Sato needling for pupillary membranes.

Dense pupillary membranes are resistant to needle-knife incision. Trauma to attached intraocular structures may be produced during discission techniques that stretch the membrane. The Sato discission technique produces membrane stabilization on a fixation needle. This allows controlled incision of the pupillary membrane at a right angle to the direction of the fibers without tension on surrounding structures. The Sato technique is not difficult, and surgical complications are minimal. Using this technique, adequate pupillary openings were produced in 62 of 65 eyes, with an average follow-up of

Thymectomy in the neonatal rat.

A method for removal of the thymus in the newborn rat employing measures aimed at reducing the mortality incurred by the surgical procedure, cannibalism and infection is described in detail. Tranquilisation of the dam, sterile technique, magnification of the operative site for thymus aspiration, warming to 37 degrees C during the recovery phase, and application of pheromones to the offspring prior to their reunion with the dam, all proved important in increasing the survival rate.

Outcome After Treatment of Unruptured Intracranial Aneurysms in a Population-Based Cohort: A Retrospective Register Study.

BACKGROUND: Population-studies indicate that the incidence of aneurysmal subarachnoid hemorrhage is declining, and that the prevalence of unruptured intracranial aneurysms (uIAs) is higher than previously assumed. This suggests the risk of aneurysmal subarachnoid hemorrhage from an uIA is lower than previously estimated. We therefore aimed to report institution- and population-specific outcomes after treatment of uIAs, and to describe time trends in treatment rates and modalities. METHODS: This is a registry-based retrospective population-based observational cohort study from the University H

Association between treatment modality and delayed cerebral ischemia in 2748 patients with aneurysmal subarachnoid hemorrhage: A 12-year single center experience.

BACKGROUND: Delayed cerebral ischemia (DCI) is a severe postoperative complication of aneurysmal subarachnoid hemorrhage (aSAH). Appropriate selection of the optimal surgical modality plays a critical role. It was widely known that two landmark clinical trials, International Subarachnoid Aneurysm Trial (ISAT) and Barrow Ruptured Aneurysm Trial (BRAT), have compared the efficacy of two major treatments for patient with aSAH. However, we still need a large-scale observational study to prove the association between treatment modality and DCI risk. Thus, we intended to validate the impact of treat

Surgical management of quadrigeminal arachnoid cysts in children: A single-center study of 15 cases according to the Cinalli classification.

Quadrigeminal arachnoid cysts (QACs) are rare intracranial lesions frequently presenting with obstructive hydrocephalus in children. This study aimed to evaluate surgical strategies and clinical outcomes in pediatric patients with QACs classified according to the Cinalli classification. We retrospectively reviewed radiological and clinical data of 15 pediatric patients undergoing surgery for QACs (endoscopic third ventriculostomy (ETV), endoscopic cyst fenestration (CF), cystoperitoneal shunting, or microsurgical fenestration via craniotomy) at a single institution. Patients were categorized a

Efficacy study of lymphaticovenular anastomosis via the lymphosome-based four-incision approach for lower limb lymphedema.

OBJECTIVE: Lower limb lymphedema (LLL) is a chronic condition with impaired lymphatic drainage. Lymphaticovenular anastomosis (LVA) is a promising microsurgical treatment for LLL. Refined surgical techniques, such as optimal incision placement and precise lymphatic vessel identification, are essential for better clinical outcomes. For patients with LLL, we performed an LVA via the lymphosome-based four-incision approach. We standardized incision positioning and identified lymphatic vessels for LVA to improve surgical outcomes. METHODS: A retrospective study was conducted on 59 patients with L

Postoperative Cognitive Alterations and Functional Brain Reorganization in Children With Middle Cranial Fossa Arachnoid Cysts: A Pilot fMRI Study.

BACKGROUND AND OBJECTIVES: Middle fossa arachnoid cysts (MFACs) are congenital benign lesions for which the indication for surgical intervention remains controversial. Although numerous studies have demonstrated that surgery can improve cognitive function in affected children, investigations into the neural mechanisms underlying these improvements are scarce. Our aim was to quantify neurocognitive outcomes and functional brain network alterations before and after microsurgical fenestration of MFAC in children. METHODS: We acquired resting-state functional MRI data from 18 pediatric MFAC patie

Hybrid Total Achilles Tendon Reconstruction Combining Acellular Dermal Matrix Placement and Free Latissimus Dorsi Flap Coverage: A Report of Two Cases.

Reconstruction of complex Achilles tendon defects involving both the tendon and the overlying soft tissues represents a challenging scenario for plastic surgeons. In this report, we present an innovative technique of hybrid total Achilles tendon reconstruction combining acellular dermal matrix placement to restore the full-thickness continuity of the tendon and free latissimus dorsi (LD) flap coverage to allow graft integration and simultaneously resurface the soft tissue defect, ultimately leading to lower limb salvage. Between August 2023 and February 2024 two patients received microsurgical

Predictors of postoperative gustatory disturbance and gustatory recovery following microsurgical resection of vestibular schwannoma.

BACKGROUND: Gustatory dysfunction is a complication of vestibular schwannoma (VS) resection and a frequent patient concern, but there is comparatively little in the literature on this topic to guide surgeons in counseling. In this study, we aimed to identify factors contributing to postoperative gustatory dysfunction and subsequent recovery. METHODS: We included consecutive VS patients who underwent microsurgical resection between July 2016 and April 2024 at our institution. Preoperative data included patient demographic information, tumor characteristics, facial nerve function (classified by

Custom Endoprosthesis-Assisted Pediatric Microsurgical Jaw Reconstruction.

Surgical treatment of pediatric maxillary and mandibular tumors can cause significant postresection disfigurement, mastication, and speech dysfunction. The need to restore form and function without compromising growth at the recipient and donor sites poses a particular reconstructive dilemma. This study evaluates outcomes of the custom endoprosthesis (CE) compared with noncustom reconstruction (NCR) and introduces an algorithm using CE to optimize available soft tissue reconstructive options. An Institutional Review Board-approved retrospective review of all patients undergoing maxillary or ma

Maximizing Aesthetic Outcomes in Delayed Breast Reconstruction: the Be.A.U.T-I.F.U.L. DIEP® Step-by-Step Inset Technique.

INTRODUCTION: In microvascular breast reconstruction, the focus has shifted to achieving aesthetically pleasing results. Delayed breast reconstruction poses challenges such as ensuring natural ptosis and avoiding a "patch" effect. The Be.A.U.T-I.F.U.L. deep inferior epigastric perforator (DIEP) flap inset, presented here, offers a systematic and sequential seven-step method to optimize breast reconstruction outcomes. METHODS: This approach emphasizes safety during flap harvest using the best (Be.) perforator identified via computed tomography angiography to minimize dissection and ensure soli

Microsurgical anatomy and approaches to thalamic gliomas. Part 2: Maximal safe resection of thalamic gliomas improves outcomes. A single-center experience.

OBJECTIVE: As presented in Part 1 of this series, thalamic gliomas (TGs) are deep-seated, difficult-to-access tumors surrounded by vital neurovascular structures. Given their high operative morbidity, TGs have historically been considered inoperable lesions. Although maximal safe resection (MSR) has become the treatment standard for lobar and even deep-seated mediobasal temporal and insular gliomas, the eloquent location of TGs has precluded this management strategy, with biopsy and adjuvant treatment being the mainstay. The authors hypothesized that MSR can be achieved with low morbidity and

The Use of Pedicular Arteriovenous Fistula (PAVF) in Microvascular Reconstruction to Enhance Flow Across the Microvascular Anastomoses.

BACKGROUND: Free flap success rates have improved dramatically over the past three decades, setting a high standard for microvascular reconstruction. However, rates of arterial or venous thrombosis, and subsequent failure, remain high in burn and trauma reconstruction when compared to autologous breast reconstruction and other elective flaps. To address the higher failure rate, we use a novel vascular approach. We create a pedicular arteriovenous fistula (PAVF), allowing the flow to be enhanced across the microvascular anastomoses. METHODS: We performed a retrospective review of 13 consecutiv
