[Acute management of spinal injuries. The thoracic and lumbar spine, surgical therapy].

The aims of internal fixation of thoracic and lumbar spinal fractures are immediate stability, preservation of spine mobility, and restoration of the anatomical form of the vertebra and of the canal diameter at acceptable risks. All these aims together can be achieved only by short posterior pedicle fixation systems, such as the external spine fixator, the various internal spine fixators, special dorsal Roy-Camille plates, Wolter plates, and the combined anterior and posterior unisegmental fusion with the USIS system. Isolated anterior procedures necessitate additional bracing. The indications

The Impact of Screw Density on Mechanical Complications in Lumbar Spine Deformity Constructs.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To determine the impact of screw density on outcomes in adult spinal deformity surgery. SUMMARY OF BACKGROUND DATA: Previous studies have suggested that decreased screw density in long lumbar constructs does not compromise mechanical robustness or increase the risk of complications such as PJK, pseudoarthrosis, or reoperation. METHODS: We retrospectively identified adults who underwent spinal fusions with an upper instrumented vertebra (UIV) of L2 or above with a minimum 2-year radiographic follow up. We collected data on UIV, Hounsfield

Surgical invasiveness, reoperation, and preoperative depression are predictive of super-utilization in adult spinal deformity surgery.

PURPOSE: A subset of adult spinal deformity (ASD) patients undergoing corrective surgery receive a disproportionate level of medical resources and incur greater costs. We examined the characteristics of such super-utilizers of health care resources among ASD patients. METHODS: This prospective, multicenter study analyzed data from ASD patients with > 4 levels of spinal fusion and a minimum 2-year follow-up. Index and total episode-of-care (EOC) costs in 2022 US dollars were calculated using average itemized direct costs obtained from administrative hospital records. Patients with total 2-year

Demographics, presentation and symptoms of patients with Klippel-Feil syndrome: analysis of a global patient-reported registry.

INTRODUCTION: Klippel-Feil syndrome (KFS) occurs due to failure of vertebral segmentation during development. Minimal research has been done to understand the prevalence of associated symptoms. Here, we report one of the largest collections of KFS patient data. METHODS: Data were obtained from the CoRDS registry. Participants with cervical fusions were categorized into Type I, II, or III based on the Samartzis criteria. Symptoms and comorbidities were assessed against type and location of fusion. RESULTS: Seventy-five patients (60F/14M/1 unknown) were identified and classified as: Type I, n

Evaluation of a Novel Flexible Cage System for C5-C6 Fixation: A Finite Element Study Against Conventional ACDF Implants.

Cervical spondylosis is a common cause of spinal cord dysfunction, and anterior cervical discectomy and fusion (ACDF) is widely employed when conservative treatment fails. Conventional implant systems such as the cervical cage with plate (CCP) and zero-profile stand-alone cage (ZPSC) are commonly used to enhance spinal stability and promote fusion, but they are associated with complications including dysphagia and adjacent segment degeneration. To address these limitations, a novel flexible plate cage system (FPCS) has been developed to optimize biomechanical performance while minimizing surgi

Taming the persistent seroma: multidisciplinary management following complex spinal revision surgery - a case report.

INTRODUCTION: Chronic seroma and pseudomeningocele formation following spinal instrumentation, particularly in the setting of prior infection and multiple revision surgeries, remains rare but pose a significant therapeutic challenge due to high recurrence rate. In this context, we present a complex case of pseudomeningocele that evolved into a persistent seroma with cutaneous fistula after multi-level lumbar fusion with a comprehensive review of sclerotherapy protocols. MATERIALS AND METHODS: A 75-year-old patient with prior L3-S1 fusion surgery presented with rod fracture and underwent surgi

Technical Nuances of Anterior Column Construction for the Treatment of Osteomyelitis-Induced Cervical Kyphoscoliotic Deformity: An Operative Video Illustration.

Pyogenic vertebral osteomyelitis is a bacterial infection of the vertebral body often caused by hematogenous spread from a distant site with 3%-11% of cases affecting the cervical spine.1,2 Patients at risk for osteomyelitis are intravenous drug users, patients with diabetes, patients in dialysis, and males older than 50 years of age. In severe cases where infection causes osseous destruction of the vertebral column lending to a loss of normal sagittal and coronal plane alignment, neurologic impairment, or spinal instability, surgical correction may be required.3 A 38-year-old woman with a med

Biomechanical study of rod stress in lumbopelvic fixation with lateral interbody fusion: an in vitro experimental study using synthetic bone models.

OBJECTIVE: Despite improvements in surgical techniques and instruments, high rates of rod fracture following a long spinal fusion in the treatment of adult spinal deformity (ASD) remain a concern. Thus, an improved understanding of rod fracture may be valuable for better surgical planning. The authors aimed to investigate mechanical stress on posterior rods in lumbopelvic fixation for the treatment of ASD. METHODS: Synthetic lumbopelvic bone models were instrumented with intervertebral cages, pedicle screws, S2-alar-iliac screws, and rods. The construct was then placed in a testing device, an

Cervical Total Disk Arthroplasty.

In the United States, cervical total disk arthroplasty (TDA) is US Federal Drug Administration (FDA) approved for use in both 1 and 2-level constructions for cervical disk disease resulting in myelopathy and/or radiculopathy. TDA designs vary in form, function, material composition, and even performance in?vivo. However, the therapeutic goals are the same: to remove the painful degenerative/damaged elements of the intervertebral discoligamenous joint complex, to preserve or restore the natural range of spinal motion, and to mitigate stresses on adjacent spinal segments, thereby theoretically l

Which functional outcome parameters correlate better with elderly patients' satisfaction after non-fusion lumbar spine surgery?

BACKGROUND: Elderly patients are increasingly operated for spinal degenerative diseases. The treatment objective is quality of life, which, in this population, is difficult to assess. Hence, patient satisfaction, although less objective, is of high importance. In this study, we have examined the relation of various functional parameters after non-fusion spinal surgery, with patient satisfaction. METHODS: A 5-year follow-up after non-fusion lumbar spine surgery on 185 elderly patients was performed. Demographics, co-morbidity factors, type of lesion and operation performed were recorded. The O

Growing rod instrumentation in the treatment of early onset scoliosis.

The goal of the treatment of early onset scoliosis (EOS) is correction of the deformity while still allowing for spinal growth. The aim of this study was to determine the safety and effectiveness of the single and dual growing rod techniques and which technique was the most effective in the management of EOS respectively. From 2003 to 2009, 23 patients underwent single (15) or dual (8) growing rod procedures using a pedicle screw construct and tandem connectors. The etiology of the patients' spinal deformities were as follows; infantile, juvenile idiopathic, congenital and neuromuscular. Clini

Surgical treatment for adult spinal deformity: projected cost effectiveness at 5-year follow-up.

BACKGROUND: In the United States, expenditures related to spine care are estimated to account for $86 billion annually. Policy makers have set a cost-effectiveness benchmark of less than $100,000/quality adjusted life year (QALY), forcing surgeons to defend their choices economically. This study projects the cost/QALY for surgical treatment of adult spinal deformity at 5-year follow-up based on 2-year cost- and health-related quality-of-life (HRQOL) data. METHODS: In a review of 541 patients with adult spinal deformity, the patients who underwent revision or were likely to undergo revision we
