We report a case of myiasis of a rectocutaneous fistula in a subject suffering from bone metastases after surgical resection of a neoplastic bladder. The fistula was infested by maggots identified as third-instar larvae of flies belonging to the family Sarcophagidae, genus Sarcophaga. The infestation was neither intestinal nor pseudointestinal; it was probably caused by a fly ovipositing on the fistula of the patient while having wound care at home, even though the possibility that the infestation might have occurred during hospitalization cannot be ruled out.

Every year in Germany approximately 12,000 people are afflicted by superficial carcinoma of the urinary bladder. In addition, the number of recurrences is not inconsiderable. The goal of optimal therapy, therefore, is not only to cure the patients but also to lower the high rate of recurrence by appropriate prophylaxis. After transurethral resection, treatment options include various topical modalities of instillation therapy, e.g., chemotherapy or immunotherapy. The Lambert-Eaton myasthenic syndrome is known to occur with, or precede, a variety of malignancies, most commonly oat cell carcinoma of the lung. We report the first case of this syndrome associated with transitional cell carcinoma of the bladder and ureter. A brief review of published reports on the presentation, diagnosis, and treatment is included.

The clinico-pathological features of nine urethral and urinary bladder polyps with prostate-type epithelium are described. The average age of the patients was 46 years. Thirty three patients with carcinoma in situ (CIS) or/and numerous recurrences of superficial bladder tumor were treated with intravesical BCG after transurethral resection. 63% of patients with CIS were free of recurrence after two years, two underwent cystectomy and one died because of progression. A 61-year-old man presented with macroscopic haematuria. Infiltrating transitional cell carcinoma of the urinary bladder was diagnosed after transurethral resection, and he was treated first with three doses of cisplatinum (100 mg/m2 at three week intervals) and then by radical cystectomy. Eleven months later he complained of progressive diplopia, which was found on computed tomography to be caused by a retro-orbital metastatic mass. There was no evidence of a space occupying lesion in the brain or of other metastases. The patient died four weeks later and permission for necropsy was refused.

This is a case report of a 67-year-old woman who had a mucinous adenocarcinoma of the urethra: she died 3 years after the clinical symptoms started owing to progression of the tumor. The tumor developed from a glandular metaplasia, which has to be regarded as a premalignant lesion. Problems of the diagnosis, including the differential diagnosis, are discussed.

We analyzed 29 cases of eosinophilic cystitis in a group of 80 patients with lower urothelial carcinoma who had been treated with mitomycin-C. Only 6 cases (20.7%) had presented symptoms and signs compatible with acute cystitis. The remaining patients had been diagnosed histopathologically by routine follow-up bladder biopsy. Urine cytology revealed abundant eosinophils in 58.6% of the patients with eosinophilic cystitis while urine culture was negative for all patients. No patient had a previous history of allergy or asthma. We discuss the possible etiopathogenic mechanisms of this rare nosol

A complete remission of widely metastatic bladder carcinoma was obtained with chemotherapy in a 41-year-old man. Severe persistent anorexia led to a 33% weight loss. The anorexia correlated with an elevated sucrose recognition threshold. Tumor-to-tumor metastases are uncommon. The most frequent donor tumor is lung cancer, while renal cell carcinoma (RCC) is by far the most common recipient. In this report, a carcinoma of the uterine cervix metastasizing to an RCC and a urothelial carcinoma of the urinary bladder metastasizing to a solitary fibrous tumor of the pleura are described. No similar cases have been found in the accessible literature. These cases are discussed and the findings are correlated with the data of the literature.
