Enhanced mucosal interleukin-6 and -8 in Helicobacter pylori-positive dyspeptic patients.

OBJECTIVES: To determine the concentrations of interleukin-1 beta, interleukin-6, and interleukin-8 in tissue homogenates of mucosal biopsy specimens from Helicobacter pylori-positive and -negative patients. METHODS: In 43 consecutive patients who underwent upper gastrointestinal endoscopy, seven antral biopsies were taken; three specimens were used for cytokine determination and the remaining four biopsies were processed for H. pylori detection. Peripheral venous blood was collected and IgG to H. pylori was assayed by an ELISA technique. RESULTS: Twenty-nine of 43 patients (67%) were histol

Fall in the prevalence of chronic gastritis over 15 years: analysis of outpatient series in Finland from 1977, 1985, and 1992.

To investigate whether the occurrence of chronic gastritis (and Helicobacter pylori acquisition) has changed in Finland in the past 15 years, the prevalence rates of chronic gastritis in biopsy specimens in consecutive series of outpatients (aged 20 or more) who had undergone diagnostic upper gastrointestinal endoscopy in 1977 (702 patients), 1985 (1309 patients), or 1992 (1447 patients) were compared. The prevalences of gastritis in these series were also compared with that in a random sample (438 subjects) of people who underwent endoscopy in 1974-76. It seemed that the prevalence of gastrit

[The pharmacodynamic and ulcer-healing action of modern anti-ulcer agents].

In a study of 159 duodenal ulcer patients the authors studied the effect of H2-blockers, antibacterial and sucralfate monotherapy on the frequency of the ulcer healing, Helicobacter pylori (HP) infection and acid-proteolytic aggression in the body and antropyloric portion of the stomach. All the drugs were found to exhibit similar antiulcer activity. Inhibition of the acid-proteolytic aggression in the gastric body was registered only under the action of H2-blockers. Antibacterial preparations inhibited the growth of HP, but did not affect the acidity and proteolysis in the gastric body. Neith

Helicobacter pylori reinfection rate, in patients with cured duodenal ulcer.

OBJECTIVE: To determine the reinfection rate of the gastric mucosa in patients previously cured of duodenal ulcers, following the eradication of Helicobacter pylori. Only those remaining H. pylori-negative beyond 12 months of follow-up were studied, to minimize the potential inclusion of patients with H. pylori recrudescence. METHODS: Patients with endoscopically proven duodenal ulcers who had been treated with triple therapy, resulting in documented eradication of H. pylori and cure of the ulcer for at least 4 years, were recalled and had their H. pylori status determined by the 14C-urea bre

[Peptic activity of gastric juice in chronic gastritis. Morpho-functional aspects].

The aim of this study has been to evaluate peptic activity in gastric juice and gastric peptic cell mass in chronic gastritis. As regard peptic activity, there is a close correlation between it and the peptic gastric cell mass considered globally and expressed as Peptic Gastric Index (PGI), resulting from the individual average between fundic peptic index (chief cells + fundic muco-peptic cells) and antral peptic index (antral muco-peptic cells), both obtained by multiplying the number of peptic cells per mm2 by the thickness of respectively fundic and antral gland layer). In particular fundic

Long-term omeprazole therapy does not affect Helicobacter pylori status in most patients.

Fifty-one patients were treated with 20-60 mg omeprazole for reflux oesophagitis resistant to H2-blocker therapy during a mean of 49 months of follow-up. With use of a standardized enzyme-linked immunosorbent assay technique specific IgG and IgG Helicobacter pylori antibodies were determined in serum obtained at the start of therapy and at the most recent visit. At the start of therapy 26 patients (51%) had evidence of H. pylori infection, as demonstrated by increased IgG and IgA antibody levels. During follow-up, 4 of these 26 patients (15%) became H. pylori seronegative. It is concluded that

Serum pepsinogens as markers of response to therapy for Helicobacter pylori gastritis.

We have investigated the effect of therapy for Helicobacter pylori gastritis on serum concentrations of pepsinogen I and II in 43 patients. In the 22 patients in whom therapy resulted in dramatic decrease in gastritis scores and in clearance of the bacteria, there was a highly significant (P = 0.0001) fall in mean serum pepsinogen II from 13.3 +/- 0.8 to 7.9 +/- 0.7 micrograms/liter, and a less pronounced fall in pepsinogen I from 89.0 +/- 5.9 to 78.5 +/- 0.4 micrograms/liter (P = 0.01). These changes resulted in a significant (P = 0.01) increase in the pepsinogen I/II ratio. In contrast, nons

Intestinal metaplasia and Helicobacter pylori: an endoscopic bioptic study of the gastric antrum.

To study the relationship between intestinal metaplasia and Helicobacter pylori infection, 2274 gastroscopic antral biopsies taken from 533 patients were examined. Overall, intestinal metaplasia was found in 135 patients (25.3%) and H pylori in 289 patients (54.2%). The prevalence of intestinal metaplasia and H pylori was age related, being more common in patients greater than or equal to 50 years compared with patients less than 50 years (intestinal metaplasia, p less than 0.001 and H pylori, p less than 0.05). Intestinal metaplasia was found more often in H pylori positive patients compared

[Secretory immunoglobulin A and pyloric helicobacteriosis in patients with duodenal ulcer].

57 patients with duodenum ulcer were examined. Secretory immunoglobulin A (SIgA) was determined by an immunofluorescent method in the gastric and duodenum mucosa depending on the population size of pyloric helicobacter (HB). The frequency of SIgA detection in the gastric mucosa depends on the HB population size: when it is considerable SIgA was not detected in 41% cases. SIgA was detected in the gastric mucosa without pathological lesions in 85% cases, in atrophic gastritis in 70% and in superficial gastritis in 54% cases.

The first cases of Helicobacter pylori (Campylobacter pylori) reported from Costa Rica.

Two gastric antrum biopsies of each of 92 consecutive patients undergoing gastroendoscopy were studied. The first biopsy was cultured for Helicobacter (Campylobacter) pylori. The second specimen was fixed and processed for histopathological analysis. The bacteria were isolated from 34 (81%) of 42 patients with nonspecific chronic gastritis, 19 (90%) of 21 cases of duodenal or gastric peptic ulcer, and from 3 (23%) of 13 normal patients. The overall frequency of isolation was 62 (67%) of the 92 cases.

Secondary prevention of coronary artery disease with antimicrobials: current status and future directions.

Over the past several decades, coronary artery disease (CAD) has become the major health problem in the Western world with more than 50% of deaths attributed to its complications. The exact causes of atherosclerosis are not clearly known, although multiple risk factors (e.g. hypertension, hyperlipidemia, diabetes mellitus, family history, and smoking) have been well described. However, these risk factors account for only about 50% of the total risk of CAD. Consequently, an ongoing search is under way to discover new risk factors for atherosclerosis as well as the basic underlying causes of pro

The Association between Vitiligo and Infections: A Systematic Review.

Vitiligo is an acquired depigmentation disease with an unknown etiology. Several theories on its etiology have been proposed, including autoimmune mechanisms, oxidative stress, genetic factors, environmental factors, and lack of melanocyte growth factor. Recent studies have demonstrated a possible correlation between microbial infections and the development or consequences of vitiligo. This study aims to investigate the potential association between microbial infections and vitiligo, focusing on identifying any explainable correlations between these two. A systematic search was performed on al

Multiple bacterial infections increase the risk of hepatic encephalopathy in patients with cirrhosis.

OBJECTIVE: Patients with liver cirrhosis (LC) are at increased risk for bacterial infections. It is not fully understood how exposure to infections induces further development of hepatic encephalopathy (HE). This study estimated risks of infection associated with HE among patients with LC. METHODS: A nested case-control study of 14,428 adult patients with LC was performed using the population-based Longitudinal Health Insurance Database 2000 in Taiwan. Cases were cirrhotic patients who developed HE during follow-up. Controls were matched to each case by age at LC diagnosis (±2 years), sex, Ch

Helicobacter canis: A Review of Microbiological and Clinical Features.

Helicobacter canis, an enterohepatic Helicobacter, has proven its role in human diseases and has been rediscussed in recent years as its zoonotic potential is increasingly described. Routine microbiological detection of this pathogen is a difficult task as its culture may fail due to fastidious growth. It is therefore supposed that many clinical laboratories under-recognize H. canis infections. A review of all clinical and microbiological literature currently available from previous relevant H. canis human clinical cases, mainly bacteremia, added with a clinical case observed at the Cliniques

Unveiling the mediating role of immune cells in the link between pathogen infections and Hashimoto's thyroiditis.

BackgroundThis study employed Mendelian randomization to investigate the relationships between pathogenic infections, immunophenotypes, and Hashimoto's thyroiditis, providing deeper insights into infection-induced Hashimoto's thyroiditis development beyond the limitations of small, inconclusive observational studies.MethodsData on pathogenic infections, immune cells, and Hashimoto's thyroiditis were obtained from public databases. The inverse variance weighted method was used as the primary analytical approach, with robustness of the findings confirmed through heterogeneity and pleiotropy test

Helicobacter pylori in Pediatric Patients.

Helicobacter pylori causes one of the most common chronic bacterial infections. Clinical manifestations include asymptomatic chronic gastritis, gastric and duodenal ulcers, adenocarcinoma, and gastric mucosa-associated lymphoid tissue lymphoma in adults. In children, most H pylori infections are asymptomatic despite being associated with microscopic gastric inflammation, and children rarely develop complications associated with infection. Due to rising resistance and lack of symptomatic improvement in the absence of peptic ulcer disease, testing and eradication therapy are recommended only for

Environmental and Physiological Barriers to Child Growth and Development.

Aggregated analyses of child growth in low- and middle-income countries (LMICs) reveal a remarkably consistent picture of serious growth failure compared to the WHO reference growth curves. Impoverished diets with low dietary diversity are a key driver of poor growth, but there are important additional environmental factors that limit the uptake and utilization of nutrients. This paper considers such factors. A large proportion of the rapid growth deterioration in later infancy can be ascribed to infections and to wider nonspecific effects of living in an unhygienic environment, including the
