[An atypical case of subacute sclerosing panencephalitis (SSPE), (with special reference to psychiatric and psychological evaluations)].

An atypical case of subacute sclerosing panencephalitis is described, with particular reference to its psychiatric and psychological aspects in the light of the neurological and laboratory data: slow-gamma electrophoresis of the spinal fluid, periodic delta signs in the EEG over about 6 yr, 1/160 blood titre of anti-measles virus antibodies. The history displayed an ingravescent dementia course and the patient's continuing survival for more than 11 yr makes the case the longest recorded in the literature. Both the psychiatric-psychological and neurological symptoms had occurred in distinct sta

Dermatological Manifestations of Emergent Viruses. Part I: Common and Vaccine-Preventable Viral Infections.

Viral infections frequently present with mucocutaneous manifestations that may provide diagnostic clues and guide timely management. However, the expanding therapeutic landscape and evolving public health recommendations highlight the need for an updated evidence-based review for dermatologists. Herein, we summarize the epidemiology, clinical presentation, diagnosis, management, and prevention of common and vaccine-preventable viral infections with dermatologic relevance, including Coronavirus Disease-2019 (COVID-19), mpox, measles, and rubella. Advances in antiviral therapies, immunomodulator

Antivirals targeting paramyxovirus membrane fusion.

The Paramyxoviridae family includes enveloped single-stranded negative-sense RNA viruses such as measles, mumps, human parainfluenza, canine distemper, Hendra, and Nipah viruses, which cause a tremendous global health burden. The ability of paramyxoviral glycoproteins to merge viral and host membranes allows entry of the viral genome into host cells, as well as cell-cell fusion, an important contributor to disease progression. Recent molecular and structural advances in our understanding of the paramyxovirus membrane fusion machinery gave rise to various therapeutic approaches aiming at inhibi

Inosine Pranobex Deserves Attention as a Potential Immunomodulator to Achieve Early Alteration of the COVID-19 Disease Course.

Since its licensing in 1971, the synthetic compound inosine pranobex has been effectively combating viral infections, including herpes zoster, varicella, measles, and infections caused by the herpes simplex virus, human papillomavirus, Epstein-Barr virus, cytomegalovirus, and respiratory viruses. With the emergence of SARS-CoV-2, new and existing drugs have been intensively evaluated for their potential as COVID-19 medication. Due to its potent immunomodulatory properties, inosine pranobex, an orally administered drug with pleiotropic effects, can, during early treatment, alter the course of t

Immune Thrombocytopenia in a Very Elderly Patient With Covid-19.

Immune thrombocytopenia (ITP) is an autoimmune disorder characterized by a decreased number of platelets and mucocutaneous bleeding. Many viruses have been identified as triggers of the autoimmune process, including human immunodeficiency virus (HIV), hepatitis C virus (HCV), Epstein-Barr virus (EBV), cytomegalovirus (CMV), parvovirus, rubella, and measles. Association with the new severe acute respiratory syndrome coronavirus, SARS-CoV-2 infection (Covid-19 infection) has been rarely reported. Here, we report the oldest case of ITP patient triggered by the novel coronavirus infection. He show

Severe Immune Thrombocytopenia Following MMR Vaccination with Rapid Recovery: A Case Report and Review of Literature.

Immune thrombocytopenia (ITP) is an autoimmune disease that occurs following viral illnesses and may also infrequently occur after measles, mumps, and rubella (MMR) vaccination. ITP typically presents with the sudden appearance of a petechial rash, bruising, and/or bleeding in an otherwise healthy-appearing child. However, ITP following MMR vaccine does not commonly cause severely depleted platelets. We report a case of ITP after MMR vaccination in a 13-month-old baby boy, who presented with petechial rash all over his body. The child had severe thrombocytopenia but was successfully treated wi

Epistaxis and gross haematuria with severe thrombocytopaenia associated with influenza vaccination.

Epistaxis may be attributed to many causes during the winter including dry mucous membranes from low indoor humidity from heating. However, epistaxis may also be due to thrombocytopaenia. Immune thrombocytopaenia purpura (ITP) is an autoimmune disorder causing thrombocytopaenia. Viral infections sometimes lead to ITP. Vaccines, predominantly the measles-mumps-rubella vaccine, have been associated with the development of ITP. There are several published case reports regarding influenza vaccine induced ITP. However, an association between ITP and influenza vaccination has not been firmly proven

Non-specific effects of vaccines: Current evidence and potential implications.

Besides protection against specific microorganisms, vaccines can induce heterologous or non-specific effects (NSE). Epidemiological data suggest that vaccination with live-attenuated vaccines such as Bacillus Calmette-Guérin (BCG), measles vaccine, and oral polio vaccine results in increased overall childhood survival, and several of these observations have been confirmed in randomized trials. Immunological mechanisms mediating NSE include heterologous lymphocyte effects and induction of innate immune memory (trained immunity). Trained immunity induces long-term functional upregulation of inna

G1m1 predominance of intrathecal virus-specific antibodies in multiple sclerosis.

We have previously shown that plasmablasts of the G1m1 allotype of IgG1 are selectively enriched in the cerebrospinal fluid of G1m1/G1m3 heterozygous patients with multiple sclerosis, whereas both allotypes are equally used in neuroborreliosis. Here, we demonstrate a strong preference for the G1m1 allotype in the intrathecal humoral immune responses against measles, rubella, and varicella zoster virus in G1m1/G1m3 heterozygous multiple sclerosis patients. Conversely, intrathecally synthesized varicella zoster virus-specific IgG1 in varicella zoster virus meningoencephalitis comprised both allo

Emerging infections and old friends: remaining prepared in South Dakota.

Recent reports of serious infection outbreaks internationally remind us of the importance of accurate information and continual vigilance. The Ebola outbreak in West Africa has captured headlines as the most severe outbreak in the history of this disease. West Nile disease, measles, pertussis and tuberculosis infect South Dakota patients on a yearly basis. A significant rise in syphilis cases has prompted recommendations for increased prenatal screening. The more unusual viral diseases, Ebola, Middle East respiratory syndrome (MERS) and Chikungunha virus, receive media attention but present mi

Re-emergence of another vaccine-preventable disease?-Two cases of rubella in older adults.

Unlike its devastating teratogenic effects, post-natal infection with rubella typically causes subclinical or inapparent illness. While rubella has been largely eliminated from the United States following the introduction of an efficacious live-attenuated vaccine in 1969, a small proportion of the population remains susceptible. Recent declining vaccination rates have resulted in a rising incidence of sporadic and outbreak-associated measles, reminding us that an increasing proportion of the population is also susceptible to, and may be reservoirs of transmission for, rubella. We describe two

Solid organ transplantation and response to vaccination.

Although early vaccination is recommended in candidates for solid organ transplantation (SOT), consensual protocols do not yet exist. We applied an SOT vaccination protocol in the Hospital Clinic of Barcelona (Spain). Serology was performed before and after vaccination and compliance with the vaccination schedule was analysed during the period 2003-2004. Two hundred and thirty seven patients (72.9% male; mean age 56.31 years, range 19-72) were included. A total of 74.5% of subjects susceptible to hepatitis B virus infection responded to hepatitis B vaccination. Most patients were protected aga

Detection of antibodies in saliva--an effective auxiliary method in surveillance of infectious diseases.

Saliva is a body fluid containing antibodies of diagnostic significance. Unlike venipuncture, saliva collection (by brushing the teeth and rubbing the gums) is painless, non-invasive, inexpensive, simple and rapid. By using sensitive immunoassays in salivary specimens it is possible to diagnose immunoglobulins against a wide range of infectious diseases e.g. hepatitis A, B and C, measles, mumps, rubella, human immunodeficiency virus, Epstein-Barr virus, parvovirus B 19, human herpesvirus 6 and Helicobacter pylori infections. Salivary antibody testing may provide better access to epidemic outbr

Xerophthalmia among hospitalized Iraqi children.

To determine the impact of sanctions on the nutritional status of Iraqi children aged < 6 years, a random sample of 700 patients (age range: 0-6 years) from the Saddam Paediatric Hospital, Diyala Province, Iraq were examined ophthalmologically for evidence of xerophthalmia. Data on the history of infection, feeding and night blindness were also collected. The prevalence of xerophthalmia was 29%, mostly among children aged 1-3 years. Xerophthalmia was significantly inversely associated with breastfeeding and highly associated with common childhood infections such as measles, diarrhoea and respi

Mumps outbreak among young people in Northern Ireland.

An outbreak of mumps occurred in the Mid-Ulster area of Northern Ireland between 1st November 1999 and 31st August 2000, with 729 cases notified. Salivary test results were obtained for 430 (59%) reported cases, of which 332 (77%) were positive for mumps IgM antibody. Three hundred and sixteen (95.2%) of these confirmed cases were in the age range 9 to 19. One hundred and eighty-four (55.4%) confirmed cases had received one dose of MMR (measles, mumps and rubella) vaccine, but only 3 (0.9%) confirmed cases had received 2 doses of MMR. The low incidence of mumps among children who had received

Herpesvirus-6 encephalitis complicated by Wernicke-Korsakoff syndrome in a pediatric recipient of unrelated cord blood transplantation.

A 10-year-old girl with M2 acute myeloid leukemia underwent an unrelated cord blood transplantation in refractory first relapse. On day +13, after 48 hours with fever, she showed a measles-like rash, and on day +15, she began experiencing neurologic symptoms (headache, tremors, weakness, nystagmus, mild confusion, speaking, taste, and behavior disturbances, and focal seizures). She also had amnesia for recent events with disability to learn, mimicking Wernicke-Korsakoff syndrome. Computed tomography of the brain and cerebrospinal fluid (CSF) and electroencephalogram were nonspecific. We found

Immunization coverage among urban and rural children in the Shimla hills.

A cross sectional study was carried out to assess immunisation coverage among 257 urban and 339 rural children aged 13-36 months who were vaccinated at pulse polio booths in Shimla hills. 84.37% urban children and 57.59% rural children were fully vaccinated. BCG scar was negative in a large number of children, especially from rural areas. Increasing maternal education was associated with complete immunisation. OPV I-III dropouts were 3% among urban children and 15% among rural. The corresponding dropout rates for DPT I-III were 1% and 8% respectively. The immunisation coverage in this region w

Calcium spirulan, an inhibitor of enveloped virus replication, from a blue-green alga Spirulina platensis.

Bioactivity-directed fractionation of a hot H2O extract from a blue-green alga Spirulina platensis led to the isolation of a novel sulfated polysaccharide named calcium spirulan (Ca-SP) as an antiviral principle. This polysaccharide was composed of rhamnose, ribose, mannose, fructose, galactose, xylose, glucose, glucuronic acid, galacturonic acid, sulfate, and calcium. Ca-SP was found to inhibit the replication of several enveloped viruses, including Herpes simplex virus type 1, human cytomegalovirus, measles virus, mumps virus, influenza A virus, and HIV-1. It was revealed that Ca-SP selectiv

Live viral vectors : construction of a replication-deficient recombinant adenovirus.

Since the use of molecular biology and genetic engineering techniques has become widespread, a new generation of candidate vaccines has been developed, including live viral vectors (1, 2). The basis of using recombinant viruses as potential vaccines involves the incorporation of specific genes from a pathogenic organism into the genome of a nonpathogenic or attenuated virus. The recombinant virus can then infect specific eukaryotic cells either in vivo or in vitro, and cause them to express the recombinant protein. In our laboratory, successful results have been obtained using replication-defi

Sequence analyses of human parainfluenza virus type 4A and type 4B fusion proteins.

cDNAs encoding human parainfluenza virus type 4A and type 4B (hPIV-4A and -4B) fusion (F) proteins were cloned and sequenced. The predicted amino acid sequences of the F proteins had similar characteristic traits to those reported for the F proteins of other paramyxoviruses. They were more closely related to the F proteins of simian virus 5 (SV5), mumps virus (MuV), hPIV-2 and Newcastle disease virus (NDV) than to the F proteins of hPIV-1, hPIV-3, Sendai virus (SV) and measles virus (MV). In addition, hPIV-4A, hPIV-4B, SV5 and MuV shared a common feature of genomic organization: there was a sm
