, 2014. Participants self-identified as breast or general surgeons and provided professional practice details. They used Likert scores (range, 1-7 with increasing likelihood to order MRI) to weigh numerous patient- and tumor-r

Patients' anesthesia preferences for Cesarean delivery: exploring the role of personality beliefs in spinal vs. General anesthesia.

BACKGROUND: This study aimed to compare the personality belief scores of patients who chose either general or spinal anesthesia during cesarean delivery (CD) and explore the relationship between personality beliefs, and anxiety levels. METHODS: This prospective study included expecting mothers, aged 18-45 years, who were classified as ASA II-III, scheduled for elective CD. Anesthesia (general or spinal) was selected based on the patient's preference. Patients with emergency CD indications, contraindications to either anesthesia type, or inability to complete the evaluation scales were exclude

Efficacy and safety of ulinastatin on cognitive dysfunction after general anesthesia in elderly patients: A protocol for systematic review and meta-analysis.

BACKGROUND: With the aging of society, the incidence of diseases increases. And along with the increase of surgery rate, the number of elderly patients with postoperative cognitive dysfunction (POCD) is also increasing. POCD seriously affects the mental state and quality of life of patients and their families. Clinical studies have shown that POCD is closely related to inflammatory reaction, and Ulinastatin can inhibit the inflammatory reaction and reduce the incidence of POCD in elderly patients under general anesthesia. However. the effect of Ulinastatin on POCD in elderly patients under gen

UK undergraduate and postgraduate breast surgery training interest and exposure: Are future doctors prepared to meet workforce demands? A national study by the PLASTICS UK Research Collaborative.

INTRODUCTION: Breast surgery encompasses oncoplastic, reconstructive and aesthetic procedures, with UK training delivered through both general surgery and plastic surgery pathways. Despite this, limited national data exist on medical student and early postgraduate exposure to breast surgery or their understanding and interest in the specialty. This study evaluates undergraduate and early postgraduate exposure, perceptions and career interest, and examines how factors such as clinical experience and training level influence these outcomes. METHOD: A national cross-sectional online survey was c

Computer Tomography (CT)-Based Study to Investigate Feasibility and Efficacy of Thoracoscopic Surgery in the Treatment of Penetrating Chest Wall Tuberculosis.

BACKGROUND: Chest wall tuberculosis may develop if tuberculous (TB) lesions spread through the chest wall and invade the thoracic cavity. The presence of a mass on the patient's chest wall may be the first indication of TB, and a chest CT scan can help diagnose external penetrating chest wall TB, the incursion of tuberculosis from the lungs into the chest wall. OBJECTIVE: This study examines the safety and efficacy of thoracoscopic-assisted surgery for the treatment of penetrating chest wall tuberculosis as a means of exploring novel concepts of minimally invasive surgery. Quality of life and patient satisfaction after laparoscopic antireflux surgery using the QOLARS questionnaire.

BACKGROUND: Currently, evaluation of patient satisfaction and quality-of-life data to estimate the outcome of laparoscopic antireflux surgery is an important issue. This study aimed first to report the midterm results for the surgical management of gastroesophageal reflux disease (GERD) by laparoscopic fundoplication and to evaluate surgical outcome, including quality of life and patient satisfaction. The second aim was to determine whether preoperative quality-of-life measurement can predict which patients will be satisfied with antireflux surgery. METHODS: The current prospective study eval

Prevalence and Risk Factors for Chronic Postsurgical Pain after Cardiac Surgery: A Single-center Prospective Cohort Study.

BACKGROUND: Chronic postsurgical pain is a common complication of surgery. The role of psychologic risk factors like depression and anxiety is substantially understudied in cardiac surgery. This study sought to identify perioperative factors associated with chronic pain at 3, 6, and 12 months after cardiac surgery. The authors hypothesize that baseline psychologic vulnerabilities have a negative influence on chronic postsurgical pain. METHODS: The authors prospectively collected demographic, psychologic, and perioperative factors in a cohort of 1,059 patients undergoing cardiac surgery at the
