A 65-year-old female patient with chronic kidney disease stage 5 and a history of spleen neoplasm with dissemination within peritoneum is presented. During 5 years of hemodialysis therapy, bilateral occlusion of brachiocephalic and iliac vein developed as a consequence of vein catheterization. An attempt to cannulate inferior vena cava was unsuccessful. A cannulation of dilated collateral abdominal veins with dialysis needles allowed to perform several hemodialysis sessions in the patient.

Tumors arising from chromaffin cells, which are derived embryologically from neural crest cells, are known as pheochromocytomas or paragangliomas. These tumors in the retroperitoneum are usually located in close proximity to the aorta and inferior vena cava. Radical surgical resection is the treatment of choice for these tumors. We present a rare case of a postmenopausal woman who presented to us with a symptomatic giant retroperitoneal mass situated in the interaortocaval region that we managed with surgical resection.

BACKGROUND: Venous thromboembolic disease rarely presents as a complication of uterine leiomyomata via inferior vena cava compression.
CASE REPORT: We describe a case associated with venous thrombotic disease and subsequent pulmonary emboli and discuss the complexities of Emergency Department (ED) management.
CONCLUSION: This case typifies the challenge of caring for the ill patient with multiple, potentially related disease processes whose treatments may actually conflict with one another.

A case is described where intracardiac thrombus was observed at the junction of the superior vena cava and the right atrium during attempted closure of an atrial septal defect (ASD) using a device similar in design to the Amplatzer septal occluder (Heart R atrial septal occluder). Thrombosis was likely related to the multiple attempts required when deploying the device and the attendant prolonged procedural time. The patient underwent subsequent urgent surgical closure of the ASD.

A case of a 53-year-old male with a long-lasting pulmonary hypertension of an unknown cause is presented. Both transthoracic and transesophageal echocardiography as well as cardiac catheterisation failed to document any cardiac defect. Finally, a multi-slice spiral computerised tomography revealed the presence of an anomalous pulmonary vein drainage to the superior vena cava with concomitant inter-atrial septal defect of a sinus venosus type. The patient was selected for surgery.

Until recently, situs inversus was considered to be an absolute contraindication for liver transplantation. However, recent reports have suggested that situs inversus should not be considered a contraindication. This study presents a successful living donor liver transplantation performed in a 4-month-old male infant with biliary atresia and situs inversus. The surgical findings revealed abdominal situs inversus with polysplenia and an absent retrohepatic inferior vena cava and intestinal malrotation.

Iatrogenic injury to the iliac vein or inferior vena cava (IVC), which may occur during abdominal operations or posterior orthopedic procedures, can have devastating consequences. Operative management is challenging and may be associated with significant morbidity. The authors report herein 3 cases of iatrogenic pelvic vein injuries that were managed with different treatment approaches. Both traditional open surgical therapy and endovascular techniques are described.

Ultrasound imaging used to facilitate performance of a femoral nerve block also affords imaging of adjacent anatomical structures. Following a fracture of the femur, an ultrasound guided femoral nerve block (UGFNB) was performed to provide analgesia; this led to the incidental finding of a previously undiagnosed femoral vein thrombosis (DVT), resulting in a change in patient management before surgery. An inferior vena cava (IVC) filter was placed before intramedullary nailing of the fracture.

A 38 years female presented with three episodes of venous thrombosis over one year--first in left femoral vein, next in splenic vein causing haematemesis and malena and lastly in inferior vena cava causing Budd-Chiari syndrome. During third episode, endoscopic cholangiopancreatography and guided biopsy established a diagnosis of cholangiocarcinoma at the mid portion of common bile duct. The disease was far advanced with cervical lymph node metastasis.

Subclavian and superior vena cava obstruction complicating SAPHO (Synovitis, Acne, Pustulosis, Hyperostosis, Osteitis) syndrome has been described. We report the first case to our knowledge of iliac vein thrombosis complicating lumbar vertebral osteitis due to SAPHO syndrome. Lumbar MRI demonstrated a large tissue mass anterior to the involved vertebras and surrounding the right iliac vein. Histology of the mass showed aseptic inflammation.

Leiomyosarcoma is an infrequent soft-tissue tumor that generally occurs in the myometrium and gastrointestinal tract. Primary tumors of the veins are unusual and generally occur in the inferior vena cava and only exceptionally in peripheral veins, such as the jugular or its branches. We report a case of leiomyosarcoma located in the left inferior turbinate. The 87 year-old male patient presented nasal obstruction and recurrent nosebleed. He had pulmonary and mediastinal metastases at the time of diagnosis.

An unusual case of primary mediastinal echinococcosis in a young female presented with non-specific symptoms. On examination, she had signs of Horner's syndrome and mild superior vena cava compression. She was operated successfully. The final diagnosis could only be made on the operating table and confirmed by histopathology. Hydatid cyst in mediastinum is uncommon but because of surrounding vital structures and potential of its complete cure should be explored without delay.

A case of massive upper gastrointestinal haemorrhage from proximal esophageal varices is reported. Upper esophageal varices occur rarely, and serve as collaterals in conditions of superior vena cava obstruction. In our patient, cervical esophageal varices were associated with portal hypertension due to occult liver cirrhosis. As the diagnosis was made at autopsy, this case serves to stress the importance of a through examination of the cervical portion of the esophagus during routine endoscopy.

A child with heterotaxia, azygous continuation of an interrupted inferior vena cava, single ventricle, and pulmonary atresia underwent the Kawashima modification of the Fontan procedure, which excluded hepatic venous return from the pulmonary blood flow. After the operation, the patient had development of pulmonary arteriovenous malformations, increasing cyanosis, and ventricular dysfunction. He underwent orthotopic heart transplantation at 7 years of age with prompt resolution of the pulmonary arteriovenous malformations.

The protein C anticoagulant pathway is an important downregulating mechanism of the blood coagulation cascade. We report a 47-yr-old male with Budd-Chiari syndrome and occlusion of the inferior vena cava. His plasma protein C antigen level was decreased to 42%, and its activity was 34%. Other coagulable factors were normal. Protein C deficiency should be considered a possible etiological factor of the Budd-Chiari syndrome.

The prevention and therapy of upper digestive hemorrhage due to rupture of esophageal varices in patients with liver cirrhosis are not yet effective enough. For their improvement, a transjugular intrahepatic portosystemic shunt (TIPS) is achieved by a new method which, without requiring general anesthesia, creates a shunt between a portal vein branch and the inferior vena cava. The indications, contraindications, outcome and eventual accidents of TIPS are analysed in the light of the latest data in the medical literature.

The clinical picture of 60 cases of primary liver cancer is reported. Distended abdominal veins found in a substantial number of cases, often point to collateral circulation due to a membrane in the inferior vena cava (budd-Chiari syndrome). Alpha-foetoprotein, alkaline phosphatase and gamma-glutamyltranspeptidase were the only reliable laboratory pointers to the disease. Ultrasound study proved to be very helpful. Thrombocytopenia was found in 5 out of 12 of our patients.

Balloon catheters with expansion capabilities from 1 mm to 6 cm have been developed. They feature torque control and can be selectively positioned within vessels as easily as the ordinary diagnostic angiographic catheters. The balloons can be utilized for hemorrhage control, conventional embolectomy, operative angiography, vena cava occlusion, and purposeful vascular thrombosis or dearterialization. In the event of unsuccessful control, the balloon can be readily deflated and removed within minutes.

A 27-year-old male patient presented with new thrombotic areas and undiagnosed partially old thrombosis of both vena femoralis, vena iliaca externa, vena iliaca communis and the vena cava inferior. He reported episodes of orchitis, recurrent oral aphthous ulcers, mild arthritis of several joints, recurrent iridocyclitis and occasionally skin pustules. The diagnosis of Adamantiades-Behçet's disease was corroborated by a positive pathergy test. Already in 1946, Benediktos Adamantiades described vascular thrombosis and later included it to his diagnostic criteria as the fourth cardinal sign of th

Fetal ultrasound studies were performed on 24 fetuses with non-immune hydrops to evaluate echocardiographic and cardiovascular Doppler parameters that may be useful in assessing hemodynamics and in predicting outcome. Of all cardiovascular parameters analyzed, only the presence of abnormal pulsations in the umbilical vein (p < 0.001) was found to be significantly different between the 11 survivors and 13 non-survivors. In a smaller subset of 12 fetuses, in whom inferior vena caval waveforms were recorded, survivors (n = 6) had a significantly lower percentage of retrograde flow in the inferior
