[Pseudoaneurysm after Bentall's procedure in acute aortic dissection type A: diagnosis using electron beam tomography].

Pseudoaneurysms are a rare complication following replacement of the ascending aorta or aorto-coronary bypass surgery. We report a case with replacement of the aorta ascendens, the aortic valve, and venous aorto-coronary bypass grafting, in whom a pseudoaneurysm developed at the site of one proximal bypass anastomosis. For the preoperative diagnosis, an ultrafast-CT (Electron Beam Tomography, EBT) was done, as neither the pseudoaneurysm, nor the venous graft could be visualized by selective coronary angiography.

Surgical treatment for acute type A aortic dissection in pregnancy: a case of aortic root replacement just after Cesarean section.

A 25-year-old woman with Marfan syndrome in the 37th week of pregnancy was referred for acute chest pain and dyspnea. An emergency Cesarean section was performed because of fetal distress. Intraoperative echocardiography at the end of the Cesarean section showed dilatation of the aortic root and dissection of the ascending aorta. The patient underwent replacement of the aortic root and the ascending aorta on the following day because of uterine bleeding. The postoperative course was uneventful for the mother and her baby.

Unexplained fever and aortic regurgitation: a rare presentation of Takayasu's aortitis--a case report.

Takayasu's aortitis is rare in male patients. The authors describe the case of a 48-year-old man with a fever of unknown origin, progressive general fatigue, and dyspnea due to aortic regurgitation. A severely thickened aorta was noted during aortic valve replacement. The diagnosis was based on a histologic examination of an operative specimen of the ascending aorta. Takayasu's aortitis should be considered even in male patients with fever of unknown origin and progressive aortic regurgitation.

Aortic intussusception: a rare presentation of type A aortic dissection evidenced by transesophageal echocardiography.

Thoracic aorta dissection involving the ascending tract (type A) is an affection which can often become lethal. The possibility of reaching immediate diagnosis and setting up surgical treatment are fundamental for the patient prognosis. In the case report, trans-esophageal echocardiography (TEE) made it possible to reach an immediate diagnosis of an unusual presentation of the dissection of type A, the intussusception of the aorta, not diagnosed by other non invasive methodologies, thus permitting timely surgical treatment.

Recurrent aortic occlusion due to malignancy.

A 62-year-old male had repeated episodes of aortic bifurcation occlusion by malignant masses of the same histologic type as the previously excised adenocarcinoma of the ascending colon. In spite of an extensive investigation of the lungs, heart and aorta no trace of the origin and the route of dissemination of the carcinomatous masses was found. Severe suspicion arises of massive malignant direct invasion of the aorta from the local recurrence of the tumor of the descending colon excised 3 years previously.

Anomalous Coronary Origin and Malignant Course in a Young Patient With Palpitation and Demand Ischemia: A Serendipity or a Syndrome Without a Name?

Anomalous coronary artery remains the second most common cause for sudden cardiac death (SCD) in young athletes. The anomaly most commonly associated with SCD is the one that courses between the aorta and pulmonary artery, the malignant course. We present a case of a young gentleman who presented with symptomatic palpitations and was found to have anomalous right coronary artery from ostial left main coronary artery coursing between the aorta and pulmonary artery.

Cardiovocal Syndrome Secondary to Thoracic Aortic Aneurysm: An Old Sign Revisited.

A 56-year-old male, chronic smoker, presented with persistent dry cough, hoarseness of voice and difficulty in swallowing. Indirect laryngoscopy revealed left vocal cord paralysis. Further evaluation revealed eccentric saccular aneurysms arising from the aortic arch and descending thoracic aorta, compressing the trachea, esophagus, left atrium and left recurrent laryngeal nerve. The patient was diagnosed with Ortner's syndrome which is an uncommon presentation of aortic aneurysm. He awaits an endovascular aorta aneurysm repair.

Valve-sparing operation for aortic root aneurysm late after Mustard procedure.

The aortic valve reimplantation technique leads to excellent clinical outcome in patients with aortic valve incompetence and aneurysms of the ascending aorta. This technique is now applied for aneurysms of ascending aorta, aortic dissection type A, and even dilatation of pulmonary autograft after the Ross operation. We report a case of aortic root dilatation late after a Mustard operation for transposition of the great arteries that was successfully managed by valve-sparing aortic root reimplantation.

Giant anastomotic pseudoaneurysm after Bentall operation causing late postoperative cardiogenic shock.

Pseudoaneurysm of the ascending aorta is a rare but severe complication occurring after composite graft surgery for combined aortic valve and ascending aorta disease. The diagnosis of this condition can be difficult because anastomotic pseudoaneurysms show highly variable clinical features depending on the site of the aortic dehiscence and on the involvement of the surrounding structures. We report an unusual case of a late postoperative aortic graft dehiscence, causing acute right heart failure.

Modification of the Norwood procedure in isomeric complete atrio-ventricular canal and hypoplastic right aortic arch.

We report a modification of the Norwood stage I procedure in a neonate with right aortic arch and complete atrio-ventricular canal in a variant of hypoplastic heart syndrome. Because of the unusual anatomy, the neo-aorta was reconstructed with a specially trimmed homograft patch and the pulmonary flow was maintained with implantation of a right ventricle to pulmonary artery shunt on the left side of the aorta. The patient had a favourable outcome and is now awaiting the stage II procedure.

Blunt traumatic avulsion of an intercostal artery: an unusual case of thoracic aortic injury.

The aortic isthmus is the most commonly injured part of the thoracic aorta in patients who survive blunt deceleration injury to that vessel long enough to reach a hospital. Occasionally, avulsion of the brachiocephalic arteries from the aortic arch is seen. The authors describe an unusual form of intrathoracic vascular injury in which mediastinal hemorrhage occurred secondary to avulsion of an intercostal artery from the descending thoracic aorta as a result of a vertical deceleration injury.

Ruptured abdominal aortic aneurysms presenting as radicular compression syndromes.

Two patients are described: one with an aneurysm of the infrarenal aorta and common iliac artery that ruptured posteriorly into the iliac vein, the other with an aneurysm of the distal abdominal aorta that ruptured posteriorly into the iliopsoas muscle. Both patients had symptoms compatible with a radicular compression syndrome. Ruptured aneurysm of one of the major abdominal arteries should be considered in the differential diagnosis of affections of the lumbosacral neural outflow, because immediate operation can be life-saving.

A Confirmed Case of Zoonotic Aortitis and Successful Repair with an Allograft.

INTRODUCTION: Streptococcus equi subspecies zooepidemicus is a β-haemolytic group C streptococcus that is a highly contagious infection in horses and other equines. Streptococcus equi infection of the native human aorta is a rare form of aortitis that has a high mortality owing to rupture. Surgical intervention with debridement and broad spectrum intravenous antibiotics are essential to improve the patient's status. REPORT: In this case study, a 71 year old man with horse contact presented with left groin pain related to S. equi aortitis and mycotic aneurysm. He was successfully treated by ex

Neonatal Death Caused by Interrupted Aortic Arch Associated With 22q11.2 Deletion Syndrome: An Autopsy Case Report.

A case of clinically unsuspected fatal interrupted aortic arch (IAA) is described. A 17-day-old Japanese girl unexpectedly entered respiratory arrest at home. On autopsy, the heart was hypertrophic, with no apparent connection between the ascending and descending aortas. The ascending aorta branched into common carotid and right subclavian arteries, whereas the left subclavian artery arose from the descending aorta, which was supplied by the ductus arteriosus, indicating type B IAA. In addition, ventricular septal defect, bicuspid aortic valve, patent foramen ovale, and thymic aplasia were ide

Chronic Periaortitis: an Update.

PURPOSE OF REVIEW: We aim to review traditional concepts and recent developments on the nosology, pathophysiology, clinical phenotypes and treatment of chronic periaortitis (CP). RECENT FINDINGS: CP is a rare disorder hallmarked by a periaortic fibro-inflammatory tissue. It can present as an isolated disease, but it can also be associated with other autoimmune and fibro-inflammatory lesions (e.g., fibrosing mediastinitis, sclerosing pancreato-cholangitis) that are part of the spectrum of IgG4-related disease. In a subgroup of patients, it also involves the thoracic aorta (so-called "diffuse p

Collagen analysis of the ascending aortic dilatation associated with bicuspid aortic valve disease compared with tricuspid aortic valve.

Dilatation of the ascending aorta is a common occurrence in patients with bicuspid aortic valve (BAV). The aim of the current study was to characterize collagen content in advanced glycation end products (AGEs) of dilated aortic tissue from two distinct areas, concave and convex aortic sites in patients with BAV and TAV. Collagen contents extracted from 100 mg tissue was isolated by enzymatic digestion using pepsin and the nondigested material was further digested using cyanogen bromide, insoluble collagen fraction (ICF) was extracted by hydrochloric acid hydrolysis. BAV tissue showed diminish

A case of neonatal arterial thrombosis mimicking interrupted aortic arch.

Neonatal arterial thrombosis is a very rare entity with clinical findings resembling coarctation of aorta or interrupted aortic arch. A two day-old male newborn was admitted to a different hospital with difficulty in sucking and sleepiness. On echocardiographic examination, a diagnosis of interrupted aortic arch was made and he was treated with prostoglandin E2. When the patient presented to our center, physical examination revealed that his feet were bilaterally cold. The pulses were not palpable and there were ecchymotic regions in the lower extremities. Echocardiography ruled out interrupte

Elective thoracotomy for pedicle screw removal to prevent severe aortic bleeding.

We present a case of a 33-year-old female who sustained multiple injuries of her spine, including spinous process fractures of C5 to C7 and a lamina fracture of C6 and C7. Her thoracic spine showed transverse process fractures of T4 to T10, a compression fracture and lamina fracture of T3, spinous process and transverse process fractures of T4 and T5, a rotation injury of T6, as well as a compression fracture of L1. Thirteen months after posterior thoracic spinal instrumentation, a pedicle screw was suspected to be in contact with the aorta, which was proved by computed tomography angiograms.
