Assessment of Patients' Quality of Life during Conservative Treatment after Distal Radius Fracture.

Introduction: This study aimed to assess patients' quality of life after distal radius fracture treatment (at least six months, but no more than ten years, after the treatment) based on the analysis of objective and subjective parameters and the influence of the fractured side on the final results. Materials and Methods: The study sample consisted of 30 women who claimed to be right-handed, divided depending on the side of the fracture (left vs. right limb). Patients were evaluated with a goniometer for active wrist movement, pronation, and supination in the elbow joint. Furthermore, the globa

Predicting and controlling risks from human exposures to vibration and mechanical shock: flag waving and flag weaving.

UNLABELLED: At work or in leisure activities, many people are exposed to vibration or mechanical shocks associated with risks of injury or disease. This paper identifies information that can be used to decide whether there may be a risk from exposure to hand-transmitted vibration or whole-body vibration and shock, and suggests actions that can control the risks. The complex and time-varying nature of human exposures to vibration and shock, the complexity of the different disorders and uncertainty as to the mechanisms of injury and the factors influencing injury have prevented the definition of

[Bruises in infants should always be accounted for].

Haematomas were observed in three male infants under 1 year of age. In the first case, no suspicion regarding child abuse was raised, but, in retrospect, the appearance of haematomas coincided with the times he had been with a childminder. When he was admitted to hospital for a different reason, namely a serious airway infection, the causal relationship could no longer be established. In the second case, the grandparents told the general practitioner about their suspicion of maltreatment. CT revealed inflicted traumatic brain injury. In the third case, inflicted traumatic brain injury was also

Nitric oxide and endothelin relationship in intestinal ischemia/reperfusion injury (II).

Endothelins ( ETs ) are potent vasoconstrictors derived from vascular endothelium. They have primary roles in many pathophysiologic states including ischemia/reperfusion (I/R) injury. The relationships between nitric oxide (NO) and ETs are still under investigation. In this study on rats we want to focus on the interaction of NO and ET especially in I/R injury. For this purpose ET-1 and PD-156252, a nonselective ET receptor blocker, were given in a mesenteric I/R model and reactive oxygen species were detected directly using chemiluminescence of the ileal tissue. ET administrations to sham and

Effect of human urinary thrombomodulin on endotoxin-induced intravascular coagulation and pulmonary vascular injury in rats.

Adult respiratory distress syndrome (ARDS) and disseminated intravascular coagulation (DIC) are serious complications of sepsis. Thrombomodulin, an important endothelial anticoagulant, binds thrombin to generate activated protein C (APC). To determine whether thrombomodulin purified from human urine (urinary thrombomodulin, UTM) is useful for the treatment of DIC and ARDS in sepsis, we examined the effect of UTM on endotoxin (ET)-induced coagulation abnormalities and pulmonary vascular injury in rats. Intravenous administration of UTM prevented the ET-induced pulmonary accumulation of leukocyt

Why does the injured drunk driver escape arrest and conviction? A case presentation and discussion by health care and law enforcement professionals.

Serious injury protects the drunk driver from prosecution and conviction for driving drunk. This escape from liability occurs despite the fact that almost half of all traffic fatalities are attributable to alcohol. Consequently, the drunk driver who is injured may resume driving as soon as he is physically able to do so, thereby becoming a renewed threat to himself and others. On the other hand, a conviction for driving while impaired (DWI) results in loss of the driving privilege, reducing exposure of other drivers to a public health hazard. A DWI conviction may also stimulate the drunk drive

Role of epidermal growth factor in protection and repair of gastric mucosal injury.

A role of epidermal growth factor (EGF) in protection and repair of gastric mucosal injury caused by p.o. administration of 0.6 N HCl was investigated in male Wistar rats. Previous to the study on the role of EGF in gastric mucosal protection and repair, changes of gastric mucosal EGF level was determined sequentially by RIA for 180 min after treatment with 0.6 N HCl and intraperitoneal injection of recombinant hEGF. Concentration of incorporated hEGF into the gastric mucosa reached the peak level at 30 min after injection of hEGF. On the other hand, the gastric endogenous EGF level fell remar

Partial rotator cuff tears in adolescents: factors affecting outcomes.

INTRODUCTION: In the adult population, rotator cuff tears are common and established treatment methods yield satisfactory results. In adolescents, however, these injuries are uncommon and few treatment methods and outcome reports exist. The purpose of this study was to examine a series of adolescent rotator cuff tears, identify associated pathology, and report treatment outcomes. METHODS: A retrospective comparative analysis of adolescent patients treated for rotator cuff tears diagnosed by magnetic resonance imaging (MRI) or arthroscopy between 2008 and 2010 was performed. Patients were divi

Workers' compensation experience of North Carolina residential construction workers, 1986-1994.

A total of 31,113 workers' compensation claims among 7,400 North Carolina Homebuilders Association (NCHA) members and their subcontractors for the period 1986-1994 were analyzed to calculate workers' compensation claim incidence density rates. For the 7 years studied, the average rate (cases/200,000 work hours) for all claims was 16.40 and the rate for medical or lost time cases was 10.78. Highest rates for cases involving medical costs or paid lost time by mechanism of injury were observed for being struck by an object (3.1), lifting/movement (1.97), falls from a different level (1.13), strik

Functional outcome thirty years after median and ulnar nerve repair in childhood and adolescence.

BACKGROUND: Age at injury is believed to be a factor that strongly influences functional outcome after nerve injury. However, there have been few long-term evaluations of the results of nerve repair and reconstruction in children. Our aim was to evaluate the long-term functional outcome of nerve repair or reconstruction at the forearm level in patients with a complete median and/or ulnar nerve injury at a young age. METHODS: Forty-five patients were assessed at a median of thirty-one years after a complete median and/or ulnar nerve injury in the forearm. The outcome was classified with a tota

Management of severe forearm injuries.

A review of principles and an operative guideline for repair of severely mutilating injuries to the forearm have been set forth. These concepts and their application have been illustrated in a series of clinical cases. The following key concepts have developed from these clinical experiences: 1. The surgeon must evaluate each case based upon the potential for return of sensation and function. One cannot justify the statement that a replanted arm is always superior to a prosthesis, even if its only purpose is cosmetic. 2. Care must be taken in the emergency room to evaluate the entire patient,

Combining Digital Camera Metadata and Artificial Intelligence to Sort Postmortem Gross Photographs: A Novel Approach in Image Classification in Forensic Pathology.

Reviewing postmortem photographs is routine in forensic pathology. Although standard photographic techniques are used, sorting postmortem photographs can be overly time-consuming, especially when reviewing large quantities of photographs. Artificial intelligence may assist in identifying body parts/locations but is challenged in determining spatial relationships such as sidedness, angle, and distance/depth. Metadata stored in modern digital cameras provides physical data that would assist in determining this information, which can be critical in the legal setting. This proof-of-concept study u

The modified fighting hypothesis of handedness: Evidence from sharp force injuries and further considerations.

The modified fighting hypothesis (MFH) proposes that most humans are right-handed because it conveyed an advantage during intraspecific fights with sharp weapons, due to the leftward location of the heart and aorta. An examination of the literature on sharp force injury showed that the thoracic region is penetrated more than any other region, and the left thorax is penetrated approximately 2.4 times more often than the right thorax. Handedness influenced the side of the thorax targeted, with most right-handers penetrating the left thorax in front of their right hand. As two thirds of the heart

The Role of Cav1.1-Ca2+-IGF-1 in EMS Preventing Denervated Intrinsic Muscle Atrophy Induced by Brachial Plexus Injury.

Functional recovery of the hand following brachial plexus injury (BPI) is highly dependent on preventing atrophy of the intrinsic muscles of the hand. Although electrical muscle stimulation (EMS) is an effective therapeutic approach for atrophy, its underlying mechanisms remain poorly elucidated. This study investigated the biological mechanisms through which EMS-induced muscle contraction inhibits the progression of denervation-induced atrophy. Using a rat model of total BPI, we validated the protective effect of EMS against atrophy in denervated intrinsic muscles. Whole-cell patch-clamp anal

Neuroprotection during Thrombectomy for Acute Ischemic Stroke: A Review of Future Therapies.

Stroke is a major cause of death and disability worldwide. Endovascular thrombectomy has been impactful in decreasing mortality. However, many clinical results continue to show suboptimal functional outcomes despite high recanalization rates. This gap in recanalization and symptomatic improvement suggests a need for adjunctive therapies in post-thrombectomy care. With greater insight into ischemia-reperfusion injury, recent preclinical testing of neuroprotective agents has shifted towards preventing oxidative stress through upregulation of antioxidants and downstream effectors, with positive r

Modification of the Zancolli Lasso Procedure for Simultaneous Correction of Wartenberg's Sign.

Ulnar nerve injury initiates an imbalance between the intrinsic muscles and extrinsic extensors of the ring and small fingers, which leads to the characteristic hyperextension of the metacarpophalangeal (MP) joints and flexion of the proximal interphalangeal joints of these 2 digits-commonly referred to as the ulnar claw hand. In addition to these changes in the static posture of the hand, ulnar nerve palsy severely impairs grasp due to deficient active MP joint flexion. In most cases, motor balance can be restored by preventing MP joint hyperextension and augmenting MP joint flexion using the

Thumb Carpometacarpal Instability.

The basal joint complex of the thumb provides the framework necessary for function of the human hand. Although its unique saddle articulation allows for a wide range of motion necessary for routine function of the hand, it is rendered inherently unstable because of poor bony congruency and reliance on its capsuloligamentous support. Painful instability of this joint can stem from several causes including traumatic dislocation, various hypermobility conditions, and chronic overuse and microtrauma. A thorough history and examination as well adequate imaging is necessary for proper evaluation of

The role of tumour necrosis factor in hepatitis B infection: Jekyll and Hyde.

Chronic hepatitis B (CHB) is a major health problem worldwide and is associated with significant long-term morbidity and mortality. The hepatitis B virus (HBV) is a hepatotropic virus that is capable of integrating in the host nucleus permanently resulting in lifelong infection. To date, there is no definitive cure for HBV, as our current treatments cannot eradicate the viral reservoir that has integrated in the liver. Elucidating the immunopathogenesis is key to finding a therapeutic target for HBV as the virus is not in itself cytopathic but the immune response to the virus causes the majori
