Group 2 pulmonary hypertension: from diagnosis to treatment.

PURPOSE OF REVIEW: Pulmonary hypertension associated with left heart disease (PHLHD) is common and associated with adverse prognosis. Proper diagnosis is critical to avoid inappropriate treatment. Practical approaches to noninvasive diagnosis are available, though invasive hemodynamics including volume loading or exercise are often necessary for definitive diagnosis. Treatment strategies and research in the field is rapidly evolving. RECENT FINDINGS: Combined pre and post capillary pulmonary hypertension is associated with remodeling of the pulmonary vasculature that is proportional to the se

Task specific dystonia - a patients' perspective.

BACKGROUND: Task specific dystonia (TSD) is a subtype of dystonia with no cure and significant limitations on treatments. Few studies have investigated the outcomes of rehabilitative therapy from the patient's perspective. PURPOSE OF THE STUDY: This study explored the interventions that patients have utilized and their perceived effectiveness in treating and managing their TSD, specifically musicians' dystonia (MD) and writer's dystonia (WD). Symptoms and the effect of TSD on the perceived performance of every day and specific tasks, and possible reasons why the condition developed, were also

Evaluation of visual evoked potentials in chronic renal failure patients with different treatment modalities.

BACKGROUND: Uremia causes central nervous system (CNS) dysfunction. Modern diagnostic methods are useful for the recognition of CNS complications in uremic patients. Data concerning visual evoked potentials (VEPs) of chronic renal failure (CRF) patients is limited. OBJECTIVE: In this study, we aimed to assess and compare VEPs abnormalities in CRF patients managed by only medical therapy (pre-dialysis) or medical therapy plus renal replacement therapy (continuous ambulatory peritoneal dialysis (CAPD) or hemodialysis (HD)). MATERIALS AND METHODS: Fifty-nine patients diagnosed with CRF of vario

Otitis media and the new guidelines.

Otitis media, including recurrent acute otitis media (AOM) and otitis media with effusion (OME), is one of the most common reasons for an illness-related visit to the primary care clinician. Until recently, antibacterial therapy was the standard treatment for most episodes of AOM and often for OME. However, in 1994, a clinical practice guideline on OME was developed by the Agency for Healthcare Policy and Research (now the Agency for Healthcare Research and Quality) and applied to normal children aged 1 to 3 years. The recommendations in the 1994 guideline were evidence based, widely discussed

[Positive interferon-gamma release assay and immunosuppression: guideline-based diagnosis and treatment of tuberculosis infection].

With the growing use of immunosuppressive therapies, clinicians are increasingly faced with the question of how to manage positive interferon-gamma release assays (IGRA). A tuberculosis infection (TBI, formerly referred to as "latent tuberculosis") is clinically silent and non-contagious but can reactivate as active tuberculosis under immunosuppressive conditions. Diagnosis involves either an IGRA or a tuberculin skin test (TST), alongside microbiological and imaging studies to exclude active disease. Screening is recommended for individuals at increased risk, including those living with human

Refractory Ulcerative Colitis Successfully Treated With a Combination of Granulocyte and Monocyte Apheresis and Mirikizumab: A Case Report.

We report the case of a 42-year-old man with treatment-refractory ulcerative colitis (UC), complicated by steroid dependence, adverse effects of immunomodulators, and significant social limitations, including inability to be hospitalized or take oral medications. Granulocyte and monocyte adsorptive apheresis (GMA) was initiated as an induction therapy, followed by the addition of mirikizumab, an anti-IL-23p19 monoclonal antibody. This sequential combination therapy resulted in significant clinical improvement and endoscopic mucosal healing. A detailed breakdown of the clinical course, objectiv

Advancements in Physical Factor Therapy and Traditional Chinese Physiotherapy for Rehabilitation of Tennis Elbow.

Tennis elbow, a typical occupational and sports injury, severely affects a patient's daily life. Despite the availability of various therapeutic approaches, patient satisfaction and treatment outcomes for conditions such as tennis elbow remain suboptimal. Recent advancements have highlighted the efficacy of physical factor therapies, a new class of non-surgical treatments, in clinical practice. In the meantime, innovative approaches to the management of tennis elbow have been made possible by the fusion of contemporary medical technology with the traditional external Chinese medicine. This stu

Review of venous thromboembolism (VTE) risk, evaluation, and treatment in individuals on estrogen-based gender-affirming hormone therapy.

Epidemiological studies of transgender women suggest that these individuals experience higher rates of venous thromboembolism than their cisgender peers. Several factors likely increase this risk, including medication use, comorbidities, and social determinants of health. Estrogen-based gender-affirming hormone therapy is critical for helping patients address unwanted secondary sexual characteristics and induce physical changes in accordance with their gender identity. However, the use of estrogen-based gender-affirming hormone therapy may increase the risk of venous thromboembolism. This revi

European S2k Guideline on Chronic Pruritus.

Pruritus is a frequent symptom in medicine. Population-based studies show that one in five persons in the general population has suffered from chronic pruritus (CP) at least once in their lifetime, with a 12-month incidence of 7%. CP, which can affect all age groups, is associated with numerous, often interdisciplinary medical conditions. It needs a precise diagnostic work-up to identify causes and relevant comorbidities. Management of CP comprises treatment of the underlying disease as well as topical and systemic therapies. Treating CP needs to be targeted, multimodal and performed in a step

Nonsteroidal Approaches for Atopic Dermatitis®: A Clinical Update.

Topical corticosteroids (TCSs) are the most widely used treatment for atopic Dermatitis® (AD), but they can have adverse effects such as skin atrophy, telangiectasias, and hypopigmentation, especially with prolonged use of higher potency steroids. Many patients also have a fear of using TCSs, known as "corticophobia." With the development of biologics and Janus kinase inhibitors, a nonsteroidal approach to the treatment of AD may be possible and may be preferred by certain patients. Given what is known about these nonsteroidal therapies, we propose a structured treatment ladder and action plan

Calciphylaxis and Intractable Pain in a Veteran with Psychological Trauma History.

Guidelines are lacking for patients with calciphylaxis on renal replacement therapy, often leading to difficulty optimally treating these patients. A 60-year-old male veteran receiving hemodialysis presented with calciphylaxis of the left lower extremity and intractable pain. His condition was complicated by chronic back pain, long-term opioid therapy, and psychological trauma history. He was ultimately transferred to a calciphylaxis treatment center but was unable to tolerate further treatments due to sepsis and hemodynamic instability. He was transitioned to comfort measures and died in the

Unraveling the complexity: Case reports of drug-induced hemolytic anemia due to ceftriaxone.

Drug-induced hemolytic anemia (DIHA) is a rare but significant condition characterized by the premature destruction of red blood cells (RBCs) triggered by certain medications. Ceftriaxone, a commonly used antibiotic, has been linked to DIHA, presenting diagnostic challenges due to its diverse clinical manifestations. This study examines three cases of DIHA caused by ceftriaxone therapy at our center. The patients presented with symptoms such as fatigue, jaundice, and dark urine following ceftriaxone therapy. Laboratory tests indicated hemolytic anemia with decreased hemoglobin, elevated lactat

Radiation enteritis: Diagnostic and therapeutic issues.

Abdominal pelvic radiation therapy can induce acute or chronic lesions in the small bowel wall, called radiation enteritis. Treatment of acute radiation enteritis is essentially symptomatic; symptoms regress when radiation is discontinued. Conversely, late toxicity can occur up to 30 years after discontinuation of radiation therapy, posing diagnostic problems. Approximately one out of five patients treated by radiation therapy will present clinical signs of radiation enteritis, including obstruction, malabsorption, malnutrition and/or other complications. Management should be multidisciplinary

[Management of plaque psoriasis in adults].

In Germany, approximately 2% of the population suffers from psoriasis, which is no longer considered only a cutaneous, but rather a systemic disease. Accordingly, common comorbidities and potential joint involvement in psoriasis must be recorded. If necessary, interdisciplinary patient care has to be organized. The use of validated scores is recommended to complete the patient's medical history. The individual treatment should include intensified topical therapies as well as short-term phototherapy in case of an acute phase. In addition to conventional systemic therapies (e.g., fumarates, meth

Behavioral therapy for chronic migraine.

Chronic migraine is a disabling condition which affects a considerable proportion of patients. Several risk factors and lifestyle habits contribute to the transformation of migraine into a chronic form. Behavioral treatments, including relaxation, biofeedback, and cognitive behavioral therapy reduce the risk of episodic into chronic migraine transformation, thus restraining the headache-related disability. The rationale of behavioral therapies is that a medical problem should be recognized and thoroughly examined by the patient to be successfully managed. Being aware of factors which precipita

HCV/HIV-coinfection--is there a state of the art after APRICOT and RIBAVIC?

Today hepatitis C is one of the unsolved medical problems and results in a significant number of deaths in the HIV population, particularly in southern Europe and the USA, where the prevalence of HCV/HIV-coinfection in the HIV population ranges from 30-50%. Recent trials using pegylated interferon alfa in combination with low dose ribavirin have achieved overall sustained response rates of up to 42%. However, high discontinuation rates, adverse events associated with mitochondrial toxicities or treatment of patients with advanced cirrhosis may decrease response to treatment and result in serio

Recurrent aortic dissection in Marfan's syndrome: possible effects of anticoagulation.

Recent reports support the role of a valve-sparing procedure in ascending aortic dissection in patients with Marfans syndrome. A 49-year-old woman with Marfans syndrome and prior aortic aneurysm repaired with a composite graft presented with sudden-onset chest pain. Following an initial negative computed tomographic (CT) scan, a long dissection involving the descending thoracic and abdominal aorta was discovered on a repeat CT scan a few hours later. Symptoms improved gradually with optimal medical management and the patient was discharged home on anticoagulant therapy. Although no direct caus

Dexamethazone-induced acute tumor lysis syndrome in a T-cell malignant lymphoma.

We report a case of steroid-induced acute tumor lysis syndrome and review the literature. A 60-year-old woman was started on steroid therapy for dyspnea due to bilateral pleural effusion and a large mass involving the anterior mediastinum. The final diagnosis was precursor T-lymphoblastic lymphoma-leukemia. Following steroid therapy, the patient developed acute renal failure and laboratory evidence of metabolic changes induced by massive cytolysis. She received vigorous hydration, diuretic and allopurinol therapy, and haemodialysis. Her diuresis, renal function and laboratory data returned to
