Evaluation of functional nerve recovery shows that allogeneic nerve graft treated with ICAM-1 and LFA-1 mAbs can be good alternative to syngeneic graft.

The objective of the study is to establish recovery results of tibial nerve defects reconstructed using allogeneic and xenogeneic graft, in host immunosuppressed with Intercellular Adhesion Molecule-1 (ICAM-1) and Lymphocyte Function Antigen-1 (LFA-1) monoclonal antibodies (mAbs). A pilot study was conducted in fifteen Fischer rats by forming a 1 cm right tibial nerve gap, then reconstructing it with 1.2 cm long grafts, namely, Wistar allogeneic, Black mouse xenogeneic, and syngeneic (n = 5/group). The main study included forty-eight rats allocated to the following groups (n = 12/group): 1) Al

Multifocal motor neuropathy: clinical and electrophysiological findings.

Multifocal motor neuropathy (MMN) can be differentiated from motor neuron disease by electrophysiological evidence of conduction block. To increase the probability of recording conduction block, we studied the whole nerve length including proximal segments in 84 patients with pure motor syndromes, using a special stimulation technique. In 8 patients, the diagnosis of MMN was confirmed by electrophysiological evidence of conduction block or temporal dispersion. The typical clinical picture of MMN with chronic progressive, asymmetrical, marked distal weakness was observed in our patients. Electr

Transsynaptic degeneration in the superficial dorsal horn after sciatic nerve injury: effects of a chronic constriction injury, transection, and strychnine.

The lumbar and cervical spinal dorsal horns of adult rats with a chronic (8 days) constriction injury of the sciatic nerve on one side (and a sham operation on the other) were examined for signs of transsynaptic degeneration. The incidence of neurons with signs of degeneration (pyknosis and hyperchromatosis; 'dark neurons') was significantly increased in the lumbar dorsal horn on both sides. The ipsilateral lumbar increase was significantly greater than the contralateral increase. There was no increase in the incidence of dark neurons in the cervical dorsal horns of the same rats. The distribu

Melorheostosis and the sclerotomes: a radiological correlation.

Melorheostosis is characterised radiologically by hyperostotic linear densities in bone. These densities have a peculiarly segmental distribution which does not correspond with the anatomical course of blood vessels or mixed nerve roots of the limbs. So far this disbritubion has lacked any valid explanation, although it has been suggested to be a developmental error as a result of an embryonic metameric disturbance. Inman and Saunders in 1944 described a sensory nerve supply to skeletal structures with 'sclerotomes' representing the zones of the skeleton supplied by individual spinal sensory n

Brainstem neurons coordinate the bladder and urethral sphincter for urination.

Urination, a vital and conserved process of emptying urine from the urinary bladder in mammals, requires precise coordination between the bladder and external urethral sphincter (EUS) that is tightly controlled by a complex neural network. However, the specific subpopulation of neurons that accounts for such coordination remains unidentified, limiting the development of target-specific therapies for certain urination disorders, for example, detrusor-sphincter dyssynergia. Here, we find that cells expressing estrogen receptor 1 (ESR1+) in the pontine micturition center (PMC) initiate voiding wh

Symptomatic extraforaminal gas-containing pseudocyst treated with unilateral biportal endoscopic spinal surgery: a case report and literature review.

BACKGROUND: Gas-containing pseudocyst is an uncommon cause of lumbar radiculopathy and most lumbar gas-containing pseudocysts locate in the spinal canal. While, extraforaminal gas-containing pseudocysts are very rare. Here, we reported a case of extraforaminal gas-containing pseudocyst, which compressed L4 exiting nerve root and caused lumbar radiculopathy. CASE PRESENTATION: A 62-year-old female presented with low back pain and radiation to anteromedial aspect of right thigh and anterior aspect of right calf. Computed tomography and magnetic resonance imaging of lumbar spine showed a gas-con

A surgical and functional approach to the pelvic gross neuroanatomy of the female Yucatan minipig.

The Yucatan minipig is gaining widespread use in studies focused on spinal cord injury. As a large animal model, it offers unique advantages for developing novel and more effective therapies. Successful neuromodulation experiments require precise access to central and peripheral neural structures, which depends on a thorough understanding of topographical anatomy and advanced surgical techniques. The current study describes the topography of the pelvic organs in the female Yucatan minipig, as well as a surgical approach to the principal nerves and branches controlling the pelvic viscera. Injectable PLGA-Coated Ropivacaine Produces A Long-Lasting Analgesic Effect on Incisional Pain and Neuropathic Pain.

The management of persistent postsurgical pain and neuropathic pain remains a challenge in the clinic. Local anesthetics have been widely used as simple and effective treatment for these 2 disorders, but the duration of their analgesic effect is short. We here reported a new poly lactic-co-glycolic acid (PLGA)-coated ropivacaine that was continuously released in vitro for at least 6 days. Perisciatic nerve injection of the PLGA-coated ropivacaine attenuated paw incision-induced mechanical allodynia and heat hyperalgesia during the incisional pain period, and spared nerve injury-induced mechani

Neurostimulation methods in the treatment of chronic pain.

The goal of this narrative review was to give an up-to-date overview of the peripheral and central neurostimulation methods that can be used to treat chronic pain. Special focus has been given to three pain conditions: neuropathic pain, nociplastic pain and primary headaches. Both non-invasive and invasive techniques are briefly presented together with their pain relief potentials. For non-invasive stimulation techniques, data concerning transcutaneous electrical nerve stimulation (TENS), transcranial direct current stimulation (tDCS), repetitive transcranial magnetic stimulation (rTMS), remot

Intrathecal Spread of Injectate Following an Ultrasound-Guided Selective C5 Nerve Root Injection in a Human Cadaver Model.

Ultrasound-guided selective C5 nerve root blocks have been described in several case reports as a safe and effective means to anesthetize the distal clavicle while maintaining innervation of the upper extremity and preserving diaphragmatic function. In this study, cadavers were injected with 5 mL of 0.5% methylene blue dye under ultrasound guidance to investigate possible proximal and distal spread of injectate along the brachial plexus, if any. Following the injections, the specimens were dissected and examined to determine the distribution of dye and the structures affected. One injection re

Ultrasound-guided interscalene catheters performed under general anesthesia in a patient with Huntington's disease.

The placement of interscalene nerve blocks in adults under general anesthesia remains controversial. There have been reports of patients who suffered permanent losses of cervical spinal cord function during this practice; however, these cases employed long needles and paresthesia or nerve stimulator techniques with high stimulation currents. As a result, practitioners currently use short needles for interscalene block and value feedback from patients who are awake. Interscalene catheters, because they require large bores and occasionally longer needles, are unlikely to be used on anesthetized

Components of the neural circuitry of the vaginocavernosus reflex in rabbits.

In mammals the mechanisms underlying female sexual and reproductive biology are poorly understood. Little attention has been paid to striated muscles and their neural regulation. The aim of the present study was to describe the components of the vaginocavernosus reflex in adult rabbits. It was found that mechanical stimulation of the glans of the clitoris and the perineal vagina induced electromyographic (EMG) responses in bulbocavernosus (Bcm) and ischiocavernosus (Icm) muscles. Unilateral and bilateral nerve transection indicated that the clitoral nerve is the main afferent path of the refle

Targeting OTUD7A-HINT1 deubiquitination activates mTOR signaling for CNS regeneration.

Axon regeneration in the central nervous system (CNS) remains limited, imposing severe constraints on functional recovery after injury. Here, we reveal that the deubiquitinase OTU deubiquitinase 7 A (OTUD7A) critically regulates CNS regeneration by modulating histidine triad nucleotide-binding protein 1 (HINT1) stability. OTUD7A stabilizes HINT1 protein through specific removal of K63-linked ubiquitin chains at lysine 7. Screening of the small-molecule deubiquitinase inhibitor PR-619 identified HINT1 as a key ubiquitination-regulated target. Notably, genetic knockdown of Hint1 alone was suffic

A novel approach to partial reduction of high-grade spondylolisthesis with offset rods - A report of two cases.

High-grade spondylolisthesis (HGS) remains a difficult entity to treat, given the high rate of complications following surgical correction of such a spinal deformity. Reduction of spondylolisthesis may be associated with traction injury to the L5 nerve root due to stretching. We report on two cases of HGS where a novel surgical technique of offset rods was used for partial reduction of spondylolisthesis thereby reducing L5 nerve root traction. Both cases had more than 50% correction of translation with good correction of slip angle and satisfactory clinical outcomes were achieved at the final

ZEB2: a multifunctional regulator of neural injury repair.

Nerve injury is a pathological condition characterized by damage or necrosis of nerve cells in injured areas due to trauma, infection, ischemia, genetic factor or other factors. Neural injury repair is precisely regulated by the complex regulatory network of body. Zinc finger E-box-binding protein 2 (ZEB2), known as a critical transcription factor, not only serves as a key participant in embryonic neural development, but also exerts a vital regulatory role in neural injury repair. This paper summarizes the structure, function, and regulatory network of ZEB2 and then elucidates its pivotal role

Proteomic analysis of synaptic protein turnover in the anterior cingulate cortex after nerve injury.

Synaptic proteins play an important role for the regulation of synaptic plasticity. Numerous studies have identified and revealed individual synaptic protein functions using protein overexpression or deletion. In neuropathic pain nociceptive stimuli conveyed from the periphery repetitively stimulate neurons in the central nerve system, brain and spinal cord. Neuronal activities change the turnover (synthesis and degradation) rate of synaptic proteins. Thus, the analysis of synaptic protein turnover rather than just expression level change is critical for studying the role of synaptic proteins

Transpedicular Approach for Ventral Epidural Abscess Evacuation in the Cervical Spine.

OBJECTIVE: Spinal epidural abscess may require prompt surgical intervention. Ventral cervical abscesses pose a particular challenge regarding the approach for surgical evacuation. The aim of this article was to describe the technical nuances of a posterior transpedicular cervical approach for evacuation of a ventral epidural abscess. METHODS: After a standard laminectomy, a foraminotomy was performed to identify the exiting nerve root. Then the medial aspect of the pedicle below the nerve was drilled. This allowed the insertion of a dissector to reach the ventral epidural space and drain the

Obturator Neuromodulation with Laparoscopic Placement of an Obturator Lead for the Treatment of Intractable Opioid-Dependent Chronic Pelvic Pain due to Obturator Neuralgia.

Chronic pelvic pain (CPP) is a common condition in women that can have a devastating effect on quality of life. Some of the most severe forms of CPP are related to peripheral nerve injuries, causing persistent neuropathic pain. We present a case of a young woman with severe opioid-dependent chronic pelvic and right groin pain due to obturator neuralgia. She had failed amultitude of treatments, including multiple medications, manual physical therapy, nerve blocks, surgical neurolysis, and spinal cord stimulation, without significant benefit. She underwent a trial of peripheral neuromodulation o

Genotype-phenotype correlation in Chinese patients with spinal and bulbar muscular atrophy.

Spinal and bulbar muscular atrophy (SBMA) is an X-linked recessive motor neuron disease characterized by slowly progressive weakness and atrophy of proximal limbs and bulbar muscles. To assess the genotype-phenotype correlation in Chinese patients, we identified 155 patients with SBMA and retrospectively examined available data from laboratory tests and neurophysiological analyses. Correlations between genotype and phenotype were analyzed. There was an inverse correlation between the length of CAG repeats and age at first muscle weakness (p<0.0001). The serum creatine kinase level showed a sig
