Dedifferentiated Carcinoma of the Ovary: A Rare Aggressive Subtype.

Dedifferentiated carcinoma of the ovary (DDOC) is a rare and aggressive malignancy characterized by both differentiated and undifferentiated carcinoma components. While its endometrial counterpart, dedifferentiated carcinoma of the endometrium, is more frequent, DDOC remains poorly understood. We present the case of a young woman who presented with acute abdominal pain and was found to have a large complex ovarian mass. Histopathological examination revealed a combination of endometrioid carcinoma and undifferentiated carcinoma. Immunohistochemical analysis demonstrated distinct staining patte

Decoding the role of transcriptomic clocks in the human prefrontal cortex.

Aging is characterized by interindividual variability quantified through aging clocks, informed by biological data and are linked to diverse health outcomes. In this study, we comprehensively profiled several transcriptomic clocks, characterize their stochastic components, compare their predictive accuracy, perform associations with mental disorders, and elucidate their functional implications in three human prefrontal cortex (PFC) datasets. Our analysis revealed substantial heterogeneity in transcriptomic age prediction across different clock signatures. Notably, deep learning models capture

RORA as a sex-specific molecular link in autism spectrum disorder: from transcriptional regulation to clinical phenotype across the lifespan.

RORA is a molecular hub in autism spectrum disorder (ASD) that integrate sex steroid signaling, neurodevelopmental gene regulation, circadian rhythm, immune function and metabolic homeostasis. Convergent evidence from postmortem brain, peripheral blood, methylation profiling, animal models and cellular studies indicates that RORA is frequently downregulated in ASD, disrupting downstream pathways involved in synaptogenesis, cortical development, aromatase activity and neuroimmune balance. RORA is bidirectionally regulated by sex hormone, where androgens suppresses and estrogens activates its ex

Decoding the heterogeneity of human undifferentiated spermatogonia reveals RAS-dependent regulation of stem cell fate.

Spermatogonial stem cells (SSCs) are essential for long-term spermatogenesis and hold therapeutic potential for treating male infertility. While rodent SSCs are well characterized, human SSCs remain poorly understood. Here, we screen antibodies against proteins encoded by genes enriched in specific subsets of human undifferentiated spermatogonia (uSPG) identified by single-cell RNA sequencing. We characterize four markers labeling distinct uSPG subsets: PIWIL4 marks primitive, quiescent uSPG; EGR4 marks uSPG at a proliferative crossroads; and PPP1R36 and NANOS3 label distinct proliferative sub

Alterations in Gut Microbiome Diversity and Composition in Patients with Epidermolysis Bullosa: A Case-Control Study.

Epidermolysis bullosa (EB) is a rare inherited genodermatosis that affects both the skin and gastrointestinal tract, potentially leading to altered gut microbiota. This study aimed to assess the diversity and composition of the gut microbiome in EB patients compared to healthy controls. Twelve EB patients-six with dystrophic epidermolysis bullosa patients (pDEB) and six with nondystrophic epidermolysis bullosa patients (pNDEB)-and 12 age- and gender-matched healthy controls were included. Fecal samples were collected, and 16S rRNA gene sequencing (V3-V4 region) was performed. Alpha diversity i

Coordinated stimulation of axon regenerative and neurodegenerative transcriptional programs by ATF4 following optic nerve injury.

Stress signaling is important for determining the fates of neurons following axonal insults. Previously, we showed that the stress-responsive kinase PERK contributes to injury-induced neurodegeneration (Larhammar et al., 2017). Here, we show that PERK acts primarily through activating transcription factor-4 (ATF4) to stimulate not only pro-apoptotic but also pro-regenerative responses following optic nerve damage. Using conditional knockout mice, we find an extensive PERK/ATF4-dependent transcriptional response that includes canonical ATF4 target genes and modest contributions by C/EBP Homolog

PIWIL4 regulates gene expression and piRNA levels in RSV-infected airway epithelial cells.

Respiratory syncytial virus (RSV) causes acute lower respiratory tract infection in young children, the elderly, and immunocompromised hosts, and no RSV vaccine or therapy exists for infants, beyond prophylaxis. PIWI-interacting RNAs (piRNAs), small non-coding RNAs acting through P element-induced wimpy testis (Piwi) proteins, remain largely unexplored. We characterized PIWIL4 expressions and function during RSV infection or poly(I:C) stimulation, a proxy of viral infection. PIWIL4 was expressed in primary and immortalized small airway epithelial cells following RSV infection. While PIWIL4 kno

Phenotype-guided Management of Post-traumatic Headache: An Update on Pathophysiology and Treatment.

PURPOSE OF REVIEW: To provide an update on the pathophysiology, phenotyping, and management of post-traumatic headache (PTH) following mild traumatic brain injury (mTBI), and to propose a phenotype-guided framework for evaluation and treatment. RECENT FINDINGS: Emerging evidence suggests that PTH represents a distinct pathophysiological entity rather than simply a variant of migraine. Recent studies involving calcitonin gene-related peptide (CGRP), pituitary adenylate cyclase-activating polypeptide (PACAP), autonomic dysfunction, and neuroimaging biomarkers suggest that PTH may comprise sever

Cholestatic liver disease in an adolescent: don't forget progressive familial intrahepatic Cholestasis-8.

Progressive familial intrahepatic cholestasis (PFIC) is an inherited disorder affecting bile acid secretion, with a heterogenous clinical presentation including pruritus, jaundice, growth failure, hepatosplenomegaly, and cirrhosis. Most patients present in infancy or early childhood, leading to progressive liver disease and potentially liver failure, often requiring liver transplantation. We present a 14-year-old girl with incidentally detected cholestasis. She had no previous jaundice, abdominal pain, clay stools, or pruritus. Establishing a robust triangulation framework to explore the relationship between hearing loss and Parkinson's disease.

The relationship between hearing loss (HL) and Parkinson's disease (PD) remains unclear. Using individual-level and summary-level data from the UK Biobank and the largest genome-wide association studies, we examined this link through observational, Mendelian randomization and genetic pleiotropy analyses. Among 158,229 participants, PD risk rose with HL severity especially in elder and males, and hearing aids significantly reduced PD risk in males. Although our results did not support a causal association, genetic correlation analysis suggested a localized genetic overlap (17q21.31). We identif

STAG1 Disease, Central Precocious Puberty, and Bone Fragility-A Case Report.

Background: Previously reported STAG1 gene-related cohesinopathies describe a range of clinical features, typically including intellectual disability (ID), facial dysmorphisms, and limb anomalies. Case presentation: We present the case of an 8-year-old girl with main findings including ID, central precocious puberty (CPP), and bone fragility. Panel genetic testing revealed a pathogenic STAG1 variant, NM_005862.3:c.2116del p.(Asp706Ilefs*15), which can only partially explain the clinical phenotype. Reports of STAG1-related cohesinopathies, including ours, have consistently described development

Diagnosis of Arrhythmogenic Cardiomyopathy in a Young Patient With Recurrent Myocarditis: The Importance of Genetic Testing.

Genetic testing for cardiomyopathies has become a pivotal tool in diagnosing conditions with diverse and often overlapping clinical presentations. Arrhythmogenic cardiomyopathy (ACM), which affects approximately 1 in 5000 individuals, is a life-threatening condition associated with life-threatening arrhythmias and sudden cardiac death (SCD). While ACM typically presents with right- or left-sided heart failure, ventricular arrhythmias, or SCD, it can also manifest as acute or recurrent myocarditis. These inflammatory episodes, known as the "hot phase" of ACM, are less common but highly relevant

Insights from a methylome-wide association study of antidepressant exposure.

This study tests the association of whole-blood DNA methylation and antidepressant exposure in 16,531 individuals from Generation Scotland (GS), using self-report and prescription-derived measures. We identify 8 associations and a high concordance of results between self-report and prescription-derived measures. Sex-stratified analyses observe nominally significant increased effect estimates in females for four CpGs. There is observed enrichment for genes expressed in the Amygdala and annotated to synaptic vesicle membrane ontology. Two CpGs (cg15071067; DGUOK-AS1 and cg26277237; KANK1) show c

Regulation of detoxifying enzymes expression and restriction of picorna-like virus infection by natural polysaccharide extracts in Drosophila cells.

The world is currently witnessing a rise in viral infections, while the availability of antiviral drugs remains limited. Traditional Chinese medicine (TCM) has historically served as a valuable source of novel compounds for disease treatment. In this study, we assessed the antiviral potential of various TCM compounds using Drosophila melanogaster as a model organism. Our findings reveal that natural polysaccharide extracts, prepared from 10 commonly used medicinal herbs or fungi, exhibit antiviral activity against two picorna-like viruses. Importantly, the antiviral effect is not directly attr

Otofaciocervical Syndrome and Its Overlap with Branchiootorenal Spectrum: An Integrated Literature Analysis of EYA1-Related Disorders, Including a Novel Case with an 8q13.2q13.3 Deletion.

Otofaciocervical syndrome (OTFCS) is a rare disorder characterized by facial, auditory, and shoulder girdle anomalies. Its significant phenotypic overlap with branchiootorenal spectrum disorders (BORSD)-both linked to EYA1 (EYA transcriptional coactivator and phosphatase 1) gene defects-has raised questions about whether they are distinct entities or part of a single clinical spectrum. We report a novel OTFCS patient with a de novo microdeletion spanning EYA1 and review all published cases of EYA1-related disorders. Our analysis reveals that all EYA1 variant types (truncating, missense, CNV, e

A proposed role for CDO1 in CNS development: Three children with rare missense variants and a neurological phenotype.

Cysteine dioxygenase type 1 (CDO1) encodes a non-heme iron dioxygenase, which is involved in cysteine metabolism. While CDO1 has been proposed to be involved in multiple physiological processes, an association with congenital disease has yet to be well defined. This study presents detailed clinical and molecular information on three individuals with overlapping neurological features. All three individuals were found to have rare, conserved, de novo variants clustered in a conserved region of the CDO1 gene with no alternative genetic etiology identified. Features present in all three individual

A zebrafish model of nicotinamide adenine dinucleotide (NAD+) deficiency-derived congenital disorders.

Developmental NAD+ deficiency is associated with diverse congenital malformations. Congenital NAD deficiency disorder (CNDD) is a multisystem developmental condition characterized by cardiac, renal, vertebral, and limb anomalies, among others. It is caused by biallelic pathogenic variants in genes involved in the nicotinamide adenine dinucleotide (NAD+) synthesis pathway. CNDD anomalies overlap with clinical features described in vertebral-anal-cardiac-tracheoesophageal fistula-renal-limb (VACTERL) association, suggesting a possible shared etiological link through NAD+ deficiency. However, the

Prenatal and postnatal findings in cerebellofaciodental syndrome: a rare genetic disorder.

INTRODUCTION: Cerebellofaciodental syndrome (CFDS) (OMIM# 616202) is an autosomal recessive neurodevelopmental disorder with an incidence of one in 10 000 000. To date, only 15 cases have been reported in the literature. It is characterized by dysmorphic features, microcephaly, short stature, intellectual disability, and central nervous system anomalies. CASE REPORT: Here, we describe a case presenting with short long bones, restricted limb movements, and growth restriction during the antenatal period. After birth, the infant exhibited facial dysmorphism, microcephaly, a bell-shaped thorax, s

Secondary and therapy-related acute myeloid leukemias: Overlapping features, distinct trajectories.

Therapy-related acute myeloid leukemia (tAML) and AML arising from previous hematologic disorders (secondary AML, sAML) share similar biological features, including karyotype abnormalities and gene specific mutations, patient-related risk factors. Older age and lower performance status also contribute to dimal prognosis, and dismal prognosis, both in terms of response rate and overall survival. However, these 2 entities significantly differ in leukemogenic trajectories. In this line, recent advances allowed for a better understanding of differential clonal progression processes in the broad la

Jumping Translocation Breakpoint Expression in Midgestation Mouse Embryos.

Jumping translocations (JTs) can lead to partial trisomies. A breakpoint within the gene known as Jumping Translocation Breakpoint (JTB) has previously been associated with JTs involving the long arm of human chromosome 1 (1q). These 1q+ amplifications are frequently observed in cancer. JTB was initially mapped to the Epidermal Differentiation Complex (EDC) at 1q21, and earlier studies primarily focused on its role in malignant or adult tissues. Using updated genomic data, we refined the mapping of JTB. We employed RNA in situ hybridization (RISH) to visualize Jtb expression with organ, tissue
