Mastoiditis Now Affects Adults: A Case Report of an Occurrence of the Disease in an 82-Year-Old Male.

Mastoiditis is typically considered a suppurative complication of otitis media seen in children. Vaccines and therapeutics can change the demographics of diseases. Childhood vaccination against Haemophilus influenza, for example, has shifted the mean age for ear, nose, and throat infections caused by this bug to age 25, whereas this used to be most prevalent in the pediatric age group previously. The authors present the case of an 82-year-old man who had mastoiditis. This case serves as a reminder to avoid anchoring bias when seeing an undifferentiated patient in the emergency department.

A rare presentation of a mucocele in a separate compartment of maxillary sinus.

Maxillary sinus is frequently presented as a single space within each maxilla. It drains directly into the nose through the middle meatus. We are reporting the second rare case of a mucocele in a separate compartment of maxillary sinus in a 26-year-old patient without a history of facial trauma. The non-contrast computed tomography of paranasal sinuses showed an expansile right infraorbital cystic lesion developed in an upper lateral position of the maxillary sinus. An excision biopsy was performed, which showed histopathological findings of a maxillary sinus mucocele.

Unusual Cause of Persistent Epistaxis with Severe Anemia in a Child.

Epistaxis in children can be caused by different systemic and local pathologies. Respiratory infections, nasal mucosa dryness, and foreign bodies are some local causes of bleeding from the nose. In developing countries, infestations still contribute a significant proportion of anemia in children. But it is very unusual for leech-causing persistent epistaxis with a consequence of severe anemia. We herein report a rare cause of severe anemia in a 5-year-old child presented to our clinic for persistent epistaxis. A leech was taken out with forceps, and his anemia was treated accordingly.

Xerostomia and Taste Alterations in COVID-19.

A recent letter published in the Ear, Nose & Throat Journal called attention to the hypothesis that the xerostomia reported in patients with the Coronavirus Disease 2019 (COVID-19) occurs due to the neuroinvasive and neurotropism potential of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). In fact, angiotensin-converting enzyme 2 (ACE-2), the main site of entry of SARS-CoV-2 into the cell, was found to be present in the ductal elements of salivary gland and several other tissues. However, some points are worth to be addressed.

The Paramedian Forehead Flap for Nasal Reconstruction: From Antiquity to Present.

Nasal reconstruction is considered the historic foundation of facial plastic surgery, and the forehead flap remains the workhorse of repair. To recreate both the aesthetic contour and function of the nose, all anatomic layers must be addressed-covering, lining, and structural support. This article reviews the noteworthy history underlying the development of the paramedian forehead flap as the primary tool in reconstruction of large nasal defects while highlighting its implications on modern nasal repair. Current developments in the use of 2-staged paramedian forehead flap reconstruction are ex

Fractionated Carbon Dioxide Laser Resurfacing as an Ideal Treatment Option for Severe Rhinophyma: A Case Report and Discussion.

Rhinophyma is a progressive, disfiguring condition that affects the nose and is caused by the hypertrophy of sebaceous glands and connective tissue. Although its exact pathogenesis remains unclear, it is generally thought to be a subtype of the chronic, inflammatory condition rosacea. To date, oral and topical treatments have been largely ineffective at treating rhinophyma. Laser resurfacing is an emerging treatment modality that offers hope for patients with severe rhinophyma. We present a case of rhinophyma treated via fractionated carbon dioxide laser resurfacing with impressive results, ex

Rhinocerebral mucormycosis associated with actinomycosis in a diabetic patient: A rare presentation.

Mucormycosis is an opportunistic fulminant fungal infection which mainly affects the immunocompromised individuals. It begins in the nose and paranasal sinuses due to the inhalation of fungal spores. The common predisposing factors include diabetes mellitus and immunosuppression. Actinomycosis is a bacterial infection caused by nonspore-forming, anaerobic or microaerophilic bacterial species of the genus Actinomyces. It is a suppurative and chronic granulomatous disease characterized by abscess formation, tissue fibrosis and draining sinuses rarely diagnosed in humans. A case of rhinocerebral

A case of stridor due to subglottic stenosis resulting from relapsing polychondritis in a young woman.

Relapsing polychondritis is a rare and severe progressive, multisystem autoimmune disease that affects cartilaginous tissues predominantly of the ear, nose, respiratory system and joints. The rarity of the disease, its unknown aetiology, the array of many possible presenting symptoms, lack of specific investigation for diagnosis and episodic nature, often cause a significant delay in diagnosis. We report a case of a young woman presenting with stridor due to subglottic stenosis of the trachea. In this case, identification of symptoms associated with the stridor facilitated the diagnosis of rel

Radiological Diagnosis of a Rare Premature Aging Genetic Disorder: Progeria (Hutchinson-Gilford Syndrome).

Hutchinson-Gilford Progeria Syndrome (HGPS) is a rare disease with a combination of short stature, bone abnormalities, premature ageing, and skin changes. Though the physical appearance of these patients is characteristic, there is little emphasis on the characteristic radiological features. In this paper, we report a 16-year-old boy with clinical and radiological features of this rare genetic disorder. He had a characteristic facial appearance with a large head, large eyes, thin nose with beaked tip, small chin, protruding ears, prominent scalp veins, and absence of hair.

Melanoma in situ and syringoma: a rare collision tumor. Clinical-pathological report of a case.

Collision or contiguous tumors, defined as two or more distinct tumors occurring at one site, are often an unexpected finding and may represent a diagnostic challenge, as clinical and histological presentations do not always coincide. Various combinations of collision tumors have been described with respect to melanocytic lesions, with the most frequently reported being the combination of nevus and basal cell carcinoma. We present an unusual case on the nose involving a melanoma in situ and a clinically-inapparent syringoma, which, to the best of our knowledge, is the first report of this comb

Challenging neck mass: non-functional giant parathyroid adenoma.

A 46-year-old man was referred to our ear, nose and throat department after the accidental discovery of a large retrotracheal mass. In order to obtain the diagnosis and to plan treatment he underwent a full battery of tests (CT, MRI, blood tests, hormonal assays, ultrasounds, thyroid scintigraphy, urine tests and fine-needle aspiration of the mass), but none of these was able to define the true nature of such cervical mass. Only after surgical excision and histological evaluation, it was diagnosed as an exceptional case of giant non-functional parathyroid adenoma.

Relapsing Polychondritis Presented with Encephalitis Followed by Brain Atrophy.

Relapsing polychondritis (RP) is a rare autoimmune disease that is characterized by inflammatory reaction of unknown etiology and destruction of cartilaginous structures. Characteristic symptoms of this disease include cartilage inflammation of the ear, nose, larynx, trachea, bronchi, joints, eyes, heart and skin. Concomitance with neurologic symptom is very rare in RP, and the detailed underlying mechanism of neurological involvement associated with RP is not fully understood. We herein described an unusual recurrent case of inflammatory brain lesions associated with RP, with attention to cli

Modified extracorporeal septoplasty: using a unilateral curved spreader graft with a septal caudal graft.

The difficulty in correcting deviated nose has been discussed for more than 50 years. Numerous techniques have been proposed for the rectification of this deformity. In this study, we describe a modification to the extracorporeal septoplasty technique that comprises a more conservative approach and results in a stable cartilaginous framework. Combination of curved spreader graft with a septal caudal graft is introduced in the method as a modified extracorporeal septoplasty. Twenty-three patients were enrolled in the study. There were no revision cases. In all cases, comparison of preoperative

Combined endoscopic transnasal and intrasphenoidal septal approach for the drainage of sphenoid sinusitis with cleft lip: technical note.

Isolated sphenoiditis is an uncommon disease, particularly in children. Immediate antibiotic therapy and/or endoscopic transnasal sphenoidotomy should be performed to avoid fatal complications. However, the commonly used transnasal route is not always available in children with a cleft lip and deviated nasal septum. We report a case of acute isolated sphenoid sinusitis in a 12-year-old boy with a cleft lip and deviated nasal septum. This patient underwent combined endoscopic transnasal and intrasphenoidal septal surgery and improved without septoplasty, which may affect the later growth of the

Seckel syndrome: a report of a case.

Seckel syndrome, first defined by Seckel in 1960, is a rare (incidence 1:10,000), genetically heterogeneous autosomal recessive disorder presenting at birth. This syndrome is characterized by a proportionate dwarfism of prenatal onset, a severe microcephaly with a "bird-headed" like appearance (beaked nose, receding forehead, prominent eyes, and micrognathia), and mental retardation. The significance of dental alterations in this syndrome resides in the defect, hypoplastic enamel, being limited to the primary dentition; in most instances the second primary molar tooth is not affected. A case o

A case of paranasal sinuses osteoma detected on bone SPECT/CT.

We present a 51-year-old woman with a history of colectomy for colon cancer who was referred to our department for bone assessment. Planar image revealed a focus of increased uptake of Tc-99m methylene diphosphonate in the nose root. According to the images of SPECT combined with low-dose CT, the abnormal radiotracer uptake at the site of left frontal sinus was considered to be osteoma. We conclude that SPECT/CT provides an accurate evaluation of the localization of the lesions and supplies anatomic information that can be valuable for diagnosis of nonmalignant bone diseases.

Bilobed flaps in nasal reconstruction.

The bilobed flap is a local transposition flap useful for the repair of nasal defects commonly seen after Mohs surgery. The bilobed flap has benefits over other nasal transposition or advancement flaps because it distributes wound closure tension over a larger surface area through the use of 2 lobes. This property is especially helpful for defects of the caudal portion of the nose, where the skin is less elastic. This article discusses the bilobed flap for nasal reconstruction along with the appropriate selection, design, and placement of the flap and the potential complications.

[Simultaneous surgery of total cleft palate in early stage].

On the basis of the literature data and authors' own experience, the methods and optimum time for simultaneous surgery on penetrating unilateral clefts of the upper lip, nose, alveolar process, and hard and soft palates are discussed. The advantages of simultaneous surgery are presented: such operations were performed on 109 of 191 patients with total cleft palate. The main advantages are considered to be as follows: the children's face harmonically develops, their speech improves, the facial appearance becomes more favorable, accompanying diseases associated with a cleft palate disappear, and

The use of Dermicol-P35 dermal filler for nonsurgical rhinoplasty.

A growing number of patients with congenital or acquired nasal defects is seeking nonsurgical procedures to correct the appearance of their nose. The use of fillers for the correction of nasal deformities is expanding because of their low risk and reversibility compared with surgery. Dermicol-P35 (Evolence; Ortho Dermatologics, Skillman, NJ) is a novel crosslinked, fibrillar type I collagen isolated from porcine tendons that has not been associated with the same degree of immunogenicity as bovine collagen. The use of Dermicol-P35 in this study of 12 subjects was associated with high tolerance,
