Prediction of dementia using CT imaging in stroke (PRODUCTS).

INTRODUCTION: A better understanding of who will develop dementia can inform patient care. Although MRI offers prognostic insights, access is limited globally, whereas CT-imaging is readily available in acute stroke. We explored the prognostic utility of acute CT-imaging for predicting dementia. PATIENTS AND METHODS: We included stroke or transient ischaemic attack (TIA) survivors from participating stroke centres in Scotland. Acute CT-scans were rated using ordinal scales for neurodegenerative and cerebrovascular changes (old infarcts, white matter lesions (WMLs), medial temporal lobe atroph

Memory Impairment, Pro-Inflammatory Host Response and Brain Histopathologic Severity in Rats Infected with K. pneumoniae or P. aeruginosa Meningitis.

Meningitis caused by Klebsiella pneumoniae and Pseudomonas aeruginosa has lately become a prevalent cause of the central nervous system (CNS) infection. Bacterial invasion into the subarachnoid space prompts the releasing mechanism of chemokines and pro-inflammatory cytokines. The present study aimed to compare K. pneumoniae and P. aeruginosa meningitis concerning the memory, pro-inflammatory mediators and brain histopathological changes at different time intervals in adult Albino rats. The animals were sacrificed at three time intervals comprising 5, 10 and 15 days after meningitis induction.

Erythrocyte Membrane-Enveloped Salvianolic Acid B Nanoparticles Attenuate Cerebral Ischemia-Reperfusion Injury.

Источник пометил публикацию как отозванную

PURPOSE: Ischemic stroke is the second leading cause of death and the third leading cause of disability worldwide. Salvianolic acid B (SAB), a water-soluble phenolic acid derived from the traditional Chinese medicine Salvia miltiorrhiza, exerted protective effects on cerebral ischemia-reperfusion injury. However, the efficacy of SAB is seriously hindered by poor blood brain barrier (BBB) permeability and short biological half-life in plasma. Brain targeted biomimetic nanoparticle delivery systems offer much promise in overcoming these limitations. METHODS: A brain targeted biomimetic nanomedi

Neuroinflammation as a target for treatment of stroke using mesenchymal stem cells and extracellular vesicles.

Ischemic stroke is the third cause of death in the developed countries and the main reason of severe disability. Brain ischemia leads to the production of damage-associated molecular patterns (DAMPs) by neurons and glial cells which results in astrocyte and microglia activation, pro-inflammatory cytokines and chemokines production, blood-brain barrier (BBB) disruption, infiltration of leukocytes from the peripheral blood into the infarcted area, and further exacerbation of tissue damage. However, some immune cells such as microglia or monocytes are capable to change their phenotype to anti-inf

Alterations in brain fluid physiology during the early stages of development of ischaemic oedema.

Oedema occurs when higher than normal amounts of solutes and water accumulate in tissues. In brain parenchymal tissue, vasogenic oedema arises from changes in blood-brain barrier permeability, e.g. in peritumoral oedema. Cytotoxic oedema arises from excess accumulation of solutes within cells, e.g. ischaemic oedema following stroke. This type of oedema is initiated when blood flow in the affected core region falls sufficiently to deprive brain cells of the ATP needed to maintain ion gradients. As a consequence, there is: depolarization of neurons; neural uptake of Na+ and Cl- and loss of K+; n

Intravenous Infusion of Autoserum-Expanded Autologous Mesenchymal Stem Cells in Patients With Chronic Brain Injury: Protocol for a Phase 2 Trial.

BACKGROUND: Brain injuries resulting from motor vehicle accidents and falls, as well as hypoxic insults and other conditions, are one of the leading causes of disability and death in the world. Current treatments are limited but include continuous rehabilitation, especially for chronic brain injury. Recent studies have demonstrated that the intravenous infusion of mesenchymal stem cells (MSCs) has therapeutic efficacy for several neurological diseases, including stroke and spinal cord injury. OBJECTIVE: The objective of our investigator-initiated clinical trial is to assess the safety and pot

Melatonin mitigates type 1 diabetes-aggravated cerebral ischemia-reperfusion injury through anti-inflammatory and anti-apoptotic effects.

INTRODUCTION: Cerebral ischemia and diabetes mellitus (DM) are common diseases that often coexist and interact with each other. DM doubles the risk of ischemic stroke, and cerebral ischemia causes stress-induced hyperglycemia. Most experimental stroke studies used healthy animals. Melatonin is neuroprotective against cerebral ischemia-reperfusion injury (CIRI) in non-DM, normoglycemic animals through anti-oxidant effect, anti-inflammation, and anti-apoptosis. Previous studies have also reported a negative correlation between hyperglycemia and urinary melatonin metabolite. OBJECTIVES: The pres

Predictors of Good Outcomes in Patients with Failed Endovascular Thrombectomy.

OBJECTIVE: Endovascular thrombectomy (EVT) fails in approximately 20% of anterior circulation large vessel occlusion (AC-LVO). Nonetheless, the factors that affect clinical outcomes of non-recanalized AC-LVO despite EVT are less studied. The purpose of this study was to identify the factors affecting clinical outcomes in non-recanalized AC-LVO patients despite EVT. MATERIALS AND METHODS: This was a retrospective analysis of clinical and imaging data from 136 consecutive patients who demonstrated recanalization failure (modified thrombolysis in cerebral ischemia [mTICI], 0-2a) despite EVT for

Early detection value of miR-29a in patients with acute myocardial infarction.

BACKGROUND: Early detection of myocardial remodeling, a critical precursor to heart failure in acute myocardial infarction (AMI), remains inadequate with current biomarkers. Preclinical studies suggest miR-29a plays a role in these processes, yet its diagnostic value in early AMI remains unclear. This study evaluates the diagnostic and prognostic potential of miR-29a as a marker for myocardial remodeling in AMI patients. METHODS: A prospective cohort study enrolled 52 patients with ST-elevation myocardial infarction (STEMI) caused by anterior descending branch occlusion, who underwent primary

Microglial Inhibition Promotes Proliferation and Differentiation of Neural Stem Cells via STAT3/SDF-1/CXCR4 Signaling Pathway in Hypoxic-Ischemic Encephalopathy.

Neonatal hypoxic-ischemic encephalopathy (HIE) may induce substantial neuronal damage. In particular, an overactivation of microglia following HIE represents a pathogenically important process. Previous studies have shown that microglial inhibitors can exert neuroprotective effects in HIE; however, the specific mechanisms underlying these effects have not yet been elucidated. Ligation of the left common carotid artery and exposure to 5% O2 were utilized to produce an HIE model in rats. A number of experimental approaches were then used to determine the effect of a microglial inhibition, achiev

Clinical profiles of MRI-negative ischemia and stroke mimics after thrombolysis: insights into diagnostic uncertainty in acute ischemic stroke care.

BACKGROUND: Intravenous thrombolysis (IVT) for suspected acute ischemic stroke is initiated before definitive imaging confirmation, resulting in treatment of patients later classified as MRI confirmed infarction (MRI +), MRI negative ischemia (MRI -), or stroke mimics. Characterization of MRI - and mimic presentations may support assessment under uncertainty. METHODS: We conducted a retrospective cohort study using a prospectively maintained stroke registry at a comprehensive stroke center between (2019-2025). Consecutive patients treated with IVT within 4.5 h of symptom onset who underwent b

Ischemic Stroke as a Possible Initial Manifestation of Primary Sjögren's Syndrome: A Case Report.

Primary Sjögren's syndrome (pSS), a chronic autoimmune disorder primarily affecting the exocrine glands, typically presents with sicca symptoms. Neurological involvement in pSS is uncommon as an initial presentation. Ischemic stroke, in particular, is a rare manifestation, while seizures are more commonly secondary to underlying structural or inflammatory central nervous system involvement. Here, we report a 41-year-old female who presented with a generalized tonic-clonic seizure preceded by left-sided tinnitus and associated with tongue biting, ocular deviation, and postictal confusion. Notab

Clinical application research of brain natriuretic peptide in patients with aneurysmal subarachnoid hemorrhage.

This study investigated the relationship between increased brain natriuretic peptide (BNP) levels following aneurysm rupture and the incidence of major adverse cardiac events (MACE), perioperative complications, and long-term neurological outcomes. We included patients with aneurysmal subarachnoid hemorrhage within 72 hours of bleeding, collecting comprehensive clinical data. Participants were divided into 2 groups based on initial serum BNP levels: elevated (>100 pg/mL) and non-elevated (≤100 pg/mL). To mitigate bias, propensity score matching was employed to balance statistically significant

Wrong-Way Eye Deviation During Eye Opening.

In most cases, eye deviation caused by brain lesions points toward the side of the lesion in supratentorial strokes and away from the lesion in infratentorial strokes. However, in rare cases of supratentorial lesions, the eyes may deviate to the opposite side, a condition known as "wrong-way eye deviation." A woman in her 60s with a history of untreated hypertension and heavy smoking was found unconscious at home and brought to the emergency department. On arrival, she was comatose, with a Glasgow Coma Scale score of 4 (E1V1M2), complete right-sided paralysis, and rightward eye deviation. Nota

Possible involvement of NAMPT in neuronal survival in cerebral ischemic injury under high-glucose conditions through the FoxO3a/LC3 pathway.

The incidence of cerebral infarction triggered by abnormal glucose tolerance has increased; however, the relationship between glucose concentration in the brain and the detailed mechanism of post ischemic cell death remains unclear. Nicotinamide phosphoribosyltransferase (NAMPT), an adipocytokine, is the rate-limiting enzyme for NAD+ synthesis in the salvage pathway. Although NAMPT activation prevents neuronal injury, the relationship between NAMPT activity, glucose metabolism disorders, and cerebral ischemia-induced neuronal cell death is unknown. In this study, we determined changes in NAMPT
