Revisiting motor unit recruitment to TMS in amyotrophic lateral sclerosis: cortical inhibition is retained during voluntary contractions.

Transcranial magnetic stimulation (TMS) has been used for many years to study the pathophysiology of amyotrophic lateral sclerosis (ALS). Based on single- or dual-pulse TMS and EMG and/or single motor unit (MU) recordings, many groups have described a loss of central inhibition as an early marker of ALS dysfunction, reflecting a state of cortical 'hyperexcitability'. This conclusion is not without its detractors, however, leading us to reexamine this issue using 4-pulse TMS, shown previously to be more effective for testing central motor pathway functional integrity. A total of 221 motor units

Intraoperative Assessment of Right Cingulum Function Using the Williams Inhibition Test During Awake Surgery for Right Frontal Gliomas.

OBJECTIVE: The cingulum is crucial for cognitive control and functionally relevant in various neurological diseases, including brain tumors. This study evaluated the usefulness of the Williams Inhibition Test (WIT), a Stroop-like task reflecting nonverbal cognitive control, in assessing cingulum function during awake surgery for right frontal gliomas. METHODS: In this retrospective study, data from 46 patients with World Health Organization grade 2 or 3 right frontal gliomas who underwent awake surgery were analyzed. WIT was administered intraoperatively to monitor cingulum function. Patients

The Influence of Vasopressor-Induced Arterial Blood Pressure Elevation on Muscle-Recorded Motor Evoked Potentials.

BACKGROUND: Transcranial electrical stimulation muscle-recorded motor evoked potentials (Tc-mMEPs) are used to monitor the spinal cord motor tracts during spinal surgery. There is considerable intra- and interindividual variability in the signals recorded, causing a high incidence of false positive warnings. Inadequate blood pressure is commonly blamed for false positive warnings and is usually managed with fluid and vasopressor therapy. The aim of the study was to systematically investigate the effects of norepinephrine and ephedrine-induced arterial blood pressure elevation on Tc-mMEPs. MET

Associations between anaesthetic dose-adjusted intraoperative EEG alpha power, processing speed, and postoperative delirium: analysis of data from three prospective studies.

BACKGROUND: We previously have shown that low intraoperative EEG alpha power is associated with impaired preoperative cognition, a delirium risk factor, and that intraoperative anaesthetic-dose-adjusted EEG bispectral index values were associated with a four-fold increased risk of postoperative delirium (POD). Yet, associations between intraoperative anaesthetic-dose-adjusted alpha power and delirium or delirium risk factors have yet to be quantified. METHODS: We examined cerebrospinal fluid (CSF) Alzheimer's disease (AD)-related biomarkers, cognitive scores, EEG recordings, and delirium data

Intraoperative Electrocorticographic Monitoring for Predicting Post-Traumatic Seizures and Prognosticating Patients With Head Injury: A Prospective Observational Study.

BACKGROUND AND OBJECTIVES: Post-traumatic epilepsy is a major complication of traumatic brain injury (TBI), with limited predictive tools and preventive strategies. Although intraoperative electrocorticography (ECoG) is widely used in epilepsy and tumor surgeries, its role in brain injury remains underexplored. This study evaluates the predictive value of intraoperative ECoG for seizure development and functional outcomes in patients with TBI. METHODS: This prospective observational study included 110 patients with head injury who underwent surgery and intraoperative ECoG evaluation. The ECoG

Shoulder kinematic and muscle recruitment pattern changes in upper trapezius myofascial trigger points: a controlled observational study.

BACKGROUND: Myofascial trigger points (MTrPs) in the upper trapezius are prevalent in individuals with shoulder pain and may interfere with neuromuscular control and kinematics during shoulder movements. The purpose of this study was to compare shoulder muscle activation patterns and kinematic parameters, particularly scapulohumeral rhythm (SHR), between individuals with upper trapezius MTrPs and the healthy group during shoulder abduction. METHODS: An observational study was conducted with participants assigned to either a trigger point group (n = 15) or a healthy group (n = 13). Surface ele

Intraoperative monitoring and early recognition of facial nerve root in vestibular schwannoma surgery.

The removal of vestibular schwannomas carries a risk of facial palsy. This study aims to evaluate the usefulness and technical aspects of intraoperative monitoring (IOM) for the facial nerve. A total of 96 patients who underwent surgery for vestibular schwannoma were retrospectively investigated. The cohort was divided into two groups: those with intraoperative facial nerve monitoring (IOM group) and those without IOM (non-IOM group). Preoperative and postoperative facial nerve functions were assessed using the House-Brackmann (HB) scale immediately after surgery, at discharge, and at the 1-ye

Transcranial electric motor evoked potential monitoring during scoliosis surgery in children with cerebral palsy and active seizure disorder: is it feasible and safe?

PURPOSE: Use of spinal cord monitoring in children with cerebral palsy (CP) and neuromuscular scoliosis is challenging. The previous reports suggest low success rates in the setting of CP, and it is unclear if transcranial electric motor evoked potentials (TcMEP) monitoring is contraindicated in patients with an active seizure disorder. The purpose of this study was to determine (1) are patients with CP able to be appropriately monitored with TcMEP? and (2) does TcMEP cause an increase in seizure activity? METHODS: This was an institutional review board-approved retrospective cohort study obs

Flat electrode contacts for vagus nerve stimulation.

The majority of available systems for vagus nerve stimulation use helical stimulation electrodes, which cover the majority of the circumference of the nerve and produce largely uniform current density within the nerve. Flat stimulation electrodes that contact only one side of the nerve may provide advantages, including ease of fabrication. However, it is possible that the flat configuration will yield inefficient fiber recruitment due to a less uniform current distribution within the nerve. Here we tested the hypothesis that flat electrodes will require higher current amplitude to activate all

Compound motor action potential guided 240 seconds plus bonus freeze for safe and durable left atrial appendage isolation in patients with recurrent persistent atrial fibrillation: How to isolate the appendage with cryoballoon (the CMAP guided ICE-B protocol).

INTRODUCTION: Pulmonary vein isolation (PVI) is the cornerstone ablation strategy for treatment of atrial fibrillation (AF). Ablation outside the PVs is sometimes needed particularly in persistent or longstanding AF. This study reported left atrial appendage isolation (LAAI) using cryoballoon (CB) focusing on technical data and short-term clinical follow-up. METHODS AND RESULTS: Patients with recurrent persistent AF after multiple AF procedures were enrolled. LAAI was performed using the second generation CB guided by compound motor action potential (CMAP) and 240 seconds plus bonus freeze (I

Somatosensory Evoked Potentials as a Stand-Alone Tool During Spine Surgery: An Egyptian Preliminary Report.

PURPOSE: Monitoring of somatosensory evoked potentials (SSEPs) serves as an early warning system to detect spinal cord injury and is correlated with postoperative sensory findings. It is an indirect indicator of motor function. This study aimed to evaluate the usefulness of intraoperative SSEPs monitoring as a stand-alone tool during spinal surgeries when motor evoked potentials are not available, to prevent and predict new postoperative neurologic deficits. Motor evoked potentials were not used as the equipment needed to record them was not available at the time of this study. METHODS: This

A-train clusters and the intermedius nerve in vestibular schwannoma patients.

OBJECTIVE: EMG "A-train" activity correlates with postoperative facial palsy after vestibular schwannoma (VS) surgery. An intermedius nerve separate from the facial nerve increases A-trains without significant impact on function. We investigate occurrence of A-train "clusters", A-trains over a majority of channels within a short time frame. METHODS: Data from 217 patients with first surgery for VS were evaluated retrospectively. Continuous EMG recorded with 9 channels was evaluated for A-train patterns. "Clusters" of A-trains were identified, i.e. A-trains within 3 seconds over a majority of
