Rationale and design of the LURIC study--a resource for functional genomics, pharmacogenomics and long-term prognosis of cardiovascular disease.

BACKGROUND AND AIM: Coronary artery disease (CAD), arterial hypertension and Type 2 diabetes mellitus are common polygenetic disorders which have a major impact on public health. Disease prevalence and progression to cardiovascular complications, such as myocardial infarction (MI), stroke or heart failure, are the product of environment and gene interaction. The LUdwigshafen RIsk and Cardiovascular Health (LURIC) study aims to provide a well-defined resource for the study of environmental and genetic risk factors, and their interactions, and the study of functional relationships between gene v

Hypertension treatment and risk of cardiovascular disease or mortality across risk equations: results from SPRINT.

The 2025 AHA/ACC blood pressure (BP) management guidelines recommend BP treatment for 10-year cardiovascular disease (CVD) ≥7.5% from the PREVENT equation, replacing the prior pooled cohort equations (PCE). We examined associations between risk in PCE and PREVENT and a composite of CVD or cardiovascular death in SPRINT. Among 1794 participants, 477 (26.6%) were at low risk in both, 119 (6.6%) were high-risk in PREVENT only, 89 (5.0%) were high-risk in PCE only, and 1109 (61.8%) were high-risk in both. Those at high risk in both equations receiving intensive treatment had a lower risk of the pr

Targeting LDL Across the Whole Spectrum of Chronic Kidney Disease: From Pathophysiology to Novel Treatments.

PURPOSE OF REVIEW: The natural fate of chronic kidney disease (CKD) is the progression to dialysis; however, most patients face fatal and nonfatal cardiovascular events throughout their lifetime. We here address the role of low-density lipoprotein cholesterol (LDL-C) in the excess cardiovascular risk and the impact of traditional and innovative LDL-lowering therapies across the whole spectrum of CKD. RECENT FINDINGS: Current guidelines on the prevention of atherosclerotic cardiovascular disease (ASCVD) from European and US cardiology societies recommend the assessment of total cardiovascular

The Role of Anti-Inflammatory Nutrition in Metabolic Syndrome and Cardiometabolic Diseases.

Background: Metabolic syndrome is closely linked to chronic systemic inflammation and intestinal dysbiosis. Anti-inflammatory nutrition is an approach based on the interaction between dietary intake, chronic low-grade inflammation, and the pathophysiological mechanisms of metabolic syndrome and cardiometabolic diseases. This narrative review evaluates the impact of plant-based dietary models, including vegan and modified Mediterranean variants, on key metabolic and inflammatory parameters. Methods: Literature data published in PubMed, Science Direct, Scopus, and Google Scholar between 2017 and

Defining Cardiovascular Endpoints in Oncology Trials: Challenges and Opportunities: A Scientific Statement From the American Heart Association.

The unprecedented expansion of approved oncology therapies has prolonged survival and transformed the prognosis for many patients diagnosed with cancer. However, cancer treatments may be associated with cardiovascular toxicities that manifest through vascular, myocardial, or metabolic pathways, potentially limiting the use of cancer therapeutics and adversely affecting outcomes. Oncology clinical trials provide an important opportunity to evaluate cardiovascular safety signals by generating data on the incidence, timing, and spectrum of toxicities. However, progress has been limited by inconsi

Residual HIV activity and host immunometabolic remodeling during antiretroviral therapy: implications for cardiovascular-kidney-metabolic risk.

Antiretroviral therapy (ART) suppresses productive HIV replication but does not eliminate viral persistence, residual transcription, or host immune perturbation. Low-level viremia (LLV), generally referring to detectable plasma HIV RNA below the threshold for virologic failure, has traditionally been interpreted in relation to assay variability, incomplete ART exposure, resistance, or impending rebound. Persistent or recurrent LLV may signal residual HIV activity or residual antigenic exposure, but it is not a direct measure of these processes and should not be equated with ongoing productive

Baseline risk and outcome benefit from intensive blood pressure targeting. A systematic review.

BACKGROUND: Randomized clinical trials (RCTs) comparing different blood pressure (BP) targets support more stringent systolic BP targets than previously recommended. We investigated the impact of baseline risk on the outcome benefit from intensive BP targeting. METHODS: We examined RCTs testing a tight systolic BP target (<120 mmHg) separately from trials testing a less tight (<125-140 mmHg) intensive target. Cardiovascular death and all-cause death were the primary outcomes of interest. We also examined stroke, heart failure and myocardial infarction. RESULTS: We included 11 trials, with a

Pharmacology in Congenital Diaphragmatic Hernia: A Focus on Cardiovascular Management.

Congenital diaphragmatic hernia (CDH) presents a complex challenge in neonatal care, requiring tailored pharmacological strategies to manage its distinct cardiorespiratory pathophysiology. CDH is commonly associated with pulmonary hypertension, impaired myocardial function, and adverse cardiorespiratory interactions, contributing to significant morbidity and mortality. Effective pharmacotherapy must address these interconnected factors while minimizing complications or side effects. Despite limited randomized controlled trial data specific to CDH, recent reports highlight the benefits of a pre

Definition, identification, implications and management of hypodynamic hypertension in pregnancy.

Hypertensive disorders of pregnancy show different maternal hemodynamic and cardiovascular profiles going from a hyperdynamic to a hypodynamic condition. These different maternal cardiovascular situations might evolve towards different outcomes and might require targeted approaches. In particular, a hypodynamic maternal cardiovascular profile might be associated to severe and early complications of pregnancy (in particular fetal growth restriction, either isolated or associated to hypertensive disorders of pregnancy). This condition is characterized by a constriction of the vessels and a low p

The lowest well tolerated blood pressure: A personalized target for all?

The optimal blood pressure (BP) target for prevention of cardiovascular complications of hypertension remains uncertain. Most Guidelines suggest different targets depending on age, comorbidities and treatment tolerability, but the underlying evidence is not strong. Results of randomized strategy trials comparing lower (i.e., more intensive) versus higher (i.e., less intensive) BP targets should drive the definition. However, these trials tested different BP targets based on systolic BP, diastolic BP or combined systolic and diastolic BP goals. Overall, the more intensive treatment targets redu

[The Influence of Antithrombotic Therapy on the Risk Factors for Cardiovascular Complications in Patients With Coronary Artery Disease And Diabetes Mellitus. Emphasis on Hypercoagulation].

Comorbid diabetes mellitus (DM) in patients with ischemic heart disease (IHD) is a serious factor that significantly impairs the life prognosis and increases the risk of cardiovascular complications (CVC) as well as the likelihood of death. The residual risk of developing CVC in such patients is largely determined by the high thrombotic status, that is associated with hypercoagulation characteristic of DM. Hypercoagulation causes activation of both platelet and coagulation pathways, which leads to an increased susceptibility to thrombosis. In this context, the combined administration of the an

Patterns of Comorbidities and Prescribing and Dispensing of Non-steroidal Anti-inflammatory Drugs (NSAIDs) Among Patients with Osteoarthritis in the USA: Real-World Study.

BACKGROUND: Osteoarthritis (OA) is a major cause of chronic pain. Non-steroidal anti-inflammatory drugs (NSAIDs) are analgesics commonly used for musculoskeletal pain; however, NSAIDs can increase the risk of certain adverse events, such as gastrointestinal bleeding, edema, heart failure, and hypertension. OBJECTIVE: The objective of this study was to characterize existing comorbidities among patients with OA. For patients with OA with and without a coexisting medical condition of interest (CMCOI), we estimated the prevalence of prescribing and dispensing NSAIDs pre-OA and post-OA diagnosis.

In silico hemodynamical simulations show additional benefits of artery wall softening induced by antihypertensive drugs.

BACKGROUND AND OBJECTIVES: Age-related arterial stiffening increases peripheral resistance and decreases arterial distensibility, thus contributing to hypertension, an important risk factor of atherosclerosis. It causes abnormal blood flow, endothelial dysfunction, higher pulse wave velocity, and consequently elevated pressure wave amplitude. METHODS: This paper presents the influence of these changes via multiscale 3D-0D transient computational fluid dynamics simulations of blood flow in five patient-specific geometries of human carotid bifurcation using archetypal flow waveforms for young a

Kidney and cardiovascular-protective benefits of combination drug therapies in chronic kidney disease associated with type 2 diabetes.

Given the substantial burden of chronic kidney disease associated with type 2 diabetes, an aggressive approach to treatment is required. Despite the benefits of guideline-directed therapy, there remains a high residual risk of continuing progression of chronic kidney disease and of cardiovascular events. Historically, a linear approach to pharmacologic management of chronic kidney disease has been used, in which drugs are added, then adjusted, optimized, or stopped in a stepwise manner based on their efficacy, toxicity, effects on a patient's quality of life, and cost. However, there are disad
