Early cranial ultrasound changes as predictors of outcome during first year of life in term infants with perinatal asphyxia.

OBJECTIVES: To identify the types of early cranial ultrasound changes that were significant predictors of adverse outcome during the first year of life in asphyxiated term infants. METHODOLOGY: This was a prospective cohort study. Shortly after birth, cranial ultrasonography was carried out via the anterior fontanelles of 70 normal control infants and 104 asphyxiated infants with a history of fetal distress and Apgar scores of less than 6 at 1 and 5 min of life, or requiring endotracheal intubation and manual intermittent positive pressure ventilation for at least 5 min after birth. Effects of parenteral cysteine and glutathione feeding in a baboon model of severe prematurity.

BACKGROUND: The availability of cysteine for glutathione synthesis is low in premature infants with respiratory distress. OBJECTIVE: The effects of gestational age, oxygen delivery, and cysteine infusion or glutathione infusion, or both, on plasma total cysteine and other methionine metabolites were studied in a baboon model of severe premature birth with respiratory distress. DESIGN: Premature baboons were studied as part of the multiinvestigator National Institutes of Health Collaborative Project on Bronchopulmonary Dysplasia. Premature baboons, 125 d (69% of term) or 140 d (78% of term) o

Hypoplastic left heart syndrome: duration of survival without surgical intervention.

BACKGROUND: Mortality rate for heart transplantation for patients with hypoplastic left heart syndrome (HLHS) has improved, but there is a considerable wait until a suitable donor is available. Thus it is important to examine the duration of survival and risk factors for early death in patients with HLHS who did not undergo surgical intervention. METHODS AND RESULTS: Twenty-six consecutive patients were studied retrospectively. Duration of survival and the 14 following variables were investigated: date of birth, body weight at birth, cardiothoracic ratio, ascending aorta diameter, interatrial

[Limit of antenatal management of intrauterine growth retarded fetus].

Recent advances in perinatal medicine contributed to the survival of extremely premature infants. Nonetheless, prognosis of the fetus with intrauterine growth retardation (IUGR) especially of preterm IUGR is not satisfactory with respect to both neonatal and long term neurodevelopmental outcome. As in utero pathophysiology of the IUGR has been revealed by noninvasive fetal monitoring technique along with invasive cordocentesis to determine fetal blood gas parameters, most of the neonatal morbidity of IUGR is thought to be originated prior to birth. Excluding the IUGR due to chromosomal abnorma

Comparing extracellular vesicle enriched plasma proteomes between term and preterm neonates over the first days of life: post hoc analysis of a prospective observational study.

BACKGROUND: While cardiopulmonary transition in neonates is well described, molecular changes which accompany it are not. The Extracellular Vesicles in Early preterm neonates and Thrombin generation (EVENT) study sought to explore molecular transition in preterm birth. How this differs from term-born neonates is not well described. METHODS: A post hoc evaluation of proteomic data generated from a subset of the prospective, single site observational EVENT study was performed comparing extracellular vesicle enriched plasma from term neonates (n=10) with preterm neonates (n=20) using a mass spec

Cerebral Blood Flow Indices During Immediate Skin-to-Skin Contact in Healthy Term Neonates: A Prospective Observational Study.

OBJECTIVE: To evaluate the effect of skin-to-skin contact (SSC) started immediately after birth on cerebral blood flow (CBF) using middle cerebral artery (MCA) Doppler and vital parameters in stable-term infants. METHODS: Clinically stable term newborns delivered through normal vaginal deliveries who only required routine care were included in this observational study through convenience sampling. Newborns with any congenital malformation or multiple gestation were excluded. Heart rate, oxygen saturation and CBF parameters of MCA [Peak systolic velocity (PSV), end diastolic velocity (EDV), me

PIEZO2-related distal arthrogryposis type 5: Longitudinal follow-up of a three-generation family broadens phenotypic spectrum, complications, and health surveillance recommendations for this patient group.

Arthrogryposis is a heterogenous condition with a wide variety of etiological causes. It has been subdivided clinically based on the presence of additional features. Dominant gain of function (GoF) pathogenic variants in PIEZO2 have been associated with several forms of arthrogryposis. Previous reports have focused on diagnosis and clinical features. We report a three-generation family with four affected individuals with a known pathogenic GoF change p.(Glu2727del) in PIEZO2. All family members presented at birth with distal arthrogryposis and ophthalmoplegia but have varied in their subsequen

Disruptions in the development of feeding for infants with congenital heart disease.

Congenital heart disease (CHD) is the most common birth defect for infants born in the United States, with approximately 36,000 affected infants born annually. While mortality rates for children with CHD have significantly declined, there is a growing population of individuals with CHD living into adulthood prompting the need to optimise long-term development and quality of life. For infants with CHD, pre- and post-surgery, there is an increased risk of developmental challenges and feeding difficulties. Feeding challenges carry profound implications for the quality of life for individuals with

Prediction of Neonatal Hyperbilirubinemia Using 1st Day Serum Bilirubin Levels.

The study was conducted on Full term neonates with birth weight > 2.5 kg born in KIMS, Hubballi with an objective to determine the first day Total Serum Bilirubin (TSB) value so as to predict subsequent development of significant hyperbilirubinemia in term neonates. All enrolled neonates were sampled for TSB and blood group on Day 1 at 20 ± 4 h and then followed up clinically by Kramer's rule and when the clinical jaundice by Kramer's rule was >10 mg/dl, TSB levels were repeated. A total of 180 newborns were enrolled for the study and 165 babies completed the study. Out of these, 17(10.3%) bab

Outcomes of infants with a birthweight less than or equal to 500 g in Northern England: 15 years experience.

AIM: We aimed to evaluate mortality and short-term neonatal morbidity of babies born ≤500 g cared for in the Northern Neonatal Network over a 15-year period. METHOD: Using regional databases, we identified all live-born babies ≥22 weeks gestation and ≤500 g, in North East England and North Cumbria from 1998 to 2012. We quantified major neonatal morbidities and survival to one year. RESULTS: We identified 104 live-born babies ≥22 weeks gestation and ≤500 g (birth prevalence 0.22/1000), of which 49 were admitted for intensive care. Overall one-year survival was 11%, but survival for those rece

Impact of fatty acid status on growth and neurobehavioural development in humans.

Many studies have been conducted to investigate the effect of n-3 long-chain polyunsaturated fatty acid (LCPUFA) supplementation during the perinatal period on the growth and neurobehavioural development of young children. Most of these intervention trials have involved infants from high-income countries, and a significant proportion have investigated supplementation of infant formulas. Generally, supplementation of infant formula for preterm rather than term infants has demonstrated more consistent, positive effects on aspects of neurobehavioural development, while the growth of both term and

Indicated preterm birth for fetal anomalies.

Between 2% and 3% of pregnancies are complicated by fetal anomalies. For most anomalies, there is no advantage to late preterm or early-term delivery. The risks of maternal or fetal complication are specific for each anomaly. Very few anomalies pose potential maternal risk. Some anomalies carry ongoing risks to the fetus, such as an increased risk of fetal death, hemorrhage, or organ damage. In a limited number of select cases, the advantages of late preterm or early-term birth may include avoiding an ongoing risk of fetal death related to the anomaly, allowing delivery in a controlled setting

Useless perinatal therapies.

With respect to Professor Bengt Robertson whose research studies were very instrumental in the development of one of the most important and evidence-based therapies in perinatal medicine, namely surfactant therapy, I thought it might be appropriate to review some useless or harmful perinatal therapies. Although the term 'neonatology' has only been in existence for about 50 years, care of the newborn in the 19th century was largely provided by nurses and obstetricians. As an example, Pierre Budin, a Parisian obstetrician, was very aware of the importance of keeping low birth weight babies warm.

[Effects of non-nutritive sucking on the physiological and behavioral states of pre-term infants during tube feeding].

PURPOSE: This study was to investigate the effects of non-nutritive sucking on physiological and behavioral state of pre-term infants during tube feeding. METHOD: This nonequivalent, non-synchronized experimental study included 50 pre-term infants. An attempt was made to match gestational age and birth weight of infants in each group. Infants in the experimental group were given a pacifier 2 minutes before, during, and for 2 minutes after tube feeding. Infants in the control group did not get a pacifier. Both groups were tested at three stages for changes in the physiologic state and behavior

Antenatal antecedents of moderate or severe neonatal encephalopathy in term infants--a regional review.

BACKGROUND: Regional audits of term infants with neonatal encephalopathy (NE) provide an opportunity to examine issues related to causation and quality of care. AIM: To document antenatal and intrapartum antecedents in a contemporary cohort of term infants with moderate or severe neonatal encephalopathy. METHODS: Term infants admitted with moderate-severe neonatal encephalopathy over 4 years were identified. The clinical records were reviewed for information about the pregnancy and birth including interpretation of monitoring and subsequent management of the labour and delivery. RESULTS: Fi

Are moderate degrees of hyperbilirubinemia in healthy term neonates really safe for the brain?

In 1994 the American Academy of Pediatrics recommended more liberal rules for the treatment of hyperbilirubinemia in healthy term newborns. Yet, the safety of moderate degrees of hyperbilirubinemia in healthy term newborns is debated. To evaluate the safety of moderate degrees of hyperbilirubinemia, we assessed neurologic condition of 20 healthy nonhemolytic term newborns with peak total serum bilirubin levels of 233-444 micromol/L and 20 control infants matched for sex and gestational age at birth. Neurologic condition was evaluated with techniques focusing on the presence of minor neurologic

Perinatal outcome of singleton term breech deliveries.

OBJECTIVE: To assess neonatal morbidity and mortality in singleton term infants delivered in breech presentation and to find a possible correlation between outcome and mode of delivery. STUDY DESIGN: Case study of 306 singleton, term (37-42 weeks), breech deliveries, that took place between 1989 and 1994 in one perinatal centre. RESULTS: 170 infants were delivered vaginally, 72 by elective and 64 by secondary cesarean section. Even after application of strict selection criteria -- i.e. prior pelvic assessment by staff obstetricians, an estimated birth weight of 2500-4000 g -- and with staff

Balloon catheter for induction of labor in nulliparous women with prelabor rupture of the membranes at term. A preliminary report.

The balloon catheter method has been described as an effective method for cervical ripening hitherto used exclusively before rupture of the membranes. We found it of interest to perform a pilot study of the balloon catheter method after rupture of the membranes. In 18 nulliparous women, with an unripe cervix (Bishop score < or = 5) 48 h after prelabor rupture of the membranes at term, cervical ripening was performed using a 26-gauge balloon catheter. Seventy six percent of these women delivered vaginally. Clinical and neonatal outcome data did not differ as compared with term pregnant women wh

Fetal pathology in human parvovirus B19 infection.

In a current Netherlands study on the effects on mother and child of infection with the human parvovirus B19 during pregnancy, 10 pregnancies have been reported. Three of them ended before term: two in fetal death and one by elective abortion. In two of these fetuses B19 infection in cells other than those of the erythroid series was demonstrated, and in the one terminated, ocular malformation and extensive inflammatory reactions in all fetal and placental tissues were found. The presence of B19 DNA was demonstrated by dot hybridization in placental and fetal tissues. In the third no gross fet
