BACKGROUND: Preoperative radio-chemotherapy (RCT) may play an important role in decreasing local recurrence and possibly improving survival for patients with advanced rectal carcinoma.
PATIENTS AND METHODS: Between 1995 and 1998 we treated 30 patients (pts) (median age 61.5 years) with advanced low rectal cancer. Radiotherapy was administered using a 10-MV linear accelerator delivering a total dose of 45 Gy to the pelvis over 5 weeks. 5-FU (500 mg/m2/d i.v.) was given concomitantly on days 1-3 and 29-31. Surgery was performed 2-4 weeks after RCT and included abdomino-perineal resection (APR)

We performed PMC-CPT-11 therapy (modified pharmacokinetic modulating chemotherapy plus irinotecan, or modified PMC) in a case of sigmoid colon cancer with local invasion and multiple hepatic metastases. This regimen combined PMC therapy which includes Hartmann's operation, simple hysterectomy and postoperative 5-fluorouracil (5-FU), with intravenous irinotecan, or CPT-11. The multiple hepatic metastatic lesions disappeared after surgery and no local recurrence has been found since. These results indicate that PMC-CPT-11 (modified PMC) therapy could be an effective regimen for cases of progress

PURPOSE: This study evaluates the accuracy of computed tomography (CT), magnetic resonance imaging (MRI), and endorectal ultrasound (ERUS) in preoperative staging of colon and rectal cancers, comparing imaging results with histopathological findings. The aim is to identify strengths and limitations of each modality to optimize therapeutic strategies, particularly for neoadjuvant therapy in locally advanced cases.
METHODS: This retrospective study analyzed 487 patients treated surgically for colon and rectal cancers between 2012 and 2018. Patients with incomplete imaging or histopathological d

BACKGROUND: The optimal interval between neoadjuvant chemoradiotherapy (nCRT) and total mesorectal excision (TME) in locally advanced rectal cancer (LARC) remains controversial. This randomized controlled trial evaluated whether prolonging the interval between nCRT and surgery improves pathological response and oncological outcomes.
METHODS: Adult patients with histologically confirmed, MRI-staged II-III rectal adenocarcinoma located within 12 cm of the anal verge were randomized to undergo surgery at 8 weeks or 12 weeks after completion of long-course nCRT (50 Gy with concurrent 5-fluorourac

BACKGROUND: Local excision as a first step in the treatment of early rectal cancer has gained interest. However, in the presence of histopathological risk factors, (inter)national guidelines recommend completion total mesorectal excision. Although oncologically safe, completion total mesorectal excision is associated with an increased end colostomy rate compared to primary total mesorectal excision, especially in distal lesions. Transanal total mesorectal excision may facilitate lower anastomoses, potentially reducing end colostomy rates.
OBJECTIVE: To compare the end colostomy rate and oncol

BACKGROUND: Risk-adapted treatment strategies have been recommended for patients with rectal cancer. In our previous phase II study, neoadjuvant chemotherapy with capecitabine and oxaliplatin resulted in substantial tumor responses and encouraging survival outcomes for patients with locally advanced rectal cancer.
OBJECTIVE: The aim of this study was to compare the survival outcomes of neoadjuvant chemotherapy and neoadjuvant chemoradiotherapy in patients with locally advanced rectal cancer.
DESIGN: Retrospective cohort analysis.
SETTING: This study was conducted at 2 high-volume hospitals

AIM: In rectal cancer, neo-adjuvant (chemo)radiotherapy (n(C)RT) should be used selectively, due to related toxicity. The decision to offer n(C)RT is mainly based on preoperative MRI staging. cN category has limited accuracy and its use when deciding on n(C)RT is controversial. This population-based study aimed to assess the association between cN category and local recurrence (LR) rate.
METHOD: Data from a national cross-sectional cohort of patients who underwent curative resection of primary rectal cancer in the Netherlands in 2016 were used. Patients were subdivided by neo-adjuvant treatme

BACKGROUND: Minimally invasive approaches to manage early-stage rectal cancer have become popular, offering effective treatment with reduced morbidity. Among them, transanal endoluminal surgery (TES) and endoscopic submucosal dissection (ESD) are widely utilized, yet their comparative outcomes from large-scale real-world data remain limited.
METHODS: We conducted a retrospective cohort study using the TriNetX United States Collaborative Network. Adults with T1 rectal cancer undergoing either TES or ESD were identified. Propensity score matching was performed on demographic and clinical variab

BACKGROUND: The presence of comorbidity affects the care of cancer patients, many of whom are living with multiple comorbidities. The prevalence of cancer comorbidity, beyond summary metrics, is not well known. This study aims to estimate the prevalence of comorbid conditions among cancer patients in England, and describe the association between cancer comorbidity and socio-economic position, using population-based electronic health records.
METHODS: We linked England cancer registry records of patients diagnosed with cancer of the colon, rectum, lung or Hodgkin lymphoma between 2009 and 2013

BACKGROUND: Although the accuracy of preoperative MRI staging has been established on follow-up histopathologic examination, the reproducibility of MRI staging has been evaluated in studies with expert radiologists reading a large sample of MRI images and therefore is not generalizable to the real-world setting.
OBJECTIVE: The purpose of this study was to evaluate the interrater reliability of MRI for distance to the mesorectal fascia, T category, mesorectal lymph node status, and extramural depth of invasion for preoperative staging of primary rectal cancer.
DESIGN: This was a prospective,

INTRODUCTION: Previous studies, including research published more than 10 years ago in Northern Alberta, have demonstrated improved outcomes with increased surgical volume and subspecialisation in the treatment of rectal cancer. We sought to examine contemporary rectal cancer care in the same region to determine whether practice patterns have changed and whether outcomes have improved.
METHODS: We reviewed the charts of all patients with rectal adenocarcinoma diagnosed between 1998 and 2003 who had a potentially curative resection. The main outcomes examined were 5-year local recurrence (LR)

BACKGROUND: Transanal endoscopic microsurgery (TEM) is considered a curative option for selected T1 rectal cancer. Although TEM is safe, local recurrence (LR) rates after TEM are unacceptably high. Evidence on selection criteria, however, is not abundant. To expand evidence on low- versus high-risk T1 rectal cancer with respect to LR, this study aimed to identify predictive histopathologic factors in a selected group of T1 rectal cancers treated with TEM only.
METHODS: The study enrolled 62 patients for whom specimens of the primary tumor containing an invasive T1 carcinoma could be reevaluat

C-reactive protein (CRP), a biomarker of inflammation, has been shown to be influenced by genetic variation in the CRP gene. In this study, we test the hypothesis that genetic variation in CRP influences both the risk of developing colon and rectal cancer and survival. Two population-based studies of colon cancer (n = 1,574 cases, 1,970 controls) and rectal (n = 791 cases, 999 controls) were conducted. We evaluated four CRP tagSNPs: rs1205 (G > A, 3' UTR); rs1417938 (T > A, intron); rs1800947 (G > C, L184L); and rs3093075 (C > A, 3' flanking). The CRP rs1205 AA genotype was associated with an

Rectal cancer is projected to have the 10th highest incidence of cancer in the United States in 2009, with 40 870 estimated new cases this year. The prognosis for patients with rectal cancer is directly related to several factors, with the most important, based on current data, the extent of primary tumor invasion, the number of lymph nodes involved, involvement of the circumferential resection margin, and the presence of metastases. Contemporary therapy is dependent upon pre-surgical diagnostic imaging modalities, which influence the indication for neoadjuvant therapy and the decision process

Transrectal ultrasound-guided prostate biopsy is the standard technique to detect prostate cancer histologically. Based on a few case reports, it is known that prostate biopsy can cause a seeding of tumor cells. We report the case of a 64-year-old man who 2 years after radical prostatectomy for prostate cancer underwent proctocolectomy because of ulcerative pancolitis. In the lamina submucosa of the distal rectum a micrometastasis of the prostate cancer was found. A possible reason for this metastasis is an implantation of prostate cancer cells due to the transrectal biopsy of the prostate.
