Iron Supplementation in Iron-Replete and Nonanemic Pregnant Women in Tanzania: A Randomized Clinical Trial.

IMPORTANCE: Anemia is common in pregnancy and increases the risk of adverse outcomes. Iron deficiency is a leading cause of anemia in sub-Saharan Africa, and iron supplementation is the standard of care during pregnancy; however, recent trials among children have raised concerns regarding the safety of iron supplementation in malaria-endemic regions. There is limited evidence on the safety of iron supplementation during pregnancy in these areas. OBJECTIVE: To evaluate the safety and efficacy of iron supplementation during pregnancy in a malaria-endemic region. DESIGN, SETTING, AND PARTICIPAN

Postpartum Experiences of Women With a History of Trauma and Substance Use Disorders.

PURPOSE: To explore how relational, health care, and sociocultural systems shape mental health and distress management in postpartum women with trauma and substance use histories. METHOD: Using a qualitative design informed by phenomenological sensibility and ecological systems theory, we conducted semi-structured interviews with seven women who had given birth within the past 5 years and self-disclosed a history of trauma, and self-identified as sober. RESULTS: Reflexive thematic analysis generated seven themes organized across five ecological systems levels: Perception of Mental Health and

Automated diagnosis of plus form and early stages of ROP using deep learning models.

Retinopathy of Prematurity (ROP) represents a critical ophthalmological pathology affecting premature infants, with established associations to low birth weight (BW) and early gestational age (GA). Elevated risk of severe ROP, which can result in irreversible vision loss, is observed in infants exhibiting lower BW and GA. This research investigates the development of an automated diagnostic system designed to classify Plus disease, a marker of abnormal retinal vascularity, and ROP staging, a determinant of disease progression. Specifically, the model facilitates binary classification of Plus d

The work of ICRP Task Group 121 - the effects of preconceptional and intrauterine exposures.

Hereditary effects in future generations following radiation exposure of parents prior to the conception of their children are an important component of the ICRP system of radiological protection as set out in the 2007 Recommendations of ICRP. Also of special interest is the exposure in utero of the embryo/fetus because not only do the consequent carcinogenic and hereditary effects need to be considered (as with individuals exposed after birth), but additionally teratogenic (developmental) effects, such as congenital malformations, need to be taken into account. ICRP Task Group 121 is examinin

Voluntary micronutrient interventions save lives: Evidence from China's free folic acid supplementation policy.

This study assesses the health impacts of a voluntary micronutrient intervention by examining China's Free Folic Acid Supplementation Program (FFASP), a nationwide policy offering free supplements to rural women planning pregnancies since 2008. Using a difference-in-differences design, we estimate that FFASP reduced infant mortality by approximately 2.8%, accounting for 11.7% of the observed narrowing in rural-urban disparities during the intervention period. A cost-benefit analysis indicates that the program averted deaths at $15,552 per life saved, a figure markedly lower than Chinese and gl

Propensity Score Matching Analysis on Interaction Between Noise Exposure and Gestational Hypertension for Preterm Birth Among Female Workers - 4 PLADs, China, 2024-2025.

WHAT IS ALREADY KNOWN ABOUT THIS TOPIC?: Preterm birth (PTB) is a major public health concern. While noise exposure and gestational hypertension (GH) are recognized risk factors for PTB, evidence on their independent and combined effects among female workers remains limited. WHAT IS ADDED BY THIS REPORT?: Using PSM and logistic regression in 7,886 female manufacturing and mining workers, this study confirmed an overall PTB prevalence of 9.3%. Occupational noise and GH independently elevated PTB risk, with a significant positive additive interaction and the highest risk in the co-exposed group

Early and delayed post-cesarean complications: an imaging review.

Cesarean delivery is a common procedure, currently accounting for one-third of live births in the US. In the acute post-operative setting, it can be difficult to distinguish changes of the expected recovery of the uterus from those of complications. A radiologist should be able to recognize expected postoperative findings and their evolution, such as endometrial cavity debris, small hematoma, or peri-incisional edema, when interpreting images at various stages of recovery. Commonly seen acute complications after cesarean delivery include subfascial and bladder flap hematomas, uterine dehiscenc

Coordinated Multidisciplinary Care Enables Safe Vaginal Delivery in Complete Heart Block: A Case Report.

Congenital atrioventricular block (CAVB) is a rare condition that presents unique challenges in pregnancy, especially in patients with permanent pacemaker implantation. Management requires careful coordination to ensure maternal and fetal safety. We report the case of a 35-year-old primigravida with isolated CAVB and a dual-chamber pacemaker (DDD mode), implanted for complete heart block. Pre-pregnancy cardiac evaluation revealed stable ventricular pacing with normal left ventricular function. A multidisciplinary team, including maternal-fetal medicine, cardiology, and electrophysiology, overs

Otology and Audiology: Complications, Challenges, and Concerns in the Patient With Cleft Lip and/or Palate.

Cleft lip and/or palate, a treatable congenital condition, affects around 1 in 700 births, placing a large burden of care on those affected. A multidisciplinary approach to treating the patient involves not only cleft surgery but also a wide array of treatments and preventative therapies to prevent further disease. In this review, the otolaryngology subspecialties, otology and audiology, are discussed in relation to orofacial clefts. As patients with clefts carry a higher risk of permanent hearing loss and speech complications caused by the pathology of their cleft, management of additional di

The potential global cost-effectiveness of prospective Strep A vaccines and associated implementation efforts.

Group A Streptococcus causes a wide range of diseases from relatively mild infections including pharyngitis to more severe illnesses such as invasive diseases and rheumatic heart disease (RHD). Our aim is to estimate the cost-effectiveness of a hypothetical Strep A vaccine on multiple disease manifestations at the global-level. Cost-effectiveness analyses were carried out by building on the potential epidemiological impact of vaccines that align with the WHO's Preferred Product Characteristics for Strep A vaccines. Maximum vaccination costs for a cost-effective vaccination strategy were estima

Congenital Zika Virus Infection Impairs Corpus Callosum Development.

Congenital Zika syndrome (CZS) is a set of birth defects caused by Zika virus (ZIKV) infection during pregnancy. Microcephaly is its main feature, but other brain abnormalities are found in CZS patients, such as ventriculomegaly, brain calcifications, and dysgenesis of the corpus callosum. Many studies have focused on microcephaly, but it remains unknown how ZIKV infection leads to callosal malformation. To tackle this issue, we infected mouse embryos in utero with a Brazilian ZIKV isolate and found that they were born with a reduction in callosal area and density of callosal neurons. ZIKV inf

Enhancing interventions for prevention of mother-to-child- transmission of hepatitis B virus.

Prevention of mother-to-child transmission of hepatitis B virus (HBV) infection is a cornerstone of efforts to support progress towards elimination of viral hepatitis. Current guidelines recommend maternal screening, antiviral therapy during the third trimester of high-risk pregnancies, universal and timely HBV birth dose vaccination, and post-exposure prophylaxis with hepatitis B immunoglobulin for selected neonates. However, serological and molecular diagnostic testing, treatment and HBV vaccination are not consistently deployed, particularly in many high endemicity settings, and models pred

Mitochondrial dynamics in the neonatal brain - a potential target following injury?

The impact of birth asphyxia and its sequelae, hypoxic-ischaemic (HI) brain injury, is long-lasting and significant, both for the infant and for their family. Treatment options are limited to therapeutic hypothermia, which is not universally successful and is unavailable in low resource settings. The energy deficits that accompany neuronal death following interruption of blood flow to the brain implicate mitochondrial dysfunction. Such HI insults trigger mitochondrial outer membrane permeabilisation leading to release of pro-apoptotic proteins into the cytosol and cell death. More recently, ke

Crossmodal reorganisation in deafness: Mechanisms for functional preservation and functional change.

The study of deafness and blindness has contributed unique knowledge to our understanding of the brain, showing that environmental experience critically shapes neural structure and function. Nevertheless, the most prevalent theories of crossmodal plasticity propose opposing views about the function of reorganised cortical regions. Some theories agree on functional preservation, where in the absence of early sensory stimulation, cortical regions respond to a different sensory modality, but perform the same function. Others propose that the absence of sensory stimulation from birth results in co
