Misaligned by design: evaluating and deploying generative AI for the real-world conditions of primary care: a Nordic and European perspective.

BACKGROUND: Large language models (LLMs) have entered clinical and consumer use faster than primary-care researchers can evaluate them, and the dominant benchmarks and deployment strategies were shaped in hospital settings rather than primary care. The evidence is mixed: GPT-4 scored below general practitioners on the Swedish family-medicine specialist examination, while Brodeur et al. (2026) found an LLM outscoring the several hundred physicians who worked the same hospital-based cases. The decisive problem is therefore not raw performance but misalignment between how these systems are evalua

Diagnostic Challenges of Cerebral Venous Thrombosis Mimicking Brain Tumor: A Case Report.

BACKGROUND Cerebral venous thrombosis is a rare but critical stroke subtype that frequently mimics brain tumors on imaging, resulting in diagnostic delays and unnecessary interventions. This case highlights a classic example of cerebral venous thrombosis mimicking a glioma, showing the diagnostic pitfalls and need for heightened clinical suspicion. CASE REPORT A 64-year-old man presented with a 1-month history of recurrent headaches and left lower-limb pain. Initial evaluation revealed markedly elevated D-dimer (7.12 mg/L; normal <0.55) and imaging findings highly suggestive of high-grade glio

A multifaceted outpatient treatment model for mood and anxiety disorders designed to optimize both treatment outcomes and access to care.

Despite the availability of evidence-based psychosocial and pharmacologic interventions for mental health concerns, access to care remains suboptimal. We present a time-efficient mental health treatment program designed to improve overall community access to evidence-based care. Quality of care within the time-efficient model was prioritized by focusing on a multifaceted program of evidence-based interventions and facilitating connections to long-term care when needed. We present the results of a proof-of-concept open trial that indicates that access to care can be improved while maintaining q

Latinx caregivers of LGBTQ people, cultural values, and mental health outcomes.

Studies have found that Latinx cultural values play a crucial role in the acceptance process of Latinx caregivers of lesbian, gay, bisexual, transgender, and queer (LGBTQ) people. Guided by minority stress framework, in this study we quantitatively examine whether Latinx cultural values buffered the effects of acceptance on mental health outcomes among Latinx caregivers of LGBTQ people. In a national survey of 315 Latinx caregivers (age: M = 37.9; SD = 9.6) of LGBTQ people (age: M = 18.9, SD = 7.0), we assessed (a) the relation between acceptance, mental health outcomes (i.e., depression and a

Using personalised brain stimulation to modulate social cognition in adults with autism-spectrum-disorder: protocol for a randomised single-blind rTMS study.

BACKGROUND: Autism spectrum disorder (ASD) is a neurodevelopmental disorder characterized by impairments of social interaction and communication as well as repetitive, stereotyped behaviour. Previous research indicates that ASD without intellectual impairment is associated with underactivity and reduced functional connectivity of the brain's mentalizing pathway, to which the right temporo-parietal junction (rTPJ) serves as an important entry point and hub. In this study, we aim to utilize functional magnetic resonance imaging (fMRI) to localize activation maxima in the rTPJ and other regions i

Imaging malaria parasites across scales and time.

The idea that disease is caused at the cellular level is so fundamental to us that we might forget the critical role microscopy played in generating and developing this insight. Visually identifying diseased or infected cells lays the foundation for any effort to curb human pathology. Since the discovery of the Plasmodium-infected red blood cells, which cause malaria, microscopy has undergone an impressive development now literally resolving individual molecules. This review explores the expansive field of light microscopy, focusing on its application to malaria research. Imaging technologies

The most influential decision factors for choosing a formal diagnostic classification system for describing personality pathology.

There are currently three models for diagnosing personality disorders for practitioners to choose from in the formal diagnostic classification systems Diagnostic and Statistical Manual of Mental Disorders (DSM) and International Classification of Diseases (ICD): the categorical model retained from DSM-IV, a dimensional model in the ICD-11, and the DSM-5 Alternative Model for Personality Disorders (AMPD) that is a hybrid of categorical and dimensional methods. We used a sequential mixed-methods approach to explore how psychologists and psychiatrists decide which model to use for diagnosing pers

Racial disparities in the efficacy of traditional versus acceptance-based behavioral weight loss.

OBJECTIVE: Standard behavioral weight loss (SBT) is a first-line intervention for obesity, but its efficacy with Black adults is limited. Some preliminary work suggests that acceptance-based behavioral therapy (ABT), which integrates mindful decision making, experiential acceptance, and reflection on values, may be a better fit for the needs of Black adults. In this fully powered secondary data analysis, we compared the efficacy of SBT and ABT for Black adults. METHOD: Black and non-Hispanic White adults (N = 737) with overweight or obesity (body mass index = 27-50 kg/m²) were enrolled in one

"Migrants are human beings and they don't sleep with animals" healthcare workers and migrants' experiences of the impact of inadequate antiretroviral therapy provision in Botswana.

BACKGROUND: Migrant populations in Botswana face considerable barriers to accessing HIV prevention and treatment services, particularly antiretroviral therapy (ART). These gaps undermine both individual health outcomes and broader public health goals. This study explores the perceptions of healthcare workers and cross-border migrants on the impact of inadequate ART provision to migrants and the consequences for the local population. METHODS: A qualitative study was conducted in Gaborone and Francistown, Botswana. In-depth interviews were carried out with 12 healthcare providers and 20 cross-bo

Higher Polyp Detection Rate and Cecum Intubation Rate in Morning Shift Compared to Afternoon Shift: A Multicenter Large Cohort Retrospective Study.

BACKGROUND: Colorectal cancer (CRC) is the third most commonly occurring cancer worldwide. There are conflicting reports on whether colonoscopies performed during the morning shift have higher adenoma detection rates. OBJECTIVES: To investigate the differences in polyp detection rate (PDR) in colonoscopies between morning and afternoon shifts. METHODS: In a retrospective, multicenter study involving a large cohort, we analyzed all colonoscopies conducted between 2016 and 2023 across seven endoscopy departments. The PDR was compared between morning and afternoon shifts. RESULTS: We included

A brief online intervention for sleep problems and heavy drinking: Impacts on alcohol- and sleep-related problems.

There is growing evidence that poor sleep contributes to increased alcohol use and related impairment among college students (DeMartini & Fucito, 2014), with heavy drinking exacerbating these effects. Efforts to reduce this impact via online personalized feedback interventions (PFIs) have had limited success, and there is a dearth of research on attempts to simultaneously address these concerns among college students using brief, web-based interventions. The present study assessed intervention efficacy in addition to the impact of sleep disturbance on alcohol outcomes using a novel, brief, onl

Heeding lessons of our past: Centering sociohistorical context and health equity in efforts to expand contraceptive access.

Contraceptive access initiatives and programs to expand contraceptive access over the past two decades, following the fervor of the CHOICE project, were often inattentive to health equity and the sociohistorical context of care. While these initiatives improved access to contraception and, in particular, long-acting reversible methods, the enthusiasm for these methods and the framing of them as a panacea for social ills contributed to the entrenchment of non-patient-centered models of contraceptive care and harmful targeting of low-income communities of color. In this commentary we describe a

Effects of self-similarity and self-generation on the perceptual prioritization of voices.

The self-prioritization effect (SPE) reflects the ability to efficiently discern self-relevant information. The self-voice emerges as a crucial identity marker because of its inherent self-relevance, and previous work has demonstrated the perceptual and cognitive advantages of the self-voice over other voices. Yet, the extent to which humans prioritize their self-voice when they hear it is because it is both self-similar ("That sounds like my voice") and self-generated ("I said that") remains understudied. Here, we examined the impacts of self-similarity and self-generation on the SPE through

It takes two to co-ruminate: Examining co-rumination as a dyadic and dynamic system.

Co-rumination-defined as when individuals perseverate on problems with each other, focus excessively on negative feelings, and cyclically discuss the causes and consequences of problems-is often examined from the perspective of the person seeking support or by assigning one rating of co-rumination to a dyad. This approach muddles how each person contributes to the "co" of co-rumination and may have implications for understanding prior work that has shown associations between co-rumination and intrapersonal and interpersonal well-being. We leveraged state space grids to examine co-rumination as

Effects of a Hyperbaric Normoxic Environment on the Retina and Choroid.

PURPOSE: Divers and individuals working in submarines and other underwater environments are exposed to normoxic hyperbaric conditions frequently. The aim of this study was to evaluate the effects of hyperbaric normoxia on the retina and choroid. METHODS: Healthy participants with no prior diving experience were exposed to 2.4 atmospheres absolute pressure in a normoxic hyperbaric chamber (HC). Optical coherence tomography was used to measure retinal thickness, the peripapillary retinal nerve fiber layer, and choroidal thickness (CT) before and within 30 minutes after HC exposure. RESULTS: Th

Adolescent executive function as a resilience factor in the family stress model among Mexican-origin families.

Mexican-origin families often face economic hardship due to systemic oppression, increasing the likelihood of adolescent marijuana use. While the family stress model provides insight into the mechanism of the association between family economic hardship and adolescent marijuana use, resilience factors are relatively unknown. The present study uses a three-wave longitudinal data set of Mexican immigrant families in the United States (Wave 1-adolescents: Mage = 12.29, SD = 0.93; mothers: Mage = 38.57, SD = 5.71) to investigate the mechanism underlying the association between family income and ad

Neurogenic Voiding Dysfunction in Spinal Cord Injury and Stroke: Urodynamic Evaluation, Functional Classification, and Therapeutic Strategies.

Many patients with injuries to their central nervous system, especially those who have had a stroke or a spinal cord injury, have neurogenic voiding dysfunction. It happens when patients can't control their bladder voluntarily, which can make them feel urgency, experience leaks, or have trouble emptying their bladder. If not treated right away, it can cause infections, damage to the bladder, or even kidney problems over time. This review will explain how this type of dysfunction works, how it is diagnosed, and what treatment options are available. It will mostly focus on stroke and spinal cord

Intrinsic Functional Connectivity Network in Children with Dyslexia: An Extension Study on Novel Cognitive-Motor Training.

Objectives: Innovative, evidence-based interventions for developmental dyslexia (DD) are necessary. While traditional methods remain valuable, newer approaches, such as cognitive-motor training, show the potential to improve literacy skills for those with DD. Verbal Working Memory-Balance (VWM-B) is a novel cognitive-motor training program that has demonstrated positive effects on reading, cognitive functions, and motor skills in children with DD. This extension study explored the neural mechanisms of VWM-B through voxel-to-voxel intrinsic functional connectivity (FC) analysis in children with

Decoronation as a Therapeutic Alternative for Ankylosis in Children and Adolescents for Vertical Bone Preservation and Growth: A Systematic Review.

Background/Objectives: Dentoalveolar ankylosis in adolescents involves a series of difficulties related to bone growth and development, especially in a vertical manner. A systematic review of studies on ankylosis in young permanent teeth treated by decoronation which considered the preservation/vertical growth of the alveolar bone as their main objective was carried out. Methods: The PRISMA 2020 guidelines were followed. Our research question was formulated using the PICO structure.
