Aetiology of cancer in Asia.

Cancer has become the leading cause of death in many Asian countries. There is an increasing trend in breast, prostate and colon cancers, which are considered as typical of economically developed countries. Although breast and prostate cancer rates are still lower than in western countries, they are particularly rapidly increasing. In this paper, we review recently published literature to identify important etiologic factors affecting the cancer risk in Asian populations. Infectious agents such as Helicobacter pylori, hepatitis B and C viruses, and human papillomavirus were shown to be associa

Patient Perspectives on the "Future Patient" Telerehabilitation Program for Atrial Fibrillation: Qualitative Study.

BACKGROUND: Atrial fibrillation (AF) is a prevalent chronic condition with increasing incidence worldwide. AF increases the risks of stroke, heart failure, and myocardial infarction and imposes a substantial burden on the health care system. Cardiac rehabilitation programs, while effective, often have low patient adherence. Recent evidence suggests that cardiac telerehabilitation, where patients are given home monitoring devices, could enhance adherence and outcomes. The program "Future Patient-Telerehabilitation of Patients with AF" (FP-AF) was created to assess the effects and potential bene

Ensuring the survival of the clinician-scientist.

Many forces threaten the survival of the clinician-scientist as an academic species, among them: (1) the changing health environment; (2) the complexity of and rapid advances in biomedical science, which necessitate that MD-PhD graduates "retool" after completing their clinical training; (3) the length and rigor of the research training required to train clinician-scientists adequately; (4) the scarcity of funding for subspecialty training positions; (5) the perception that the successful clinician-scientists in academic medicine are those who focus on basic, rather than clinical, research; (6

Prenatal alcohol exposure increases the susceptibility to develop aggressive prolactinomas in the pituitary gland.

Excess alcohol use is known to promote development of aggressive tumors in various tissues in human patients, but the cause of alcohol promotion of tumor aggressiveness is not clearly understood. We used an animals model of fetal alcohol exposure that is known to promote tumor development and determined if alcohol programs the pituitary to acquire aggressive prolactin-secreting tumors. Our results show that pituitaries of fetal alcohol-exposed rats produced increased levels of intra-pituitary aromatase protein and plasma estrogen, enhanced pituitary tissue growth, and upon estrogen challenge d

Pre-implantation alcohol exposure and developmental programming of FASD: an epigenetic perspective.

Exposure to alcohol during in-utero development can permanently change the developmental programming of physiological responses, thereby increasing the risk of neurological illnesses during childhood and later adverse health outcomes associated with fetal alcohol spectrum disorder (FASD). There is an increasing body of evidence indicating that exposure to alcohol during gestation triggers lasting epigenetic alterations in offspring, long after the initial insult; together, these studies support the role of epigenetics in FASD etiology. However, we still have little information about how ethano

Young people and caregivers' perceptions of an intervention program for children who deliberately light fires.

A significant number of children and adolescents engage in deliberate fire setting, beyond the scope of curiosity and experimentation. Interventions developed to respond to the needs of such fire setters generally involve educational and/or psychosocial approaches. Research evaluating the effectiveness of these interventions is dominated by outcome studies which rely on recidivism rates determined by either official records or parent reports. There has however, been no process evaluation studies published. This study presents a process analysis which aimed to identify the strengths and weaknes

Drug Therapy in Obstructive Sleep Apnea.

There are several reasons to develop a pharmacologic remedy in obstructive sleep apnea (OSA); but so far, there is no generally effective drug available. Previous attempts to find a drug in OSA therapy were serendipity driven in small pilot trials. There is growing literature on phenotyping pathophysiologic mechanisms of OSA that may be exploited in strategic drug development programs. The current review addresses potential pitfalls encountered in previous studies and highlights several drug candidates under development in the field.

Evaluation of the Ebola Virus Disease (EVD) preparedness and readiness program in Uganda: 2018 to 2019.

INTRODUCTION: the Democratic Republic of Congo (DRC) declared its 10thoutbreak of Ebola virus disease (EVD) in 42 years on August 1st 2018. The rapid rise and spread of the EVD outbreak threatened health security in neighboring countries and global health security. The United Nations developed an EVD preparedness and readiness (EVD-PR) plan to assist the nine neighboring countries to advance their critical preparedness measures. In Uganda, EVD-PR was implemented between 2018 and 2019. The World Health Organization commissioned an independent evaluation to assess the impact of the investment in

An internet-based adolescent depression preventive intervention: study protocol for a randomized control trial.

BACKGROUND: The high prevalence of major depressive disorder in adolescents and the low rate of successful treatment highlight a pressing need for accessible, affordable adolescent depression prevention programs. The Internet offers opportunities to provide adolescents with high quality, evidence-based programs without burdening or creating new care delivery systems. Internet-based interventions hold promise, but further research is needed to explore the efficacy of these approaches and ways of integrating emerging technologies for behavioral health into the primary care system. Participatory Development of a Speech-Language Telerehabilitation Intervention Combined With Home-Based Transcranial Direct Current Stimulation for Primary Progressive Aphasia: A Qualitative Study.

PURPOSE: Intensive speech-language therapy (SLT) delivered as telerehabilitation in combination with transcranial direct current stimulation (tDCS) may be an effective treatment option for people with primary progressive aphasia (pwPPA). However, such combined treatment approaches are technically challenging, and feasibility for pwPPA has not yet been established. We aimed to involve stakeholders in the development of a novel approach combining remotely supervised but independently self-administered home-based tDCS with an intensive aphasia telerehabilitation (naming and communicative-pragmati

Guidance on take-home naloxone distribution and use by community overdose responders in Canada.

BACKGROUND: The increasing toxicity of opioids in the unregulated drug market has led to escalating numbers of overdoses in Canada and worldwide; takehome naloxone (THN) is an evidence-based intervention that distributes kits containing naloxone to people in the community who may witness an overdose. The purpose of this guidance is to provide policy recommendations for territorial, provincial and federal THN programs, using evidence from scientific and grey literature and community evidence that reflects 11 years of THN distribution in Canada. METHODS: The Naloxone Guidance Development Group

Stimulation rate reduction and auditory development in poorly performing cochlear implant users with auditory neuropathy.

OBJECTIVE: Patients with auditory neuropathy spectrum disorder (ANSD) exhibit altered neural synchrony in response to auditory stimuli. It has been hypothesized that a slower rate of electrical stimulation in programming strategies for cochlear implant (CI) users with ANSD may enhance development of neural synchrony and speech perception abilities. STUDY DESIGN: Retrospective case series. SETTING: Tertiary otologic practice. PATIENTS: Twenty-two patients with ANSD underwent CI. Patients with complete postoperative audiometric data and at least 2 years of follow-up were included in further a

Specific Early Childhood Experiences Predict Executive Function Skills During Later Childhood and Adolescence: Evidence from the ECHO Cohort.

The healthy development of executive function (EF) skills in childhood and adolescence provides a crucial foundation for later outcomes, from mental and physical health to academic achievement and socio-emotional functioning. Although meta-analyses have identified associations between children's EF skills and early experiential factors, these are often examined in terms of broad categories (e.g., socioeconomic status) or cumulative risk factors (e.g., adverse childhood experiences; ACEs). The current study leverages longitudinal data (N = 1295) from six cohorts from the Environmental influence

Sex modulates the long-term effects of delivery mode on microbiota-gut barrier crosstalk and colitis susceptibility in mice.

Sexual dimorphism and mode of delivery are key determinants of gut physiology and microbiota development and may differentially affect predisposition to gut-related diseases. Cesarean section delivery markedly shapes early-life microbiota, predisposing individuals to higher risk of immune and metabolic comorbidities later in life. Although both sex and delivery mode are known to influence gut barrier-microbiota crosstalk, whether delivery mode modulates or counter-regulates sex-specific features of this interaction remains, to our knowledge, largely unexplored. Here, we investigated how sex im

Developing evidence-informed pathways for long-term engagement in cycling for the United States.

Cycling participation in the United States remains lower than in comparable nations despite widespread bicycle ownership. This manuscript examines the fragmented structure of cycling in the U.S. and proposes the adoption of evidence-informed Long-Term Athlete Development (LTAD) pathways to transform cycling from a niche pursuit into an integrated cultural practice. Drawing on structured expert discussions among coaches, program leaders, and stakeholders, this perspectives paper synthesizes practice-based insights to inform the development of a scalable framework. Supporting Decision Making in Early Years Initiatives: Co-Designing a Child Health Evidence Dashboard.

ISSUE ADDRESSED: Research has trialled many initiatives to support caregivers to shape young children's health behaviours. However, translating evidence to public health practice and policy is a challenge, in part due to access to such evidence. We aimed to co-design a child health evidence dashboard, and to understand the expected enablers and barriers to use of the dashboard with next-users (e.g., Australian child service managers, program planners). METHODS: Using an integrated knowledge translation approach, we conducted a mixed-methods study involving five online co-design workshops. Nex
