Survey Study to Differentiate Vasospastic Angina From Coronary Microvascular Dysfunction in Real-World Management of Ischemia With NonObstructive Coronary Artery Disease.

The appropriate management of ischemia with nonobstructive coronary artery disease (INOCA) has now become a globally recognized issue. However, there is a lack of studies examining how cardiologists manage INOCA by differentiating between vasospastic angina and coronary microvascular dysfunction (CMD) endotypes in real clinical practice, as well as the extent to which they are satisfied with their current diagnostic and management approaches. This survey collected data through a web-based questionnaire conducted between October 15, 2024, and November 15, 2024, targeting Japanese cardiologists

Variation in tuberculosis screening and infliximab use in Kawasaki disease in Japan.

BACKGROUND: This study examined nationwide variation in tuberculosis (TB) screening and infliximab (IFX) use in the management of Kawasaki disease (KD) in Japan. Immunosuppressive agents such as IFX, which is used to treat refractory KD, may elevate the risk of TB infection. Although guidance exists for IFX use in KD patients, standardized protocols for TB screening, particularly regarding test selection and timing, are lacking. METHODS: A nationwide, cross-sectional web-based survey was conducted from October 2023 to February 2025. Eligible respondents were physicians involved in the care of

Using implementation science to decrease variation and high opioid administration in a surgical ICU.

BACKGROUND: High doses and prolonged duration of opioids are associated with tolerance, dependence, and increased mortality. Unfortunately, despite recent efforts to curb outpatient opioid prescribing because of the ongoing epidemic, utilization remains high in the intensive care setting, with intubated patients commonly receiving infusions with a potency much higher than doses required to achieve pain control. We attempted to use implementation science techniques to monitor and reduce excessive opioid prescribing in ventilated patients in our surgical intensive care unit (SICU). METHODS: We

Patterns, determinants, and outcomes of early use of antidementia drugs: A 6-year multicenter cohort study in Thailand.

BACKGROUND: Evidence on antidementia drugs (ADD) use in developing countries, where accessibility to ADD is challenging, is limited. Our aim was to examine prescribing patterns, factors, and outcomes associated with the early-ADD use (within 3 months from diagnosis) in people with dementia. METHODS: A retrospective cohort study included individuals aged ≥ 60 years with dementia from three hospitals in Thailand between 2015 and 2020. Participants were categorized as non-ADD users, early-ADD users, and delayed users. ADD switching and one-year persistence were analyzed. Multivariable logistic r

Antibiotic Diversity Index: A novel metric to assess antibiotic variation among hospitalized children.

BACKGROUND: Despite nationally endorsed treatment guidelines and stewardship programs, variation and deviation from evidence-based antibiotic prescribing occur, contributing to inappropriate use and medication-related adverse events. Measures of antibiotic prescribing variability can aid in quantifying this problem but are not adequate. OBJECTIVE: The objective of this study is to develop a standardized metric to quantify antibiotic prescribing variability (diversity) within and across children's hospitals, and to examine its association with outcomes. METHODS: We performed a cross-sectional

Increasing Naloxone Access and Prescribing for Patients on High-Dose Opioids From a Managed Care Pharmacy Health Plan Perspective.

Background: Opioid overdoses decrease when communities have access to naloxone. Clinicians play a key role in offering naloxone to high-risk chronic opioid patients. Managed care pharmacists within our health plan noted disproportionate processing for claims of opioid utilizers compared to claims of naloxone prescriptions. Objective: To increase naloxone access and prescribing to members who classify at a dosage with a higher risk for opioid overdose, defined as over 90 morphine milligram equivalents (MME). Methods: Multiple system-wide initiatives were implemented to improve naloxone access.

Pragmatic Emergency Department Intervention Reducing Default Quantity of Opioid Tablets Prescribed.

INTRODUCTION: The opioid epidemic is a major cause of morbidity and mortality in the United States. Prior work has shown that emergency department (ED) opioid prescribing can increase the incidence of opioid use disorder in a dose-dependent manner, and systemic changes that decrease default quantity of discharge opioid tablets in the electronic health record (EHR) can impact prescribing practices. However, ED leadership may be interested in the impact of communication around the intervention as well as whether the intervention may differentially impact different types of clinicians (physicians

Opioid prescribing in the Netherlands during the COVID-19 pandemic: a national register-based study.

OBJECTIVES: The COVID-19 pandemic and related lockdown measures disrupted global healthcare provision, including opioid prescribing. In North America, opioid sales declined while opioid-related deaths increased. In Europe, the effect of the pandemic on prescribing is not yet known. Given the ongoing increase in opioid-related harm and mortality, it is crucial to analyse the impact of the COVID-19 crisis and lockdown measures on opioid prescribing. Therefore, the objective of this study was to characterise opioid prescribing in the Netherlands during the COVID-19 pandemic. DESIGN: A nationwide

Appropriate antimicrobial prescribing for bacteriuria before urological procedures: A room for improvement.

In the presence of a positive preoperative urine culture, the prescription of a preoperative antibiotic therapy is recommended. The choice of antibiotic therapy and prescription are usually made by the urologist or the general practitioner (GP). The objective of the treatment is urinary sterilization rather than parenchymal treatment, and the treatment choice is key to reduce selective pressure and antimicrobial resistance. The objective of this study was to evaluate prescription patterns made by urologists and GPs, then to compare the antibiotics chosen by urologists or GPs to the "ideal" tre

Opioid prescribing patterns and the effect of chronic kidney disease in pediatric urology population: A retrospective cohort analysis.

BACKGROUND: Significant efforts have been undertaken to decrease opioid prescribing, but there is little research into patient-specific factors presenting as barriers in the pediatric surgical population. Non-steroidal anti-inflammatory drugs (NSAIDs) have been shown to be a safe and effective alternative to opioids for pain control, however, concerns about their impact on renal function limit their use in patients with chronic kidney disease (CKD). Data is limited on the interplay of CKD on opioid prescribing. OBJECTIVE: We hypothesized that opioid reduction efforts were successful, but pati

The Effects of Removing the Requirement for Prior Reimbursement Approval on Anticoagulant Use in Ireland: A Cross-Sectional Study.

PURPOSE: Prior approval for reimbursement is a policy of cost containment while ensuring oversight and governance of medicines. It has been employed in Ireland to address financial challenges due to the shift from warfarin to direct oral anticoagulants (DOACs). Studies assessing the effectiveness of this policy are limited. Thus, we aimed to examine the effectiveness of prior approval for reimbursement of DOACs (apixaban, rivaroxaban) as a cost containment policy in Ireland. METHODS: The Irish Health Service Executive-Primary Care Reimbursement Service database was used in this cross-sectiona
