Current landscape for outcome prediction in patients with cardiogenic shock.

Cardiogenic shock (CS) is a critical state of primary cardiac dysfunction characterized by tissue hypoperfusion and cellular hypoxia, with in-hospital mortality rates persistently between 30% and 50%. This review synthesizes contemporary evidence to redefine risk stratification through a dynamic, multi-modal lens, moving beyond static snapshots to prioritize longitudinal assessment aimed at anticipating clinical instability, therapeutic response, or physiological recovery. The evolution of CS staging is evaluated, specifically the Society for Cardiovascular Angiography and Interventions (SCAI)

Ultrastructural analysis of transjugular biopsies in MASLD reveals core hepatocellular stress and cytoskeletal adaptations.

BACKGROUND AND AIMS: Electron microscopy (EM) has provided key insights into hepatocellular alterations in MASLD, but most studies relied on percutaneous biopsies and primarily focused on selected features such as megamitochondria or autophagic vesicles. We aimed to evaluate the feasibility of ultrastructural analysis in transjugular biopsies and to provide a comprehensive description of hepatocellular changes in patients with MASLD. METHODS: Patients with MASLD, well characterized by hepatic venous pressure gradient (HVPG) and stratified by fibrosis stage, portal hypertension, and MASH statu

Pulmonary Comorbidities in Children and Young Adults with Fontan Circulation: The Limitations of General Cardiology Clinics Compared with Dedicated Fontan Clinics.

Pathogenesis of pulmonary comorbidities post-Fontan surgery is multifactorial, including long-term, non-pulsatile blood flow, history of multiple sternotomies, abnormal airway lymphatics, associated scoliosis, and recurrent laryngeal nerve or vagal nerve injury as complications of cardiac surgery. A retrospective, cross-sectional analysis of 128 patients under the age of 30 years with a single ventricle status post-Fontan procedure. These patients were evaluated in our multidisciplinary Fontan clinic. Variables relating to demographics, pulmonary diseases, pulmonary function testing measuremen

Impact of early CVP monitoring on 1-year mortality in patients with congestive heart failure in the ICU: a retrospective analysis based on the MIMIC-IV2.2 database.

BACKGROUND: Central venous pressure (CVP) monitoring is critical for fluid management in critically ill patients. This study evaluated the impact of CVP monitoring on 1-year mortality in intensive care unit (ICU) patients with congestive heart failure (CHF). METHODS: Data from the Medical Information for Critical Care IV (MIMIC-IV) database were analyzed for ICU patients admitted for the first time with a stay > 24 h. Patients were categorized into CVP and no-CVP groups based on CVP measurement. Logistic regression analyses were performed, with propensity score matching (PSM) and overlap weig

Predictivity of Each Readiness-to-Explant Criterion for Successful Venoarterial Extracorporeal Membrane Oxygenation Explantation.

BACKGROUND: The use of temporary mechanical circulatory support (tMCS) has revolutionized the management of cardiogenic shock (CS). However, standardized readiness-to-explant criteria for venoarterial extracorporeal membrane oxygenation (VA-ECMO) have not been established. METHODS AND RESULTS: We performed a retrospective analysis of 37 patients with CS who were explanted from VA-ECMO at Okayama University Hospital from December 2018 to May 2024 to evaluate the diagnostic performance of each readiness-to-explant criterion for explant success or failure. Explant success was defined as 30-day s

Hemodynamic Impact of Mechanical Ventilation in Anesthetized Patients With Cavopulmonary Connection: A Physiologic Crossover Study.

OBJECTIVES: In patients with single-ventricle hearts and cavopulmonary connection, both pulmonary and systemic blood flow rely on passive venous drainage through the lungs. Conventional wisdom teaches that spontaneous breathing, as opposed to positive-pressure mechanical ventilation, enhances cardiac output in these patients. However, the hemodynamic effects of spontaneous versus mechanical ventilation under general anesthesia have not been systematically studied. DESIGN: A prospective, crossover, single-center clinical study. SETTING: The cardiac catheterization laboratory, Children's Hospi

Innovative use of PreserFlo microshunt in an infant with Sturge-Weber syndrome-related glaucoma: Case report.

INTRODUCTION: Sturge-Weber syndrome (SWS) is a rare congenital disorder characterized by leptomeningeal angiomas, port-wine stains, and glaucoma, affecting up to 70 % of pediatric cases. Elevated episcleral venous pressure disrupts aqueous outflow, leading to a persistently high intraocular pressure (IOP). Conventional treatments, such as trabeculectomy and Ahmed valve implantation, often fail because of fibrosis, hypotony, and vascular anomalies. The PreserFlo MicroShunt, a minimally invasive glaucoma surgery device, offers controlled IOP reduction with fewer complications; however, its use i

COVID-19-induced extracorporeal circulation coagulation during continuous renal replacement therapy: A cross-sectional study.

After the control policies of the COVID-19 epidemic were lifted in China from December 5th, 2022, there was an increase in the demand for hemodialysis and continuous renal replacement therapy (CRRT) at our center, and patients experienced hypercoagulable blood states more frequently. This study aimed to investigate the effect of COVID-19 on extracorporeal coagulation during CRRT. All CRRT records were gathered from the Hemodialysis Center at our hospital from December 5th, 2022 to February 4th, 2023, and analyzed the incidence and risk factors associated with extracorporeal coagulation. High plasma aldosterone concentration is associated with worse 24-h ambulatory blood pressure profile in patients with primary aldosteronism.

Patients with primary aldosteronism (PA) have a higher risk of cardiovascular disease (CVD) than essential hypertension due to underlying hyperaldosteronism. However, the association between high plasma aldosterone concentrations (PACs) and diurnal blood pressure (BP) variation has not been fully elucidated. Because abnormal ambulatory blood pressure monitoring (ABPM) profiles are associated with increased CVD risk, we investigated the association between PACs and the ABPM profile in 36 patients with PA diagnosed by confirmatory tests who underwent adrenal venous sampling (AVS). The clinical p

Guidance document: risk assessment of patients with cirrhosis prior to elective non-hepatic surgery.

As a result of the increasing incidence of cirrhosis in the UK, more patients with chronic liver disease are being considered for elective non-hepatic surgery. A historical reluctance to offer surgery to such patients stems from general perceptions of poor postoperative outcomes. While this is true for those with decompensated cirrhosis, selected patients with compensated early-stage cirrhosis can have good outcomes after careful risk assessment. Well-recognised risks include those of general anaesthesia, bleeding, infections, impaired wound healing, acute kidney injury and cardiovascular comp

Serum concentration of taurochenodeoxycholic acid predicts clinically significant portal hypertension.

BACKGROUND & AIMS: Severity of portal hypertension is usually quantified by measuring the hepatic venous pressure gradient (HVPG). However, due to its invasiveness, alternative markers are being sought. Bile acids (BA), being synthesized, metabolized, and transported by the liver, seem to have the potential to serve as endogenous markers. The aim of the present study was to determine whether serum BA reflect the severity of portal hypertension. METHODS: We correlated serum concentrations of individual BA with portal pressure (as HVPG) in an exploratory cohort of 21 cirrhotic patients with por

Influence of genetic variants on remifentanil sensitivity in Chinese women.

WHAT IS KNOWN AND OBJECTIVE: Significant individual differences have been observed in pain sensitivity and analgesic effect of opioids. Previous studies have shown that genetic factors contributed to analgesics requirement obviously. Therefore, we investigated the role of genetic polymorphisms in the sensitivity to the analgesic effect of remifentanil in this study. METHODS: One hundred thirty-seven patients undergoing gynaecological surgery were observed. Before procedures, we measured the basal pain threshold of each patient, including the pressure pain threshold and pressure pain tolerance

Effect of five nights of sleep extension on peripheral vascular function: a randomized crossover investigation into long sleep duration.

Long sleep duration, defined as ≥9 h, is associated with increased cardiovascular mortality. We sought to determine the effect of sleep extension on peripheral vascular health. Twelve middle-aged adults were randomly assigned to spend five nights with 8 h (control) or 10+ hours time in bed (TIB) in a crossover fashion. Sleep was assessed using wrist actigraphy. Peak reactive hyperemia in the forearm was measured using venous-occlusion plethysmography as an index of microvascular vasodilation. Nighttime and morning blood pressure was recorded along with pulse wave velocity (arterial stiffness).

Dural sac shrinkage signs on spinal magnetic resonance imaging indicate overdrainage after lumboperitoneal shunt for idiopathic normal pressure hydrocephalus.

BACKGROUND: We previously found the usefulness of dural sac shrinkage signs (DSSSs), which are the anterior shift of the spinal cord and dura mater behind the cord, detected by magnetic resonance imaging (MRI) at the thoracic level for the diagnosis of spontaneous intracranial hypotension (IH). This is a retrospective survey on the usefulness of DSSSs for the early detection of iatrogenic IH caused by overdrainage through a lumboperitoneal shunt (LPS) for patients with idiopathic normal pressure hydrocephalus (INPH). METHODS: Forty-five INPH patients had an LPS using a pressure programmable v

Clinical features and management of children with dengue-associated obstructive shock syndrome: A case report.

RATIONALE: Dengue obstructive shock syndrome is a fatal complication commonly observed in the late critical phase of dengue infection and is associated with a high mortality rate. The main pathogenesis involves a dramatic increase in chest pressure, owing to severe plasma leakage and mechanical respiratory support, hampering the heart's ability to pump effectively and impeding adequate blood venous return to the heart chambers. To date, there is a paucity of clinical data about Dengue obstructive shock syndrome reported. PATIENT CONCERNS: The 2 reported patients presented with prolonged and d

Optimal NIV Medicare Access Promotion: Patients With Thoracic Restrictive Disorders: A Technical Expert Panel Report From the American College of Chest Physicians, the American Association for Respiratory Care, the American Academy of Sleep Medicine, and the American Thoracic Society.

The existing coverage criteria for noninvasive ventilation (NIV) do not recognize the benefits of early initiation of NIV for those with thoracic restrictive disorders and do not address the unique needs for daytime support as the patients progress to ventilator dependence. This document summarizes the work of the thoracic restrictive disorder Technical Expert Panel working group. The most pressing current coverage barriers identified were: (1) delays in implementing NIV treatment; (2) lack of coverage for many nonprogressive neuromuscular diseases; and (3) lack of clear policy indications for

Optimal cannula positioning of HeartMate 3 left ventricular assist device.

Cannula position in HeartMate II and HeartWare left ventricular assist devices (LVADs) is associated with clinical outcome. This study aimed to investigate the clinical implication of the device positioning in HeartMate 3 LVAD cohort. Consecutive patients who underwent HeartMate 3 LVAD implantation were followed for one year from index discharge. At index discharge, chest X-ray parameters were measured: (a) cannula coronal angle, (b) height of pump bottom, (c) cannula sagittal angle, and (d) cannula lumen area. The association of each measurement of cannula position with one-year clinical outc
