Decompression of benign orbital apex lesion via medial endoscopic approach.

The use of endoscopic orbital and optic nerve decompression for traumatic optic neuropathy and dysthyroid orbitopathy have been well documented; however, reports on endoscopic decompression for benign orbital apex lesions are scarce. The records of two patients who underwent endoscopic decompression of the bony orbit for progressive visual loss were reviewed. Patient 1 had fibrous dysplasia and presented with headache and visual field defects. Patient 2 had sphenoid wing meningioma and multiple previous attempts of transcranial tumor resection and orbital decompression. Both had progressive vi

Lyme-associated orbital inflammation presenting as painless subacute unilateral ptosis.

A 90-year-old woman presented with subacute painless left ptosis. Examination of the left eye revealed ptosis with loss of the superior eyelid sulcus, 2 mm of proptosis, mild tenderness with retropulsion, and optic disc edema. Levator function and extraocular movements were normal, and there was no relative afferent pupillary defect. MRI demonstrated thickening of the extraocular muscles in the left orbit with lacrimal gland enlargement and mild enhancement of the optic nerve sheath. Serology revealed a positive enzyme-linked immunosorbent assay for Lyme antibodies and a positive Western blot

Orbital dissemination of Lemierre syndrome from gram-positive septic emboli.

A 45-year-old patient presented with bilateral orbital abscesses. He was found to have Lemierre syndrome, a condition involving septic thrombophlebitis of the internal jugular vein. The patient developed severe proptosis, sepsis, and cavernous sinus thrombosis. Despite aggressive antibiotic and anticoagulation therapy, visual loss was rapid, and the patient ultimately died. Lemierre syndrome, previously thought to be rare, is now becoming more commonly reported. Its prompt diagnosis and treatment are essential for patient survival.

Bilateral spontaneous subperiosteal orbital hemorrhages following endoscopic retrograde cholangiopancreatography.

Spontaneous subperiosteal orbital hemorrhage is rare. The authors describe a case of acute bilateral eyelid swelling, eyelid ptosis, and double vision in a patient immediately following endoscopic retrograde cholangiopancreatography. CT revealed bilateral subperiosteal orbital hemorrhages. The patient demonstrated no signs of optic nerve compromise, and the hemorrhages resolved spontaneously without sequela. Previous reports in the literature have documented subperiosteal hemorrhages following increased intracranial venous pressure, coagulopathies, and prone positioning during procedures. To t

Case report of orbital violation with placement of ethmoid drug-eluting stent.

BACKGROUND: Delivery of topical therapy is a mainstay in the treatment of chronic rhinosinusitis (CRS). Ethmoid sinus stenting has recently been introduced as a minimally invasive mechanism to deliver medication. We review available literature on ethmoid stenting and present a case report involving orbital violation and a fixed, dilated pupil. METHODS: Case report and literature review. RESULTS: A 37 year-old female underwent endoscopic placement of bilateral ethmoid sinus drug-eluting stents for CRS at an outside facility. Postoperatively she complained of right-sided ocular pain and pressu

Delayed immediate surgery for orbital floor fractures: Less can be more.

Orbital floor fractures can result in diplopia, enophthalmos, hypoglobus and infraorbital dysthesia. Currently, the most common treatment for orbital floor fractures is immediate surgical intervention. However, there are a number of well-documented cases of unoperated orbital floor fractures in the literature, culminating in diplopia or enophthalmos in few patients. Of these, none reported the diplopia or enophthalmos to be bothersome. As reported previously in the ophthalmology literature, most orbital floor fracture-induced diplopia resolves as the swelling settles, and the few patients with

Remote penetrating orbital trauma due to a snooker cue through the mouth.

PURPOSE: This is a unique case report of a self-inflicted orbital injury that presented as a simple upper vestibulum laceration. A 43-year-old man presented to the Oral and Maxillofacial Surgery Unit with a small laceration in his mouth and complaints of pain in his left eye due to a snooker cue penetration. Upon admission, clinical findings included a small laceration on the upper left vestibulum and a subdermal hematoma in the left eye lid with restricted movements of the left eye. Further examination revealed remote trauma to the orbit, penetrating through the oral cavity, passing the maxil

Periprosthetic bleeding 18 years post-silicone reconstruction of the orbital floor.

BACKGROUND: Periprosthetic orbital haemorrhage is an uncommon complication of the alloplastic implants used in post-traumatic orbital floor repair. The small case series or individual reports provide no definite causative explanation for this delayed bleeding around silicone implants. It is likely that it is related to the disruption of fine capillaries within the pseudocapsule surrounding the implant, since the material does cause low-grade irritation with evidence of chronic inflammation. CASE REPORT: We report the case of a patient who developed a spontaneous periprosthetic bleeding 18 yea

Adult orbital xanthogranulomatous disease: adult-onset xanthogranuloma of periorbital location.

Adult orbital xanthogranulomatous disease (AOXGD) is a rare granulomatous disorder, which has four subtypes: adult-onset xanthogranuloma (AOX), adult-onset asthma with periocular xanthogranuloma, necrobiotic xanthogranuloma and Erdheim-Chester disease. We report a 42-year-old woman who presented with yellowish nonulcerative nodules on her eyelids. On histopathological examination of a nodule, mild degeneration of collagen fibres was seen, with surrounding infiltration of numerous foam cells and Touton giant cells in the deep dermis. Lymphoid follicles were seen in the reticular dermis. There w

[Orbital and ocular adnexal lymphomas. Clinico-pathological correlation in 25 cases].

Clinical, histological features and outcome of a cohort of patients with orbital and adnexal lymphoproliferative tumors were evaluated. Twenty-five cases in an oncologic referral center from 1995 to 2008, were included in the study. Each case had detailed immunophenotypic analysis using a panel of monoclonal antibodies (CD45, CD20, CD3, CD5, CD23, BCL2, BCL6, BCL10, Ki67, CD30, CD15, BCL1, Kappa, Lambda, CD138). Lesions were classified by using WHO (2008) lymphomas classification. Twenty-three patients were found to have primary and two secondary lymphomas. Histological subtypes were: 16 patie

A case series of unilateral orbital aspergillosis in three cats and treatment with voriconazole.

Feline orbital aspergillosis is increasingly described in the published reports and may be an emerging disease. Orbital mycotic infections were diagnosed in three cats with exophthalmos, significant dorso-temporal globe deviation and pronounced resistance to retropulsion. Advanced imaging was performed in all three cases to evaluate the extent of disease as well as to obtain guided orbital biopsies in two cases. Surgical intervention in the form of a lateral orbitotomy was pursued in the first case with the other two cases treated with enucleation or medical management alone. The available pub

[Correction of minor exophthalmos by enlargement of orbital volume through conjunctive approach].

OBJECTIVE: To investigate the treatment of minor exophthalmos by enlargement of orbital volume through conjunctive approach. METHODS: Through the conjunctive approach medial to the eye ball, the dissection was performed subperiosteally to expose the ethmoid bone at the medial orbital wall. The exposure bone was 2.5-3.0 cm in length and 2.0-2.5 cm in width. Then the ethmoid bone was fractured to enlarge the orbital volume. From June 2006 to April 2008, 3 cases were treated. RESULTS: The patients recovered in a short time with no visible skin scar. The technique was mini-invasive, and didn't a
