Factors influencing monoamine metabolites and tryptophan in patients with alcohol dependence.

Cerebrospinal fluid 5-hydroxyindoleacetic acid (5-HIAA), homovanillic acid (HVA) and tryptophan (TRY) were measured in 36 female patients with alcohol dependence defined by strict operational criteria. Results were compared with a control group of 32 women hospitalized in a neurological unit for peripheral diseases. Several disorders presumably interfering with monoamine metabolism were excluded and diet, psychomotor activity, diurnal variation, nutritional state and similar nonspecific variables controlled in both groups. Only 5-HIAA was found to be decreased in development patients as compar

Growth hormone during catch-up growth and failure of catch-up growth in rats.

Male Long-Evans rats were fasted or given cortisone injections beginning at 37 days of age in order to produce growth retardation. They were then allowed to recover for periods of up to 28 days. GH concentration was measured in trunk blood plasma of rats decapitated after minimal stress. During the recovery period there was a significant increase in plasma GH in both experimental groups. Organ weight/body weight ratios for liver and heart, protein/tissue, DNA/tissue, and DNA/protein ratios of liver, heart, and skeletal muscle were found to remain normal or to return to normal values during the

U.S. Army Mortuary Affairs Soldiers: Predeployment Traumatic Exposures, Mental Health, and Substance Use.

INTRODUCTION: U.S. Army Mortuary Affairs (MA) soldiers collect, identify, and evacuate the remains of deceased personnel from the theater of war. They are exposed to violent death, human remains, information about the deceased, combat, and other dangerous environments, and deployment-related stressors. Understanding the predeployment health and history of trauma exposures is important to planning deployment and postdeployment health. This study examined predeployment traumatic exposures, mental health, and substance use in MA soldiers, including population characteristics, exposure to dead bod

Combining custom retraction, fiducial markers, and splinted implant scan bodies for mandibular complete arch implant scanning: A dental technique.

Intraoral scanning of a severely atrophic mandible remains challenging because of limited crestal landmarks, mobile soft tissues, and restricted scanner access. This dental technique describes a workflow that combines a patient-specific 3-dimensionally printed retractor, temporary parasymphyseal miniscrews as fiducial markers, and implant scan bodies (ISBs) splinted with an orthodontic elastomeric chain and flowable resin for a severely atrophic mandibular arch. The retractor improves exposure of the peri-implant mucosa, the miniscrews provide detectable landmarks for alignment, and the splint

Sarcoidosis presenting as multiple osseous lesions.

Sarcoidosis is a multisystem inflammatory condition presenting with the formation of noncaseating granulomas. These granulomas can be found in nearly every organ of the body, but in 90% of cases the lungs are involved. Osseous manifestations are seen in only 3% to 13% of cases and are typically seen alongside the more common pulmonary manifestations. These lesions can be misdiagnosed as metastatic cancer so biopsy, along with clinical correlation and exclusion of other diseases, is necessary to make the diagnosis. Most patients with primary osseous sarcoidosis remain asymptomatic but routine m

Clitoroplasty in the correction of clitoral hypertrophy: description of a technique that spares neurovascular structures.

Clitoral hypertrophy is a condition that has a negative impact on a womans intimate life and can cause embarrassment and impact on her sexual life. The article describes a surgical technique of clitoroplasty performed with a 360° circumferential subcoronal incision only in the skin and Dartos tunica to avoid neuronal damage, followed by degloving to the base of the clitoris, preserving the dorsal neurovascular bundle. The body of the clitoris was amputated, preserving 0.5 cm of the stump of the clitoral shaft, and the glans was sutured to the rest of the remaining spongy tissue. Finally, it is

Primary Canalicular MALT Lymphoma Masquerading as Chronic Canaliculitis.

A 67-year-old female presented with chronic canaliculitis and underwent canalicular marsupialization. During the procedure, a fleshy mass was found in the canaliculus, which was excised completely and sent to pathology. Histology confirmed the diagnosis of extranodal marginal zone mucosa-associated lymphoid tissue lymphoma. The patient underwent staging with positron emission tomography/CT scan, which did not show any hypermetabolic foci elsewhere in the body, so the patient elected to undergo close observation without further treatment. At 12 months of follow-up, the patient has remained dise

Carotid Body: The Primary Peripheral Arterial Chemoreceptor.

The carotid body (CB) is a polymodal chemosensory organ that plays an essential role in initiating respiratory and cardiovascular adjustments to maintain blood gas homeostasis. Much of the available evidence suggests that chronic hypoxia induces marked morphological and neurochemical changes within the CB, but the detailed molecular mechanisms by which these affect the hypoxic chemosensitivity still remain to be elucidated. Dysregulation of the CB function and altered oxygen saturation are implicated in various physiological and pathophysiological conditions. Knowledge of the morphological and

Expression of taste signaling elements in jejunal tissue in subjects with obesity.

Taste-signaling proteins, which are expressed throughout the digestive tract, are involved in regulating metabolism and immunity. This study aimed to determine if these genes are expressed and altered in jejunal tissues from patients with extreme obesity who received bariatric surgery. Reverse transcription polymerase chain reaction revealed that phospholipase C beta 2 and transient receptor potential channel M5 expression was downregulated in the jejunum of patients with a body mass index above 50, whereas gustducin expression remained unchanged. Our data suggest that taste-signaling dysregul

A case of positional central sleep apnea due to compression of the left vertebral artery on brainstem.

Studies evaluating the association between Central Sleep Apnea (CSA) and positional sleep apnea are not commonly described and are barely understood. We report a case of a 51-year-old-male with moderate Obstructive Sleep Apnea (OSA) and severe CSA probably secondary to brainstem compression, which responded to the adoption of strict lateral body posture. The addition of Continuous Positive Airway Pressure (CPAP) optimally resolved the remaining obstructive respiratory events. We suggest including Magnetic Resonance Imaging (MRI) in the work-up plan of patients with positional CSA.

A Quick Reference on Phosphorus.

Phosphorus, or phosphate, is the body's major intracellular anion involved in numerous biological processes. Most phosphate is intracellular, with the remaining amount contained within soft tissues and the extracellular space. Parathyroid hormone, calcitriol, calcitonin, and phosphatonins regulate normal phosphate homeostasis by adjusting renal and/or gastrointestinal absorption and/or excretion. Hypophosphatemia occurs secondary to decreased gastrointestinal absorption, transcellular shifts, increased renal excretion, or some combination of these general mechanisms. Hyperphosphatemia results

Caruncular Leishmaniasis--An Unusual Case.

CASE: A 50-year-old patient presented with a left eye caruncular mass of 5-year duration. Examination revealed a fleshy mass originating from the left nasal upper and the lower lid, involving the nasal conjunctiva and the whole of the cornea. The mass was excised and histopathological examination revealed the presence of LD bodies, the hallmark feature of leishmaniasis. CONCLUSION: Ocular leishmaniasis is a potentially blinding disease and delay in diagnosis and treatment can cause irreversible damage to the eye and adnexae. Due to the uncommon occurrence of this disease, its diagnosis and tr

Strangulated tension viscerothorax with gangrene of the stomach in missed traumatic diaphragmatic rupture.

Acquired diaphragmatic hernias are usually posttraumatic in occurrence. In patients who have blunt trauma and associated diaphragmatic hernia, the diagnosis may be missed or delayed, often leading to poor treatment outcomes. We present a rare occurrence of tension viscerothorax due to missed traumatic diaphragmatic rupture in a 25-year-old woman whose condition was complicated by gangrene and perforation of the fundus as well as questionable viability of the anterior wall of the body of the stomach. The patient had a successful emergency transabdominal suture plication of the diaphragm and gas

A review of non-cystic fibrosis pediatric bronchiectasis.

With the implementation of vaccination programs and the use of antibiotics, developed countries have seen a decline in infection-related pediatric bronchiectasis. However, significant morbidity from bronchiectasis is still seen and both infectious and noninfectious causes of bronchiectasis in the pediatric population remain. A review of the literature will be presented including causes of pediatric bronchiectasis, clinical symptoms and signs, laboratory evaluation and imaging, as well as treatment options. This review stresses the importance of early evaluation and treatment in children with r

[Infective endocarditis due to Bartonella quintana].

We present the clinical case of a man of 68 years who was admitted for dizziness and sensation of loss of conscience. The clinical examination revealed a body temperature of 37.5 degrees C and a murmur of mitral regurgitation. The echocardiogram showed a severe mitral regurgitation and left cavitie's dilatation; transesophageal echocardiogram showed a vegetation in the anterior leaflet of the mitral valve. In blood cultures grew a Gram-negative bacteria identified as Bartonella spp. A PCR demonstrated that it was a Bartonella quintana. The patient was treated with gentamicin, doxiciclin and ce

Synovial chondromatosis of the temporomandibular joint with middle cranial fossa extension.

Synovial chondromatosis of the temporomandibular joint (TMJ) is relatively rare. An unusual case with extension through the glenoid fossa and into the middle cranial fossa is reported. Invasion of the infratemporal fossa and the middle cranial fossa was seen on both computed tomography and magnetic resonance imaging. Complete removal of the loose bodies with excision of the affected synovium is the accepted treatment of synovial chondromatosis. A conservative approach should be followed while trying to eliminate any remaining lesion in the infratemporal fossa and the middle cranial fossa. An o

Caring for the foot mobile: holistic foot and nail management.

Proper foot and nail management is essential to maintaining a good quality of life and remaining independent as we age. Basic and ongoing care of the feet and proper shoe selection are critical not only for preventing diabetic foot problems, but also for helping to lead productive, balanced lives. When our feet hurt, the rest of our body most likely hurts too. Because proper foot and nail care and education are critical for preventing ulceration and amputation, especially in persons with diabetes, this outcome case study presents a holistic approach to the management of foot and nail problems.

Considerations of isolated destruction of the short process of the incus in a case of cholesteatoma.

Cholesteatoma occurs frequently with the destruction of the ossicles. The most commonly eroded ossicle has been reported to be the incus. Many studies report destruction of the long process and body of the incus. However, isolated erosion of the short process of the incus is an unusual finding. We present such a case with a slight conductive hearing loss that remained unchanged at 2 1/2 years postoperatively. The short process of the incus may contribute to hearing up to 10-15 dB. It is also possible that it plays a role in epitympanic aeration by supplying an attachment surface to the incudal

Sonography of the knee: intra-articular pathology.

MRI is the gold-standard imaging technique for evaluation of the intra-articular structures of the knee, and the use of sonography remains controversial. Sonography nevertheless is a useful alternative in several conditions: Inflammatory joint diseases, where it is appropriate for early diagnosis and follow-up of joint effusion and synovitis; peri-articular masses, where it is the best modality for guidance of needle puncture or biopsy; suspected meniscus or ligament lesion, where it may provide a positive diagnosis but is not sufficient to exclude intra-articular lesions; and loose bodies, wh

Schwannian crystalline-like inclusions bodies (Fardeau-Engel bodies) revisited in peripheral neuropathies.

In 1969, Fardeau and Engel described polygonal organelles containing crystalline-like structures located in the Schwann cell cytoplasm of unmyelinated fibers. Such inclusions were reported in various conditions, mainly with axonal lesions. They were also reported in a few cases of peripheral neuropathy associated with primary mitochondriopathy. Although they are surrounded by a double membrane, their mitochondrial origin is not definitely proven. Their significance remains obscure but they deserve to be better known as Fardeau-Engel bodies, so as not to be mistaken with mitochondrial crystalli
