Kidney transplantation is one of several treatment options available for patients with end stage kidney disease. Management of patients following kidney transplantation is a complex, multi-faceted endeavor that requires the involvement of all members of the multidisciplinary team. Medications used to prevent transplant rejection come with a myriad of side effects and monitoring considerations. In addition to immunosuppression, these patients are also more susceptible to infections and require antimicrobial prophylaxis in the early post-transplant period. Understanding the common medications an

The Cardiovascular-Kidney-Metabolic (CKM) syndrome reframes cardiovascular, kidney, and metabolic disease as an integrated continuum, yet its management relies on reactive laboratory markers with substantial resource burdens. Retinal oculomics offers a non-invasive window into this continuum, grounded in structural and functional parallels between the retina and kidney. Their shared vulnerability to metabolic and hemodynamic stressors allows the retina to reflect subclinical CKM injury. Evidence is strongest for cardiovascular endpoints, where retinopathy and quantitative vessel metrics are as

Ferroptosis is a distinct form of regulated cell death driven by lipid peroxidation and redox imbalance. Since its formal recognition in 2012, ferroptosis has emerged as a central pathway linking metabolic stress and oxidative injury to both physiologic and pathologic processes. Its functions extend from tissue sculpting during embryogenesis and tumor suppression to pathologic contributions in neurodegeneration, cardiovascular disease, liver and kidney injury, cancer, and inflammatory disorders. Despite these advances in our understanding of ferroptosis, critical questions remain regarding its

OBJECTIVES: Pregnancy posttransplant remains high risk because of potential maternal, fetal, and graft-related complications. We retrospectively evaluated the clinical course, maternal-fetal outcomes, and graft-related events of pregnancies occurring after liver and kidney transplant at our center.
MATERIALS AND METHODS: We evaluated 3538 kidney transplants and 760 liver transplants performed at Başkent University between November 3, 1975, and November 3, 2024. Female recipients who became pregnant posttransplant were identified through hospital electronic medical records, the transplant data

We present the case of a 75-year-old patient who presented with recurrent arthralgia. Additional findings included chronic renal impairment of unknown cause and a history of partial small bowel resection. Arthrosonography raised suspicion of crystal arthropathy; however, no crystals were detected in the knee aspirate. Further investigation revealed previous findings of oxalate nephropathy, most likely as a result of increased reabsorption following abdominal surgery. Subsequently, Von Kossa staining of the aspirate confirmed oxalate arthropathy.

AKI is commonly seen among hospitalized patients, many of which the VIR is consulted to assess for a potential intervention. Rates are increased for critically ill patients and those with pre-existing CKD, advanced age, or significant comorbidities. Developing an AKI is associated with increased morbidity, mortality, and risk of progression to chronic kidney disease. Physiologically, renal function depends on the coordinated activity of nephrons, which regulate fluid balance, electrolyte homeostasis, and metabolic waste filtration. Disruption of renal perfusion, injury to the microscopic and m

This article examines the ethical and cultural logics shaping living donor kidney transplantation in Japan, where donation is restricted to kin and deceased donation remains rare. Drawing on ethnographic fieldwork in a Tokyo-area hospital, it analyzes how Japan's kin-based transplantation regime contrasts with the utilitarian model dominant in the United States, where donor pools are expanded through non-kin and anonymous exchange. I argue that Japan's restriction to family donors reflects an ethic of care rooted in family-based welfare and intergenerational reciprocity. At the same time, the

Pregnancy in women with kidney disease is high risk and associated with adverse maternal and fetal outcomes. Kidney disease in pregnancy continues to remain under-recognized and underdiagnosed, and disparities exist in access to kidney care for pregnancy patients. Health disparities with kidney disease and pregnancy are connected to differences in social, economic, cultural, and environmental factors; geographical location; and race/ethnicity. The review discusses health disparities in conditions related to kidney disease and pregnancy. We aim to advocate for interventions and policies to miti

OBJECTIVE: To assess heterologous booster (HB) with Janssen Ad26.COV2.S in kidney transplant recipients (KTRs) with low antibody levels after multiple COVID-19 mRNA vaccinations.
PATIENTS AND METHODS: This multicenter study examined anti-spike protein antibody levels in KTRs with two or more doses of mRNA vaccinations. Enrollment was from April 11, 2022, to September 19, 2023. Kidney transplant recipients with low levels (LL, 0.4-250 IU/mL) were offered HB with Janssen Ad26.COV2.S vaccine. Those with undetectable levels (UL, <0.4 IU/mL) were randomized to HB with or without immunosuppression

The Korean Society of Nephrology has embraced the principle of "Green Nephrology" and launched campaigns addressing environmental sustainability in kidney care. These include initiatives to monitor resource usage, reduce waste generation in treatment facilities, and minimize dialysis-related environmental effects. The Society aims to raise awareness about the environmental impact of kidney disease and its treatments while emphasizing the importance of sustainable practices. Collaboration between health authorities and related academic societies is planned for the development of national strate

INTRODUCTION: When one kidney is lost, the other works to compensate. The renally excreted antibiotic vancomycin (VCM) requires therapeutic drug monitoring to avoid renal injury and ensure efficacy. However, no studies have clarified whether the dosing regimen for patients with two kidneys can be used in patients with one kidney. We investigated whether patients with one kidney are able to receive VCM with the same dosing regimen as patients with two kidneys.
METHODS: This retrospective case series included patients with one kidney who were treated with VCM at Chiba University Hospital. Chang

Kidney diseases are among the most prevalent conditions worldwide, impacting over 850 million individuals. They are categorized into acute kidney injury and chronic kidney disease. Current preclinical and clinical trials have demonstrated that endothelin (ET) is linked to the onset and progression of kidney disease. In kidney diseases, pathological conditions such as hyperglycemia, acidosis, insulin resistance, and elevated angiotensin II levels lead to an increase in ET. This elevation activates endothelin receptor type A, resulting in harmful effects like proteinuria and a reduced glomerular

BACKGROUND: Kidney transplantation is the best option for overcoming kidney replacement therapy in the end stage kidney disease population. Yet the number of available kidney donors remains limited. The use of aggressive kidney offers could serve as an alternative to address the shortage of donors and reduce the rate organ non-use.
METHODS: We retrospectively analyzed ten kidneys that were aggressively offered to our transplant center before being included in the non-usable organ pool. They were transplanted in a short period of time. Donor and recipient demographics, intraoperative data, and

Obesity is arguably the premier global public health problem of our era. It is also an independent risk factor for the development and progression of chronic kidney disease (CKD) and adverse outcomes in persons with CKD. Fortunately, effective treatment options for obesity have proliferated in just the last few years. These newer therapies in conjunction with older treatments offer great potential in treating obesity and CKD, although many questions remain about their use, efficacy, and place within the overall therapeutic plan. This article will provide a discussion of the definition of obesi

Replacement of kidney function by dialysis maintains the lives of millions of patients. Clinical results however are still unsatisfactory with mortality rates and complications well above the matched control population. This has led to a continuous quest for more effective dialysis techniques and new biomaterials. In the 1980s, the production of synthetic membranes with higher water permeability ( i.e ., high flux) leveraged the development of techniques, such as hemodiafiltration (HDF), with improved solute removal, thanks to combination of diffusion and convection. Efficacy trials have demon

INTRODUCTION: An increasing number of elderly patients are undergoing either primary kidney transplantation (PrKT) or retransplantation (ReKT).
METHODS: Single-center retrospective cohort study of all deceased donor KTs (DDKTs) performed in elderly patients (age ≥65 years).
RESULTS: From December 2004 through August 2022, we performed 668 DDKTs in elderly patients including 39 ReKTs and 629 PrKTs. Mean donor age was lower in the ReKT group (44 ± 17 ReKT vs. 54 ± 13 years PrKT), as was KDPI (58 ± 24 vs. 74 ± 21% PrKT, both p < 0.05). A total of 44% of ReKT patients had a cPRA level above 50%

Background: Finerenone has emerged as a promising nonsteroidal mineralocorticoid receptor antagonist for patients with chronic kidney disease (CKD), yet its safety and efficacy in kidney transplant recipients remain unstudied. Methods: A total of 1750 kidney transplant recipients were screened, and 39 were prescribed finerenone alongside angiotensin-converting enzyme inhibitors or angiotensin II receptor blockers. Fifteen patients who met inclusion criteria and used finerenone consistently for at least six months were compared with 15 matched controls. Proteinuria, renal function, serum potass
