Prophylactic high-flow nasal oxygen vs. conventional oxygen for prevention of postoperative respiratory complications: a randomized trial.

BACKGROUND: Postoperative pulmonary complications (PPCs) are a major cause of morbidity after lung resection, particularly in older adults. Following extubation, noninvasive respiratory support strategies are increasingly used to prevent respiratory deterioration, but the role of prophylactic high-flow nasal cannula (HFNC) in this setting remains uncertain. METHODS: In this single-center, open-label randomized controlled trial, patients undergoing video-assisted thoracoscopic surgery (VATS) lobectomy or segmentectomy for stage I-IIIa non-small cell lung cancer (NSCLC) were randomized to HFNC

Postoperative lactate trajectories predict in-hospital adverse outcomes and differ by surgical approach in cardiac surgery: a propensity score-matched study.

BACKGROUND: Totally thoracoscopic cardiac surgery (TTCS) employs peripheral cardiopulmonary bypass (CPB) with retrograde aortic perfusion, yet whether its net effect on early tissue perfusion is superior to open sternotomy remains uncertain. This study identified postoperative arterial lactate trajectory phenotypes, examined their association with surgical approach, and evaluated their prognostic value for in-hospital adverse outcomes. METHODS: A single-centre, retrospective, propensity score-matched cohort study enrolled 1,264 patients (632 per group). Group-based trajectory modelling (GBTM)

Extension of the minimally invasive pectus repair to adult patients: strategies for success.

Pectus excavatum (PE) is the most common congenital deformity of the anterior chest wall and is increasingly recognized as a clinically relevant condition in adult patients. While minimally invasive repair of pectus excavatum (MIRPE) is well established in pediatric and adolescent populations, early attempts to apply this technique in adults were associated with higher complication rates. These challenges were largely attributed to increased chest wall rigidity, more severe and asymmetric deformities, and greater mechanical forces acting on intrathoracic support bars. Over the past two decades

Thoracic endoscopic spine surgery: systematic review of the literature and exploring the margin of benefit.

BACKGROUND: Thoracic disc herniations are rare and challenging to treat surgically due to their complex regional anatomy, and conventional open approaches are associated with considerable morbidity. While full endoscopic approaches have demonstrated success in the cervical and lumbar spine, their role in the thoracic spine is not as well defined. We performed a systematic review of the literature on full endoscopic surgery for thoracic disc herniations in order to elucidate its safety, efficacy, and potential benefit over conventional open approaches. METHODS: The PubMed, Web of Science, and

Pathological complete response following neoadjuvant chemoimmunotherapy in cancer of unknown primary: a case report.

BACKGROUND: Cancers of unknown primary (CUP) are a heterogeneous group of metastatic malignancies in which a primary tumor cannot be identified despite comprehensive diagnostic evaluation. Most CUP patients have a poor prognosis, and only 15-20% can be classified into favorable subgroups. Although molecularly guided therapy (MGT) has shown survival benefits in global phase II studies, the overall outcomes remain unsatisfactory. In particular, intrathoracic lymph node metastasis of unknown origin represents a clinical dilemma, with limited evidence guiding treatment decisions. CASE DESCRIPTION

A 58-Year-Old Woman's Odyssey With Bilateral Nodular Opacities Comes to a Diagnosis.

Benign metastasizing leiomyoma (BML) is an uncommon entity in which histologically benign smooth muscle tumors appear at distant sites, most frequently the lungs, in women with a history of uterine leiomyoma treated surgically. Despite its benign pathology, BML often presents radiologically in a pattern that mimics metastatic malignancy, resulting in substantial diagnostic uncertainty. Awareness of this condition is essential for avoiding unnecessary or overly aggressive interventions. We describe the case of a 58-year-old woman and active smoker with a significant 40-pack-year history who was

Situs inversus totalis and lung cancer: a case report of surgical resection after neoadjuvant chemoimmunotherapy for stage IIIB squamous cell carcinoma.

Immune checkpoint inhibitors (ICIs) have emerged as a promising therapeutic strategy for lung cancer. This case report describes a patient with situs inversus totalis (SIT) and initially unresectable, advanced lung squamous cell carcinoma (LSCC) who successfully underwent surgical resection following neoadjuvant therapy with pembrolizumab and chemotherapy. The patient was incidentally found to have SIT and a 6.5 × 5.0 cm mass in the right upper lobe (RUL) with hilar and mediastinal lymphadenopathy on a local chest CT scan. PET-CT scan revealed the RUL lesion (approximately 6.6 cm, SUVmax 13.7)

Impact of patient-specific three-dimensional reconstruction on surgical planning and perioperative outcomes in early-stage lung cancer: a randomised clinical trial.

BACKGROUND: Anatomical segmentectomy requires precise identification of the segmental bronchi and vessels; however, conventional two-dimensional (2D) CT provides limited visualisation of their three-dimensional (3D) anatomical relationships and variations. METHODS: In this prospective randomised controlled trial, patients with early-stage non-small cell lung cancer (NSCLC) who were scheduled to undergo uniportal thoracoscopic anatomical segmentectomy were assigned to either the 2D CT group or the 3D reconstruction group. The trial was registered with the Chinese Clinical Trial Registry before

Catastrophic Esophageal Tumor Perforation within One Week after First-Line Chemo-Immunotherapy for Advanced Esophageal Squamous Cell Carcinoma: A Case Report.

INTRODUCTION: Esophageal perforation secondary to advanced esophageal squamous cell carcinoma (ESCC) is a life-threatening oncologic emergency that can rapidly progress to mediastinitis, empyema, and septic shock. With the increasing use of first-line chemo-immunotherapy, early tumor necrosis-particularly in ulcerated or deeply invasive lesions-may precipitate catastrophic perforation. CASE PRESENTATION: A man in his 70s was diagnosed with unresectable middle thoracic ESCC with distant metastases to the iliopsoas muscle and left supraclavicular lymph node (cT3brN2M1b, cStage IVB). First-line

Surgical Treatment of Tuberculous Pleural Empyema in Children and Adolescents: A Retrospective Cohort Study.

BACKGROUND: While non-specific pleural empyema in children can be successfully managed with conservative drainage and thoracentesis in 80-84% of cases, tuberculous pleural empyema (TB empyema) exhibits high resistance and frequently requires extensive thoracic surgery. The choice of surgical scope depends directly on disease stage and secondary complications. METHODS: We conducted a retrospective cohort study of 85 pediatric patients (aged 4 to 17 years) with confirmed TB empyema treated between 1984 and 2022, and followed until 2025. Disease staging followed a predefined adaptation of the Am

Does surgical margin affect recurrence and survival after pulmonary segmentectomy for cT1 lung cancer?

BACKGROUND: During segmentectomy for early-stage non-small cell lung cancer (NSCLC), it is advised that the tumor be at least 1 cm away from the intersegmental plane, with a margin-to-tumor (M/T) ratio of at least 1. This study aims to investigate how surgical margin influences the risk of recurrence and overall survival (OS) following pulmonary segmentectomy. METHODS: This is a single-center study including consecutive patients who underwent pulmonary segmentectomy for cT1N0M0 NSCLC between January 2017 and December 2022. Surgical margin distance and M/T ratio were measured on deflated lung

Early versus late chest tube removal after surgery for primary spontaneous pneumothorax-a systematic review and meta-analysis.

BACKGROUND: The optimal timing of postoperative chest tube removal remains disputable. Traditionally, chest tubes are left in place for several days for adequate pleurodesis and recurrence prevention after surgery for primary spontaneous pneumothorax (PSP). Currently, early tube removal, defined as immediate removal upon cessation of air leakage, is increasingly practiced. This study aimed to evaluate the safety of early chest tube removal in terms of recurrence in these patients. METHODS: MEDLINE (PubMed), EMBASE and Cochrane databases were searched until January 2024. Inclusion criteria enc

Hybrid Atrial Fibrillation Ablation: A Decade-Long Single-Center Experience.

BACKGROUND: Atrial fibrillation (AF) represents a major public health burden, especially in its long-standing persistent form, which is often resistant to pharmacological or catheter-based therapies. Hybrid ablation, which integrates minimally invasive surgical and endocardial catheter techniques, has been introduced to address these complex cases. However, data evaluating the long-term comparative effectiveness of immediate versus staged ablation strategies remain limited, and the specific contribution of adjunctive targets, such as Bachmann's bundle (BB), remains unclear. METHODS: In this s

A Case of Legionella Pneumonia Suspected as Typical Pneumonia After Lung Cancer Surgery, Diagnosed by a Repeat Urinary Legionella Antigen Test.

Legionella pneumophila is a leading cause of severe atypical pneumonia and is associated with high mortality when initial treatment is inadequate. The urinary Legionella antigen test serves as a rapid, first-line diagnostic tool; however, early results may be negative, and antigen shedding can be intermittent. In these situations, timely retesting is essential, especially in postoperative or high-risk patients. We report the case of a 74-year-old man, a former smoker with diabetes and moderate aortic stenosis, who underwent thoracoscopic left lower lobectomy and was discharged on postoperative

Electromagnetic navigation bronchoscopy for localization of bilateral multiple pulmonary nodules: a comparative evaluation of safety and efficacy.

BACKGROUND: Lung cancer frequently presents with bilateral multiple pulmonary nodules (BMPNs), whose accurate intraoperative localization during surgery is challenging, making reliable preoperative localization essential. This retrospective study evaluates the safety and efficacy of electromagnetic navigation bronchoscopy-guided dye marking (ENBDM) compared with conventional computed tomography-guided percutaneous dye marking (CTPDM) for localizing BMPNs. METHODS: A retrospective cohort study was conducted on patients with BMPNs who underwent preoperative localization (either ENBDM or CTPDM)

Oncologic outcomes and predictors of recurrence following minimally invasive and open surgery for thymoma.

BACKGROUND: Thymomas are rare neoplasms of the mediastinum with an incidence of 0.15 per 100,000 person-years but represent the most common tumor of the anterior mediastinum in adults. Complete surgical removal is the mainstay treatment for thymomas. While minimally invasive surgeries (MIS) for thymoma are increasing, it remains unknown if MIS is oncologically equivalent to traditional open surgical techniques (OTs). The aim of the present study was to characterize outcomes and identify risk factors associated with recurrent disease in patients undergoing surgery for early-stage thymoma, with
