Understanding resistance in glioblastoma: insights into personalized and targeted therapeutic strategies.

Glioblastoma multiforme (GBM) is the most common and aggressive primary malignant brain tumor. Despite combined treatments, including surgical removal followed by radiation and chemotherapy, the prognosis remains poor. Even with temozolomide, the current standard for GBM treatment, the disease is still incurable because of GBM's highly invasive nature and resistance to therapy. This review examines the contributions of key DNA repair pathways, including O6-methylguanine-DNA methyltransferase, base excision repair, and homologous recombination, to the resolution of DNA lesions induced by therap

Fusobacterium nucleatum as a two-hit amplifier in colorectal cancer pathogenesis: intersection of local and systemic mechanisms via the gut-kidney axis.

Colorectal cancer (CRC) is a complex malignancy driven by genetic, environmental, and lifestyle factors. A critical contributor to CRC tumor initiation and progression is gut microbial dysbiosis. Fusobacterium nucleatum, an oral commensal bacterium, is a potent promoter of malignancy rather than a passive colonizer. This review examines the molecular mechanisms by which F. nucleatum promotes CRC progression, focusing on one indirect systemic and three direct local mechanisms. First, the lipopolysaccharide-Toll-like receptor 4 axis sustains a chronic inflammatory loop that remodels the tumor mi

Immunotherapy advancements in high-grade serous ovarian cancer: From promise to practice.

High-grade serous ovarian carcinoma (HGSC) is the most common epithelial ovarian cancer subtype, with high recurrence rates and poor overall survival. Despite progress with cytoreductive surgery, platinum-based chemotherapy, and poly(ADP-ribose) polymerase (PARP) inhibitors, treatment outcomes remain poor, underscoring the need for novel approaches. Immunotherapy is currently the standard of care for the management of several solid tumors, but its role in HGSC is still emerging. This review synthesizes current evidence on the immune landscape of HGSC, predictive biomarkers, resistance mechanis

Target, silence, replace: a review on RNA-based drugs in modern medicine.

RNA therapies have evolved into a revolutionary approach in contemporary medicine for treating various diseases by directly targeting RNA molecules engaged in disease pathogenesis. These therapeutic agents regulate biological processes through diverse mechanisms, including modulation of RNA function and gene expression. Medical applications of RNA are greatly enhanced by its structure, adaptability, and capacity for targeted binding. Among these traits is its ability to bind to certain molecules unique to those chemicals. RNA-based treatments have emerged from advancements in the production, m

Cross-reactive tissue-resident memory T lymphocytes-concepts, evidence, and open questions.

BACKGROUND: Tissue-resident memory T (Trm) cells have emerged as a distinct lymphocyte lineage that provides rapid, frontline immune surveillance in peripheral tissues while also contributing to durable systemic immunity. A critical and evolving aspect of Trm cell biology is their inherent cross-reactivity-the ability of a single T cell receptor (TCR) to recognize and respond to multiple related or unrelated antigens beyond its primary target. MAIN BODY: This review synthesizes current knowledge on the dual roles of cross-reactive Trm cells, which can mediate broad protection against heterolo

Mechanism-matched Helicobacter pylori treatment in the potassium-competitive acid blocker era.

Helicobacter pylori infection remains common worldwide and causes chronic gastritis, peptic ulcer disease, gastric mucosa-associated lymphoid tissue lymphoma, and gastric cancer. Eradication therapy has become less reliable as resistance to clarithromycin, metronidazole, and levofloxacin has reduced the performance of older empiric regimens. Potassium-competitive acid blockers (P-CABs), molecular resistance testing, fecal susceptibility assays, and new antimicrobial strategies have changed treatment selection, but none removes the need to match regimen design to microbial biology and antibioti

Toll like receptor agonist effects on human CD8+ T cell activation and expression of T cell checkpoint receptors.

We have previously found that murine CD8+ T cells activated with cognate antigen increase the expression of PD-1 and other checkpoint receptors, and this can be attenuated in the presence of specific Toll-like receptor (TLR) agonists. In the current report, we sought to investigate whether TLR agonists similarly affect the expression of T cell checkpoint receptors on human CD8+ T cells and their function. Cryopreserved human peripheral blood mononuclear cells (PBMCs) containing CD8+ T cells reactive to human cytomegalovirus (CMV) or Epstein-Barr virus (EBV) were stimulated with virus-specific

Mechanisms of action and therapeutic potential of PCSK9-regulating drugs.

CONTEXT: Proprotein convertase subtilisin/kexin type 9 (PCSK9) regulates plasma low-density lipoprotein cholesterol (LDL-C) metabolism and is a key target for cardiovascular therapies. It also plays roles in inflammation, cancer, and metabolic disorders, prompting interest in repurposing PCSK9-targeting drugs for non-lipid conditions. OBJECTIVE: This review comprehensively summarizes PCSK9-regulating medications, delves into their mechanisms of action, and explores their increasingly expanding therapeutic potential across multiple organ systems, such as the liver, immune system, small intesti

Antimycobacterial and immunomodulatory activities of sorafenib in a preclinical mouse model of TB infection through CD4+CD25low and CD8+CD25low effector T cells.

Tuberculosis is a communicable disease caused by Mycobacterium tuberculosis (Mtb). It is one of the major global public health problems that leads to a high morbidity and mortality rate. Drug resistance in Mycobacterium tuberculosis (Mtb) is another significant and persistent public health concern. The development of effective TB vaccines and treatments requires a better understanding of the intricate interactions between M. tuberculosis and host immunity. We previously reported that sorafenib (SRB) reduces bacterial growth by allosterically inhibiting ornithine acetyltransferase (MtArgJ), an

Making cups and rings: the 'stalled-wave' model for macropinocytosis.

Macropinocytosis is a broadly conserved endocytic process discovered nearly 100 years ago, yet still poorly understood. It is prominent in cancer cell feeding, immune surveillance, uptake of RNA vaccines and as an invasion route for pathogens. Macropinocytic cells extend large cups or flaps from their plasma membrane to engulf droplets of medium and trap them in micron-sized vesicles. Here they are digested and the products absorbed. A major problem - discussed here - is to understand how cups are shaped and closed. Recently, lattice light-sheet microscopy has given a detailed description of t

Immune escape of head and neck cancer mediated by the impaired MHC-I antigen presentation pathway.

Tumor immune evasion is a hallmark of Head and Neck Cancers. The advent of immune checkpoint inhibitors (ICIs) in the first-line setting has transformed the management of these tumors. Unfortunately, the response rate of Head and Neck Squamous Cell Carcinomas (HNSCC) to ICIs is below 15%, regardless of the human papillomavirus (HPV) status, which might be partially related with impaired antigen presentation machinery (APM). Mechanistically, HNSCC cells are usually defective in the expression of MHC-I associated APM, while this transcriptional pathway is critical for the activation of tumor-kil

Microfluidic formulation, cryoprotection and long-term stability of paclitaxel-loaded π electron-stabilized polymeric micelles.

Controlled manufacturing and long-term stability are key challenges in the development and translation of nanomedicines. This is exemplified by the mRNA-nanoparticle vaccines against COVID-19, which require (ultra-)cold temperatures for storage and shipment. Various cryogenic protocols have been explored to prolong nanomedicine shelf-life. However, freezing typically induces high mechanical stress on nanoparticles, resulting in aggregation or destabilization, thereby limiting their performance and application. Hence, evaluating the impact of freezing and storing on nanoparticle properties alre

Enhancement of mucosal innate and adaptive immunity following intranasal immunization of mice with a bovine adenoviral vector.

INTRODUCTION: Nonhuman adenoviral (AdV) gene delivery platforms have significant value due to their ability to elude preexisting AdV vector immunity in most individuals. Previously, we have demonstrated that intranasal (IN) immunization of mice with BAd-H5HA, a bovine AdV type 3 (BAdV3) vector expressing H5N1 influenza virus hemagglutinin (HA), resulted in enhanced humoral and cell-mediated immune responses. The BAd-H5HA IN immunization resulted in complete protection following the challenge with an antigenically distinct H5N1 virus compared to the mouse group similarly immunized with HAd-H5HA

Hepatitis B and circadian rhythm of the liver.

The circadian rhythm in humans is determined by the central clock located in the hypothalamus's suprachiasmatic nucleus, and it synchronizes the peripheral clocks in other tissues. Circadian clock genes and clock-controlled genes exist in almost all cell types. They have an essential role in many physiological processes, including lipid metabolism in the liver, regulation of the immune system, and the severity of infections. In addition, circadian rhythm genes can stimulate the immune response of host cells to virus infection. Hepatitis B virus (HBV) infection is the leading cause of liver dis

Phagocytes produce prostaglandin E2 in response to cytosolic Listeria monocytogenes.

Listeria monocytogenes is an intracellular bacterium that elicits robust CD8+ T-cell responses. Despite the ongoing development of L. monocytogenes-based platforms as cancer vaccines, our understanding of how L. monocytogenes drives robust CD8+ T-cell responses remains incomplete. One overarching hypothesis is that activation of cytosolic innate pathways is critical for immunity, as strains of L. monocytogenes that are unable to access the cytosol fail to elicit robust CD8+ T-cell responses and in fact inhibit optimal T-cell priming. Counterintuitively, however, activation of known cytosolic p

Malignant Pleural Mesothelioma: State-of-the-Art on Current Therapies and Promises for the Future.

Malignant pleural mesothelioma (MPM) is a rare, aggressive cancer of the pleural surface associated with asbestos exposure. The median survival of MPM patients is a mere 8-14 months, and there are few biomarkers and no cure available. It is hoped that, eventually, the incidence of MPM will drop and remain low and constant, given that most nations have banned the use of asbestos, but in the meantime, the incidence in Europe is still growing. The exact molecular mechanisms that explain the carcinogenicity of asbestos are not known. Standard therapeutic strategies for MPM include surgery, often c

Systemic adjuvant therapy for adult patients at high risk for recurrent cutaneous or mucosal melanoma: an Ontario Health (Cancer Care Ontario) clinical practice guideline.

BACKGROUND: Previous versions of the guideline from the Program in Evidence-Based Care (pebc) at Ontario Health (Cancer Care Ontario) recommended that the use of high-dose interferon alfa 2b therapy be discussed and offered to patients with resected cutaneous melanoma with a high risk of recurrence. Subsequently, several clinical trials in patients with resected or metastatic melanoma found that immune checkpoint inhibitors and targeted therapies have a benefit greater than that with interferon. It was therefore considered timely for an update to the guideline about adjuvant systemic therapy i

Human Papillomaviruses; Epithelial Tropisms, and the Development of Neoplasia.

Papillomaviruses have evolved over many millions of years to propagate themselves at specific epithelial niches in a range of different host species. This has led to the great diversity of papillomaviruses that now exist, and to the appearance of distinct strategies for epithelial persistence. Many papillomaviruses minimise the risk of immune clearance by causing chronic asymptomatic infections, accompanied by long-term virion-production with only limited viral gene expression. Such lesions are typical of those caused by Beta HPV types in the general population, with viral activity being suppr
