BACKGROUND: Preeclampsia is the most common complication of pregnancy, significantly affecting maternal and fetal health, and is characterized by placental and systemic endotheliopathy. Patients with preeclampsia have elevated levels of VWF (von Willebrand Factor), which is associated with poor clinical outcomes. However, whether VWF serves as a marker for endotheliopathy or contributes to the pathogenesis of preeclampsia remains poorly understood.
METHODS: We investigated the role of hyperadhesive VWF in the development of preeclampsia by studying patients, evaluating mouse models, and perfo

During pregnancy, the mother-placenta relationship involves intricate and dynamic exchanges. Over the course of gestation, the maternal body encounters numerous fetal materials secreted by the placenta, such as hormones, growth factors, and extracellular vesicles like exosomes. These exosomes are carriers of key biomolecules, including proteins, lipids, nucleic acids (DNA), and microRNA (miRNA), capable of influencing maternal cellular activity. Although the exact functions of placental exosomes during pregnancy remain under investigation, existing research indicates that they contribute signi

BACKGROUND AND AIMS: Preeclampsia (PE) is a serious medical condition that usually causes high blood pressure and affects multiple organs. Considering the adverse effect of oxidative stress on the process of PE in pregnant women and regarding the role of the Nrf2 gene in placental oxidative pathways, this study was conducted to investigate the DNA methylation status of Nrf2 in PE and healthy pregnant women.
MATERIALS AND METHODS: The present case-control study consisted of 70 PE and 70 healthy pregnant women. Blood and placenta samples were taken from all subjects, and the percentage of the N

INTRODUCTION: The incidence of single fetal demise in monochorionic diamniotic twin pregnancies is approximately 7.5%. In single fetal demise cases, the placental region of the demised fetus is almost infarcted or necrotic at the time of delivery. Further, it has been reported that in some cases, a surviving fetus uses all regions of the placenta after a single fetal demise. Thus, in the present study, we investigated the incidence and natural prognosis of placental recruitment in spontaneous single fetal demise cases over a period of 11 years.
METHODS: All 306 monochorionic diamniotic twin p

INTRODUCTION: Accumulating epidemiological studies support that Vitamin D deficiency is associated with the pathogenesis of preeclampsia. However, it is unknown whether vitamin D can be used as a treatment for preeclampsia. This study aimed to explore whether vitamin D supplementation could improve the rat model of preeclampsia.
METHODS: LPS was used to establish a rat model of preeclampsia. Inflammatory cytokines were examined by QRT-PCR and ELISA assays, and the concentration of sfit-1 and NO was assessed by ELISA. Analyzing the pathological features of the placenta with hematoxylin-eosin.

Twin-to-twin transfusion syndrome (TTTS) is characterized by an unbalanced blood transfer through placental abnormal vascular connections. Prenatal ultrasound (US) is the imaging technique to monitor monochorionic pregnancies and diagnose TTTS. Fetoscopic laser photocoagulation is an elective treatment to coagulate placental communications between both twins. To locate the anomalous connections ahead of surgery, preoperative planning is crucial. In this context, we propose a novel multi-task stacked generative adversarial framework to jointly learn synthetic fetal US generation, multi-class se

BACKGROUND: The ability to predict risk factors for blood transfusion after postpartum hemorrhage could enhance the performance of lifesaving procedures in patients who experience postpartum hemorrhage. Therefore, this study aimed to evaluate these risk factors and create a scoring system for blood transfusion evaluations and risk in obstetric patients.
STUDY DESIGN AND METHODS: Diagnosis and blood transfusion data of 14,112 women who delivered between January 1, 2015, and December 31, 2015, were analyzed. A binary logistic regression model was used. We conducted univariate analyses of each r

Placenta previa, low-lying placenta, and placenta accreta are aberrations in the normal development of the placenta. Diagnosis in the prenatal period is essential because each of these pathologies can have a profound impact on the management of labor, birth, and the third stage. In recent years, there has been an increase in the occurrence of these placental abnormalities, with the increase in the cesarean rate considered to be a main cause of this phenomenon. Comprehensive risk assessment, combined with recent advances in ultrasonography, can provide earlier detection of impaired placental im

The semiquantitative reverse transcription polymerase chain reaction was employed to detect the expression of transforming growth factor beta (TGF-beta) and insulin-like growth factor (IGF) in complete hydatidiform mole, normal first-trimester villi, the normal term placenta (after vaginal/abdominal deliver) and the preeclamptic placenta at term. The expression of IGF-I mRNA was seen in all five tissues, but its level was much lower in the term placental tissues with preeclampsia than in other tissues. The content of IGF-I mRNA in villous tissues from molar pregnancy was slightly higher than i

BACKGROUND: Placenta Accreta Spectrum (PAS) involves abnormal placental adhesions, leading to severe complications like postpartum hemorrhage. Such cases often require cesarean hysterectomies, which are associated with significant risks of urinary tract injuries.
METHODS: This retrospective study reviewed medical records of 220 patients diagnosed with PAS at Rafik Hariri University Hospital, Beirut, from 2007 to 2022. Patients were categorized based on ultrasound findings into two groups: Group 1 included 84 with Placenta Previa Accreta; Group 2 comprised 72 with Low Risk Placenta Previa, 52

It is important to develop a better understanding of placental insufficiency given its role in common maternofetal complications such as preeclampsia and fetal growth restriction. Functional magnetic resonance imaging offers unprecedented techniques for exploring the placenta under both normal and pathological physiological conditions. Dynamic contrast-enhanced magnetic resonance imaging (DCE MRI) is an established and very robust method to investigate the microcirculatory parameters of an organ and more specifically its perfusion. It is currently a gold standard in the physiological and circu

Until recently, trophoblast invasion during human placentation was characterized by and restricted to invasion into uterine connective tissues and the uterine spiral arteries. The latter was explained to connect the arteries to the intervillous space of the placenta and to guarantee the blood supply of the mother to the placenta. Today, this picture has dramatically changed. Invasion of endoglandular trophoblast into uterine glands, already starting at the time of implantation, enables histiotrophic nutrition of the embryo prior to perfusion of the placenta with maternal blood. This is followe

Velamentous insertion of the cord, or vasa previa, is a malady where fetal vessels tranverse membranes ahead of the fetal part. The incidence of vasa previa is 1: 2000-3000 deliveries. Fetal mortality is over 50-75%. Early diagnosis is needed because these deliveries require emergency cesarean section; it is especially more common with placenta percreta, uterine atony and hemorrhage. Intravascular infusion of red blood cells (RBCs) into the fetus is one of the most successful means of in utero therapy for severe fetal anemia caused by RBC alloimmunization. We performed four fetal intrauterine

Corticotropin-releasing factor (CRF) produced in placenta has paracrine effects within placenta, decidua, and myometrium and endocrine effects on mother and fetus. CRF is a potent local regulator of myometrial contractility and of prostaglandin release, Recently, urocortin, a new member of the CRF family, has been localized in human placenta and membranes. Urocortin mimics some of the local effects of CRF in intrauterine tissues, that is, increase of adrenocorticotrophic hormone (ACTH) and prostagiandin release and myometrial contractility. A local CRF-BP modulates the paracrine effects of CRF

UNLABELLED: An analysis of indications for obstetric hysterectomy in Central Teaching Hospital of Ministry of Internal Affairs in Warsaw between 1985-1997 is presented. There have been 15 caesarean or post partum hysterectomies over that period, which makes 0.11% of all deliveries and 0.67% of caesarean sections. All operations followed caesarean sections. In two patients (13.3%) procedure was performed electively because of myomas. In the remainder emergency or post-partum hysterectomy was necessary. The most common indication was haemorrhage connected with placenta praevia, adherent placenta

This is a prospective study in order to evaluate the association between placenta grade III changes and the neonatal outcome. A total of 370 patients divided into two groups were included: the case group included 182 patients with placenta grade III changes (placental senescence) and the control group included 188 cases with grade 0, I or II placenta. The patients with grade III placenta had more cases of oligohydramnios (37 vs 24%), cardiotocographic non reactive tests (35 vs 17 cases) and an elevated frequency of cesarean sections (83%), (P < 0.05). The newborns in both groups had not differ

Twenty-three cases of advanced abdominal pregnancy were managed at Harare Hospital, Zimbabwe, over a 10 year period. The incidence was 1 in 9500 total deliveries and 1 in 60 ectopic pregnancies. There were no maternal deaths but perinatal mortality was 83%. Of the 5 liveborn infants, 4 had pressure deformities and 3 were small for gestational age. There was one neonatal death due to pulmonary hypoplasia. Diagnosis is difficult and is often first suggested by failed induction of labour in a case of presumed intrauterine death. Abnormal presentation of the fetus, displacement of the cervix and a

Basement membrane (type IV) collagens were extracted from a mouse tumour with acetic acid and from human placenta after limited enzymatic digestion. Antisera were produced against both collagens in rabbits and guinea-pigs and examined by various assays. These antisera were found to be specific for basement membrane collagen and showed little or no cross-reactions with the interstitial collagens, types I, II and III or with human placenta collagen consisting of alpha A and alpha B chains. Varying degrees of cross-reaction were observed between antisera to human and mouse type IV collagen. Immun

The fetus is dependent upon the placenta for transport to it of all nutrients for energy and growth. The primary nutrients crossing the placenta are glucose, lactic acid, free amino acids, free fatty acids, and ketone bodies. Under normal circumstances, it appears that glucose is the primary energy source. In abnormal conditions there is the possibility that energy requirements may be met also by protein-amino acid oxidation and ketone body metabolism. The fetal brain may be quite adept in the use of ketone bodies. In all transport phenomena, the placenta intervenes by diverting nutrients into

Temporary cross-clamping of the infrarenal abdominal aorta to reduce bleeding during hysterectomy following cesarean section in patients with placenta previa-accreta is a relatively simple procedure; therefore, it can be an option when intravascular balloon catheter placement is impossible or encountered massive bleeding of the unexpected placenta accreta.
