Hamstring strain injuries: recommendations for diagnosis, rehabilitation, and injury prevention.

UNLABELLED: Hamstring strain injuries remain a challenge for both athletes and clinicians, given their high incidence rate, slow healing, and persistent symptoms. Moreover, nearly one third of these injuries recur within the first year following a return to sport, with subsequent injuries often being more severe than the original. This high reinjury rate suggests that commonly utilized rehabilitation programs may be inadequate at resolving possible muscular weakness, reduced tissue extensibility, and/or altered movement patterns associated with the injury. Further, the traditional criteria use

Comparison between distally based peroneus brevis and sural flaps for reconstruction of foot, ankle and distal lower leg: an analysis of donor-site morbidity and clinical outcome.

BACKGROUND: Skin defects of the foot, ankle and distal lower leg often require coverage by local or distant flaps. We aimed to compare functional outcome and donor-site morbidity following transfer of distally based delayed sural (DSFs) or peroneus brevis flaps (PBFs). METHODS: Between 2003 and 2006, 52 patients (24 DSFs and 28 PBFs) were included. For increased reliability, all extended sural flaps were delayed for 3-15 days. At the end of the follow-up period (minimum 12 months), patients were asked to fill out a modified foot and ankle score (Foot and Ankle Outcome Score (FAOS)) questionna

[The surgeon as a hand patient: the clinical and psychological impact of hand and wrist fractures].

To gain insight into the management of patients with hand and wrist injuries, a series of clinical and psychological analyses was performed on 9 surgeons, each of whom underwent operative fixation of a hand or wrist fracture. The results of these analyses suggest that the functional outcome after hand surgery was affected by the surgeons' personality, motivation, and ability to accept and adapt to the injury, the nature of the injury, and the importance of the hand to the surgeons' careers. Surgeons are highly motivated and compulsive individuals who consider their career involvement a major s

Measuring wrist and hand function: common scales and checklists.

INTRODUCTION: A hand and wrist disorder affects a patient's overall well-being and health-status. One concept serves as the foundation for all further consideration: in order to have confidence in your results when assessing patients with wrist and hand limitations, the clinician and researcher must choose standardised patient-oriented instruments that address the primary aims of the study. In this paper, we assess the quality of reviews published on patient oriented instruments in current use for assessing function of the hand and wrist joint. We highlight features of commonly used scales tha

Effects of the different frequencies of whole-body vibration during the recovery phase after exhaustive exercise.

AIM: This study was to investigate the effects of vibration exercise on the oxygen consumption (VO2) and heart rate variability (HRV) during the recovery phase after exhaustive exercise. METHODS: Twenty male college students volunteered as subjects to participate in the study. The subjects were randomly crossover assigned to perform three 10 min vibration exercises, namely non-vibration (CON, 0 Hz, 0 mm), low-frequency (LFT, 20 Hz, 0.4 mm) and high-frequency (HFT, 36 Hz, 0.4 mm) treatments immediately after an incremental exhaustive cycling exercise in separated days. Fixed-angle locking plating of displaced proximal humerus fractures.

Proximal humerus fractures are common, especially among patients older than 60 years who have osteoporosis. Several techniques have been used within the past several decades to treat displaced proximal humerus fractures. Many different implants have been investigated and tested, all with inadequate results; debate continues as to the best method of fixation. Fixed-angle locking plates offer a relatively new treatment option for these complicated fractures, especially in patients with osteoporosis. The use of locking plates for treating comminuted proximal humerus fractures has biologic and bio

Lateral subvastus approach with osteotomy of the tibial tubercle for total knee replacement: a two-year prospective, randomised, blinded controlled trial.

The lateral subvastus approach combined with an osteotomy of the tibial tubercle is a recognised, but rarely used approach for total knee replacement (TKR). A total of 32 patients undergoing primary TKR was randomised into two groups, in one of which the lateral subvastus approach combined with a tibial tubercle osteotomy and in the other the medial parapatellar approach were used. The patients were assessed radiologically and clinically using measurement of the range of movement, a visual analogue patient satisfaction score, the Western Ontario McMasters University Osteoarthritis Index and th

Effects of explicit and implicit perseverative cognition on cardiac recovery after cognitive stress.

Slow cardiovascular (CV) recovery after stress is a predictor of adverse CV outcomes. Perseverative cognition (PC) about stress has been hypothesized to co-determine slow recovery. In the present study, it was investigated whether two types of trait PC, i.e. trait worry and trait rumination, predicted delayed cardiac recovery after a cognitive stressor. Furthermore, it was examined whether explicit state PC (i.e. negative intrusive thoughts) or implicit state PC (i.e. automatic vigilance) additionally predicted delayed cardiac recovery. Fifty-nine participants performed a stressful task, which

The promotion of peripheral nerve regeneration by chitooligosaccharides in the rat nerve crush injury model.

Chitooligosaccharides (COSs), the biodegradation product of chitosan, have shown many biological functions. In this study, we examined the possible benefits of treatment with COSs (M.W. 800) on regeneration of rat crushed sciatic nerves. The rats with sciatic nerve crush injury were administered intraperitoneally daily with 3 or 6mg/kg body weight of COSs over a 3-week period. During and at the end of COSs treatment, a series of functional and histological examinations, including the measurement of withdrawal reflex latency (WRL) values, walking track analysis, electrophysiological assessments

Comparing the effects of exercise program and low-level laser therapy with exercise program and polarized polychromatic non-coherent light (bioptron light) on the treatment of lateral elbow tendinopathy.

BACKGROUND DATA: The use of low-level laser therapy (LLLT) and polarized polychromatic non-coherent light as supplements to an exercise program has been recommended for the management of lateral elbow tendinopathy (LET). OBJECTIVE: To investigate whether an exercise program supplemented with LLLT is more successful than an exercise program supplemented with polarized polychromatic non-coherent light in treating LET. MATERIALS AND METHODS: Patients with unilateral LET for at least 4 wk were sequentially allocated to receive either an exercise program with LLLT or an exercise program with pola

Minimally invasive management of unstable proximal femoral extracapsular fractures using reverse LISS femoral locking plates.

The efficacy of the reverse Less Invasive Plating System in the management of unstable proximal femoral extracapsular fractures was retrospectively evaluated. Twenty-seven patients with complex proximal femoral fractures were identified. There were three open fractures. The mean age was 71 years (range; 65-79). The mean follow up was 24 months (range; 15-32). The main outcome measures were union, union time, requirement for secondary procedures, development of deep infection, pain, and functional impairment. Nonunion was observed in one patient. The average Harris hip score at the last assessm

[Neuropsychological aspects of acquired diffuse brain injury in paediatric childhood].

INTRODUCTION AND DEVELOPMENT: Acquired brain injury is one of the most common causes of long-term disability in childhood and traumatisms are the most prevalent cause. Diffuse axonal injury is associated with poor neuropsychological outcome. In contrast to early focal lesions, young age confers no advantage in the outcome of severe diffuse brain injury. Plasticity is incomplete for structural and functional deficits in children. The traditional view of enhanced reorganization of function after localized brain injury it can't be applied in early severe diffuse brain injury. In the paediatric po

Timing for brachial plexus injury: a personal experience.

This article is a presentation of personal experience and resultant views by the author on the timing of surgery for nerve injury, especially that for brachial plexus injuries. The author presents arguments for early and delayed surgery based on the type of injury encountered and examines how early nerve repair on all might preclude spontaneous recovery. On the other hand, neglect of repair or greatly delayed repair is also deleterious because many useful outcomes can be gained by a well timed nerve repair or transfer. This is part of a Point-Counterpoint discussion with Dr. Rolfe Birch's pres

Nerve root replantation.

Traumatic avulsion of nerve roots from the spinal cord is a devastating event that usually occurs in the brachial plexus of young adults following motor vehicle or sports accidents or in newborn children during difficult childbirth. A strategy to restore motor function in the affected arm by reimplanting into the spinal cord the avulsed ventral roots or autologous nerve grafts connected distally to the avulsed roots has been developed. Surgical outcome is good and useful recovery in shoulder and proximal arm muscles occurs. Pain is alleviated with motor recovery but sensory improvement is poor
