PURPOSE: Proximal instrumentation failure is a challenge in posterior spinal fusions (PSFs) crossing the cervicothoracic junction. High rates of proximal junctional kyphosis (PJK) and loss of fixation have been reported. In this single-center retrospective cohort study, we evaluate the utility of anterior cervical discectomy and fusion (ACDF) in addition to traditional PSF crossing the cervicothoracic junction in order to mitigate implant-related complications.
METHODS: All patients who underwent PSF across the cervicothoracic junction with ACDF with 2 years of follow-up data were reviewed. W

BACKGROUND: The use of modern rigid instrumentation in pediatric cervical fusions decreases the risk of implant-related complications, both acute and long term. However, previous studies have indicated that acute implant-related issues still occur in the adult population. Reports of pediatric acute implant complications, occurring within 3 months of surgery, are under-represented in the literature. The purpose of this study is to document the prevalence of acute implant-related complications in a pediatric cervical fusion population.
METHODS: A retrospective review of instrumented cervical fu

BACKGROUND: Limb-threatening lower extremity traumatic injuries can be devastating events with a multifaceted impact on patients. Therefore, evaluating patient-reported outcomes (PROs) in addition to traditional surgical outcomes is important. However, currently available instruments are limited as they were not developed specific to lower extremity trauma patients and lack content validity. The LIMB-Q is being developed as a novel PRO instrument to meet this need, with the goal to measure all relevant concepts and issues impacting amputation and limb-salvage patients after limb-threatening lo

OBJECTIVE: Assessment of physiological tremor during neurosurgical procedures may provide further insights into the composites of surgical expertise. Virtual reality platforms may provide a mechanism for the quantitative assessment of physiological tremor. In this study, a virtual reality simulator providing haptic feedback was used to study physiological tremor in a simulated tumor resection task with participants from a "skilled" group and a "novice" group.
DESIGN: The task involved using a virtual ultrasonic aspirator to remove a series of virtual brain tumors with different visual and tac

BACKGROUND AND OBJECTIVES: Vaginal cuff dehiscence may be a vascular-mediated event, and reports show a higher incidence after robot-assisted total laparoscopic hysterectomy (RATLH), when compared with other surgical routes. This study was conducted to determine the feasibility of using laser angiography to assess vaginal cuff perfusion during RATLH.
METHODS: This was a pilot feasibility trial incorporating 20 women who underwent RATLH for benign disease. Colpotomy was made with ultrasonic or monopolar instruments, whereas barbed or nonbarbed suture was used for cuff closure. Time of instrume

BACKGROUND: The management of lumbar foraminal/far lateral discs (FOR/FLD) with stenosis remains controversial. Operative choices should be based on each patient's preoperative dynamic X-ray findings, magnetic resonance (MR), and computed tomography (CT) studies. Here we reviewed several options for decompression alone vs. decompression with fusion.
METHODS: Safe excision of FOR/FLD with stenosis should begin at the level above the disc herniation, as identification of the superior, foraminally, and far laterally exiting nerve root is critical. Performing an undercutting laminectomy and utili

OBJECTIVE: A potential long-term complication of lumbar fusion is the development of adjacent segment disease (ASD), which may necessitate second surgery and adversely affect outcomes. The objective of this is to determine the incidence of ASD following instrumented fusion in adult patients with lumbar spondylolisthesis and to identify the risk factors for this complication.
PATIENTS AND METHODS: We retrospectively assessed adult patients who had undergone decompression and instrumented fusion for lumbar spondylolisthesis between January 2006 and December 2012. The incidence of ASD was analyz

BACKGROUND: The vacuum phenomenon within an intervertebral disc is not an uncommon radiographic finding in the elderly. However, no reports in the English literature have focused on the effect of an anterior vacuum disc in relation to surgical outcome of same-segment spondylolisthesis. We hypothesized that instrumented posterolateral fusion is not adequate in this situation and that additional interbody fusion with cages would provide better radiographic and clinical outcomes.
METHODS: The medical records of 72 patients who underwent instrumented fusion for one-segment degenerative lumbar spo

BACKGROUND: Geriatric cachexia is distinct from other age-related muscle wasting syndromes; however, detection and therefore treatment is challenging without the availability of valid instruments suitable for application in the clinical setting. This study assessed the sensitivity and specificity of a newly developed screening instrument utilising portable assessments against previously defined and commonly accepted diagnostic criteria for detection of geriatric cachexia.
METHODS: Cross-sectional analyses from 71 older adults' post-surgical fixation for hip fracture were performed. The diagno

More than one million people develop pressure ulcers (PU) every year in the US - a major problem that leads to increased morbidity and mortality. A hospital-acquired PU (HAPU) is defined as any ulcer noted 24 or more hours after hospital admission. The purpose of this retrospective study was to compare the incidence of HAPUs between two 1-year periods of time (March 1, 2007 through March 17, 2008 and March 18, 2008 through March 31, 2009) at an acute care facility. Records from all admitted adult medical-surgical and critical care patients along with routinely submitted quality

BACKGROUND: Vertebral artery injury (VAI) can be a devastating complication during cervical spine surgery. Although considered a rare occurrence overall, incidences of VAI have been reported in the ranges of 0.07% to 8%. Such injuries have the potential for catastrophic consequences, including blood loss, permanent morbid neurologic injury, and even death. The introduction of intraoperative navigation using either preoperative or intraoperative imaging has now been widely adopted in current practice so as to try and minimize adverse outcomes while giving real-time, dynamic information of the o

BACKGROUND: Posterior decompression, instrumentation, and posterolateral fusion are surgical procedures for the treatment of degenerative lumbar diseases. Solid fusion usually causes adjacent problems. This study investigated the clinical outcome and radiographic fate of the L5-S1 segment in patients who underwent posterior instrumented surgery for degenerative lumbar diseases.
METHODS: From January 1999 to December 2000, 181 patients (average age 59.4 years, range 45-79 years) underwent posterior decompression, posterior instrumentation, and posterolateral fusion for degenerative lumbar dise
