Buccal bifurcation cyst in a 7-year-old: surgical management and 14-month follow-up.

BACKGROUND: The buccal bifurcation cyst (BBC) is a paradental cyst that typically presents on the buccal aspect of mandibular permanent first molars in children 6 to 11 years of age. Histopathologic features are non-specific and closely resemble other inflammatory odontogenic cysts. The aim of this article is to report a case of a BBC and to review the management of such lesions. METHODS: A 7-year-old white male, complaining of lower right tooth pain and swelling, presented to the Nationwide Children's Hospital Dental Clinic. A clinical examination revealed no caries and swelling localized to

Radiographic examination of dentigerous cysts in the transitional dentition.

OBJECTIVES: To examine radiographically the relationship between the deciduous tooth and dentigerous cyst of the permanent successor during the transitional dentition. METHODS: From a retrospective review of all patients who visited our institution from April 1988 to August 2001, 70 patients under 16 years of age who had histologically confirmed dentigerous cysts that had developed from the central incisor to the second premolar were identified. These 70 patients were investigated using panoramic and periapical radiographs. RESULTS: In most cases (54 cases; 77.1%) the cyst was in the premola

Influence of the maxillary canine on mandibular fracture.

The length of the root of the mandibular canine tooth has been considered by many authors as being a source of weakness in the mandible. It has also been suggested that a direct blow or a bending force around this tooth can result in traumatic injury. We advance a theory that implicates the maxillary canine tooth as directly contributing to the mandibular canine region fracture pattern.

Bisphosphonate-associated osteonecrosis of jaw reoccurrence after methotrexate therapy: a case report.

INTRODUCTION: Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is a well-known complication caused by amino-bisphosphonate therapy. We document one case of BRONJ associated with oral administration of methotrexate, a known immunosuppressive drug used to treat rheumatoid arthritis. METHODS: A 66-year-old woman was referred for evaluation and endodontic surgery of recently re-treated tooth 13. Tooth 14 was extracted 3 months prior, and the extraction site had not completely healed. Her medical history revealed rheumatoid arthritis and osteoporosis. She had been taking Fosamax (alendronat

Actinomyces endogenous endophthalmitis in a cat following multiple dental extractions.

An 8-year-old, brachycephalic, mixed breed cat underwent full mouth tooth extractions for the treatment of tooth root abscessation. Subsequently, the cat developed anterior uveitis refractory to topical therapy that eventually necessitated enucleation. Actinomyces species were isolated from both the tooth root abscesses and the anterior chamber after enucleation. Histopathology of the enucleated eye revealed panophthalmitis with abundant intralesional bacteria morphologically consistent with Actinomyces. Between the time of tooth root extraction and enucleation (20 weeks), the cat was diagnose

In vitro evaluation of a method for obtaining periapical radiographs for diagnosis of external apical root resorption.

This in vitro study was conducted to evaluate a method to obtain reproducible periapical radiographs, using individualized positioning devices, of upper central incisors submitted to simulated orthodontic movement. Linear measurements of tooth length were carried out on 29 extracted human central incisors. The teeth were radiographed in a tissue simulator, at different inclinations in the bucco-lingual direction (0 initial, 0 control, -10, -5, +5, and +10 degrees). The radiographs were digitized and quantitative analysis of tooth length was carried out using an electronic ruler. The linear mea

Common dental emergencies.

Dental caries, a bacterial disease of teeth characterized by destruction of enamel and dentine, is often the underlying cause of dental pain. When a carious lesion impinges on the dental pulp, pulpitis follows and, ultimately, necrosis of the pulp occurs. Untreated necrosis may lead to a localized abscess or a spreading infection into the surrounding soft tissue that results in cellulitis. Immediate treatment involves antibiotic therapy for cellulitis, perhaps with drainage of abscesses, while definitive treatment requires root canal therapy or extraction of the involved tooth. Pericoronitis i

Influence of the number of teeth on the cognitive function of older adults: an intermediary analysis based on dietary diversity.

BACKGROUND: Tooth loss has been associated with cognitive function in older adults. We aimed to explore the mechanism between tooth loss and cognitive decline in these specific populations, which has been scarcely studied. METHODS: This study used the longitudinal data of the Chinese Longitudinal Healthy Longevity Survey (CLHLS) to build a linear mixed effects model to explore the impact of tooth loss on cognitive function in older adults. The effect of wearing dentures on the above effects was also evaluated. In addition, through mediation analysis, this study studied the mediating role of d

Matrix-assisted laser desorption/ionization imaging mass spectrometry revealed traces of dental problem associated with dental structure.

Periodontal disease is a serious dental problem because it does not heal naturally and leads to tooth loss. In periodontal disease, inflammation at periodontal tissue is thought as predominant, and its effect against tooth itself remains unclear. In this study, we applied matrix-assisted laser desorption/ionization imaging mass spectrometry (IMS) to teeth for the first time. By comparing anatomical structure of tooth affected with periodontal disease with normal ones, we analyzed traces of the disease on tooth. We found signals characteristic of enamel, dentin, and dental pulp, respectively, i

Conservative management of an accidentally resected root: a 3-year follow-up.

Despite all the effort by dentists to provide high levels of dental care in clinical practice, there is always the possibility of accidents occurring. This report describes a referred case that presented with irreversible pulpitis in tooth #19. The situation developed after accidental resectioning of the distal root of tooth #19 during the surgical extraction of tooth #18 by another dentist. The resultant open apex in the distal root of tooth #19 was managed conservatively. A positive apical stop was prepared at the root end of the distal root and sterile calcium hydroxide powder was used to c

Image accuracy of plain film radiography and computerized tomography in assessing morphological abnormality of impacted teeth.

This study compares the image accuracy of computerized tomography (CT) with that of plain film radiography (PFR) in analyzing the 3-dimensional shape of impacted teeth suspected of being malformed. Eighteen patients with 22 malformed teeth were studied by PFR and CT. Crown shape, root shape, crown/root relationship, and tooth inclination and structure were assessed by PFR and CT. CT was found to be superior to PFR in showing the multiplanar shape of tooth crown and root, crown/root relationship, and tooth inclination (P <.001). No difference was found in the representation of tooth structures.

Effects of prenatal malnutrition on development of oral tissue in neonatal rats.

The effects of prenatal malnutrition on the growth of incisor and molar tooth germs and tongue were investigated in newborn rats. Compared with the results on the young of well-nourished mothers, the young of the malnourished animals had fewer and smaller cells in the molar tooth germs and fewer cells in incisor tooth germs. The tongue of the malnourished animals had fewer but larger cells. 45Ca uptake studies and alkaline and acid phosphatase activities suggested that prenatal malnutrition affected incisor and molar tooth germs in different ways. Alkaline phosphatase activity in the tongue di

Management of intrusive luxation in the primary dentition by surgical repositioning: an alternative approach.

Intrusive luxation is the most common trauma during early infancy which results in the displacement of the tooth into its alveolus. Depending on the severity of the intrusion, the American Academy of Pediatric Dentistry recommends either extraction or spontaneous re-eruption for the primary tooth. This case report provides a brief insight into surgical repositioning as an alternative treatment option for the management of intruded primary tooth in a 4-year-old girl.

Primary failure of eruption: a case report.

The long-term effect of extracting infra-occluded (submerged) first permanent molars on the subsequent movement of the second permanent molars was studied in a member of a family with previously noted primary failure of tooth eruption. The case is described with the use of radiographs and diagrams and the conclusion reached that in some pathological conditions tooth movement takes place in an apical direction, an observation that may be termed 'paradoxical' tooth movement.

Study of secondary dentine deposition in central incisors as an age estimation method for adults.

This work aimed to assess the pulp/tooth area ratio's utility in the upper central incisors using orthopantomograms. A convenience sample of 801 adult patient orthopantomograms was studied. Image J® software was used to measure the pulp/tooth area ratio, and a regression model was developed. Our results conclude that the methodology assessing upper incisors' pulp/tooth area ratio using orthopantomograms can lead to age overestimation and statistically significant differences between chronological and estimated age. For those over 50, no correlation between pulp/tooth area ratio and chronologic

Bacterial meningitis after dental extraction in a 17-year-old horse.

Dental extractions in horses may result in bacteremia, which can lead to systemic complications. Bacterial meningitis following oral cheek tooth extractions in a 17-year-old Thoroughbred gelding is described in this report. The bacterial meningitis was confirmed by histopathology. The gelding was presented for evaluation of intermittent fever, loose feces, and mild colic signs which started 5 days after cheek tooth extraction. This case illustrates a rare complication associated with oral tooth extraction in a horse and highlights the unusual presenting features of meningitis. Key clinical mes
