Reimagining dementia screening: A stakeholder-informed perspective on artificial intelligence, digital biomarkers, and real-world implementation.

The approval of disease-modifying treatments for Alzheimer's disease demands a rethinking of cognitive screening. Drawing on over 180 stakeholder interviews from the NSF National I-Corps program, this perspective highlights barriers in current workflows, from time constraints in primary care to learning effects in long-term care, and presents innovation pathways centered on AI and digital biomarkers. Speech analysis, in particular, offers a scalable and cost-effective screening tool aligned with existing CPT codes. We outline implementation strategies and emphasize the urgent opportunity to al

Anti-MDA5 Antibody-Positive Clinically Amyopathic Dermatomyositis Associated With Multiple Heterologous Carcinomas: A Case Report.

Dermatomyositis (DM), an autoimmune disorder, is linked to increased malignancy risk. A 53-year-old man with anti-melanoma differentiation-associated gene 5 (MDA5)-positive clinically amyopathic dermatomyositis (CADM) and rapidly progressing interstitial lung disease (RP-ILD) developed heterochronous gastric and colorectal cancers. Early endoscopic screenings led to successful curative resections, preventing recurrence. Despite low cancer incidence assumptions in patients with anti-MDA5-positive RP-ILD, this case advocates for reevaluation and periodic cancer screenings to enhance management,

[Chinese expert consensus on the visual acuity testing for infants and preverbal children (2023)].

Abnormal visual experience in the first 3 years of life may lead to permanent visual impairment without early screening and timely intervention. It is of great clinical significance to evaluate the visual acuity of infants and preverbal children with suspected visual abnormality. However, the evaluation is difficult due to the inability of these children to express their complaints and the lack of unified standards. In order to promote the standardization of the visual acuity testing in infants and preverbal children, including the screening and follow-up examinations, a consensus has been rea

Telemedicine in diabetic retinopathy: Access to rural India.

Diabetic retinopathy (DR) is a growing concern in India. The first step in management of DR is timely screening. With 10% prevalence in rural India, 11 million people are likely to have DR by the year 2030. With limited resources and skilled manpower, it will not be possible to have routine eye examination to identify and treat these patients on a regular basis. Telemedicine is a possible answer in these situations where patients could be remotely screened and appropriately advised. With the advent of several technological advances such as low cost hand-held nonmydriatic camera, increased capa

RNA interference for the identification of ectoparasite vaccine candidates.

Ectoparasites present a major challenge for disease management globally. With drug resistance increasingly observed in many disease-causing species, the need for novel control measures is pressing. Ever-expanding genomic resources from 'next generation' sequencing are now available for a number of arthropod ectoparasites, necessitating an effective means of screening these data for novel candidates for vaccine antigens or targets for chemotherapeutics. Such in vitro screening methods must be developed if we are to make discoveries in a timely and cost-effective manner. This review will discuss

First trimester prediction of ischemic placental disease.

Ischemic placental disease is characterized by one or more of the clinical manifestations of preeclampsia, fetal growth restriction, and/or placental abruption, resulting in indicated preterm delivery. Since over half of the indicated preterm deliveries are due to ischemic placental disease, accurate early prediction of the disease is of paramount importance in developing prevention strategies. This review article focuses on studies that have used the first trimester aneuploidy screening timing window to predict those patients who later develop ischemic placental disease. Emphasis was given to

Migration and tuberculosis in the UK: targeting screening for latent infection to those at greatest risk of disease.

Active tuberculosis (TB) in the UK predominantly affects the non-UK born, but is generally not manifest at the time of UK entry. Strategies to detect latent TB infection (LTBI) in this population are, therefore, important. To date, targeted screening has focused on TB risk estimates based on the incidence in the country of origin. Using TB incidence in the UK and migration data, we estimated the numbers needed to be tested and treated for LTBI to prevent one case of TB disease. Numbers were the lowest in Somalian and the highest in South African and Filipino migrants, which contrasts with TB r

[Predicting pregnancy outcomes based on first trimester screening].

Pregnancy surveillance in the western world focuses on the second and third trimesters. Recently, a few studies demonstrated that several pregnancy outcomes can be predicated by first trimester screening, especially in the time period between 11 + 0 to 13 + 6 weeks of gestation. These outcomes include the risk of developing preeclampsia, small for gestational age (SGA) and macrosomic fetuses, risk of abortion or intrauterine fetal death, risk of preterm labor and the risk of developing gestational diabetes. This review focuses on these findings and examines if it is feasible to expand the nuch

Preparticipation physical evaluation: getting to the heart of the matter.

The cardiovascular evaluation is an important component of the preparticipation physical examination of the athlete. The history should be standardized to include questions ranging from those about chest pain and palpitations to ones about syncope. The physical examination should not be a perfunctory one, but rather a careful search for findings that might include a low grade heart murmur that accentuates with Valsalva'a maneuver (suggestive of hypertrophic cardiomyopathy, the leading cause of sudden death in a competitive athlete). The cardiovascular risks to the athlete are known. Fortunatel

[Methods for rapid evaluation of the effectiveness of drugs showing promise for urgent prevention and treatment of plague].

The choice of efficient antibacterial drugs useful in special prophylaxis and treatment of plague includes the stage of the in vivo screening whose main disadvantages are long-term and hazardous tests. The paper presents the data showing that a useful chemotherapeutic for the treatment of plague could be safely screened within 48-72 hours. The methods are based on two-fold retrobulbar or intraperitoneal administration of a drug within 24 hours to albino mice infected with highly virulent strains of Y. pestis. The dose should not exceed 1.10(7) live microbes. It was shown that the administratio

Understanding experiences and perceptions of perinatal mental health screening tools among under-served groups: A qualitative study of women from ethnic minority communities in the United Kingdom.

BACKGROUND: Perinatal mental health problems, such as depression and anxiety, are major global public health concerns. Screening for mental health problems in pregnancy supports early detection and treatment, improving outcomes for women and their families. However, mental ill health is not consistently identified in routine maternal practice, particularly among women from ethnic minority communities in high income countries like the United Kingdom. Although research has explored mental health screening tools among majority ethnic populations, there remains a notable lack of evidence regarding

Knowledge of cervical cancer screening, barriers to awareness, and the role of technological interventions in improving screening uptake: a scoping review protocol.

BACKGROUND: Cervical cancer remains a major public health concern despite being largely preventable through early detection, regular screening, timely diagnosis, and effective treatment of precancerous lesions. Globally, cervical cancer continues to be one of the leading causes of cancer-related morbidity and mortality among women, particularly in low- and middle-income countries where access to preventive services remains limited. Although screening guidelines from the World Health Organization (WHO) and the South African Department of Health (DOH) are widely available, inadequate knowledge a

Cost-effectiveness of artificial intelligence-assisted liquid-based cytology testing for cervical cancer screening in China.

BACKGROUND: The 2021 World Health Organization (WHO) guidelines for cervical cancer screening recommend human papillomavirus (HPV) DNA or mRNA testing. Artificial intelligence (AI)-assisted liquid-based cytology (LBC) systems also have the potential to facilitate rapid scale-up of cervical cancer screening. We aimed to evaluate the cost-effectiveness of AI-assisted LBC testing, compared with the manual LBC and HPV-DNA testing, for primary cervical cancer screening in China. METHODS: We developed a Markov model for a cohort of 100,000 women aged 30 years over a lifetime to simulate the natural

Agreement between GLIM and PG-SGA for diagnosis of malnutrition depends on the screening tool used in GLIM.

BACKGROUND & AIM: The Global Leadership Initiative on Malnutrition (GLIM) has suggested a process for the diagnosis of malnutrition. The process consists of applying an existing screening tool for malnutrition screening, followed by malnutrition diagnostics, and finally categorization of malnutrition severity (moderate or severe) according to specific GLIM criteria. However, it is not known how well the GLIM process agrees with other diagnostic tools used in the current clinical practice. The aim of this study was to validate the GLIM process against the Patient Generated-Subjective Global Ass
